Segmentations of MRI images of the female pelvic floor: A study of inter- and intra-reader reliability
Purpose: To describe the inter- and intra-operator reliability of segmentations of female pelvic floor structures. Materials and Methods: Three segmentation specialists were asked to segment out the female pelvic structures in 20 MR datasets on three separate occasions. The STAPLE algorithm was used to compute inter- and intra-segmenter agreement of each organ in each dataset. STAPLE computed the sensitivity, specificity, and positive predictive values (PPV) for inter- and intra-segmenter repeatability. These parameters were analyzed using intra-class correlation analysis. Correlation of organ volume to PPV and sensitivity was also computed. Results: Mean PPV of the segmented organs ranged from 0.82 to 0.99, and sensitivity ranged from 33 to 96%. Intra-class correlation ranged from 0.07 to 0.98 across segmenters. Pearson correlation of volume to sensitivity were significant across organs, ranging from 0.54 to 0.91. Organs with significant correlation of PPV to volume were bladder (−0.69), levator ani (−0.68), and coccyx (−0.63). Conclusion: Undirected manual segmentation of the pelvic floor organs are adequate for locating the organs, but poor at defining structural boundaries. J. Magn. Reson. Change in Overactive Bladder Symptoms After Surgery for Stress Urinary Incontinence in Women
OBJECTIVE: To assess change in overactive bladder (OAB) symptoms up to 5 years after surgery and to identify associated predictors of change from baseline.
METHODS: This is a secondary analysis of data from three multicenter urinary incontinence (UI) surgical trials of women with stress-predominant mixed UI assigned to Burch colposuspension, autologous fascial sling, or retropubic or transobturator midurethral slings. The primary outcome was improvement of 70% or greater from baseline in symptoms measured by the Urinary Distress Inventory-Irritative subscale. Surgical groups were compared within respective trials. Generalized linear models were fit using 1-year and up to 5-year data.

CONCLUSION: Most women with stress-predominant mixed UI experienced significant improvement in OAB symptoms after incontinence surgery although this initial improvement diminished over time. Obesity blunted symptom improvement.

Holocene pollen records from the central Arctic Foothills, northern Alaska: testing the role of substrate in the response of tundra to climate change
1 To explore the role of edaphic controls in the response of arctic tundra to climate change, we analysed Holocene pollen records from lakes in northern Alaska located on glaciated surfaces with contrasting soil texture, topography and tundra communities. Using indicator taxa, pollen accumulation rates (PARs) and multivariate comparison of fossil and modern pollen assemblages, we reconstructed the vegetational changes at Upper Capsule Lake (Sagavanirktok surface) and Red Green Lake (Itkillik II surface) in response to increased effective moisture between the early and middle Holocene. 2 In the Red Green record, low PARs and the continuous presence of taxa indicative of prostrate-shrub tundra (PST; Equisetum , Polypodiaceae, Thalictrum and Rosaceae) indicate that the vegetation resembled PST throughout the Holocene. During the warm, dry early Holocene (11 300–10 000 cal years BP), PST also occurred on Sagavanirktok surfaces, as evidenced by PST indicators (Bryidae, Polypodiaceae, Equisetum and Rosaceae) in this interval of the Upper Capsule record. However, PARs increased, suggesting increased vegetation cover, PST taxa declined and taxa indicative of dwarf-shrub tundra (DST; Rubus chamaemorus and Lycopodium annotinum ) increased between 10 000 and 7500 cal years BP. 3 We hypothesize that between the early and middle Holocene the fine-textured soils and smooth topography of Sagavanirktok surfaces led to increased soil moisture, greater vegetation cover, permafrost aggradation, anoxic and acidic soil conditions, slower decomposition and the development of a thick organic layer. In contrast, soil moisture remained low on the better-drained Itkillik II surface, and vegetational changes were minor. 4 Landscape-scale substrate variations have an effect on how tundra responds to climate change, suggesting that the response of arctic ecosystems to future variability may be spatially heterogeneous. Frequent Fires in Ancient Shrub Tundra: Implications of Paleorecords for Arctic Environmental Change
Understanding feedbacks between terrestrial and atmospheric systems is vital for predicting the consequences of global change, particularly in the rapidly changing Arctic. Fire is a key process in this context, but the consequences of altered fire regimes in tundra ecosystems are rarely considered, largely because tundra fires occur infrequently on the modern landscape. We present paleoecological data that indicate frequent tundra fires in northcentral Alaska between 14,000 and 10,000 years ago. Charcoal and pollen from lake sediments reveal that ancient birch-dominated shrub tundra burned as often as modern boreal forests in the region, every 144 years on average (+/− 90 s.d.; n = 44). Although paleoclimate interpretations and data from modern tundra fires suggest that increased burning was aided by low effective moisture, vegetation cover clearly played a critical role in facilitating the paleofires by creating an abundance of fine fuels. These records suggest that greater fire activity will likely accompany temperature-related increases in shrub-dominated tundra predicted for the 21st century and beyond. Increased tundra burning will have broad impacts on physical and biological systems as well as on land-atmosphere interactions in the Arctic, including the potential to release stored organic carbon to the atmosphere
A multicentered comparison of measurements obtained with microtip and external water pressure transducers
This study compared simultaneous intravesical pressure readings obtained with catheter-mounted microtip transducers and external water pressure transducer catheters during filling cystometry. Women undergoing multichannel urodynamic testing were randomly assigned to one of three groups: two microtip catheters, two external water pressure transducer catheters, or one of each type. Intravesical pressure was measured simultaneously for each transducer combination in each subject for minimal and maximal Valsalva effort and minimal, moderate, and maximal cough effort at two sequential bladder volumes (150 and 300 ml). Paired t tests were used to compare the means of the intravesical pressure obtained by the two types of catheters. The largest mean differences were observed when comparing microtip and water pressure transducers. Correlations of maximum pressure were consistently high between two microtip transducers and two water pressure transducers but lower for the microtip–water combination. Excellent reproducibility was demonstrated with transducers of similar types for intravesical pressures recorded during Valsalva and cough in women without prolapse. However, considerable variability was seen in pressures recorded by different transducers, particularly dependent on the water catheter manufacturer, indicating that intravesical pressure recordings from microtip and water-based systems are not interchangeable.Peer Reviewedhttp://deepblue.lib.umich.edu/bitstream/2027.42/45850/1/192_2005_Article_27.pd
