    A field vegetable transplanter for use in both tilled and no-till soils

    A commercial, manually fed vegetable transplanter was modified and adapted to work in no-till soils. Details of the modifications are described in this article. The aim of this research was to evaluate the performance of this transplanter under actual field conditions in both tilled and no-till soils. The draft force in the no-till soil was measured for different working tools mounted for tilling or loosening a narrow band of soil before passing with the furrower. The combination of a ripper shank opener with a straight nose point resulted in the lowest draft force values per unit of working depth. The transplanter accuracy and transplant success rate were evaluated in different vegetable crops. The accuracy parameters (multiple index, miss index, quality of feed index, and precision) were estimated and in general were similar in the tilled and no-till soils, indicating that the transplanter was able to operate in both soil conditions with the same accuracy. The transplant success rates were also similar in both soil conditions. The transplanter thus offers a satisfactory technical solution for transplanting vegetables in both tilled and no-till soils

    Legume Cover Crop Alleviates the Negative Impact of No-Till on Tomato Productivity in a Mediterranean Organic Cropping System

    The ecosystem services a cover crop (CC) provides depend enormously on species choice and tillage system. Here, we evaluated the impact of (a) three winter CCs-rye (Secale cereale L.) and squarrose clover (Trifolium squarrosum L.) monocultures and their mixture, and (b) two tillage systems-roller-crimping of CC residue as dead mulch for no-till (NT) systems and incorporating CC residue into the soil as green manure for conventional tillage (CT) systems-on the performance of organic processing tomato, i.e., plant growth, nutrient accumulation, fruit yield, and weed biomass. The assessments took place over two years in field experiments conducted under Mediterranean conditions. At the termination time, rye and mixture were the most productive and the best weed-suppressive CCs. During tomato growing season, squarrose clover regardless of tillage system stimulated tomato growth, Nitrogen content and uptake, and the yield relative to the other cover crops. Nevertheless, NT generally impaired the tomato nutritional status and increased weed biomass compared to CT despite some potential weed control by cover crops. These two aspects caused a significant drop in tomato yield in all NT systems. The results suggested that, despite the multiple benefits the compared CCs can offer in Mediterranean agroecosystems, legume CCs could be the key to developing feasible organic vegetable no-till systems

    First Lensed Quasar Systems from the VST-ATLAS Survey: One Quad, Two Doubles, and Two Pairs of Lensless Twins

    We have analyzed images from the VST-ATLAS survey to identify candidate gravitationally lensed quasar systems in a sample of WISE sources with W1 - W2 ≥ 0.7.. Results from follow-up spectroscopy with the Baade 6.5 m telescope are presented for eight systems. One of them is a quadruply lensed quasar, and two are doubly lensed systems. Two are projected superpositions of two quasars at different redshifts. In one system, two quasars, although at the same redshift, have very different emission line profiles and constitute a physical binary. In two systems, the component spectra are consistent with the lensing hypothesis, after allowing for microlensing. However, as no lensing galaxy is detected in these two systems, we classify them as lensless twins. More extensive observations are needed to establish whether they are in fact lensed quasars or physical binaries

    Transient Arterial Hypertension Induced by Gonadotropin-Releasing Hormone Agonist Treatment for Central Precocious Puberty.

    Background: Gonadotropin-releasing hormone agonists (GnRHa) are a safe and effective treatment for precocious puberty. Triptorelin is one of the long lasting GnRHa, which reversibly suppresses the pituitary-gonadal axis. Triptorelin-induced hypertension (HTN) has rarely been reported in the literature. Clinical Case/Methods: We report a 10-year-old girl with central precocious puberty who, during treatment with triptorelin, developed an asymptomatic stage II HTN. Initial workup showed no renal, thyroid, or electrolytes abnormalities. The renal ultrasound showed no parenchymal disease and no increased renal resistance index suggestive of a renal artery stenosis. Echocardiography and ocular fundoscopy were normal. HTN (stage II) was confirmed with ambulatory blood pressure monitoring (ABPM). After extensive literature review, we found 3 other cases of HTN secondary to GnRHa, improving with endocrine treatment cessation. Therefore, antihypertensive treatment was not started immediately in our patient. Indeed, after completion of her treatment with triptorelin, we observed a complete normalization of her blood pressure (confirmed with ABPM) without any medication. Conclusion: Concomitantly to GnRHa treatment, our patient developed HTN, which completely subsided after stopping triptorelin. The complete normalization of her blood pressure, together with a negative workup for HTN strongly speaks for a causal effect of her endocrine treatment. In this setting, estrogen depletion might play a role, although this remains debated

    Differentially loaded radiostereometric analysis (DLRSA) in torsion adds essential information in diaphyseal bone healing: the example of a tibial osteotomy

    BACKGROUND: Clinical assessment and conventional imaging, which are currently used to monitor fracture healing, do not provide information on the mechanical properties of the healing construct. This limits their use in patient management decisions. Differentially loaded radiostereometric analysis (DLRSA) is a technique developed to assess the mechanical properties of healing fractures in vivo. DLRSA measures the relative micromotion of tantalum beads inserted into bone fracture fragments in response to load across the fracture site. To date, these loads have been applied axially, although in fractures fixed with devices which are highly resistant to axial loads, such as locked intramedullary nails, torsional testing may be more sensitive to healing. The aim of this study was to establish a method to investigate DLRSA using torsional loading for clinical application. METHODS: A device was designed and built to apply torsional loads to the tibia. The test case was an oblique plain corrective osteotomy of a tibial diaphysis stabilized with an intramedullary nail and with tantalum beads inserted into the two adjacent bone segments. Post surgical examinations were made at 2 weeks, 2, 4, and 6 months as well as 1 and 2 years. Healing was monitored with the use of plain film radiographs, computed tomography (CT) and DLRSA. Axial loads of 30kg and an external torsion of 5Nm were applied during DLRSA examinations and the resultant displacement and stiffness were calculated. RESULTS: Torsional DLRSA demonstrated progressive changes in angular displacements and torsional stiffness consistent with the fracture healing observed by CT. By contrast, axial DLRSA was not informative and was more reflective of the stability of the fixation than healing bone. CONCLUSION: The addition of torsional assessments to DLRSA provides an important investigative option in assessing the biomechanical properties of bone healing in vivo.M.J. Chehade, I.A. Vakaci, S.A.     Personal digital health hubs for multiple conditions

    Multimorbidity is the presence of more than one chronic disease condition in an individual. Health-related, socioeconomic, cultural and environmental factors, as well as patient behaviour, all influence the outcomes of multimorbidity. Addressing these complex and often interacting biopsychosocial factors therefore requires a shift in treatment from a physical damage model towards person-centred integrated care with increased patient agency. Education influences behaviour and can be used to empower patients and their carers with greater agency, thus allowing greater responsibility for and control over the management of patient care. In this paper we reflect on our own learning as a community of health practitioners from different disciplines. Recognizing the increasing importance of patient agency in driving the evolution of health care, we describe the concept of a web-based personal digital health hub for integrated patient care. Informed by collaboration between patient, health and education communities, we share our early experience in the implementation of a health hub around a cohort of patients with hip fractures. We also describe a vision for future health care based on the co-creation of digital health hubs centred on patients’ and carers’ needs. The health hub could allow important advances and efficiencies to be achieved in workforce practice and education; patient and carer engagement in self-care; and the collection of patient-reported health data required for ongoing research and improvements in health care.Mellick J Chehade, Lalit Yadav, Asangi Jayatilaka, Tiffany K Gillc and Edward Palme

    BACKGROUND: Autosomal dominant polycystic kidney disease (ADPKD) is a genetic disorder characterized by numerous fluid-filled cysts that frequently result in end-stage renal disease. While promising treatment options are in advanced clinical development, early diagnosis and follow-up remain a major challenge. We therefore evaluated the diagnostic value of Fetuin-A as a new biomarker of ADPKD in human urine. RESULTS: We found that renal Fetuin-A levels are upregulated in both Pkd1 and Bicc1 mouse models of ADPKD. Measurement by ELISA revealed that urinary Fetuin-A levels were significantly higher in 66 ADPKD patients (17.5 ± 12.5 μg/mmol creatinine) compared to 17 healthy volunteers (8.5 ± 3.8 μg/mmol creatinine) or 50 control patients with renal diseases of other causes (6.2 ± 2.9 μg/mmol creatinine). Receiver operating characteristics (ROC) analysis of urinary Fetuin-A levels for ADPKD rendered an optimum cut-off value of 12.2 μg/mmol creatinine, corresponding to 94% of sensitivity and 60% of specificity (area under the curve 0.74 ; p = 0.0019). Furthermore, urinary Fetuin-A levels in ADPKD patients correlated with the degree of renal insufficiency and showed a significant increase in patients with preserved renal function followed for two years. CONCLUSIONS: Our findings establish urinary Fetuin-A as a sensitive biomarker of the progression of ADPKD. Further studies are required to examine the pathogenic mechanisms of elevated renal and urinary Fetuin-A in ADPKD

    Utilising digital health technology to support patient-healthcare provider communication in fragility fracture recovery: systematic review and meta-analysis

    The objective of this review is to evaluate the effectiveness of digital health supported targeted patient communication versus usual provision of health information, on the recovery of fragility fractures. The review considered studies including older people, aged 50 and above, with a fragility fracture. The primary outcome was prevention of secondary fractures by diagnosis and treatment of osteoporosis, and its adherence. This review considered both experimental and quasi-experimental study designs. A comprehensive search strategy was built to identify key terms including Medical subject headings (MeSH) and applied to the multiple electronic databases. An intention to treat analysis was applied to those studies included in the meta-analysis and odds ratio was calculated with random effects. Altogether, 15 studies were considered in the final stage for this systematic review. Out of these, 10 studies were Randomised controlled trials (RCT) and five were quasi experimental studies, published between the years 2003 and 2016 with a total of 5037 participants. Five Randomised control trails were included in the meta-analysis suggesting that digital health supported interventions were overall, twice as effective when compared with the usual standard care (OR 2.13, 95% CI 1.30-3.48), despite the population sample not being homogeneous.
