    Factors influencing seniors' willingness to pay intention for exercise in the civil sports and recreation centers

    BackgroundThe increasing trend for an older population is a phenomenon that is well recognized around the world. The percentage of senior citizens participating in sports, recreation, and leisure activities is also increasing in order to sustain a healthier society. As long as seniors recognize the importance and benefits of keeping healthy, they would prefer to be active in various sports and fitness activities. The seniors could engage in making decisions about their own health by exercising regularly and by encouraging their similar-age peers. The civil sports and recreation centers (CSRCs) in Taiwan have adopted operating transfer (OT) and are located mainly in metropolitan areas, with programs consisting of indoor sports offering people daily exercise. The major aims of the CSRC are to keep the citizens in good health and not to be the city government's financial burden. The extant literature examines the determinants of general consumer behavior, neglecting studies on older adults. This study aims to explore the factors influencing seniors' willingness to pay (WTP) more for exercise in CSRCs outside of free hours.MethodsThis mixed-method study consists of quantitative and qualitative analyses. For the quantitative component, we invited five experts in the sports industry from the industry and academia to evaluate the validity of the questionnaire. A questionnaire consisting of motivation, exercise involvement, and constraint constructs in the quantitative method was administered to 193 older adults in CSRCs. The collected data were analyzed through descriptive statistics, t-test, one-way analysis of variance (ANOVA), and structural equation model (SEM).ResultsEmpirical results indicate that WTP in groups of people aged 75–79, 70–74, and 65–69 years is higher than in the group of people aged 85 years and above. Exercise involvement and motivation had more of a positive impact on seniors' WTP.DiscussionsDeveloping a regular exercise habits and having social interaction in exercise courses drive female seniors to pay more for body health. The results of this study provide managers of CSRCs with decision-making recommendations: (1) initiate entry-level sports courses, such as quick chair exercise, to cultivate among seniors the habit of exercise; (2) motivate seniors to participate in sports with other seniors in order to enhance their social activity and raise their WTP more for sports consumption outside of free scheduled periods; and (3) recruit seniors as exercise coaches or personal trainers to pass their experiences with other people of their age. In addition to expanding seniors' social connections, it encourages peers to exercise. Managerial implications and future research are also discussed.ConclusionThis study contributes to exploring the determinants of seniors' WTP intention. Motivation and exercise involvement had a significantly positive impact on seniors' WTP intention

    Refractory gastric variceal bleeding secondary to splenic vein occlusion associated with abdominal lymphadenopathy

    SummarySplenic vein occlusion caused by abdominal lymphadenopathy is rare. We herein present the case of a 80-year-old man with refractory isolated gastric variceal bleeding in the absence of pancreatic or liver disease. Left-sided portal hypertension was confirmed by angiography, and para-aortic lymphadenopathy compressing the splenic vein was identified by serial abdominal computed tomography. Endoscopic sclerosing therapy failed to treat the recurring gastric variceal hemorrhage. Therefore, splenectomy was suggested and the patient was successfully treated. The patient had been variceal bleeding free for 12 months since the surgery. In patients with isolated gastric varices but without advanced liver disease, a variety of diagnostic techniques should be attempted to elucidate the nature of portal hypertension, and left-sided portal hypertension should be suspected. For those cases in which endoscopic treatment failed to treat refractory gastric variceal bleeding, splenectomy can be an effective option

    <p>Abstract</p> <p>Objective</p> <p>Endocervical adenocarcinomas (ECA) and endometrial adenocarcinomas (EMA) are uterine malignancies that have differing biological behaviors. The choice of an appropriate therapeutic plan rests on the tumor's site of origin. In this study, we propose to evaluate whether PR adds value to the performance and test effectiveness of the conventional 3-marker (ER/Vim/CEA) panel in distinguishing between primary ECA and EMA.</p> <p>Methods</p> <p>A tissue microarray was constructed using paraffin-embedded, formalin-fixed tissues from 38 hysterectomy specimens, including 14 ECA and 24 EMA. Tissue microarray (TMA) sections were immunostained with 4 antibodies, using the avidin-biotin complex (ABC) method for antigen visualization. The staining intensity and extent of the immunohistochemical (IHC) reactions were appraised using a semi-quantitative scoring system.</p> <p>Results</p> <p>The three markers (ER, Vim and CEA) and their respective panel expressions showed statistically significant (p < 0.05) frequency differences between ECA and EMA tumors. Although the additional ancillary PR-marker also revealed a significant frequency difference (p < 0.05) between ECA and EMA tumors, it did not demonstrate any supplementary benefit to the 3-marker panel.</p> <p>Conclusion</p> <p>According to our data, when histomorphological and clinical doubt exists as to the primary site of origin, we recommend that the conventional 3-marker (ER/Vim/CEA) panel is easier, sufficient and appropriate to use in distinguishing between primary ECA and EMA. Although the 4-marker panel containing PR also reveals statistically significant results, the PR-marker offers no supplemental benefit to the pre-existing 3-marker (ER/Vim/CEA) panel in the diagnostic distinction between ECA and EMA.</p

    Localized tail state distribution and hopping transport in ultrathin zinc-tin-oxide thin film transistor

    Carrier transport properties of solution processed ultra thin (4 nm) zinc-tin oxide (ZTO) thin film transistor are investigated based on its transfer characteristics measured at the temperature ranging from 310K to 77K. As temperature decreases, the transfer curves show a parellel shift toward more postive voltages. The conduction mechanism of ultra-thin ZTO film and its connection to the density of band tail states have been substantiated by two approaches, including fitting logarithm drain current (log ID) to T-1/3 at 310K to 77K according to the two-dimensional Mott variable range hopping theory and the extraction of density of localized tail states through the energy distribution of trapped carrier density. The linear dependency of log ID vs. T-1/3 indicates that the dominant carrier transport mechanism in ZTO is the variable range hopping. The extracted value of density of tail states at the conduction band minimum is 4.75 x 10(20) cm(-3) eV(-1) through the energy distribution of trapped carrier density. The high density of localized tail states in the ultra thin ZTO film is the key factor leading to the room-temperature hopping transport of carriers among localized tail states.     Aberrant Sensory Gating of the Primary Somatosensory Cortex Contributes to the Motor Circuit Dysfunction in Paroxysmal Kinesigenic Dyskinesia

    Paroxysmal kinesigenic dyskinesia (PKD) is conventionally regarded as a movement disorder (MD) and characterized by episodic hyperkinesia by sudden movements. However, patients of PKD often have sensory aura and respond excellently to antiepileptic agents. PRRT2 mutations, the most common genetic etiology of PKD, could cause epilepsy syndromes as well. Standing in the twilight zone between MDs and epilepsy, the pathogenesis of PKD is unclear. Gamma oscillations arise from the inhibitory interneurons which are crucial in the thalamocortical circuits. The role of synchronized gamma oscillations in sensory gating is an important mechanism of automatic cortical inhibition. The patterns of gamma oscillations have been used to characterize neurophysiological features of many neurological diseases, including epilepsy and MDs. This study was aimed to investigate the features of gamma synchronizations in PKD. In the paired-pulse electrical-stimulation task, we recorded the magnetoencephalographic data with distributed source modeling and time-frequency analysis in 19 patients of newly-diagnosed PKD without receiving pharmacotherapy and 18 healthy controls. In combination with the magnetic resonance imaging, the source of gamma oscillations was localized in the primary somatosensory cortex. Somatosensory evoked fields of PKD patients had a reduced peak frequency (p &lt; 0.001 for the first and the second response) and a prolonged peak latency (the first response p = 0.02, the second response p = 0.002), indicating the synchronization of gamma oscillation is significantly attenuated. The power ratio between two responses was much higher in the PKD group (p = 0.013), indicating the incompetence of activity suppression. Aberrant gamma synchronizations revealed the defective sensory gating of the somatosensory area contributes the pathogenesis of PKD. Our findings documented disinhibited cortical function is a pathomechanism common to PKD and epilepsy, thus rationalized the clinical overlaps of these two diseases and the therapeutic effect of antiepileptic agents for PKD. There is a greater reduction of the peak gamma frequency in PRRT2-related PKD than the non-PRRT PKD group (p = 0.028 for the first response, p = 0.004 for the second response). Loss-of-function PRRT2 mutations could lead to synaptic dysfunction. The disinhibiton change on neurophysiology reflected the impacts of PRRT2 mutations on human neurophysiology
