    Teaching-learning process from service teaching of mental health nursing: experience report

    Objective: Describe the conceptions of resident nurses about the nurse's role in mental health services. Method: Descriptive/reflexive study of the experience-report type, carried out from experiences of nurses from the Multiprofessional Residency Program in Mental Health, guided by the theoretical presuppositions of the Brazilian Psychiatric Reform and structured from the Arch of Charles Maguerez. Results: In-service teaching provided reflections on the residents' conceptions of nurses' performance in mental health services. The key posts were the insertion in the specific nursing care, the construction of the multiprofessional work process, the recognition of their professional identity. Therapeutic relationship and communication, receptiveness, co-responsibility of care and the construction of links with clients, family and work colleagues for the solution of the problems.  Conclusions: The Arch of Charles Maguerez facilitated the teaching process in service. In addition, it provided the perception of the action-reflection-action movement as essential to understand fundamentals of care practices in nursing and to collaborate to the insertion of the nurse and the process of changes, from the daily services of mental health

Rio de Janeiro, RJ, Brasil.Uma das principais dificuldades da Estratégia Saúde da Família (ESF) diz respeito às práticas profissionais que privilegiam ações sobre as doenças em detrimento da integralidade do sujeito doente e das ações de promoção e proteção da saúde no seu contexto de vida, contrariando, desse modo, os preceitos conceituais da Atenção Primária à Saúde (APS). A formação especializada em Saúde da Família desenvolvida em uma perspectiva da política de Educação Permanente em Saúde (EPS) é um dos principais instrumentos de qualificação dos profissionais de saúde, desenvolvendo competências que conduzem à mudança de práticas do cuidado no âmbito da Atenção Básica, em vista à construção de uma nova práxis em saúde. O estudo objetivou analisar a mudança nas práticas de profissionais de saúde, egressos do “Curso de Especialização de Saúde da Família – CESF 2005”, que atuavam na ESF do município de João Pessoa, Paraíba. Participaram do estudo vinte egressos do CESF, vinculados aos cinco Distritos Sanitários do referido município. A investigação partiu da análise documental sobre o curso e a gestão municipal de saúde. Os dados primários foram obtidos por meio da técnica de Grupo Focal (GF), e foi utilizado o método da Análise Coletiva do Trabalho (ACT), que possibilita aos trabalhadores falarem e compreenderem sobre como são realizadas suas atividades de trabalho. As falas são gravadas, transcritas e, posteriormente, validadas pelos profissionais, em sessão de GF. Como conclusões, o estudo revelou que, embora o curso tenha propiciado novas competências e habilidades para o trabalho na ESF, houve mais dificuldades do que facilidades para a ocorrência de mudança de práticas. Entre as dificuldades, os aspectos ligados à gestão pública da saúde municipal e às dimensões socioculturais da população, além do perfil dos trabalhadores que atuam na ESF foram as mais evidentes. Sobre as facilidades, alguns aspectos ligados à gestão pública da saúde municipal; o processo de formação do CESF 2005; as práticas integrativas e complementares, assim como a formação de parcerias constituíram as mais significativas. A análise das monografias de conclusão do curso possibilitou constatar que suas competências atingiram plenamente os objetivos declarados e integraram teoria e prática. O método da ACT mostrou-se potente, por ter revelado outras questões não definidas previamente no projeto, as quais foram denominadas de “Outras singularidades sintomáticas do trabalhador da ESF.” Trata-se de questões relacionadas, principalmente, ao sofrimento e adoecimento vivenciados pelos profissionais no cotidiano do processo de trabalho. A fala dos egressos sobre o trabalho que realizam na ESF e sobre a mudança nas práticas foi uma oportunidade de reflexão profunda sobre a formação, assim como sobre as necessidades do trabalho em saúde, apontando para a necessária busca de adequação, ressaltando questões que, na maioria das vezes, tendem a ser invisíveis para a gestão, causando sofrimento aos profissionais, e tornando as atividades menos atrativas e motivantes para esses.One of the main difficulties of the Family Health Strategy (FHS) makes reference to the professional practices that privilege actions about the diseases to the detriment of the integrality of the sick individual and the actions of health promotion and protection in their life context in opposition to the conceptual precepts of Primary Health Care (PHC). The specialized formation in Family Health, developed in a policy perspective of Permanent Education in Health (PEH), is one of the main instruments for qualifying health professionals, developing competences that lead to the modification of care practices regarding the Basic Attention with the purpose of making up a new praxis in healthcare. The study aimed at analyzing the change in the practices of health professionals, as former students of the “Specialization Course in Family Health (SCFH) – 2005”, who acted at the FHS in the city of João Pessoa, Paraíba. A total of twenty previous SCFH students took part in this study, connected to the five Sanitary Districts of the referred municipality. The investigation derived from the secondary data analysis about the course and the municipal health management. The primary data were collected using the Focal Group (FG) technique, along with the Collective Analysis of Work (CAW) method, which enables workers to talk and understand how their working activities are carried out. The speeches are recorded, transcribed and afterwards validated by the professionals in a FG session. As to the conclusions, though the course has promoted new competences and skills for the work at FHS, it was revealed that the course presented more difficult aspects than easy ones for changing the practices. Amongst the difficulties, the aspects that refer to public health management and the population´s sociocultural aspects, as well as the profile of workers from the FHS were the most prevalent ones. As to the facilities, some characteristics of the town‟s health public management; the formation process of the SCFH 2005; the complementary and integrating practices; and the formation of partnerships were the most noteworthy ones. The analysis of end-of-graduationcourse papers, in the format of monographs, enabled to realize that its competences fully attained the mentioned objectives and integrated theory and practice. The CAW procedure proved to be solid, by revealing other matters not previously defined in the project, which were named “other FHS worker´s symptomatic singularities”. These are issues especially related to suffering and illness experienced in the work process. The speeches of the earlier undergraduates, about the labor fulfilled at FHS and the change in practices, were an opportunity of profound reflection about the formation, as well as the needs of health work, pointing out for the necessary pursue of appropriateness, highlighting issues that, most of the times, tend to remain invisible for the management, causing the professionals to suffer and making the activities less attractive and motivating
