    Recurrence of Mitotically Active Cellular Fibroma of the Ovary

    Background. 10% of ovarian fibromatous tumours typically exhibit increased cellularity, mitotic activity, and less frequently nuclear atypia. Therefore, the classification within the group of fibromatous tumours may represent some difficulties, thus, one or several of these features should appear. Case. We introduce the clinical and pathologic features based on one case of recurrence of a mitotically active cellular ovarian fibroma (MACF) in the pararectal fossa. This recurrence took place six years after primary surgery. Macroscopically, the tumour was firm, fibrous, well delimited, yellow-white without gross necrosis. On microscopic examination, it was composed of a densely cellular proliferation of fibrolastic-like cells with bland nuclear features and arranged in a fascicular pattern. There was no sign of significant atypia or necrosis. Conclusion. Recently, this case is the first report of a recurrence of MACF, following primary surgery with no tumoral rupture or surgical difficulty. The clinical outcome of ovarian cellular fibromas (CFs) and MACFs is typically uneventful. This case, however, strongly suggests maintaining a long-term clinical follow-up even though the principal tumour was surgically treated without tumour rupture or in the absence of adherence or any surgical difficulty

    1. (1) The quantitative amino acid pattern of three macroglobulins (IgM) present in the serum of patients suffering from Waldenström paraproteinemia, and of two macroglobulins present in the serum of patients with symptomatic macroglobulinemia has been investigated by chromatography on columns of ion-exchange resin, and compared with a sample of immunoglobins G (IgG). 2. (2) The purity of each preparation of IgM has been controlled by analytical centrifugation, double immunodiffusion in agar, and by immunoelectrophoresis. 3. (3) Chemical analysis appears to support the hypothesis that the greater individual variation in the amino acid patterns of the three IgM in the serum of patients with Waldenström's disease, than observed in the two IgM of the serum of symptomatic macroglobulinemia patients reveals a greater homogeneity in the Waldenström group, owing to a common clonal origin. At any rate, their Chromatographic patterns show sufficient common characteristics to clearly classify them, and perhaps even to determine their genetic type (I or II). 1. (1) Les auteurs ont analysé par la technique de Spackman et al. la composition en acides aminés de trois macroglobulines (IgM) extraites du sérum de patients atteints de la maladie de Waldenström, de deux IgM extraites du sérum de patients présentant une macroglobulinémie symptomatique, ainsi que d'un échantillon d'immunoglobines G (IgG). 2. (2) La pureté des six préparations d'IgM a été contrôlée au préalable par ultra-centrifugation, double immunodiffusion en gélose, et immunoélectrophorese. 3. (3) L'analyse chimique tend à confirmer une hypothèse déjà formulée: la variabilité de la composition en acides aminés des paraprotéines de la maladie de Waldenström implique une grande homogénéité moléculaire et par conséquent l'unicité d'origine pour chaque cas considéré séparément. I1 subsiste toutefois suffisamment de traits communs dans leur profil chromatographique pour ne pas hésiter devant leur parenté de classe (IgM) et savoir même peut-être en reconnaître le type (I ou II). © 1966.SCOPUS: ar.jinfo:eu-repo/semantics/publishe

    Effect of insulin on acid and pepsin secretion in vagotomized and non-vagotomized patients already stimulated by pentagastrin

    The effects of insulin on acid and pepsin secretion were studied in 16 non-vagotomized patients and 14 completely vagotomized patients, during the continuous intravenous infusion of pentagastrin ('Peptavlon ICI' In non-vagotomized patients, insulin did not alter the acid response to pentagastrin, whereas it strongly stimulated pepsin secretion. In contrast, insulin inhibited acid and pepsin secretion in vagotomized patients. The data presented suggest that after vagotomy, the inhibitory effect of insulin on acid secretion may determine the pattern of acid response during an insulin test. © 1971 Informa UK Ltd.SCOPUS: ar.jinfo:eu-repo/semantics/publishe
