    Noninvasive prediction of Blood Lactate through a machine learning-based approach.

    We hypothesized that blood lactate concentration([Lac]blood) is a function of cardiopulmonary variables, exercise intensity and some anthropometric elements during aerobic exercise. This investigation aimed to establish a mathematical model to estimate [Lac]blood noninvasively during constant work rate (CWR) exercise of various intensities. 31 healthy participants were recruited and each underwent 4 cardiopulmonary exercise tests: one incremental and three CWR tests (low: 35% of peak work rate for 15 min, moderate: 60% 10 min and high: 90% 4 min). At the end of each CWR test, venous blood was sampled to determine [Lac]blood. 31 trios of CWR tests were employed to construct the mathematical model, which utilized exponential regression combined with Taylor expansion. Good fitting was achieved when the conditions of low and moderate intensity were put in one model; high-intensity in another. Standard deviation of fitting error in the former condition is 0.52; in the latter is 1.82 mmol/liter. Weighting analysis demonstrated that, besides heart rate, respiratory variables are required in the estimation of [Lac]blood in the model of low/moderate intensity. In conclusion, by measuring noninvasive cardio-respiratory parameters, [Lac]blood during CWR exercise can be determined with good accuracy. This should have application in endurance training and future exercise industry

    A HO-IRT BASED DIAGNOSTIC ASSESSMENT SYSTEM WITH CONSTRUCTED RESPONSE ITEMS

    ABSTRACT The aim of the present study was to develop an on-line assessment system with constructed response items in the context of elementary mathematics curriculum. The system recorded the problem solving process of constructed response items and transfered the process to response codes for further analyses. An inference mechanism based on artificial intelligence was implemented with the system to diagnose the bugs in the problem solving process automatically. To examine the performance of the system, a "Multiplication of Fraction" test was constructed and administered to 158 six graders in Taiwan. The results showed that the mean of classification accuracies of the bugs is above 97%, which implies that the proposed system identifies leaning bugs accurately and efficiently. In addition to bug identification, a high-order item response theory (HO-IRT) was applied to estimate the overall and domain abilities. The correlations between the abilities estimated with HO-IRT and the number of bugs were highly correlated, which suggests that the more learning bugs children possessed the lower his/her mathematic abilities would be.     Assessment of median nerve mobility by ultrasound dynamic imaging in carpal tunnel syndrome diagnosis

    2013 Joint UffC, EFTF and PFM SymposiumCarpal tunnel syndrome (CTS) is a common entrapment neuropathy. Nerve conduction studies (NCS) have been used as a standard for CTS diagnosis. Complementing NCS, ultrasound imaging provides anatomic information on pathologic changes of the median nerve, such as the reduced median nerve mobility. Motion of median nerve is dependent on mechanical characteristics, and body movements. The purpose of this study was therefore to measure transverse sliding patterns of the median nerve during fingers flexion and extension in ultrasound B-mode images for distinguishing healthy from CTS subjects, and to investigate any correlation between NCS severity and median nerve motion. Transverse ultrasound images were acquired from 19 normal, 15 mild, and 10 severe CTS subjects confirmed by NCS. In two-second acquisition, their fingers were initially in natural position; the median nerve was then moved toward the ulnar side and radius side in fingers flexion and extension, respectively. The displacements of the median nerve were calculated by the multilevel block-matching pyramid algorithm and averaged. All the average displacements at different acquisition times were then accumulated to obtain cumulative displacements, which were curve-fitted by polynomial function. To differentiate the normal from CTS cases, the R-squared, curvature, and amplitude of the fitted curves were computed, to evaluate the goodness, variation, and maximum value of the fit, respectively. Compared to the CTS patients, the normal subjects had higher R-square, curvature, and amplitude estimates. The three parameters were then inputted to a fuzzy c-means algorithm to classify normal cases and CTS ones. The diagnostic efficiency had an accuracy of 93.2%, a specificity of 100%, and a sensitivity of 88%. Further study includes measuring mechanical strain and stress at different neural sites to provide elasticity of the median nerve. © 2013 IEEE.published_or_final_versio

    Investigation of a Potential Scintigraphic Tracer for Imaging Apoptosis: Radioiodinated Annexin V-Kunitz Protease Inhibitor Fusion Protein

    Radiolabeled annexin V (ANV) has been widely used for imaging cell apoptosis. Recently, a novel ANV-Kunitz-type protease inhibitor fusion protein, ANV-6L15, was found to be a promising probe for improved apoptosis detection based on its higher affinity to phosphatidylserine (PS) compared to native ANV. The present paper investigates the feasibility of apoptosis detection using radioiodinated ANV-6L15. Native ANV and ANV-6L15 were labeled with iodine-123 and iodine-125 using Iodogen method. The binding between the radioiodinated proteins and erythrocyte ghosts or chemical-induced apoptotic cells was examined. ANV-6L15 can be radioiodinated with high yield (40%−60%) and excellent radiochemical purity (>95%). 123I-ANV-6L15 exhibited a higher binding ratio to erythrocyte ghosts and apoptotic cells compared to 123I-ANV. The biodistribution of 123I-ANV-6L15 in mice was also characterized. 123I-ANV-6L15 was rapidly cleared from the blood. High uptake in the liver and the kidneys may limit the evaluation of apoptosis in abdominal regions. Our data suggest that radiolabled ANV-6L15 may be a better scintigraphic tracer than native ANV for apoptosis detection

    THE EFFECT OF INSULIN AND CARBOHYDRATE SUPPLEMENTATION ON GLYCOGEN REPLENISHMENT AMONG DIFFERENT HINDLIMB MUSCLES IN RATS FOLLOWING PROLONGED SWIMMING

    In the present study we investigated the interactive effects of insulin and carbohydrate on glycogen replenishment in different rat hindlimb muscles. Forty male Sprague Dawley rats were assigned to 5 groups, including 1) sedentary control with carbohydrate supplement (2 g glucose · kg body wt-1), 2) sedentary rats with 16 hours recovery, carbohydrate and insulin (0.5 U · kg body wt-1), 3) swimming without recovery, 4) swimming with 16 hours recovery and carbohydrate supplement, and 5) swimming with 16 hours recovery, carbohydrate and insulin. The swimming protocol consisted of two 3 h swimming sections, which were separated by a 45 min rest. The insulin and carbohydrate were administered to the rats immediately after exercise. At the end of the experiment, the soleus (S), plantaris (P), quadriceps (Q) and gastrocnemius (G) were surgically excised to evaluate glycogen utilization and replenishment. We observed that glycogen utilization was significantly lower in G and Q than S and P during swimming (p <0.05), and S showed the greatest capacity of glycogen resynthesis after post-exercise recovery (p <0.05). In the sedentary state, the glycogen synthesis did not differ among hindlimb muscles during insulin and carbohydrate treatments. Interestingly, with insulin and carbohydrate, the glycogen resynthesis in S and P were significantly greater than in Q and G following post-exercise recovery (p <0.05). We therefore concluded that the soleus and plantaris are the primary working muscles during swimming, and the greatest glycogen replenishment capacity of the soleus during post-exercise recovery is likely due to its highest insulin sensitivity

    Inhibitory Effects of Terminalia catappa on UVB-Induced Photodamage in Fibroblast Cell Line

    This study investigated whether Terminalia catappa L. hydrophilic extract (TCLW) prevents photoaging in human dermal fibroblasts after exposure to UVB radiation. TCLW exhibited DPPH free radical scavenging activity and protected erythrocytes against AAPH-induced hemolysis. In the gelatin digestion assay, the rates of collagenase inhibition by TCL methanol extract, TCLW, and its hydrolysates were greater than 100% at the concentration of 1 mg/mL. We found that serial dilutions of TCLW (10–500 μg/mL) inhibited collagenase activity in a dose-dependent manner (82.3% to 101.0%). However, TCLW did not significantly inhibit elastase activity. In addition, TCLW inhibited MMP-1 and MMP-9 protein expression at a concentration of 25 μg/mL and inhibited MMP-3 protein expression at a concentration of 50 μg/mL. TCLW also promoted the protein expression of type I procollagen. We also found that TCLW attenuated the expression of MMP-1, -3, and -9 by inhibiting the phosphorylation of ERK, JNK, and p38. These findings suggest that TCLW increases the production of type I procollagen by inhibiting the activity of MMP-1, -3 and -9, and, therefore, has potential use in anti-aging cosmetics

    Enhanced Antifungal Bioactivity of Coptis Rhizome Prepared by Ultrafining Technology

    The aim of this study was to identify and quantify the bioactive constituents in the methanol extracts of Coptis Rhizome prepared by ultrafining technology. The indicator compound was identified by spectroscopic method and its purity was determined by HPLC. Moreover, the crude extracts and indicator compound were examined for their ability to inhibit the growth of Rhizoctonia solani Kühn AG-4 on potato dextrose agar plates. The indicator compound is a potential candidate as a new plant derived pesticide to control Rhizoctonia damping-off in vegetable seedlings. In addition, the extracts of Coptis Rhizome prepared by ultrafining technology displayed higher contents of indicator compound; they not only improve their bioactivity but also reduce the amount of the pharmaceuticals required and, thereby, decrease the environmental degradation associated with the harvesting of the raw products

    SummaryAcquired hemophilia A (AHA) is a rare condition that predisposes affected patients to a bleeding tendency, even after a trivial physical insult. We present our experience with a 45-year-old male patient who was referred to our institute because of persistent bleeding from a left forearm surgical wound after fasciotomy. He was diagnosed as having AHA. Surgical treatment in combination with recombinant activated factor VII (rFVIIa) led to a satisfactory result. Clinical awareness and multidisciplinary professional connections are necessary in the treatment of AHA. Acquired hemophilia should be considered in the differential diagnosis of patients with uncontrolled bleeding episodes
