    Successful treatment of an early invasive oral squamous cell carcinoma with topical 5-aminolevulinic acid-mediated photodynamic therapy

    Our previous studies showed successful treatment of a series of 36 oral verrucous hyperplasia lesions and of an extensive oral verrucous carcinoma with a topical 5-aminolevulinic acid (ALA)-mediated photodynamic therapy (topical ALA-PDT) protocol (with a fluence rate of 100 mW/cm2 and a light exposure dose of 100 J/cm2) using a 635-nm light-emitting diode (LED) light source. In this case report, we tested whether an enhanced topical ALA-PDT protocol (with a fluence rate of 200 mW/cm2 and a light exposure dose of 200 J/cm2) could be used to treat an early invasive oral squamous cell carcinoma (OSCC) with a verrucous appearance of the left lower posterior edentulous alveolar mucosa of a 67-year-old male former areca-quid chewer and ex-smoker. The main verrucous lesion showed complete regression after eight treatments with PDT. However, 10 extra treatments were needed to eradicate the multiple residual leukoplakia lesions on the edentulous alveolar mucosa. Moderate to severe post-PDT pain was noted during the initial eight treatments, and the patient needed analgesics (codeine phosphate, 30 mg three times daily) to control the pain. No recurrence of the OSCC lesion was found after a follow-up period of 4 years. We suggest that our enhanced topical ALA-PDT protocol may have good potential to be used as a treatment of choice for a superficially invasive OSCC without regional or distant metastasis before the commencement of other effective therapies

    Learning Discriminative Shrinkage Deep Networks for Image Deconvolution

    Most existing methods usually formulate the non-blind deconvolution problem into a maximum-a-posteriori framework and address it by manually designing kinds of regularization terms and data terms of the latent clear images. However, explicitly designing these two terms is quite challenging and usually leads to complex optimization problems which are difficult to solve. In this paper, we propose an effective non-blind deconvolution approach by learning discriminative shrinkage functions to implicitly model these terms. In contrast to most existing methods that use deep convolutional neural networks (CNNs) or radial basis functions to simply learn the regularization term, we formulate both the data term and regularization term and split the deconvolution model into data-related and regularization-related sub-problems according to the alternating direction method of multipliers. We explore the properties of the Maxout function and develop a deep CNN model with a Maxout layer to learn discriminative shrinkage functions to directly approximate the solutions of these two sub-problems. Moreover, given the fast-Fourier-transform-based image restoration usually leads to ringing artifacts while conjugate-gradient-based approach is time-consuming, we develop the Conjugate Gradient Network to restore the latent clear images effectively and efficiently. Experimental results show that the proposed method performs favorably against the state-of-the-art ones in terms of efficiency and accuracy

    Significant association of hematinic deficiencies and high blood homocysteine levels with burning mouth syndrome

    Background/PurposeBurning mouth syndrome (BMS) is characterized by a burning sensation of the oral mucosa in the absence of clinically apparent mucosal alterations. In this study, we evaluated whether there was an intimate association of the deficiency of hemoglobin (Hb), iron, vitamin B12, or folic acid; high blood homocysteine level; and serum gastric parietal cell antibody (GPCA) positivity with BMS.MethodsBlood Hb, iron, vitamin B12, folic acid, and homocysteine concentrations and the serum GPCA level were measured in 399 BMS patients and compared with the corresponding levels in 399 age- and sex-matched healthy control individuals.ResultsWe found that 89 (22.3%), 81 (20.3%), 10 (2.5%), and six (1.5%) BMS patients had deficiencies of Hb (men: <13 g/dL, women: <12 g/dL), iron (<60 μg/dL), vitamin B12 (<200 pg/mL), and folic acid (<4 ng/mL), respectively. Moreover, 89 (22.3%) BMS patients had abnormally high blood homocysteine level and 53 (13.3%) had serum GPCA positivity. BMS patients had a significantly higher frequency of Hb, iron, or vitamin B12 deficiency; of abnormally elevated blood homocysteine level; or of serum GPCA positivity than the healthy control group (all p < 0.001 except for vitamin B12 deficiency, for which p = 0.004). However, no significant difference in frequency of folic acid deficiency (p = 0.129) was found between BMS patients and healthy control individuals.ConclusionWe conclude that there is a significant association of deficiency of Hb, iron, and vitamin B12; abnormally high blood homocysteine level; and serum GPCA positivity with BMS

    Giant lipoma arising from deep lobe of the parotid gland

    BACKGROUND: Lipomas are common benign soft tissue neoplasms but they are found very rarely in the deep lobe of parotid gland. Surgical intervention in these tumors is challenging because of the proximity of the facial nerve, and thus knowledge of the anatomy and meticulous surgical technique are essential. CASE PRESENTATION: A 71-year-old female presented with a large asymptomatic mass, which had occupied the left facial area for over the past fifteen years, and she requested surgical excision for a cosmetically better facial appearance. The computed tomography (CT) scan showed a well-defined giant lipoma arising from the left deep parotid gland. The lipoma was successfully enucleated after full exposure and mobilization of the overlying facial nerve branches. The surgical specimen measured 9 × 6 cm in size, and histopathology revealed fibrolipoma. The patient experienced an uneventful recovery, with a satisfying facial contour and intact facial nerve function. CONCLUSION: Giant lipomas involving the deep parotid lobe are extremely rare. The high-resolution CT scan provides an accurate and cost-effective preoperative investigative method. Surgical management of deep lobe lipoma should be performed by experienced surgeons due to the need for meticulous dissection of the facial nerve branches. Superficial parotidectomy before deep lobe lipoma removal may be unnecessary in selected cases because preservation of the superficial lobe may contribute to a better aesthetic and functional result

Immobilization of BCNU on a nanocarrier might increase the thermal stability of BCNU and extend its half-life.Methods: Nanosized graphene oxide (GO) could be modified by polyacrylic acid (PAA) to improve the aqueous solubility and increase the cell penetration efficacy of the nanocarrier. PAA&amp;ndash;GO intended as a drug carrier for BCNU was prepared and characterized in this study. The size and thickness of PAA&amp;ndash;GO was investigated by transmission electron microscopy and atomic force microscopy, and the presence of PAA functional groups was confirmed by electron spectroscopy for chemical analysis and thermogravimetric analysis. BCNU was conjugated to PAA&amp;ndash;GO by covalent binding for specific killing of cancer cells, which could also enhance the thermal stability of the drug.Results: Single layer PAA&amp;ndash;GO (about 1.9 nm) with a lateral width as small as 36 nm was successfully prepared. The optimum drug immobilization condition was by reacting 0.5 mg PAA&amp;ndash;GO with 0.4 mg BCNU, and the drug-loading capacity and residual drug activity were 198 &amp;micro;g BCNU/mg PAA&amp;ndash;GO and 70%, respectively. This nanocarrier significantly prolonged the half-life of bound BCNU from 19 to 43 hours compared with free drug and showed efficient intracellular uptake by GL261 cancer cells. The in vitro anticancer efficacy of PAA&amp;ndash;GO&amp;ndash;BCNU was demonstrated by a 30% increase in DNA interstrand cross-linking and a 77% decrease in the IC50 value toward GL261 compared with the same dosage of free drug.Conclusion: Nanosized PAA&amp;ndash;GO serves as an efficient BCNU nanocarrier by covalent binding.     Higher power of sensorimotor rhythm is associated with better performance in skilled air-pistol shooters

Reduced cortical activity in the sensorimotor area has been related to less variability in the movement preparation that is conducive to skilled motor performance. This study proposes sensorimotor rhythm (SMR), 12–15 Hz of the electroencephalography (EEG) in the sensorimotor area, may be used to investigate psychomotor efficiency in sports performance. Method Twenty-four skilled air pistol shooters were recruited to fire 40 shots while EEG and shooting accuracy were recorded. Results The data show that improved performance of skilled shooters is associated with higher SMR power during the last second and lower coherence on high alpha power at Fz-T3 before action initiation. A negative relationship is also exhibited between the SMR power and the shooting performance during the aiming. Conclusions This finding suggests that reduced interference from sensorimotor processing, as reflected by elevated SMR power, may be related to improved processing efficiency during the aiming period. We conclude that SMR may be used to understand psychomotor efficiency underlying air-pistol shooting performance

    Impact of emotional and motivational regulation on putting performance: a frontal alpha asymmetry study

    Background The efficacy of emotional and motivational regulation can determine athletic performance. Giving the short duration and fast changing nature of emotions experienced by athletes in competition, it is important to examine the temporal dynamics of emotional and motivational regulation. The aim of this study was to investigate emotional and motivational regulation as measured by frontal alpha asymmetry in skilled golfers during putting performance after a performance failure. Methods Twenty skilled university golfers were recruited and requested to perform 40 putts at an individualized difficulty level of 40–60% successful putting rate. Trials immediately after a failed putt were selected for analysis. Successful performances were those trials where a hole was and unsuccessful performances were those that failed. The frontal alpha asymmetry index of LnF4-LnF3 was derived for statistical analysis. Results (1) Successful performance was preceded by a larger frontal alpha asymmetry index at T2 than that of T1, and (2) a larger frontal alpha asymmetry index was observed for unsuccessful performance than for successful performance at T1. Discussion The results suggest that successful emotional and motivational regulation was characterized by a progressive increase of frontal alpha asymmetry, which led to subsequent putting success when facing an emotionally provocative putting failure. These findings shed light on the application of frontal alpha asymmetry for the understanding and enhancement of emotional and motivational regulation during sport performance
