    Treatment and Prognosis of Hepatitis B Virus Concomitant with Alcoholism

    Hepatitis B virus (HBV) infection is a global disease worldwide. The Asia-Pacific region has a high prevalence of viral hepatitis, and Taiwan is a region of high prevalence of chronic hepatitis B (CHB) with increasing alcoholic liver disease. We have investigated the prognosis and treatment of patients with concomitant hepatitis B virus (HBV) infection and alcoholism. The 10-year cumulative incidence of hepatocellular carcinoma (HCC) is much higher in patients with concomitant alcoholism and HBV infection than in those with alcoholism or HBV infection alone. Treatment with antiviral therapy and abstinence may be started in patients with decompensated cirrhosis and compensated cirrhosis with high HBV DNA. In pre-cirrhotic cases, treatment with antiviral therapy and abstinence may be started in patients with persistently elevated ALT levels and high HBV DNA, and significant fibrosis with minimal elevated or normal ALT levels and mild high HBV DNA. Treatment with antiviral therapy and abstinence reduces the incidence of HCC in patients with concomitant HBV infection and alcoholism. In conclusion, patients with concomitant HBV infection and alcoholism have high incidence of cirrhosis, HCC, and mortality. Treatment with antiviral therapy and abstinence may be started to reduce the incidence of cirrhosis, HCC, and mortality in these patients

    Prognostic Factors Influencing the Patency of Hemodialysis Vascular Access: Literature Review and Novel Therapeutic Modality by Far Infrared Therapy

    In Taiwan, more than 85% of patients with end-stage renal disease undergo maintenance hemodialysis (HD). The native arteriovenous fistula (AVF) accounts for a prevalence of more than 80% of the vascular access in our patients. Some mechanical factors may affect the patency of hemodialysis vascular access, such as surgical skill, puncture technique and shear stress on the vascular endothelium. Several medical factors have also been identified to be associated with vascular access prognosis in HD patients, including stasis, hypercoagulability, endothelial cell injury, medications, red cell mass and genotype polymorphisms of transforming growth factor-β1 and methylene tetrahydrofolate reductase. According to our previous study, AVF failure was associated with a longer dinucleotide (GT)n repeat (n ≥ 30) in the promoter of the heme oxygenase-1 (HO-1) gene. Our recent study also demonstrated that far-infrared therapy, a noninvasive and convenient therapeutic modality, can improve access flow, inflammatory status and survival of the AVF in HD patients through both its thermal and non-thermal (endothelial-improving, anti-inflammatory, antiproliferative, antioxidative) effects by upregulating NF-E2-related factor-2-dependent HO-1 expression, leading to the inhibition of expression of E-selectin, vascular cell adhesion molecule-1, and intercellular adhesion molecule-1

    Tree Ring Characteristics of 30-Year-Old Swietenia Macrophylla Plantation Trees

    Ring characteristics of mahogany (Swietenia macrophylla K.) plantation trees grown in Taiwan were explored. Significant differences in average ring width (RW) and ring density (RD) occurred among three tree-diameter classes and three radial stages of ring numbers. RW in the radial direction decreased from the pith outward to the bark and followed a distinctive three-stage variation pattern (juvenile, transition, and mature zones). RD in the radial direction increased slowly from the pith outward to the bark. Wider tree rings and lower density are associated with juvenile wood close to the pith, whereas narrower tree rings and higher density are typical for mature wood outward toward the bark. RD in overtopped trees was higher than that in dominant trees. However, RW in dominant trees was wider than that in intermediate and overtopped trees. Earlywood density, latewood density, maximum density, and minimum density were the most important factors determining overall RD. There was a weak relationship between RW and RD, indicating that it is unlikely for growth rates of mahogany plantation trees to have a significant impact on wood density

    Set voltage distribution stabilized by constructing an oxygen reservoir in resistive random access memory

    In this letter, the instability mechanism of RRAM was investigated, and a technique was developed to stabilize the distribution of high resistance state (HRS) and better concentrate the SET voltage. In previous research, we found that an interface-type switching characteristic was observed on the I-V curve beneath the filament-type switching behavior, owing to the oxygen accumulation effect. In this letter, this interface-type switching characteristic is used to fit the natural distribution of HRS for an analysis of the instability mechanism. According to the results, the reason for the HRS distribution is the accumulation of extra oxygen ions which are left over from a lower degree of oxygen and oxygen vacancy recombination during the reset process. We propose a solution which creates an extra oxygen reservoir by changing the surface topography of the electrode to store the surplus oxygen ions from the reset process, eliminating the accumulation effect, and indeed improving stability.     A Developmental Learning Approach of Mobile Manipulator via Playing

    Inspired by infant development theories, a robotic developmental model combined with game elements is proposed in this paper. This model does not require the definition of specific developmental goals for the robot, but the developmental goals are implied in the goals of a series of game tasks. The games are characterized into a sequence of game modes based on the complexity of the game tasks from simple to complex, and the task complexity is determined by the applications of developmental constraints. Given a current mode, the robot switches to play in a more complicated game mode when it cannot find any new salient stimuli in the current mode. By doing so, the robot gradually achieves it developmental goals by playing different modes of games. In the experiment, the game was instantiated into a mobile robot with the playing task of picking up toys, and the game is designed with a simple game mode and a complex game mode. A developmental algorithm, “Lift-Constraint, Act and Saturate,” is employed to drive the mobile robot move from the simple mode to the complex one. The experimental results show that the mobile manipulator is able to successfully learn the mobile grasping ability after playing simple and complex games, which is promising in developing robotic abilities to solve complex tasks using games
