    Background Mental health problems are leading contributors to the global disease burden in adolescents. This study aims to highlight (1) salient context-specific factors that influence stress and coping among school-going adolescents across two urban sites in India; and (2) potential targets for preventing mental health difficulties. Methods Focus group discussions were undertaken with a large sample of 191 school-going adolescent boys and girls aged 11–17 years (mean = 14 years), recruited from low- and middle-income communities in the predominantly urban states of Goa and Delhi. Framework analysis was used to identify themes related to causes of stress, stress reactions, impacts and coping strategies. Results Proximal social environments (home, school, peers and neighborhood) played a major role in causing stress in adolescents’ daily lives. Salient social stressors included academic pressure, difficulties in romantic relationships, negotiating parental and peer influences, and exposure to violence and other threats to personal safety. Additionally, girls highlighted stress from having to conform to normative gender roles and in managing the risk of sexual harassment, especially in Delhi. Anger, rumination and loss of concentration were commonly experienced stress reactions. Adolescents primarily used emotion-focused coping strategies (e.g., distraction, escape-avoidance, emotional support seeking). Problem-focused coping (e.g., instrumental support seeking) was less common. Examples of harmful coping (e.g., substance use) were also reported. Conclusions The development of culturally sensitive and age-appropriate psychosocial interventions for distressed adolescents should attend to the challenges posed by home, school, peer and neighborhood environments. Enhancements to problem- and emotion-focused strategies are needed in order to bolster adolescents’ repertoire of adaptive coping skills in stressful social environments

    Evaluation of concentrated growth factor as an adjuvant on crestal bone levels around dental implants

    Background: Dental implants have a long-term success rate when the osteointegration is maintained with minimal crestal bone loss yearly. The present study evaluates the crestal bone loss around Osseointegrated implants using a concentrated growth factor as an added factor to preserve the crestal bone levels.Methods: Total of 20 patients with a single edentulous site were included in the study and divided into test and control groups. Test group concentrated growth factor is placed along with implant, and the control group implant placed without concentrated growth factor. Soft tissue parameters like modified sulcus bleeding index (mSBI) and modified plaque index (mPI) were done three and nine-month after implant placement. Hard tissue parameters like crestal bone levels (CBL), bone density, and volume were done immediately after implant placement and nine-month of implant placement using cone-beam computed tomography (CBCT).Results: Intragroup comparison from baseline to nine-month in the test group showed a significant difference at nine months at different points compared to the control group in hard and soft tissue parameters. On intergroup comparison was statically insignificant.Conclusions: Concentrated growth factor aids in enhancing bone density and faster healing around dental implants, so it can be used as an advancement in personalized medicine and promoting osseous regeneration by increasing the density and volume of bone around the dental implants.

    Genomic Profiling Identifies Novel Mutations and SNPs in ABCD1 Gene: A Molecular, Biochemical and Clinical Analysis of X-ALD Cases in India

    X-linked adrenoleukodystrophy (X-ALD) affects the nervous system white matter and adrenal cortex secondary to mutations in the ABCD1 gene that encode the peroxisomal membrane protein. We conducted a genomic and protein expression study of susceptibility gene with its clinical and biochemical analysis. To the best of our knowledge this is the first preliminary comprehensive study in Indian population that identified novel mutations and SNPs in a relatively large group. We screened 17 Indian indigenous X-linked adrenoleukodystrophy cases and 70 controls for mutations and SNPs in the exonic regions (including flanking regions) of ABCD1 gene by direct sequencing with ABI automated sequencer along with Western blot analysis of its endogenous protein, ALDP, levels in peripheral blood mononuclear cells. Single germ line mutation was identified in each index case in ABCD1 gene. We detected 4 novel mutations (2 missense and 2 deletion/insertion) and 3 novel single nucleotide polymorphisms. We observed a variable protein expression in different patients. These findings were further extended to biochemical and clinical observations as it occurs with great clinical expression variability. This is the first major study in this population that presents a different molecular genetic spectrum as compared to Caucasian population due to geographical distributions of ethnicity of patients. It enhances our knowledge of the causative mutations of X-ALD that grants holistic base to develop effective medicine against X-ALD

    FORMULATION AND EVALUATION OF GLICLAZIDE NANOSPONGES

    Objective: The objective of the present study was to develop and characterize an optimal stable nanosponges of Gliclazide (GLZ) by using the emulsion solvent diffusion method and aimed to increase its bioavailability and release the drug in sustained and controlled manner. Methods: The GLZ nanosponge was prepared by emulsion solvent diffusion method using different drug-polymer ratios (1:1 to 1:5) Eudragit S100 is used as a polymer. Differential scanning calorimetry (DSC) and Fourier transform infrared spectroscopy (FTIR) estimated the compatibility of GLZ with polymer. All formulations evaluated for production yield, entrapment efficiency, in vitro drug release, scanning electron microscopy (SEM) and stability studies. Results: The DSC and FTIR Studies revealed that no interaction between drug and polymer. The Production yield of all batches in the range of 73.8±0.30 to 85.6±0.32. Batch F3 showed the highest production yield, the entrapment efficiency of batch F3 70.6±0.77. The average particle size ranges from 303±2.36 to 680±2.50 nm. By the end of 10th hour F3 formulation shown highest drug release was found to be 94.40±1.12%. The release kinetics of the optimized formulation shows zero-order drug release. The stability study indicates no significant change in the in vitro dissolution profile of optimized formulation. Conclusion: The results of various evaluation parameters, revealed that GLZ nanosponges would be possible alternative delivery systems to conventional formulation to improve its bioavailability, the emulsion solvent diffusion method is best method for preparation of nanosponges and release the drug in sustained and controlled manner

    Atypical presentation of Hand foot mouth disease (HFMD) caused by enterovirus serotype Coxsackievirus A6, in India

    A 27-year-old male presented in the OPD of Naval Hospital in Port Blair, Andaman Islands, India, in 2011 with a history of low-grade fever associated with malaise and a pruritic skin rash. Case 2 – A 17-year-old male student reported to the OPD at Naval Hospital, Kochi Kerala, India, in August 2015. He presented with eruptions on both the palm and soles with a history of high-grade fever for the past 3–4 days. Clinically, both the cases were diagnosed as hand, foot, and mouth disease (HFMD). Both samples were tested against measles virus and varicella-zoster IgM antibodies by enzyme immunoassay and found negative. Stool sample (case 1) and lesion swab (case 2) were processed by enterovirus reverse transcription polymerase chain reaction and phylogenetic analysis, and both were positive for enterovirus human coxsackievirus A6 (CVA6) (untranslated region [UTR]). Phylogenetic analysis also confirmed that both the CVA6 etiology belonged to the genotype F. HFMD in adults often asymptomatic and very few patients get atypical symptoms. Clinical diagnosis is often troublesome to identify HFMD in such cases. An epidemiological surveillance/vigilance is essential to document these atypical cases in near future in developing countries like India

    Background: A class of uncommon IgG4-related systemic diseases known as vasculitis caused by antineutrophil cytosolic antibodies (ANCA) is distinguished by the enlargement of small to large blood vessels. Objective: This review article offers an in-depth analysis of the most recent developments in ANCA-associated vasculitis, covering a range of topics from pathophysiology and diagnostics to treatment and long-term results.   Methods: In this review we investigated the existing work on ANCA associated vasculitis by different sources such as Science Direct, Scopus, Pubmed, Web of Science, Google scholar and SciHub. Results: A number of diseases, including eosinophilic granulomatosis with polyangiitis (EGPA), microscopic polyangiitis (MPA), and granulomatosis with polyangiitis (GPA), can have significant morbidity and mortality if they are not appropriately diagnosed and treated. Understanding the pathophysiology, clinical presentation, and curative options for ANCA-associated vasculitis have advanced significantly during the past ten years. The development of specific immunosuppressive medications has been largely responsible for the remarkable evolution in recent years in the treatment of ANCA-associated vasculitis. Conclusion: This review article has provided a detailed examination of the therapy for AAV, alternatives available, including induction and maintenance regimens, as well as the accompanying advantages and disadvantages. Additionally, the growing importance of biologic drugs like rituximab was examined, emphasizing its potential as supplements or replacements for traditional medical treatments
