    Laparoscopic management of a giant adrenal cyst

    AbstractAdrenal cysts, a rare entity, are usually nonfunctional and asymptomatic. Here, we report a 32-year-old woman who presented with mild breathlessness and vague abdominal fullness and discomfort that had lasted 6 months. Chest radiography showed elevation of the left side of the diaphragm. Abdominal sonography showed an anechoic lesion of >15cm, without a septum, that was situated over the left abdominal area. Abdominal computed tomography (CT) showed a 20cm suprarenal cyst located between the pancreas, spleen, and left kidney, causing anterior displacement of the pancreas and inferior displacement of the left kidney. Magnetic resonance imaging confirmed the CT and sonographic findings, and revealed that the cystic mass was originating from the left adrenal gland. Fluid analysis of the cystic lesion showed no evidence of infection or malignancy. A laparoscopic adrenalectomy was performed. Histopathologically, the adrenal tissue showed no evidence of adenoma or malignancy, and the cystic component showed a flattened endothelial lining with fibrotic tissue, consistent with an endothelial cyst. The postoperative course was uneventful and the patient had no evidence of recurrence during a 6-year follow-up. We discuss the diagnosis and management of adrenal cysts

    Applications of Differential CDMA Schemes and Control Technology for Distribution Substations

    This paper presents an approach to achieve monitoring and control of distribution systems in a distribution substation using power-line communication (PLC) combined with Hadamard code. Four different techniques, i.e., binary phase shift keying (BPSK), quadrature phase shift keying (QPSK), 16-quadrature amplitude modulator (QAM) and 64-QAM in code division multiple access (CDMA), are employed. With spreading-spectrum modulation and demodulation in the studied PLC system, the proposed approach can achieve reliable high-speed information transmission through power lines. With Hadamard code, the signals corresponding to different relays are orthogonal to each other and the interference among them can be reduced. The proposed approach has the advantages of high-speed detection, bi-direction communication, reading and backup data, control and turn-off functions, displaying the real-time system information, etc. When 100 kHz is used as the carrier frequency for 256 relays under power-line noise below 14 dB, the simulation results show that the bit error rate (BER) is less than 10-5 . The proposed scheme can be applied to the smart-grid distribution substation of the studied distribution systems

    Neel tensor torque at the ferromagnet/antiferromagnet interface

    Antiferromagnets (AFMs) exhibit spin arrangements with no net magnetization, positioning them as promising candidates for spintronics applications. While electrical manipulation of the single-crystal AFMs, composed of periodic spin configurations, is achieved recently, it remains a daunting challenge to characterize and to manipulate polycrystalline AFMs. Utilizing statistical analysis in data science, we demonstrate that polycrystalline AFMs can be described using a real, symmetric, positive semi-definite, rank-two tensor, which we term the Neel tensor. This tensor introduces a unique spin torque, diverging from the conventional field-like and Slonczewski torques in spintronics devices. Remarkably, Neel tensors can be trained to retain a specific orientation, functioning as a form of working memory. This attribute enables zero-field spin-orbit-torque switching in trilayer devices featuring a heavy-metal/ferromagnet/AFM structure and is also consistent with the X-ray magnetic linear dichroism measurements. Our findings uncover hidden statistical patterns in polycrystalline AFMs and establishes the presence of Neel tensor torque, highlighting its potential to drive future spintronics innovations.Comment: main text 18 pages, supplementary information 10 page

    Atypical Presentation of Spinal Epidural Abscess—Prolonged and Intractable Abdominal Pain

    SummaryDespite advances in medicine, early diagnosis of spinal epidural abscess remains a challenge to clinicians. The most common symptoms of spinal epidural abscess include back pain, fever, and neuralgic deficits. However, spinal epidural abscess can also present with vague and nonspecific symptoms. In this case, a 68-year-old male had abdominal pain in the right upper quadrant lasting 3 weeks and was diagnosed with a gastric ulcer. After treatment, his symptoms did not resolve. Fever and back pain became evident as his disease progressed, followed by right lower limb weakness and the inability to walk. He was taken to the emergency department of our hospital, and the weakness of his lower extremities worsened during hospitalization. His right leg became completely paralyzed despite the use of intravenous antibiotics. A spinal computed tomography scan was performed emergently (magnetic resonance imaging was unavailable) and revealed an epidural abscess involving T5–6 with adjacent osteomyelitis. The patient underwent posterior decompressive laminectomy with pus drainage in the T4–7 region. His neuralgic examinations improved soon after the operation, but ambulation remained limited. Early diagnosis is crucial to the prognosis of spinal epidural abscess, because delayed diagnosis usually results in complete paralysis even death.
