    A clinical study of primary caesarean section at a tertiary care hospital in coastal city: an observational study

    Background: Caesarean section represents the most significant operative intervention in obstetrics and its development and application has saved the lives of countless mothers and infants. However there has been a steady increase in the rate of caesarean sections worldwide. In this study, we aim to find the primary caesarean section rates and strategies to cut it down.Methods: This was a prospective observational study done from January 2018 to June 2019 at Justice K. S. Hegde Charitable Hospital, Deralakatte, Mangaluru which included 379 cases of primary cesarean sections. All patients undergoing primary caesarean section with gestational age >28 weeks confirmed by dates, clinical examination and ultrasound were included in this study. Patients who had undergone previous caesarean section or were less than 18 years of age or were less than 28 weeks of gestation were excluded from the study.Results: In this study the rate of primary caesarean section was found to be 14.5% and the majority of the study subjects belonged to the age group of 18-27 years (62.3%). With respect to parity, primigravida were high in number (71.2%), followed by multigravida. The number of emergency caesarean sections were (79%) more than elective (21%). The most common intra-operative complication was post-partum haemorrhage (PPH) in 2.8% and the most common postoperative complication was wound gape in 2.1%.Conclusions: Maximum efforts should be made to allow progression of vaginal delivery in primigravida which helps us to bring down the primary caesarean rate

    VALIDATED STABILITY INDICATING HIGH PERFORMANCE LIQUID CHROMATOGRAPHIC METHOD FOR THE QUANTIFICATION OF ASENAPINE MALEATE

    Objective: A stability indicating high performance liquid chromatography method using the photodiode array detector is developed and validated for the analysis of asenapine in bulk and in its tablet formulation. Methods: The method utilized a BDS Y Persil C18 (250 x 4.6 mm, 5 ”m) column with 0.1% orthophosphoric acid and methanol (65:35 v/v) as mobile phase. The analysis was performed at 30 oC column temperature with a flow rate of 1 ml/min and detection at 227 nm. The method was validated for specificity, sensitivity, precision, linearity, accuracy and robustness. Asenapine was subjected to stress degradation studies under acidic, basic, oxidative, thermal and photolytic conditions. Results: Under optimized experimental conditions, asenapine was eluted from the column at a retention time of 6.781 min. The method was linear in the range of 100-300 ”g/ml. The linear regression data showed good relationship (correlation coefficient (R2) = 0.9999). The relative standard deviation and mean recovery values were within limits. The analytical performance of the method was not affected when small variations in the experimental parameters were made. Degradation products resulting from stress degradation studies did not interfere with the detection of asenapine. Conclusion: The proposed stability indicting high performance liquid chromatography method is sensitive, precise, accurate, robust and specific. This method can be used for quantification of asenapine in bulk drug and in tablet dosage form in the presence of its stress degradation products

    Study of conjunctival impression cytology in assessing goblet cell density in dry eyes

    Background: Dry eye is a chronic multi-factorial condition characterized by disturbances in the tear film and ocular surface with a decrease in goblet cells which can be assessed by conjunctival impression cytology (CIC) which is as effective as biopsy for diagnostic purposes. It helps establish not only the diagnosis of dry eye but also aids in grading the severity. This is a highly sensitive method to detect pathological changes in the conjunctival surface and confirm the clinical diagnosis. The aim of the study was to determine the goblet cell density by impression cytology, grading the severity of dry eyes and comparing the study with other studies.Methods: Study was conducted for a period of 2 yrs from August 2011 to July 2013 in 80 patients above 20 yrs age with clinically established diagnosis of dry eye who were subjected to conjunctival imprint cytology and stained with PAS to estimate he goblet cell density.Results: Out of the 80 cases of dry eye studied 45 were females and 35 were males with a female to male ratio of 1.28: 1 with a slight female preponderance. Age of the patients ranged from 21yrso 73 yrs. CIC showed positivity in 43 cases (53.75%) and negative in 37 cases (46.25%). 39 cases (90.70%) of positive CIC were above the age of 40 years.Conclusions: Dry eyes were found to be more common in age group above 40yrs with slight female preponderance. CIC showed decreased goblet cell density with increasing age in clinically diagnosed dry eyes

    Prediction of Chronic Kidney Disease using SVM and CNN

    Chronic kidney disease is one of the deadliest diseases today and it is vital to have a good diagnosis as soon as possible. In medical treatment, machine learning has been reported to be effective. A doctor can diagnose the disease early by using machine learning classifier algorithms. This study investigated the chronic disease prognosis of this concept. Disease data was taken from the University of California, Irvine. Other measurement algorithms used in this study include C5.0, Chi-square automatic interaction detector, line extraction, SVM line with L1 and L2 flap, and neural network random tree. The database was also submitted to a feature selection program that merited the database. Scores are computer generated for each category segment using the following methods: Full Version, (ii) Link-Based Feature Selection, (iii) Folder Feature Selection, (iv) Minimal Collapse and Selected Optional Retrospective Features, (v) integrated small oversampling method with very small reduction features and selected bias on the selected operator, and (vi) how to do multiple samples combined with full functions. In the full multi-sample processing process, the findings show that L2-loaded LSVM has a very high accuracy of 96.86 percent. The graph shows the results of different methods, as well as precision, precision, recall, F-score, area under the curve, and GINI coefficient. The minimum absolute reduction and selection regression operation selected features using the synthetic minority oversampling approach produced the best results after using the synthetic minority oversampling method with full features. The support vector machine achieved a high accuracy of 96.46 percent in the process of making very large samples with very small turndowns and selected operator features. Machine learning methods used with convolutional neural networks and SVM classifier models on the same database, with 96.7 percent of high-definition support machine models and networks are used

    FORMULATION AND DEVELOPMENT OF MICROSPHERES FOR THE TREATMENT OF FAMILIAL ADENOMATOUS POLYPOSIS

    Objective: Familial adenomatous polyposis (FAP) also known as familial polyposis coli, is a hereditary disease characterized by progressive appearance of numerous polyps mainly in the large intestine. Polyps are initially benign but can easily become cancerous and as such it is a life threatening condition. Celecoxib, a cyclooxygenase-2 (COX-2) inhibitor is thought to induce cell death, and thus prevent or delay the growth of polyps. So in the present study celecoxib loaded microspheres were prepared using control release Hydroxy propyl methyl cellulose (HPMC K4M) and pH dependent polymer eudragit L 100-55 in different ratios (1:1 to 1:4) respectively. The main objective of the study is to identify the polymer concentration required to prevent the drug release in stomach region and promotes in intestinal region.Methods: Emulsification solvent evaporation method was selected for the preparation and all the optimized formulations were evaluated for drug-polymer interactions, percentage yield, micrometric properties, entrapment efficiency, particle size analysis, differential scanning calorimetry and in vitro dissolution study.Results: Drug and polymer interactions were evaluated by using FTIR and DSC. The FTIR spectrum and DSC thermograms stated that drug and polymer are compatible to each other. The micrometric properties of drug loaded microspheres were carried out and they were found to be as the angle of repose (18.26 °-40.69 °), bulk density (0.2846-0.3875), tapped density (0.4111-0.5428), Carr's index (9.66-14.77), Hausner's ratio (1.112-1.2642) which were within the limits. In vitro dissolution, drug release was found to be from 4.5 to 6.5 h for the prepared four formulations (F1ñ€“F4). From the kinetic data modeling the order of drug release was found to be zero order and korsmeyer-peppas with n value above 0.5 for all the formulations indicating non-fickian diffusion.Conclusion: All the result demonstrated that celecoxib microspheres can be effectively used in the treatment of familial adenomatous polyposi

    This review paper focuses to ascertain a new approach in turbulence generation on the structure of premixed flames and external combustion using a fractal grid pattern. This review paper discusses the relationship between fractal pattern and turbulence flow. Many researchers have explored the fractal pattern as a new concept of turbulence generators, but researchers rarely study fractal turbulence generators on the structure premixed flame. The turbulent flow field characteristics have been studied tand investigated in a premixed combustion application. In terms of turbulence intensity, most researchers used fractal grid that can be tailored so that they can design the characteristic needed in premixed flame. This approach makes it extremely difficult to determine the exact turbulent burning velocity on the velocity fluctuation of the flow. The decision to carry out additional research on the effect circle grid fractal plate as a turbulent generator for premixed flame should depends on the blockage ratio and fractal pattern of the grid. 1

    Confirmation of Parity Violation in the Gamma Decay of 180Hfm^{180}Hf^{m}

    This paper reports measurements using the technique of On Line Nuclear Orientation (OLNO) which reexamine the gamma decay of isomeric 180^{\rm 180}Hfm^{\rm m} and specifically the 501 keV 8−^{\rm -} -- 6+^{\rm +} transition. The irregular admixture of E2 to M2/E3 multipolarity in this transition, deduced from the forward-backward asymmetry of its angular distribution, has for decades stood as the prime evidence for parity mixing in nuclear states. The experiment, based on ion implantation of the newly developed mass-separated 180^{\rm 180}Hfm^{\rm m} beam at ISOLDE, CERN into an iron foil maintained at millikelvin temperatures, produces higher degrees of polarization than were achieved in previous studies of this system. The value found for the E2/M2 mixing ratio, Ï”\epsilon = -0.0324(16)(17), is in close agreement with the previous published average value Ï”\epsilon = - 0.030(2), in full confirmation of the presence of the irregular E2 admixture in the 501 keV transition. The temperature dependence of the forward-backward asymmetry has been measured over a more extended range of nuclear polarization than previously possible, giving further evidence for parity mixing of the 8−^{\rm -} and 8+^{\rm +} levels and the deduced E2/M2 mixing ratio.Comment: 28 pages, 9 figures, accepted for publication in Physical Review

    A digital health record technique is one application which will bring great convenience in healthcare. Within this paper, we introduce one cryptographic primitive named as conjunctive keyword search with designated tester and timing enabled proxy re-file file encryption function, which is a type of sometime-dependent SE plan. We design one searchable file encryption plan supporting secure conjunctive keyword search and approved delegation function. The searchable file encryption (SE) plan may well be a technology to include security protection and favorable operability functions together, that may play a huge role within the e-health record system. As opposed to existing schemes, the task is able to do timing enabled proxy re-file file encryption with effective delegation revocation. The security and privacy within the sensitive private information would be the major concerns within the users that could hinder further development and broadly adoption within the systems. It might enable patients to delegate partial access legal rights along with other individuals to function search functions over their records in the while period. How big time-frame for your delegate to look and decrypt the delegator’s encrypted documents may be controlled. The comparison and extensive simulations show it provides a small computation and storage overhead. We formulate a method model along with a security model for your suggested Re-dtPECK plan to exhibit it's competent plan proven secure within the standard model. The experimental results and security analysis indicate our plan holds much greater security compared to existing solutions by having an acceptable overhead for cloud applications
