    mdTLS: How to Make middlebox-aware TLS more efficient?

    The more data transmission over TLS protocol becomes increasingly common in IT Systems, the more middleboxes are deployed in networks. These middleboxes have several advantages, however, they become the target of cyber-attacks. Many researchers proposed revised versions of TLS protocols to make them secure, however, their approaches had some limitations. In this paper, we propose a middlebox-delegated TLS (mdTLS) protocol to improve performance based on the middlebox-aware TLS (maTLS), one of the most secure TLS protocols. We found out that the computational complexity of mdTLS is about twice as low as that of maTLS. Furthermore, we formally verified that our proposal meets newly defined security goals as well as those verified by maTLS.     Expression and Distribution Pattern of Retinoic Acid Receptors in the Nasal Mucosa

    Background and Objectives Retinoids are naturally occurring vitamin A derivatives that regulate cellular processes and metabolism. In particular, retinoids play a key role in cellular proliferation by binding to retinoic acid receptors (RAR)-alpha, beta, and gamma. Considering the functional role of nasal mucosa where active cell regeneration occurs, RAR may play a role in tissue remodeling of the human nasal mucosa. Methods In this study, we investigated the expression and distribution pattern of RAR using reverse transcription-polymerase chain reaction (RT-PCR), immunohistochemistry (IHC) and Western blot in normal ethmoid mucosa (NE), chronic rhinosinusitis (IE) and polyp (P). Results IE and P samples showed higher expression levels of RAR in RT-PCR and Western blot than NE samples. RAR reactivity was also observed in the NE group, which indicates that cell regeneration also occurs in normal condition. Through IHC, we found the localization of RAR. RAR-α was distributed in the epithelial cells, submucosal glands, and endothelial cells. RAR-β was located in the basal epithelium, while RAR-γ was present in goblet cells and submucosal glands. The staining intensity of RAR-α, β and γ was higher than that in the NE group. Especially in the P group, RARs were abundantly distributed in the stalks of polyps. Conclusion The stalk region contains a lot of collagen and fibroblasts to support polyp formation, and the greater amount of RAR in the stalk suggested that RARs may be associated with angiogenesis and cell proliferation. Accordingly, elevated RAR levels in chronic rhinosinusitis could indicate that RARs play a critical role in cell regeneration, angiogenesis and immunomodulation under inflammatory conditions in the human nasal mucosa

    Prognostic significance of natural killer cell-associated markers in gastric cancer: quantitative analysis using multiplex immunohistochemistry

    Abstract Background Natural killer (NK) cells mediate the anti-tumoral immune response as an important component of innate immunity. The aim of this study was to investigate the prognostic significance and functional implication of NK cell-associated surface receptors in gastric cancer (GC) by using multiplex immunohistochemistry (mIHC). Methods We performed an mIHC on tissue microarray slides, including 55 GC tissue samples. A total of 11 antibodies including CD57, NKG2A, CD16, HLA-E, CD3, CD20, CD45, CD68, CK, SMA, and ki-67 were used. CD45 + CD3-CD57 + cells were considered as CD57 + NK cells. Results Among CD45 + immune cells, the proportion of CD57 + NK cell was the lowest (3.8%), whereas that of CD57 + and CD57- T cells (65.5%) was the highest, followed by macrophages (25.4%), and B cells (5.3%). CD57 + NK cells constituted 20% of CD45 + CD57 + immune cells while the remaining 80% were CD57 + T cells. The expression of HLA-E in tumor cells correlated with that in tumoral T cells, B cells, and macrophages, but not CD57 + NK cells. The higher density of tumoral CD57 + NK cells and tumoral CD57 + NKG2A + NK cells was associated with inferior survival. Conclusions Although the number of CD57 + NK cells was lower than that of other immune cells, CD57 + NK cells and CD57 + NKG2A + NK cells were significantly associated with poor outcomes, suggesting that NK cell subsets play a critical role in GC progression.     Treatment shortening of drug-sensitive pulmonary tuberculosis using high-dose rifampicin for 3 months after culture conversion (Hi-DoRi-3): a study protocol for an open-label randomized clinical trial

    Background : The standard treatment regimen for drug-sensitive tuberculosis (TB), comprising four companion drugs, requires a minimum duration of 6 months, and this lengthy treatment leads to poor adherence and increased toxicity. To improve rates of adherence, reduce adverse events, and lower costs, a simplified and shortened treatment regimen is warranted. Methods : This study is a multicenter, open-label randomized clinical trial of non-inferiority design that compares a new regimen with the conventional regimen for drug-sensitive pulmonary TB. The investigational group will use a regimen of high-dose rifampicin (30 mg/kg/day) with isoniazid and pyrazinamide, and the treatment will be maintained for 12 weeks after the achievement of negative conversion of sputum culture. The control group will be treated for 6 months with a World Health Organization-endorsed regimen consisting of isoniazid, rifampicin (10 mg/kg/day), ethambutol, and pyrazinamide. The primary endpoint is the proportion of unfavorable outcomes at 18 months after randomization. Secondary outcomes include time to unfavorable treatment outcome, time to culture conversion on liquid medium, treatment success rate at the end of treatment, proportion of recurrence at 18 months after randomization, time to recurrence after treatment completion, and adverse events of grade 3 or higher during the treatment. We predict a 10% unfavorable outcome for the control group, and 0% difference from the investigational group. Based on 80% verification power and a 2.5% one-sided significance level for a non-inferiority margin of 6%, 393 participants per group are required. Considering the 15% dropout rate, a total of 926 participants (463 in each group) will be recruited. Discussion : This study will inform on the feasibility of the treatment regimen using high-dose rifampicin with a shortened and individualized treatment duration for pulmonary TB.     Different Roles of Dendritic Cells for Chronic Rhinosinusitis Treatment According to Phenotype

    Dendritic cells (DCs) are antigen-presenting cells derived from the bone marrow that play an important role in the association between the innate and adaptive immune responses. The onset and development of chronic rhinosinusitis (CRS) involve a serious imbalance in immune regulation and mechanical dysfunction caused by an abnormal remodeling process. Recent studies have shown that an increase in DCs in CRS and their function of shaping the nasal mucosal immune response may play an important role in the pathogenesis of CRS. In this review, we discuss DC subsets in mice and humans, as well as the function of DCs in the nasal sinus mucosa. In addition, the mechanism by which DCs can be used as targets for therapeutic intervention for CRS and potential future research directions are also discussed

    The complete chloroplast genome sequence of Lychnis kiusiana (Caryophyllaceae)

    The complete chloroplast genome sequence of Lychnis kiusiana Makino (Caryophyllaceae) was determined. The genome was 151,831 bp long, consisting of a large single-copy region (83,875 bp) and a small single-copy region (17,591 bp) separated by two inverted repeats (25,331 bp). The plastome contained 124 genes; 82 encode proteins, 34 encode tRNA, and eight encode rRNA. The gene quantity and order resembled those of a typical Caryophyllaceae plastome. Phylogenetic analysis using 79 gene sequences from 14 previously reported genomes of Caryophyllaceae showed that Lychnis clades were nested within the Silene genus, suggesting that Lychnis is a lower taxonomic rank than genus

    En aquest estudi, s'investiga la figura dins de l'estructura de les agències, integrant les opinions dels professionals en la planificació estratègica. Per conèixer amb més detall la situació actual del planner a Espanya, s'ha entrevistat a cinc planners. Ells han indicat que la incorporació del planner a Espanya és recent i la figura encara està en desenvolupament. Mentre el seu paper tradicional s'enfocava en la capacitat analítica, que segueix sent una de les qualitats del planner, a més, actualment també es té en compte l'aspecte polivalent, el punt creatiu, sintètic i sobretot l'estratègic.En este estudio, en base a la investigación de la figura del planner dentro de la estructura de agencias, se han integrado las opiniones de los profesionales de la planificación estratégica. Para conocer la situación actual del planner en España, se han entrevistado cinco planners. Ellos han indicado que la incorporación del planner en España es reciente y la figura todavía está en desarrollo. Mientras su papel tradicional se enfocaba en la capacidad analítica, ahora se valora más el aspecto polivalente, el punto creativo, sintético y sobre todo el estratégico.This study, based on the investigation of the planner in the agency structure, contains the opinions from the professionals in the strategic planning. To get to know more about the actual situation of planners in Spain, a total of five planners were interviewed. They indicated that the introduction of planner in Spain has been not so long and the role of planner is still growing. While the traditional role of planner has been focused on the analytic capacity which is still one of the attributes for planner, now being polyvalent becomes more and more important -being creative, synthetic and, above all, being strategic
