    Persisting Cough as the Single Presenting Symptom of an Intrathoracic Tumor in a Nine-Month-Old Child with Adenovirus Airway Infection

    We report on a nine-month-old girl who presented with persisting cough, and diminished ventilation of the left hemithorax. Viral pneumonia was suspected after Adenovirus detection by PCR, but chest X-rays showed a persistent shadowing of the left hemithorax and persistent coughing despite clinical improvement. Because of the discrepancy between clinical and radiological signs further investigations by ultrasound and CT scan were performed, which visualized an intrathroracic tumor. Histopathology confirmed diagnosis of a teratoma. This case highlights the need for careful evaluation by the treating physicians. If the chest X-ray provides a discrepancy to the clinical findings or persistent pathologies exist, differential diagnosis should be discussed and further diagnostics be performed

    Markarian 421's Unusual Satellite Galaxy

    We present Hubble Space Telescope (HST) imagery and photometry of the active galaxy Markarian 421 and its companion galaxy 14 arcsec to the ENE. The HST images indicate that the companion is a morphological spiral rather than elliptical as previous ground--based imaging has concluded. The companion has a bright, compact nucleus, appearing unresolved in the HST images. This is suggestive of Seyfert activity, or possibly a highly luminous compact star cluster. We also report the results of high dynamic range long-slit spectroscopy with the slit placed to extend across both galaxies and nuclei. We detect no emission lines in the companion nucleus, though there is evidence for recent star formation. Velocities derived from a number of absorption lines visible in both galaxies indicate that the two systems are probably tidally bound and thus in close physical proximity. Using the measured relative velocities, we derive a lower limit on the MKN 421 mass within the companion orbit (R \sim 10 kpc) of 5.9 \times 10^{11} solar masses, and a mass-to-light ratio of >= 17. Our spectroscopy also shows for the first time the presence of H\alpha and [NII] emission lines from the nucleus of MKN 421, providing another example of the appearance of new emission features in the previously featureless spectrum of a classical BL Lac object. We see both broad and narrow line emission, with a velocity dispersion of several thousand km s^{-1} evident in the broad lines.Comment: LaTeX (aaspp4 style), 28 pages, 8 figures, to appear in AJ.     Departures From Axisymmetric Morphology and Dynamics in Spiral Galaxies

    New HI synthesis data have been obtained for six face-on galaxies with the Very Large Array. These data and reanalyses of three additional data sets make up a sample of nine face-on galaxies analyzed for deviations from axisymmetry in morphology and dynamics. This sample represents a subsample of galaxies already analyzed for morphological symmetry properties in the R-band. Four quantitative measures of dynamical nonaxisymmetry are compared to one another and to the quantitative measures of morphological asymmetry in HI and R-band to investigate the relationships between nonaxisymmetric morphology and dynamics. We find no significant relationship between asymmetric morphology and most of the dynamical measures in our sample.     Oxygen and Nitrogen in Leo A and GR 8

    We present elemental abundances for multiple HII regions in Leo A and GR 8 obtained from long slit optical spectroscopy of these two nearby low luminosity dwarf irregular galaxies. As expected from their luminosities, and in agreement with previous observations, the derived oxygen abundances are extremely low in both galaxies. High signal-to-noise ratio observations of a planetary nebula in Leo A yield 12 + log(O/H) = 7.30 +/- 0.05; "semi-empirical" calculations of the oxygen abundance in four HII regions in Leo A indicate 12 + log(O/H) = 7.38 +/- 0.10. These results confirm that Leo A has one of the lowest ISM metal abundances of known nearby galaxies. Based on results from two HII regions with high signal-to-noise measurements of the weak [O III] 4363 line, the mean oxygen abundance of GR 8 is 12 + log(O/H) = 7.65 +/- 0.06; using "empirical" and "semi-empirical" methods, similar abundances are derived for 6 other GR 8 HII regions. Similar to previous results in other low metallicity galaxies, the mean log(N/O) = -1.53 +/- 0.09 for Leo A and -1.51 +/- 0.07 for GR 8. There is no evidence of significant variations in either O/H or N/O in the HII regions. The metallicity-luminosity relation for nearby (D < 5 Mpc) dwarf irregular galaxies with measured oxygen abundances has a mean correlation of 12 + log(O/H) = 5.67 - 0.151 M_B with a dispersion in oxygen about the relationship of 0.21. These observations confirm that gas-rich low luminosity galaxies have extremely low elemental abundances in the ionized gas-phase of their interstellar media. Although Leo A has one of the lowest metal abundances of known nearby galaxies, detection of tracers of an older stellar population indicate that it is not a newly formed galaxy as has been proposed for some other similarly low metallicity star forming galaxies.Comment: 36 pages, 7 figures.     The Tully-Fisher Relation as a Measure of Luminosity Evolution: A Low Redshift Baseline for Evolving Galaxies

    We use optical rotation curves to investigate the R-band Tully-Fisher properties of a sample of 90 spiral galaxies in close pairs. The galaxies follow the Tully-Fisher relation remarkably well, with the exception of eight distinct 3-sigma outliers. Although most of the outliers show signs of recent star formation, gasdynamical effects are probably the dominant cause of their anomalous Tully-Fisher properties. Four outliers with small emission line widths have very centrally concentrated line emission and truncated rotation curves; the central emission indicates recent gas infall after a close galaxy-galaxy pass. These four galaxies may be local counterparts to compact, blue galaxies at intermediate redshift. The remaining galaxies have a negligible offset from the reference Tully-Fisher relation, but a shallower slope (2.6-sigma significance) and a 25% larger scatter. We characterize the non-outlier sample with measures of distortion and star formation to search for third parameter dependence in the residuals of the TF relation. Severe kinematic distortion is the only significant predictor of TF residuals; this distortion is not, however, responsible for the slope difference from the reference distribution. Because the outliers are easily removed by sigma clipping, we conclude that even in the presence of some tidal distortion, detection of moderate luminosity evolution should be possible with high-redshift samples the size of this 90-galaxy study.     Neutral Gas Distribution and Kinematics of the Nearly Face-on Spiral Galaxy NGC 1232

    We have analyzed high velocity resolution HI synthesis observations of the nearly face-on Sc galaxy NGC 1232. The neutral gas distribution extends well beyond the optical extent of the galaxy. As expected, local peaks in the HI column density are associated with the spiral arms. Further, the HI column density drops precipitously near the center of the galaxy. Closed contours in the velocity field suggest either that the system is warped, or that the rotation curve declines. The velocity dispersion is approximately constant throughout the system, with a median value of 9.9 +/- 1.8 km/s. When corrected for rotational broadening, there is no indication of a radial trend in the neutral gas velocity dispersion in this galaxy.Comment: 14 pages of text, 10 pages of figures.     Varicella-zoster virus infections in immunocompromised patients - a single centre 6-years analysis

    Background: Infection with varicella-zoster virus (VZV) contemporaneously with malignant disease or immunosuppression represents a particular challenge and requires individualized decisions and treatment. Although the increasing use of varicella-vaccines in the general population and rapid initiation of VZVimmunoglobulins and acyclovir in case of exposure has been beneficial for some patients, immunocompromised individuals are still at risk for unfavourable courses. Methods: In this single center, 6-year analysis we review incidence, hospitalization and complication rates of VZVinfections in our center and compare them to published data. Furthermore, we report three instructive cases. Results: Hospitalization rate of referred children with VZV-infections was 45%, among these 17% with malignancies and 9% under immunosuppressive therapy. Rate of complications was not elevated in these two high-risk cohorts, but one ALL-patient died due to VZV-related complications. We report one 4-year old boy with initial diagnosis of acute lymphoblastic leukemia who showed a rapidly fatal outcome of his simultaneous varicella-infection, one 1.8-year old boy with an identical situation but a mild course of his disease, and an 8.5-year old boy with a steroiddependent nephrotic syndrome. This boy developed severe hepatic involvement during his varicella-infection but responded to immediate withdrawl of steroids and administration of acyclovir plus single-dose cidofovir after nonresponse to acyclovir after 48 h. Conclusion: Our data show that patients with malignant diseases or immunosuppressive therapy should be hospitalized and treated immediately with antiviral agents. Despite these measures the course of VZV-infections can be highly variable in these patients. We discuss aids to individual decision-making for these difficult situations

    P67-phox (NCF2) Lacking Exons 11 and 12 Is Functionally Active and Leads to an Extremely Late Diagnosis of Chronic Granulomatous Disease (CGD)

    Two brothers in their fifties presented with a medical history of suspected fungal allergy, allergic bronchopulmonary aspergillosis, alveolitis, and invasive aspergillosis and pulmonary fistula, respectively. Eventually, after a delay of 50 years, chronic granulomatous disease (CGD) was diagnosed in the index patient. We found a new splice mutation in the NCF2 (p67-phox) gene, c.1000+2T→G, that led to several splice products one of which lacked exons 11 and 12. This deletion was in frame and allowed for remarkable residual NADPH oxidase activity as determined by transduction experiments using a retroviral vector. We conclude that p67-phox which lacks the 34 amino acids encoded by the two exons can still exert considerable functional activity. This activity can partially explain the long-term survival of the patients without adequate diagnosis and treatment, but could not prevent progressing lung damage
