    “Crisis” or “opportunity”? COVID-19 pandemic's impact on environmentally sound invention efficiency in China

    IntroductionThe environmentally sound invention (ESI) is a “bridge” between environmental sound technologies (ESTs) and green productions. This study investigates the COVID-19 pandemic's impact on ESI efficiency using a multi-methods model in three stages.MethodsThe ESI efficiency is measured using the Slack-Based Measure (SBM) method in the first stage. By excluding the environmental effect of the pandemic on each province using the stochastic frontier analysis (SFA) model's results in the second stage, this study compares the ESI efficiency change with or without the influence of the pandemic in the third stage.ResultsThe results show that the pandemic can be a “crisis” in the short term, but an “opportunity” in the long term. First, the SBM efficiency results in the first stage show a decrease in the number of the average efficient provinces in which the pandemic is more severe during 2020-2021. Second, results of the spatial Tobit and SFA models provide evidence that the COVID-19 pandemic negatively impacts the ESI efficiency during 2020, this impact is decreasing in 2021, and this impact has a spatial diffusion effect.DiscussionBased on these results, this study discussed the theoretical and political implications. This paper enriches the knowledge of ESTs research and development by proposing a three-stage approach with multi-methods to investigate the influence of the pandemic's impact on ESI efficiency

    A systematic review and Bayesian meta-analysis of the antibiotic treatment courses in AECOPD

    Background: No consensus exists on the antibiotic treatment course for patients with acute exacerbations of chronic obstructive pulmonary disease (AECOPD). Former studies indicate that shorter courses might have the same efficacy with fewer adverse events, which is inconsistent with guidelines and general practice. Existing evidence allows us to conduct a systematic review and Bayesian analysis on this topic.Methods: Four databases were searched from their inception to January 5, 2023. All statistical estimations were performed using R. “Gemtc” was the core package of analysis. CINeMA was used to assess the grade of confidence of the results.Results: Fourteen studies were included in the Bayesian meta-analysis. No difference in the clinical success rate of antibiotic treatment was observed from a super short course (1–3 days) to a long course (≄10 days). Considering the adverse events, the short course (4–6 days) might be the safest. The majority of results were of high or moderate confidence grade.Conclusion: Short course might cause the fewest adverse events. The clinical efficacy of antibiotics might not depend on the course length. Undeniably, more systematic explorations are warranted to investigate the clinical application of a shorter course of antibiotic treatment

    Efficacy and safety of three species of Rhodiola L. in patients with chronic obstructive pulmonary disease: A systematic review and meta-analysis

    Background: Chronic obstructive pulmonary disease (COPD) is characterized by chronic hypoxia, inflammation, oxidative stress, and irreversible airflow limitations. Rhodiola L. is a genus of botanical drugs used in traditional medicine that may influence COPD.Objective: A systematic review of the safety and efficacy of Rhodiola L. in patients with COPD.Material and methods: We searched the PubMed, Embase, Cochrane Library, Web of Science, Scopus, China National Knowledge Infrastructure (CNKI), Chongqing VIP, Wanfang, and SinoMed databases. The search strategy used terms including “COPD” and “Rhodiola.” Two independent reviewers conducted the literature screening, data extraction, and risk of bias assessment, with a third reviewer involved to resolve disagreements. Statistical analysis was conducted in Review Manager (version 5.4.1), following the Cochrane Handbook.Results: This review included nine studies, of which two focused on Rhodiola crenulata (Hook.f. and Thomson) H. Ohba (R. crenulata) and two on Rhodiola kirilowii (Regel) Maxim (R. kirilowii); the remaining five focused on Rhodiola wallichiana (Hook.) S.H.Fu (R. wallichiana). Compared with the placebo, patients who received Rhodiola L. presented no more adverse events (p = 0.65) but showed significant improvement in the percentage of forced expiratory volume in 1 s at prediction (FEV1%pred), forced expiratory volume in 1 s (FEV1), the ratio of forced expiratory volume in 1 s on forced vital capacity (FEV1/FVC), saturation of oxygen in arterial blood, partial pressure of oxygen in arterial blood (PaO2), partial pressure of carbon dioxide in arterial blood (PaCO2), systolic pulmonary arterial pressure, diastolic pulmonary arterial pressure, COPD assessment test, efficient rate, C-reactive protein, and N-terminal pro-B-type natriuretic peptide (all p < 0.01). Compared with ambroxol, R. kirilowii provided additional benefits to patients with COPD in FEV1%pred, FEV1, FEV1/FVC, PaO2, PaCO2, 8-iso-prostaglandin F2α, superoxide dismutase, glutathione, malondialdehyde, and total antioxidant capacity (all p < 0.01).Conclusion: Among the Rhodiola L. genus, this review included R. wallichiana, R. crenulata, and R. kirilowii, which might be safe and effective in COPD.     MARVEL: A Randomized Double‐Blind, Placebo‐Controlled Trial in Patients Undergoing Endovascular Therapy: Study Rationale and Design

    BACKGROUND Steroids have pleiotropic neuroprotective actions including the regulation of inflammation and apoptosis which may influence the effects of ischemia on neurons, glial cells, and blood vessels. The effect of low‐dose methylprednisolone in patients with acute ischemic stroke in the endovascular therapy era remains unknown. This trial investigates the efficacy and safety of low‐dose methylprednisolone (2 mg/kg IV for 3 days) as adjunctive therapy for patients with acute ischemic stroke undergoing endovascular therapy within 24 hours from symptom onset. METHODS The MARVEL(Methylprednisolone as Adjunctive Therapy for Acute Large Vessel Occlusion: A Randomized Double‐Blind, Placebo‐Controlled Trial in Patients Undergoing Endovascular Therapy) trial is an investigator‐initiated, prospective, randomized, double‐blind, placebo‐controlled multicenter clinical trial. Up to 1672 eligible patients with anterior circulation large‐vessel occlusion stroke presenting within 24 hours from symptom onset are planned to be consecutively randomized to receive methylprednisolone or placebo in a 1:1 ratio across 82 stroke centers in China. RESULTS The primary outcome is the ordinal shift in the modified Rankin scale score at 90 days. Secondary outcomes include 90‐day functional independence (modified Rankin scale score, 0–2). The primary safety end points include mortality rate at 90 days and symptomatic intracerebral hemorrhage within 48 hours of endovascular therapy. CONCLUSION The MARVEL trial will provide evidence of the efficacy and safety of low‐dose methylprednisolone as adjunctive therapy for patients with anterior circulation large‐vessel occlusion stroke undergoing endovascular therapy

    Methylprednisolone as Adjunct to Endovascular Thrombectomy for Large-Vessel Occlusion Stroke

    Importance It is uncertain whether intravenous methylprednisolone improves outcomes for patients with acute ischemic stroke due to large-vessel occlusion (LVO) undergoing endovascular thrombectomy. Objective To assess the efficacy and adverse events of adjunctive intravenous low-dose methylprednisolone to endovascular thrombectomy for acute ischemic stroke secondary to LVO. Design, Setting, and Participants This investigator-initiated, randomized, double-blind, placebo-controlled trial was implemented at 82 hospitals in China, enrolling 1680 patients with stroke and proximal intracranial LVO presenting within 24 hours of time last known to be well. Recruitment took place between February 9, 2022, and June 30, 2023, with a final follow-up on September 30, 2023.InterventionsEligible patients were randomly assigned to intravenous methylprednisolone (n = 839) at 2 mg/kg/d or placebo (n = 841) for 3 days adjunctive to endovascular thrombectomy. Main Outcomes and Measures The primary efficacy outcome was disability level at 90 days as measured by the overall distribution of the modified Rankin Scale scores (range, 0 [no symptoms] to 6 [death]). The primary safety outcomes included mortality at 90 days and the incidence of symptomatic intracranial hemorrhage within 48 hours. Results Among 1680 patients randomized (median age, 69 years; 727 female [43.3%]), 1673 (99.6%) completed the trial. The median 90-day modified Rankin Scale score was 3 (IQR, 1-5) in the methylprednisolone group vs 3 (IQR, 1-6) in the placebo group (adjusted generalized odds ratio for a lower level of disability, 1.10 [95% CI, 0.96-1.25]; P = .17). In the methylprednisolone group, there was a lower mortality rate (23.2% vs 28.5%; adjusted risk ratio, 0.84 [95% CI, 0.71-0.98]; P = .03) and a lower rate of symptomatic intracranial hemorrhage (8.6% vs 11.7%; adjusted risk ratio, 0.74 [95% CI, 0.55-0.99]; P = .04) compared with placebo. Conclusions and Relevance Among patients with acute ischemic stroke due to LVO undergoing endovascular thrombectomy, adjunctive methylprednisolone added to endovascular thrombectomy did not significantly improve the degree of overall disability.Trial RegistrationChiCTR.org.cn Identifier: ChiCTR210005172

    Hybrid Wireless Sensor Network Coverage Holes Restoring Algorithm

    Aiming at the perception hole caused by the necessary movement or failure of nodes in the wireless sensor actuator network, this paper proposed a kind of coverage restoring scheme based on hybrid particle swarm optimization algorithm. The scheme first introduced network coverage based on grids, transformed the coverage restoring problem into unconstrained optimization problem taking the network coverage as the optimization target, and then solved the optimization problem in the use of the hybrid particle swarm optimization algorithm with the idea of simulated annealing. Simulation results show that the probabilistic jumping property of simulated annealing algorithm could make up for the defect that particle swarm optimization algorithm is easy to fall into premature convergence, and the hybrid algorithm can effectively solve the coverage restoring problem

    WSNs Microseismic Signal Subsection Compression Algorithm Based on Compressed Sensing

    For wireless network microseismic monitoring and the problems of low compression ratio and high energy consumption of communication, this paper proposes a segmentation compression algorithm according to the characteristics of the microseismic signals and the compression perception theory (CS) used in the transmission process. The algorithm will be collected as a number of nonzero elements of data segmented basis, by reducing the number of combinations of nonzero elements within the segment to improve the accuracy of signal reconstruction, while taking advantage of the characteristics of compressive sensing theory to achieve a high compression ratio of the signal. Experimental results show that, in the quantum chaos immune clone refactoring (Q-CSDR) algorithm for reconstruction algorithm, under the condition of signal sparse degree higher than 40, to be more than 0.4 of the compression ratio to compress the signal, the mean square error is less than 0.01, prolonging the network life by 2 times
