Self-reported clinical history of misdiagnosed leprosy cases in the State of Mato Grosso, Brzil, 2016-2019
This study aimed to analyze the self-reported clinical history of patients misdiagnosed with leprosy in the State of Mato Grosso, Brazil. This is a cross-sectional study of new leprosy cases diagnosed in the State of Mato Grosso from 2016 to 2019, with individuals who were released from multidrug therapy due to misdiagnosis after starting treatment. Data were collected via telephone interviews. Over the study period, 354 leprosy cases were released from treatment due to misdiagnosis, of which 162 (45.8%) could be interviewed. All interviewees expressed dissatisfaction with their treatment, which prompted them to seek a reevaluation of their diagnosis before they were released due to "misdiagnosis". Among them, 35.8% received a final diagnosis of a musculoskeletal or connective tissue disease - mainly fibromyalgia and degenerative changes in the spine - followed by 13.6% with diagnoses of skin and subcutaneous tissue diseases. For 23.5% of the respondents, no alternative diagnosis was established, whereas 7.4% were later re-diagnosed with leprosy. Fibromyalgia and spinal problems were the most common alternative diagnoses for erroneous leprosy. Although the diagnosis of leprosy is usually clinical and does not require access to technical infrastructure in most cases, some more complex situations require diagnostic support via complementary tests, as well as close collaboration between primary care and reference services
Relating pseudospin and spin symmetries through charge conjugation and chiral transformations: the case of the relativistic harmonic oscillator
We solve the generalized relativistic harmonic oscillator in 1+1 dimensions,
i.e., including a linear pseudoscalar potential and quadratic scalar and vector
potentials which have equal or opposite signs. We consider positive and
negative quadratic potentials and discuss in detail their bound-state solutions
for fermions and antifermions. The main features of these bound states are the
same as the ones of the generalized three-dimensional relativistic harmonic
oscillator bound states. The solutions found for zero pseudoscalar potential
are related to the spin and pseudospin symmetry of the Dirac equation in 3+1
dimensions. We show how the charge conjugation and chiral
transformations relate the several spectra obtained and find that for massless
particles the spin and pseudospin symmetry related problems have the same
spectrum, but different spinor solutions. Finally, we establish a relation of
the solutions found with single-particle states of nuclei described by
relativistic mean-field theories with scalar, vector and isoscalar tensor
interactions and discuss the conditions in which one may have both nucleon and
antinucleon bound states.Comment: 33 pages, 10 figures, uses revtex macro

Abstract
We report a case of CD8(+)/V beta 5.1(+) T-cell large granular lymphocyte leukemia (T-LGL leukemia) presenting with mild lymphocytosis, severe autoimmune neutropenia, thrombocytopenia, polyarthritis and recurrent infections with a chronic disease course. Immunophenotyping showed an expansion of CD3(+)/TCR alpha beta(+)/CD8(+bright)/CD11c(+)/CD57(-)/CD56(-) large granular lymphocytes with expression of the TCR-V beta 5.1 family. Southern blot analysis revealed a clonal rearrangement of the TCR beta-chain gene. Hematopoietic growth factors, high dose intravenous immunoglobulin and corticosteroids were of limited therapeutic benefit to correct the cytopenias. During the disease course, the patient developed a severe cutaneous leg ulcer and bilateral vascular mammary skin lesions. Treatment with 2-deoxycoformycin resulted in both clinical and hematological complete responses, including the resolution of vascular skin lesions. Combined immuno-staining with relevant T-cell associated and anti-TCR-V beta monoclonal antibodies proved to be a sensitive method to assess the therapeutic effect of 2-deoxycoformicin and to evaluate the residual disease.

Unusual magnetic relaxation behavior in La0.5Ca0.5MnO3 and Nd0.5Sr0.5MnO3
We have carried out a systematic magnetic relaxation study, measured after
applying and switching off a 5 T magnetic field to polycrystalline samples of
La0.5Ca0.5MnO3 and Nd0.5Sr0.5MnO3. The long time logarithmic relaxation rate
(LTLRR), decreased from 10 K to 150 K and increased from 150 K to 195 K in
La0.5Ca0.5MnO3. This change in behavior was found to be related to the complete
suppression of the antiferromagnetic phase above 150 K and in the presence of a
5 T magnetic field. At 195 K, the magnetization first decreased, and after a
few minutes increased slowly as a function of time. Moreover, between 200 K and
245 K, the magnetization increased throughout the measured time span. The
change in the slope of the curves, from negative to positive at about 200 K was
found to be related to the suppression of antiferromagnetic fluctuations in
small magnetic fields. A similar temperature dependence of the LTLRR was found
for the Nd0.5Sr0.5MnO3 sample. However, the temperature where the LTLRR reached
the minimum in Nd0.5Sr0.5MnO3 was lower than that of La0.5Ca0.5MnO3. This
result agrees with the stronger ferromagnetic interactions that exist in
Nd0.5Sr0.5MnO3 in comparison to La0.5Ca0.5MnO3. The above measurements
suggested that the general temperature dependence of the LTLRR and the
underlying physics were mainly independent of the particular charge ordering
system considered. All relaxation curves could be fitted using a logarithmic
law at long times. This slow relaxation was attributed to the coexistence of
ferromagnetic and antiferromagnetic interactions between Mn ions, which
produced a distribution of energy barriers.Comment: Accepted to PRB as a regular article, 10 figures, Scheduled Issue: 01
