SYNOVIAL FLUID D-LACTATE — BACTERIAL-SPECIFIC MARKER FOR INFECTION OF NATIVE AND PROSTHETIC JOINTS
Infection of native and prosthetic joints remains a critical disease, associated with both significant mortality and morbidity. The diagnosis of joints infection is extremely difficult since presentation and preoperative tests are not always obvious and precise, while correct and timely diagnosis of septic etiology is crucial. In this case a rapid and accurate test would be helpful.Purpose of the study — тo evaluate the analytical performance and diagnostic capabilities of measuring the synovial fluid D-lactate for early diagnosis of infection in native and prosthetic joints.Material and methods. Test group of patients (n = 86) contained two subgroups – patients with periprosthetic infection (PPI) (n = 58) and patients with bacterial arthritis (BA) (n = 28). Control group (n = 104) also included two subgroups – patients with aseptic instability of implant components (n = 75) and patients with osteoarthritis (OA) (n = 29).Results. The authors observed that SF D-lactate ≥1,2 mmol/l was the optimal cutoff value for identifying patients with bacterial causes. The higher SF levels of D-lactate were observed in patients with BA compared to aseptic causes, (p<0,0001), as well as in patients with PJI in contrast to aseptic loosening of prosthesis (p<0,0001). In patients with native joints, SF D-lactate had better sensitivity (92,8%) compared to SF leucocytes (66,6%) and percentage of neutrophils (44,4%). D-lactate had better sensitivity for diagnosis of PJI (96,5%, 89,6% and 60,3% respectively). There were no significant differences in SF D-lactate levels due to different bacterial strains.Conclusion. The study demonstrated high analytical performance and diagnostic capabilities of measuring of synovial fluid D-lactate for diagnosis of BA and PJI. It is a rapid and accurate test for differentiating bacterial joint infection from the aseptic inflammatory joint diseases. This procedure can be carried out within less than one hour and be helpful in outpatient setting
EPIDEMIOLOGY OF PRIMARY HIP ARTHROPLASTY: REPORT FROM REGISTER OF VREDEN RUSSIAN RESEARCH INSTITUTE OF TRAUMATOLOGY AND ORTHOPEDICS
The paper presents data analysis of the Hip Arthroplasty Register of Vreden Russian Research Institute of Traumatology and Orthopedics, namely information on 37373 primary THA performed at the Vreden Institute and at several other orthopedic centers and 1200 hip replacements at other hospitals of St. Petersburg.There were 1.5 times more women in the studied cohort than men. A significant predominance of women with dysplastic osteoarthritis (72.4%) and rheumatoid arthritis (82.1%) was reported. A male predominance was noted in patients with secondary osteoarthritis (53.1%), post-traumatic changes of hip (61.0%) and osteonecrosis of the femoral head (68.6 %). The mean age of patients was 58.0±12.9 years (95% CI from 57.9 to 58.1, median 59 years). Age data of the study revealed that patients were 10-12 years younger than reported in the national arthroplasty registers of other countries.Total hip arthroplasty was performed in the absolute majority of patients – 37295 cases (99,8%). Uncemented implants were used in 59.3% of cases, hybrid – in 29.6%, cemented – in 10.2%, reverse-hybrid – in 0.9% of all patients. The most common bearing used was metal on crosslink polyethylene, which was applied in 50.1% of all cases of arthroplasty. The type of fixation of the implant, and the use of different bearings varied in different age groups. The paper presents not only the absolute numbers of the data, but also demonstrated the dynamics of the changes in time starting from 2007.The present epidemiological study does not claim the absolute completeness of the presented data, but contains the analysis of the large number of cases, comparable with follow-ups of patients in some national registers of certain European countries. OTsENKA IZMENENIY MINERAL'NOY PLOTNOSTI KOSTNOY TKANI U PATsIENTOV S DISPLASTIChESKIM KOKSARTROZOM, NAKhODIVShIKhSYa NA LEChENII V SPETsIALIZIROVANNOM STATsIONARE TRAVMATOLOGO-ORTOPEDIChESKOGO PROFILYa
Цель. Оценить изменения минеральной плотности костной ткани (МПКТ) у пациентов с диспластическим коксартрозом, находившихся на лечении в «ФЦТОЭ» в 2015г. Материал и методы. Проведен анализ МПКТ у 344 пациентов с диспластическим коксартрозом, в возрасте от 22 до 78 лет, средний возраст - 54,6±12,4 (из них женщин 248 - 72%, мужчин 96 - 28%) с градацией по возрастному критерию: до 45 лет - 11,4%, от 45 до 65 лет - 36%, более 65 лет - 52,6%. Пациентам выполнялась двухэнергетическая рентгеновская абсорциометрия поясничного отдела позвоночника и проксимального отдела бедренной кости на аппарате EXСELL XR-46 («Norland», США) с оценкой по Т-критерию, согласно рекомендациям ВОЗ. Результаты. Среди пациентов исследованной группы частота сниженной МПКТ составила 73,7% - 253 человека (остеопороз 23% - 79 человек, остеопения 50,7% - 174 человека). Снижение показателей до уровня остеопороза среди женщин выявлено у 62 человек, что составляет 25%, среди мужчин у 17 человек, что составляет 18%. Снижение показателей до уровня остеопении среди женщин выявлено у 130 человек, что составляет 52,4%, среди мужчин у 44 человек, что составляет 46%, это выше средних популяционных показателей в Российской Федерации (женщины -43,3%, мужчины - 44,1%). Доля пациентов с нормальными показателями МПКТ составила 26,3% - 91 человек (среди женщин 22,6% - 56 человек, среди мужчин 36,4% - 35 человек). По возрастному критерию распространенность остео-пороза составила: до 45 лет случаев не выявлено, от 45 до 65 лет - 13,6%, более 65 лет - 31,2%, распространенность остеопении составила: до 45 лет - 42,8%, от 45 до 65 лет -54,5%, более 65 лет - 50,0%. Выводы. Полученные результаты свидетельствуют о высокой частоте сниженной МПКТ у пациентов с диспластическим коксартрозом. Это определяет в последующем актуальность формирования групп риска для проведения превентивной фармакотерапии среди пациентов стационара травматолого-ортопедического профиля с возрастным критерием от 45 лет, с целью замедления темпов снижения МПКТ и риска развития переломов, как до операции, так и в послеоперационном периоде
MINERAL'NAYa PLOTNOST' KOSTNOY TKANI U DETEY S DIAGNOZOM DTsP POSLE REKONSTRUKTIVNYKh OPERATsIY NA TAZOBEDRENNOM SUSTAVE
Цель. Оценка состояния минеральной плотности костной ткани (МПКТ) у детей с диагнозом ДЦП после реконструктивных операций на тазобедренном суставе. Материал и методы. В исследовании приняли участие 15 детей с диагнозом ДЦП с оценкой по международной шкале двигательных возможностей GMFCS III-V. Все пациенты находились на восстановительном лечении в детском травматолого-ортопедическом отделении Центра после проведения реконструктивных операций на тазобедренном суставе. Оценивалось физическое развитие: масса и длина тела, индекс массы тела (ИМТ). Для определения МПКТ детям выполнялась двухэнергетическая рентгеновская абсорциометрия поясничного отдела позвоночника L1-L4 позвонков в прямой проекции (Spine AP) и всего тела (Whole body) на аппарате EXСELL XR-46 («Norland», США) с оценкой по Z-критерию, согласно рекомендациям ВОЗ. Результаты. Средний возраст детей 7,4±1,9 лет. Из них 7 девочек и 8 мальчиков. Антропометрические параметры у данной группы пациентов: средняя масса тела 19,8±7,1 кг, средняя длина тела 118,6±14,8 см, средний ИМТ 13,8±2,7, что ниже, чем у здоровых сверстников. В анамнезе у 3 пациентов (20%) наблюдались низкоэнергетические переломы. По результатам двухэнергетической рентгеновской абсорциометрии по программе Whole body у 11 пациентов (73,3%) отмечалось снижение костной массы по сравнению с возрастной нормой (Z-критерий менее -2,0 SD), среднее значение -3,74±0,81. В тоже время, среднее значение Z-критерия МПКТ L1-L4 позвонков находилось в пределах возрастной нормы и составило -0,77±0,641. Заключение. У детей с тяжелой формой церебрального паралича (GMFCS III-V) в восстановительном периоде после реконструктивных операции на тазобедренном суставе наблюдается значительное снижение МПКТ. Это усугубляется длительным периодом иммобилизации после оперативного лечения. Таким образом, необходимо дооперационное определение МПКТ у данной категории пациентов с целью коррекции выявленных нарушений и снижения риска внепозвонковых переломов в послеоперационном периоде
Ambulatory blood pressure and arterial stiffness web-based telemonitoring in patients at cardiovascular risk. First results of the VASOTENS (Vascular health ASsessment Of The hypertENSive patients) Registry
The VASOTENS Registry is an international telehealth-based repository of 24-hour ambulatory blood pressure monitorings (ABPM) obtained through an oscillometric upper-arm BP monitor allowing combined estimation of some vascular biomarkers. The present paper reports the results obtained in 1200 participants according to different categories of CV risk. Individual readings were averaged for each recording and 24-hour mean of brachial and aortic systolic (SBP) and diastolic blood pressure (DBP), pulse wave velocity (PWV), and augmentation index (AIx) obtained. Peripheral and central BP, PWV and AIx values were increased in older participants (SBP only) and in case of hypertension (SBP and DBP). BP was lower and PWV and AIx higher in females. PWV was increased and BP unchanged in case of metabolic syndrome. Our results suggest that ambulatory pulse wave analysis in a daily life setting may help evaluate vascular health of individuals at risk for CV disease
