    Current Rehabilitation Therapies in Parkinson’s Disease

    Rehabilitation is one of the important non-pharmacological interventions for Parkinson’s disease (PD). At the time of diagnosis, an appropriate exercise regimen can be prescribed based on the patient’s symptoms. Rehabilitative therapies should be continued throughout the disease course. This chapter summarized the standard specifications and research progression on PD from perspectives of assessment and treatment of rehabilitation. The physical therapy, occupational therapy, speech-language therapy, and neuromodulation therapy are the focus of the introduction. Accurate and comprehensive functional assessment is the premise of rehabilitation plan. Various approaches are used individually or in combined targeted at one or more dysfunction. Although there is still no consensus about the optimal approach about intensity, the frequency of treatment sessions, and complexity, rehabilitation is proved to be able to induce short-term, but clinically important benefits, particularly for gait and balance. The rehabilitative program for PD should be targeted to practicing and learning specific activities in the core areas and be tailored to the individual patients’ characteristics. In addition to improving patient’s performance, environmental modification and alleviation of caregivers are also included in rehabilitation intervention. Innovative techniques have been recently proposed: virtual reality and exergaming, motor imagery and action observation, robot-assisted physiotherapy, and nonconventional therapies

    Designing the audience journey through repeated experiences

    We report on the design, premiere and public evaluation of a multifaceted audience interface for a complex non-linear musical performance called Climb! which is particularly suited to being experienced more than once. This interface is designed to enable audiences to understand and appreciate the work, and integrates a physical instrument and staging, projected visuals, personal devices and an online archive. A public premiere concert comprising two performances of Climb! revealed how the audience reoriented to the second performance through growing understanding and comparison to the first. Using trajectories as an analytical framework for the audience ‘journey’ made apparent: how the trajectories of a single performance are embedded within the larger trajectories of a concert and the creative work as a whole; the distinctive demands of understanding and interpretation; and the potential of the archive in enabling appreciation across repeated performances

    Worldwide productivity and research trend of publications concerning electroactive materials and spinal cord injury: A bibliometric study

    Purpose: We investigated the current state and trends in the area during the previous 10 years using bibliometric approaches to evaluate the global scientific output of research on electroactive materials and spinal cord injury.Methods: Studies on spinal cord injury in electroactive materials that were published between 2012 and 2022 were located using the Web of science (WOS) datebase. The software programs bibliometrix R-package and CiteSpace were used to do quantitative analyses of annual publications, nation, author, institution, journal source, co-cited references, and keywords. The studies were categorized by the research’s main points using a qualitative analysis, and publications having more than 10 citations each year.Results: In the final analysis, 1,330 relevant papers or reviews were included. There is an increased tendency in both the average annual citation rate and the number of publications in the discipline. The United States and the University of Toronto are the countries and institutions that have contributed the most to this discipline, respectively. The majority of authors are from the China and United States. Zhang Y is the author with the most published articles and holds the top position in the cited author h-index species. The journal with the highest number of published articles is “Disability and rehabilitation”; the journal is divided into four main areas including physics, materials, chemistry, molecular, and biology. The keyword analysis revealed a shift in research hotspots from schwann cell, fracture, and urinary disorders to carbon-based materials, functional recovery, and surgery. Analysis of qualitative data revealed that the role and mechanism of injectable conductive hydrogels in spinal cord healing after damage is a hot topic of current study, with the mechanism primarily focusing on the inhibition of oxidative stress (Nrf2) and apoptosis (Casepase 3).Conclusion: Our bibliometric analysis indicates that research on electroactive materials for spinal cord injury remains an active field of study. Moreover, contemporary research is concentrated on carbon-based materials, functional rehabilitation, and surgery

    Performance of next-generation sequencing for diagnosis of blood infections by Klebsiella pneumoniae

    ObjectiveKlebsiella pneumoniae (Kp) bloodstream infections (BSI) can be a life-threatening opportunistic infection. We aimed to evaluate the diagnostic accuracy of metagenomic next-generation sequencing (mNGS) for Kp BSI.MethodsWe retrospectively analyzed 72 patients suspected with bloodstream infection and mNGS Kp positive in peripheral blood, who were hospitalized in our hospital from January 2022 to January 2023. Clinical data and laboratory parameters were collected. All patients had blood drawn and other samples for blood mNGS, blood cultures (BC) and other cultures (OC). The accuracy of mNGS results was analyzed according to infection site, clinical indicators, therapeutic effect and routine culture results. The detection of pathogenic microorganisms by blood mNGS and routine culture was compared.ResultsAmong 72 infection patients, 29 cases (40.28%) were BC positive, 43 cases (59.72%) were other culture (OC) positive, 16 cases (22.22%) were both BC and OC positive, 56 cases were positive for both mNGS and routine culture. Among the 56 double-positive cases, mNGS and conventional cultures were completely consistent in 27 cases, partially consistent in 15 cases, and completely inconsistent in 14 cases. Using the clinical diagnosis as the reference standard, There were 51 cases consistent with the results of mNGS with Kp BSI, the clinical consistency was 70.83% (51/72). The coincidence rate of mNGS and clinical diagnosis was higher than that of BC (54.17%, 39/72), indicating a statistically significant difference between the two methods (P<0.01).ConclusionsCurrent evidence indicates that mNGS exhibits excellent accuracy for the diagnosis of Kp BSI. Although it cannot replace blood culture detection technology, it can be used as a supplement to provide stronger diagnostic capabilities for BSI and optimize treatment

    Design, Analysis and Model Predictive Control of an Axial Field Switched-Flux Permanent Magnet Machine for Electric Vehicle/Hybrid Electric Vehicle Applications

    In this paper, an axial field switched-flux permanent magnet (AFSFPM) machine with stator-PM, which has a high power/torque density and efficiency feature as well as shorter axial length, is designed, analyzed and controlled. The topology, operating principle and design procedure of the AFSFPM machine are labored and discussed. The electromagnetic performance, including the flux linkage, back-EMF, cogging torque, winding inductance and field-control capability, are studied based on 3-D finite-element analysis (FEA). In order to investigate the operating performance of the machine in the whole speed range, a continuous-control-set model predictive control (MPC) method for the AFSFPM machine is proposed. Based on the stage control targets, the maximum torque per ampere (MTPA) and maximum output power flux-weakening strategies are presented in constant torque and constant power regions, respectively. Finally, a prototype of AFSFPM machine has been manufactured and experimentally evaluated and the results show that the MTPA strategy increases the load capability and the flux-weakening strategy broadens the constant power operation range. Moreover, the anti-load-disturbance capacity and dynamic response performance are improved under the MPC method. As a result, the proposed AFSFPM machine drive system is excellent alternative for electrical vehicles (EVs) or hybrid EVs (HEVs)

    Effects of Harvesting Intensity on the Growth of <i>Hydrilla verticillata</i> and Water Quality

    The effects of harvesting intensity on the growth of Hydrilla verticillata (L. fil.) Royle as well as water quality were studied in controlled experiments to provide a reference for managing submerged vegetation and purifying the water. The results showed that harvesting had a significant effect on the recovery of shoot growth and H. verticillata height. The harvested group recovered completely or mostly after two harvests, but the recovery time was significantly longer than the control group. The final biomasses of the harvested groups (15%, 30%, 45%, 60%, and 75% harvested) decreased to 66.61%, 49.13%, 43.95%, 43.77%, and 29.94% of the control group, respectively. The greater the harvesting intensity, the fewer the winter buds. Harvesting reduced the number of H. verticillata branches. Repeated harvesting at medium and low intensities during the rapid growth of H. verticillata effectively improved the water quality and inhibited the propagation and growth of phytoplankton. These results show that harvesting controlled the growth of H. verticillata, and that medium and low harvesting intensities were best when considering water quality

    Autoclavable Albumin-Based Cryogels with Uncompromising Properties

    The development of autoclavable hydrogels has been driven by the need for materials that can withstand the rigors of sterilization without compromising their properties or functionality. Many conventional hydrogels cannot withstand autoclave treatment owing to the breakdown of their composition or structure under the high-temperature and high-pressure environment of autoclaving. Here, the effect of autoclaving on the physical, mechanical, and biological properties of bovine serum albumin methacryloyl (BSAMA) cryogels at three protein concentrations (3, 5, and 10%) was extensively studied. We found that BSAMA cryogels at three concentrations remained little changed after autoclaving in terms of gross shape, pore structure, and protein secondary structure. Young’s modulus of autoclaved BSAMA cryogels (BSAMAA) at low concentrations (3 and 5%) was similar to that of BSAMA cryogels, whereas 10% BSAMAA exhibited a higher Young’s modulus value, compared with 10% BSAMA. Interestingly, BSAMAA cryogels prolonged degradation. Importantly, cell viability, drug release, and hemolytic behaviors were found to be similar among the pre- and post-autoclaved cryogels. Above all, autoclaving proved to be more effective in sterilizing BSAMA cryogels from bacteria contamination than UV and ethanol treatments. Thus, autoclavable BSAMA cryogels with uncompromising properties would be useful for biomedical applications

    Endobronchial Valve Treatment of Tuberculous Cavities in Patients with Multidrug-Resistant Pulmonary Tuberculosis: A Randomized Clinical Study

    Background: Multidrug-resistant pulmonary tuberculosis (MDR-PTB) has become a major cause of high morbidity and mortality related to TB. Conventional drug regimens are ineffective for the treatment of MDR-PTB patients with cavities. This study aimed to evaluate the clinical efficacy and safety of one-way endobronchial valves (EBVs) for the treatment of cavities in MDR-PTB patients. Methods: MDR-PTB patients with positive sputum cultures, sputum smears, and cavities were treated with EBVs in the drainage bronchus of the pulmonary cavity between November 2013 and March 2018. The participants comprised those who had failed previous anti-tuberculosis therapy, as determined by drug susceptibility testing. Results: Thirty-five MDR-PTB patients were included, three of whom were lost during follow-up. The size of the lung cavity was reduced in all of the patients after EBV implantation, including the three lost to follow-up. In the remaining 32 patients, the sputum culture conversion (SCC) rate reached 100%, and the cavity closure rate was 68.8%. There were no significant differences in the cavity closure rate between patients aged ≀40 and >40 years, between the upper and lower lobes, or between the use and non-use of linezolid groups (p > 0.05). Interestingly, the cavity closure rate was higher in women than in men (p = 0.005). Moreover, the cavity closure rate correlated with the time to SCC (correlation coefficient, 0.8933; p < 0.0001). There were no severe adverse events in the patients treated with EBV implantation. Conclusion: EBV installation is effective and safe for the treatment of cavities in MDR-PTB patients. The efficacy of EBV treatment may not be affected by age, disease course, or the location of the lung lobe in the cavity
