    Rhinovirus Infections in Hematopoietic Stem Cell Transplant Recipients with Pneumonia

    Little is known about the impact of human rhinovirus (HRV) and coronavirus infections in hematopoietic stem cell transplant (HSCT) recipients. We tested bronchoalveolar lavage (BAL) samples obtained from HSCT recipients with acute pulmonary infiltrates for HRV (n = 122) and coronavirus (n = 46) by reverse-transcriptase polymerase chain reaction. HRV RNA was detected in 6 (8%) of 77 patients, and coronavirus RNA was detected in 0 of 46 of BAL samples from HSCT recipients. The fatality rate in HRV-infected patients was high (83%), but all patients had significant coinfections, and the overall mortality rate was not different from that of patients who were negative for HRV in BAL samples. These results suggest that HRV may be a cause of lower respiratory tract infections in HSCT recipients and that its detection in BAL samples is associated with frequent copathogens.     Seismic recurrence intervals and timing of aseismic subduction inferred from emerged corals and reefs of the Central Vanuatu (New Hebrides) Frontal Arc

    The recognition and dating of corals that have been killed by tectonic uplift allow us to date paleoseismic uplifts in the Vanuatu island arc. We recognize corals that record paleouplifts by their similarity to those known to have died during contemporary sudden uplifts and date them (1) by counting annual coral growth bands (only if part of the coral is alive at the time of collection) or (2) by newly developed techniques for obtaining ^(230)Th ages by mass spectrometry. The mass spectrometric method produces isotopic ages with precisions of ±3 to ±9 years (2σ) in the 0–1000 years B.P. time range. The ^(230)Th ages in this time range appear to be accurate. Samples whose ages are known by counting coral growth bands give ^(230)Th ages that are indistinguishable from their growth band ages. By dividing the average increment of uplift for the latest Holocene uplifts by the mean Holocene uplift rate, we can estimate average seismic uplift recurrence intervals for the past 6000 years. The results for each of four central Vanuatu arc segments are (1) North Santo emerged 1.2 m in 1866 A.D. and 0.6 m 107 years later in 1973 A.D. The average coseismic uplift of 0.9 m and mean Holocene uplift rate of 4.3 mm yr^(−1) suggest a longer recurrence interval of 212 years. (2) South Santo emerged 0.29 m in 1946 and 0.26 m 19 years later in 1965, including the related 1971 event. Here the mean Holocene uplift rate is 5.5 mm yr^(−1). The uplift data suggest a longer average recurrence interval of about 51 years. (3) North Malekula emerged 1.23 m near 1729 A. D. and 1.05 m 236 years later in 1965. The mean Holocene uplift rate of 2.7 mm yr^(−1) and mean coseismic uplift of 1.14 m for dated events suggest a longer recurrence interval of 422 years. (4) Part of southernmost Malekula has uplifted continuously or episodically by about 0.35 m from about 1957 until at least mid-1983 A.D. The maximum uplift of 2.7 mm yr^(−1) occurs near a nest of small earthquakes. Both the earthquakes and rapid uplift suggest that interplate slip beneath south Malekula may be continuous, rather than episodic. Episodes of 0.35 m uplift would have to recur every 130 years to maintain the 2.7 mm yr^(−1) uplift rate. In contrast, we find no evidence of interseismic vertical movements for the other three blocks. The most reasonable interpretation of these results is that the seismic recurrence intervals and processes for accommodation of slip are quite different on adjacent arc segments. We have used the most widely accepted moment magnitude relationship to evaluate the accumulated seismic slip caused by large earthquakes occurring since 1920. In all four arc segments this analysis suggests that the seismically radiated moments account for less than one-third to one-tenth of the slip associated with plate convergence. The similarity between the paleoseismic record of uplifts and the contemporary record of coseismic uplifts suggests that this analysis can be generalized to times before 1920. For the northern three segments of central Vanuatu, aseismic slip probably occurs in the same years as large earthquakes because the contemporary coral record records uplifts only in years having large historic earthquakes. This suggests that aseismic slip is not continuous and does not occur at rates which vary slowly over the course of the earthquake cycle. The south Santo segment may have the highest proportion of seismic slip because the mean recurrence interval of 51 years is shortest and the mean Holocene uplift rate of at least 5.5 mm yr^(−1) is the fastest

    Intraoperative identification of esophageal sentinel lymph nodes with near-infrared fluorescence imaging

    ObjectiveIn esophageal cancer, selective removal of involved lymph nodes could improve survival and limit complications from extended lymphadenectomy. Mapping with vital blue dyes or technetium Tc-99m often fails to identify intrathoracic sentinel lymph nodes. Our purpose was to develop an intraoperative method for identifying sentinel lymph nodes of the esophagus with high-sensitivity near-infrared fluorescence imaging.MethodsSix Yorkshire pigs underwent thoracotomy and received submucosal, esophageal injection of quantum dots, a novel near-infrared fluorescent lymph tracer designed for retention in sentinel lymph nodes. Six additional pigs underwent thoracotomy and received submucosal esophageal injection of CW800 conjugated to human serum albumin, another novel lymph tracer designed for uptake into distant lymph nodes. Finally, 6 pigs received submucosal injection of the fluorophore-conjugated albumin with an endoscopic needle through an esophagascope. These lymph tracers fluoresce in the near-infrared, permitting visualization of migration to sentinel lymph nodes with a custom intraoperative imaging system.ResultsInjection of the near-infrared fluorescent lymph tracers into the esophagus revealed communicating lymph nodes within 5 minutes of injection. In all 6 pigs that received quantum dot injection, only a single sentinel lymph node was identified. Among pigs that received fluorophore-conjugated albumin injection, in 5 of 12 a single sentinel lymph node was revealed, but in 7 of 12 two sentinel lymph nodes were identified. There was no dominant pattern in the appearance of the sentinel lymph nodes either cranial or caudal to the injection site.ConclusionNear-infrared fluorescence imaging of sentinel lymph nodes is a novel and reliable intraoperative technique with the power to assist with identification and resection of esophageal sentinel lymph nodes

    Heavy Fermion Behavior, Crystalline Electric Field Effects, and Weak Ferromagnetism in SmOs_{4}Sb_{12}

    The filled skutterudite compound SmOs_{4}Sb_{12} was prepared in single crystal form and characterized. The SmOs_{4}Sb_{12} crystals have the LaFe_{4}P_{12}-type structure with lattice parameter a = 9.3085 Angstroms. Specific heat measurements indicate a large electronic specific heat coefficient of ~880 mJ/mol K^{2}, from which an enhanced effective mass m^{*} ~ 170 m_{e} is estimated. The specific heat data also suggest crystalline electric field (CEF) splitting of the Sm^{3+} J = 5/2 multiplet into a Gamma_{7} doublet ground state and a Gamma_{8} quartet excited state separated by 37 K. Electrical resistivity rho(T) measurements reveal a decrease in rho(T) below ~50 K that is consistent with CEF splitting of ~33 K between a Gamma_(7) doublet ground state and Gamma_{8} quartet excited state. Specific heat and magnetic susceptibility measurements display a possible weak ferromagnetic transition at ~2.6 K, which could be an intrinsic property of SmOs_4Sb_{12} or possibly due to an unknown impurity phase.Comment: 24 pages, 11 Postscript figures, to be published in Physical Review

    Port-access coronary artery bypass grafting: A proposed surgical method

    AbstractMinimally invasive surgical methods have been developed to provide patients the benefits of open operations with decreased pain and suffering. We have developed a system that allows the performance of cardiopulmonary bypass and myocardial protection with cardioplegic arrest without sternotomy or thoracotomy. In a canine model, we successfully used this system to anastomose the internal thoracic artery to the left anterior descending coronary artery in nine of 10 animals. The left internal thoracic artery was dissected from the chest wall, and the pericardium was opened with the use of thoracoscopic techniques and single lung ventilation. The heart was arrested with a cold blood cardioplegic solution delivered through the central lumen of a balloon occlusion catheter (Endoaortic Clamp; Heartport, Inc., Redwood City, Calif.) in the ascending aorta, and cardiopulmonary bypass was maintained with femorofemoral bypass. An operating microscope modified to allow introduction of the 3.5× magnification objective into the chest was positioned through a 10 mm port over the site of the anastomosis. The anastomosis was performed with modified surgical instruments introduced through additional 5 mm ports. In the cadaver model (n = 7) the internal thoracic artery was harvested and the pericardium opened by means of similar techniques. A precise arteriotomy was made with microvascular thoracoscopic instruments under the modified microscope on four cadavers. In three other cadavers we assessed the exposure provided by a small anterior incision (4 to 6 cm) over the fourth intercostal space. This anterior port can assist in dissection of the distal internal thoracic artery and provides direct access to the left anterior descending, circumflex, and posterior descending arteries. We have demonstrated the potential feasibility of grafting the internal thoracic artery to coronary arteries with the heart arrested and protected, without a major thoracotomy or sternotomy.     The First Definitive Binary Orbit Determined with the Hubble Space Telescope Fine Guidance Sensors: Wolf 1062 (Gliese 748)

    The M dwarf binary, Wolf 1062 (Gliese 748), has been observed with the Hubble Space Telescope (HST) Fine Guidance Sensor 3 in the transfer function scan mode to determine the apparent orbit. This is the first orbit defined fully and exclusively with HST, and is the most accurate definitive orbit for any resolved, noneclipsing system. The orbital period is 2.4490 ± 0.0119 yr and the semimajor axis is 01470 ± 00007—both quantities are now known to better than 1%. Using the weighted mean of seven parallax measurements and these HST data, we find the system mass to be 0.543 ± 0.031 M⊙, where the error of 6% is due almost entirely to the parallax error. An estimated fractional mass from the infrared brightness ratio and infrared mass-luminosity relation yields a mass for the primary of 0.37 M⊙, and the secondary falls in the regime of very low mass stars, with a mass of only 0.17 M⊙
