    「回首.動情.傳承」長者生命故事計劃

    嶺南大學亞太老年學研究中心獲華人永遠墳場管理委員會(「華永會」)資助為期一年的「回首.動情.傳承」長者生命故事計劃(「計劃」)。此計劃旨在讓青年人認識長者生命經驗,學習克服困難與挫折以提升抗逆力,建立正向人生觀。 近年,主流媒體經常批評年輕人的負面人生觀,例如:「躺平主義」、「享樂主義」、「犬儒心態」等,亦不時看到青年人輕生的新聞。我們曾在大學內處理過不少受情緒困擾及企圖自殺的個案,與學生深入交流後,發現他們面對着沉重的學業壓力、財政困難或複雜的家庭關係,內心充滿掙扎不安。 此計劃讓嶺大學生與長者導師進行深度的對談,透過了解長者走過的路、他們經歷過的挫折和教訓,給予年輕人生命的啟示。如果我們以旅遊比喻人生,長者就像環遊世界的資深背包客,即使大家遊覽不同的地點、觀賞過不同的風景,他們總能夠分享一些旅遊的心得,讓新手遊客走少一點冤枉路,或領悟到旅遊的樂趣和意義。長者亦可以藉由敍述人生片段回顧他們生命中的故事,學習接納過去,增加自我認同感。青年人創作生命教育書冊,將長者積極的人生觀傳給年輕一代,並藉此鼓勵其他長者豁達地度過餘年。 我們於2022年初招募嶺南大學學生接受「生命故事敍述」培訓,內容包括:本港的人口老化現象、敍述治療理論、與長者溝通的技巧及模擬實踐練習等,以裝備同學的知識和技巧。本中心再向屯門、元朗區的長者機構發邀請信,誠邀長者擔任生命導師接受訪問。 嶺大安排同學以兩人一組的小隊形式,於2022年6至7月期間前往長者中心、日間護理中心、嶺南大學或長者家中,與十二位長者進行深入訪談。訪談結束後,同學根據訪談的內容,為長者書寫他們獨特的生命故事。例如在人離鄉賤的異國環境下,努力打拼事業的Alfred;堅持不懈持續進修的淑芹和馮春林;即使沒機會求學,仍憑一雙巧手闖出一片天的譚惠;在文化大革命的漩渦中,憑着熱忱而改變命運的蘭英;還有為家人無私奉獻的鳳群、歐婆婆、雅芳及細女;離鄉別井勇闖異地的阿美和阿水;即使被家人賣去做「妹仔」,仍能以「阿Q精神」面對的諒餘。 為保障長者的私隱權益,本書內所有刊登之故事皆經過受訪者或社工審閱,部份受訪者選擇以化名的形式來分享自己的故事,我們亦移除了部份敏感的個人資料。https://commons.ln.edu.hk/apias_guide/1008/thumbnail.jp

    Colistin Resistance of Pseudomonas aeruginosa Isolated from Snakes in Taiwan

    This study included fifty-eight isolates of P. aeruginosa from the oral cavity of snakes that were recruited from clinical cases, captive and wild snakes. The minimum inhibitory concentrations (MICs) for the determination of susceptibility were identified by the broth microdilution method. Polymerase chain reaction (PCR) was employed to detect β-lactamases genes. With regard to antipseudomonal antibiotics, the lowest nonsusceptible rates were in aztreonam (15%), piperacillin/tazobactam (12%), and amikacin (9%). The nonsusceptible rates were high in gentamicin (33%) and colistin (55%). Meanwhile, blaTEM presented in 100% of isolates where blaAmpC, blaOXA-1, and blaOXA-10 came at 94.8%, 89.7%, and 27.6%, respectively. Emergence of multidrug resistant (MDR) strains and colistin-resistant strains highlights the potential breach of public health as P. aeruginosa could be transmitted through either direct contact or indirect dissemination through the environment. This study reports that the highly resistant P. aeruginosa from snakes’ oral cavity were discovered for the very first time in Taiwan

    Risk Perception and Knowledge in Fire Risk Reduction in a Dong Minority Rural Village in China: A Health-EDRM Education Intervention Study

    Abstract Fire is one of the major disasters in rural communities but evidence of the effectiveness of education interventions against fire risks is limited. This was a 2-year study assessed the effectiveness of face-to-face Health Emergency and Disaster Risk Management (Health-EDRM) education interventions for raising fire risk reduction knowledge in a fire-prone rural ethnic minority community. The study was conducted in various pre-set time points of an intervention-based project in a Dong-based community in Nanjiang Village, Guizhou Province in 2015 and 2016 to increase knowledge among the villagers about how to reduce general- and electrical-fire risks. Pre- and post-intervention questionnaires were used to evaluate the effectiveness of increasing fire risk-related knowledge through these interventions, immediately after the 2015 and 2016 interventions, and 17 months after the 2015 intervention. The knowledge of using fire blanket, recalling the correct emergency telephone number, unplugging unused electrical appliances, and not using water to extinguish electrical fires had immediately improved after the interventions. Subjects demonstrated a better understanding that fire blankets can fight a blaze if used appropriately, and that knowledge was sustained for 17 months. The interventions were effective in improving fire prevention and response knowledge. Targeted interventions should be organized according to communities’ culture, the evolution of economic prosperity and lifestyle practices

    Implementation of Public Funded Genome Sequencing in Evaluation of Fetal Structural Anomalies

    With the advancements in prenatal diagnostics, genome sequencing is now incorporated into clinical use to maximize the diagnostic yield following uninformative conventional tests (karyotype and chromosomal microarray analysis). Hong Kong started publicly funded prenatal genomic sequencing as a sequential test in the investigation of fetal structural anomalies in April 2021. The objective of the study was to evaluate the clinical performance and usefulness of this new service over one year. We established a web-based multidisciplinary team to facilitate case selection among the expert members. We retrospectively analyzed the fetal phenotypes, test results, turnaround time and clinical impact in the first 15 whole exome sequencing and 14 whole genome sequencing. Overall, the molecular diagnostic rate was 37.9% (11/29). De novo autosomal dominant disorders accounted for 72.7% (8/11), inherited autosomal recessive disorders for 18.2% (2/11), and inherited X-linked disorders for 9.1% (1/11). The median turnaround time for ongoing pregnancy was 19.5 days (range, 13–31 days). Our study showed an overall clinical impact of 55.2% (16/29), which influenced reproductive decision-making in four cases, guided perinatal management in two cases and helped future family planning in ten cases. In conclusion, our findings support the important role of genome sequencing services in the prenatal diagnosis of fetal structural anomalies in a population setting.     Iron Deficiency among School-Aged Adolescents in Hong Kong: Prevalence, Predictors, and Effects on Health-Related Quality of Life

    Iron deficiency (ID) is a prevalent nutritional deficiency affecting children/adolescents worldwide. We reported (1) the prevalence of ID and ID with anemia (IDA) among Chinese school-aged adolescents, (2) clinical and dietary predictors of iron status, and (3) its impact on health-related qualities of life (HRQoL). This cross-sectional study recruited 183 boys and 340 girls (mean age = 17.55) from 16 schools in Hong Kong. ID is defined as serum ferritin <15 μg/L. The participants reported their dietary habits, menstrual patterns (girls), and HRQoL using structured questionnaires. The overall prevalence of ID was 11.1%. None of the boys had ID or IDA. Among girls, the rate of ID was 17.1% and IDA was 10.9%. One-third (36.3%) reported a regular habit of skipping ≥1 meal/day. Lower ferritin was found in adolescents who skipped meals (Est = −35.1, p = 0.017). Lower ferritin is correlated with poorer school functioning (Est = 0.81, p = 0.045) and fatigue (Est = 0.92, p = 0.016). Skipping meals is associated with poorer physical (p = 0.0017) and school functioning (p = 0.027). To conclude, 1 in 10 school-aged adolescents in Hong Kong are iron-deficient. The ID rate in girls (17.1%) is similar to that in other industrialized countries (5.2–16.6%). Future work should promote awareness on the potential health consequences of poor dietary habits on ID and the well-being of adolescents

    CD44+ Cancer Stem-Like Cells in EBV-Associated Nasopharyngeal Carcinoma

    <div><p>Nasopharyngeal carcinoma (NPC) is a unique EBV-associated epithelial malignancy, showing highly invasive and metastatic phenotype. Despite increasing evidence demonstrating the critical role of cancer stem-like cells (CSCs) in the maintenance and progression of tumors in a variety of malignancies, the existence and properties of CSC in EBV-associated NPC are largely unknown. Our study aims to elucidate the presence and role of CSCs in the pathogenesis of this malignant disease. Sphere-forming cells were isolated from an EBV-positive NPC cell line C666-1 and its tumor-initiating properties were confirmed by <em>in vitro</em> and <em>in vivo</em> assays. In these spheroids, up-regulation of multiple stem cell markers were found. By flow cytometry, we demonstrated that both CD44 and SOX2 were overexpressed in a majority of sphere-forming C666-1 cells. The CD44+SOX2+ cells was detected in a minor population in EBV-positive xenografts and primary tumors and considered as potential CSC in NPC. Notably, the isolated CD44+ NPC cells were resistant to chemotherapeutic agents and with higher spheroid formation efficiency, showing CSC properties. On the other hand, microarray analysis has revealed a number of differentially expressed genes involved in transcription regulation (e.g. <em>FOXN4</em>, <em>GLI1</em>), immune response (<em>CCR7</em>, <em>IL8</em>) and transmembrane transport (e.g. <em>ABCC3</em>, <em>ABCC11</em>) in the spheroids. Among these genes, increased expression of CCR7 in CD44+ CSCs was confirmed in NPC xenografts and primary tumors. Importantly, blocking of CCR7 abolished the sphere-forming ability of C666-1 <em>in vitro</em>. Expression of CCR7 was associated with recurrent disease and distant metastasis. The current study defined the specific properties of a CSC subpopulation in EBV-associated NPC. Our findings provided new insights into developing effective therapies targeting on CSCs, thereby potentiating treatment efficacy for NPC patients.</p> </div
