Rola adiponektyny w patogenezie zespołu metabolicznego oraz chorób układu krążenia
The aim of this review is to present the up-to-date data about
adiponectin and it’s role in pathogenesis of cardiovascular
disease. Adiponectin is a hormone derived from adipose
tissue which regulates energy metabolism, and plays an
important role in the pathogenesis of insulin resistance.
Serum levels of adiponectin are reduced in obesity, hypertension,
metabolic syndrome and type 2 diabetes. Moreover,
plasma adiponectin concentration is inversely associated
with LDL-cholesterol, TG and is positively related to HDLcholesterol.
Recent studies have indicated that adiponectin
has antiinflammatory and antiatherogenic properties. Review
of the data confirmed the hypothesis that adiponectin plays an important role in pathogenesis of cardiovascular
disease.Celem pracy jest przedstawienie aktualnego stanu wiedzy
na temat roli adiponektyny w patogenezie zespołu metabolicznego
oraz jej wpływu na rozwój chorób układu krążenia.
Adiponektyna jest hormonem produkowanym przez
tkankę tłuszczową, który reguluje metabolizm organizmu
oraz odgrywa istotną rolę w patogenezie insulinooporności.
Wykazano obniżone stężenie adiponektyny w otyłości,
u chorych z nadciśnieniem tętniczym, zespołem metabolicznym
i cukrzycą typu 2. Ponadto stwierdzono odwrotną
zależność między stężeniem adiponektyny i cholesterolu
frakcji LDL, TG, a dodatnią korelację ze stężeniem cholesterolu
frakcji HDL w surowicy. Ostatnie badania wskazują
również na przeciwzapalne i antymiażdżycowe właściwości
adiponektyny. Z przeprowadzonego przeglądu piśmiennictwa wynika, że adiponektyna odgrywa istotną rolę
w patogenezie chorób układu krążenia
Związek między depresją a cukrzycą — rola osi podwzgórzowo-przysadkowo-nadnerczowej i przewlekłego stanu zapalnego
Depresja i cukrzyca należą do najczęstszych chorób w ludzkiej populacji. U osób z chorobami przewlekłymi, w tym z cukrzycą, często stwierdza się zaburzenia nastroju. Zaobserwowano, że u pacjentów, u których współwystępują te dwa schorzenia, wyrównanie cukrzycy jest gorsze, a ryzyko powikłań i zgonu większe niż u osób, które chorują tylko na cukrzycę. Związek między cukrzycą a depresją jest złożony. Na częste współwystępowanie chorób mogą wpływać czynniki psychologiczne oraz zaburzenia hormonalne i immunologiczne. W depresji obserwuje się zaburzenia osi podwzgórzowo-przysadkowo-nadnerczowej, co w konsekwencji prowadzi do obwodowej hiperkortyzolemii. Nadmiar kortyzolu powoduje nasilenie glikogenolizy w wątrobie i zmniejszenie wrażliwości tkanek obwodowych na insulinę. Udowodniono, że depresji towarzyszy przewlekły stan zapalny o małym nasileniu. W niniejszej pracy przedstawiono dane dotyczące zależności między hiperkortyzolemią, subklinicznym stanem zapalnym a depresją u chorych na cukrzycę typu 1 i typu 2
Plasma levels of soluble tumor necrosis factor-α receptors are related to total and LDL-cholesterol in lean, but not in obese subjects
BACKGROUND: Tumor necrosis factor-α (TNFα) is a mediator of insulin resistance. Plasma levels of soluble TNFα receptors (sTNFR1 and sTNFR2) probably reflect paracrine action of the cytokine. TNFα is also a regulator of lipid metabolism, however, data about impact of obesity on the relationships between TNFα and plasma lipids remain controversial. AIM: The purpose of the present study was to examine the associations of TNFα system with plasma lipids in lean and obese subjects with normal glucose metabolism. METHODS: We examined 63 subjects, 33 lean (BMI<25 kg × m(-2)) and 30 with marked overweight or obesity (BMI>27.8 kg × m(-2)). Anthropometric and biochemical parameters were measured. Oral glucose tolerance test and euglycemic hyperinsulinemic clamp were also performed. RESULTS: Obese subjects were markedly more insulin resistant and had higher levels of both TNFα receptors. Total (TC) and LDL-cholesterol (LDL-C), triglycerides (TG) and non-esterified fatty acids (NEFA) were also higher in the obese group. In obese subjects, both receptors were significantly related to TG and HDL-cholesterol (HDL-C), while sTNFR2 was also associated with NEFA. All those correlations disappeared after controlling for insulin sensitivity. In lean subjects, both receptors were related to TC, HDL-C and LDL-C. In that group, sTNFR1 predicted values of all those parameters independently of BMI, plasma glucose and insulin, and insulin sensitivity. CONCLUSION: We conclude that TNFα receptors are associated with plasma lipids in different way in lean and in obese subjects. TNFα system is probably important in determining cholesterol levels in lean subjects, while in obese this effect might be masked by other metabolic abnormalities
Neuroendocrine tumors of gastrointestinal tract in own material
Guzy neuroendokrynne należą do rzadkich schorzeń przewodu
pokarmowego, które często sprawiają kłopoty diagnostyczne
i lecznicze. Celem pracy było opisanie 2 przypadków
z guzem neuroendokrynnym przewodu pokarmowego
operowanych w 2005 roku w II Klinice Chirurgii Ogólnej
i Gastroenterologicznej AM w Białymstoku.
U 63-letniej pacjentki, u której wstępnie rozpoznano przerzuty
do wątroby guza neuroendokrynnego, nie udało się
w badaniach przedoperacyjnych zlokalizować dokładnie
ogniska pierwotnego. Dopiero podczas operacji stwierdzono
obecność guza neuroendokrynnego w końcowym odcinku
jelita krętego z przerzutami do wątroby i sieci większej.
Wykonano hemikolektomię prawostronną oraz częściową
resekcję segmentów VII i VIII wątroby wraz z guzami
przerzutowymi. W przebiegu pooperacyjnym obserwowano
zaburzenia neurologiczne o niejasnym podłożu
bez zmian ogniskowych w tomografii komputerowej.
U 57-letniego chorego operowanego z powodu wznowy
węzłowej guza żołądka rozpoznano histopatologicznie rakowiaka.
Wycięty 6 lat wcześniej guz żołądka był zdiagnozowany
jako Adenocarcinoma G2 pT2N0M0. Po zabiegu limfadenektomii
obserwowano u pacjenta rozsiew do wątroby
i węzłów chłonnych w jamie brzusznej, potwierdzony
oktreoskanem. Rozpoczęto terapię analogiem somatostatyny
(oktreotydem LAR). Mimo leczenia nastąpił zgon.
Autorzy niniejszej pracy prezentują własne doświadczenia,
obrazujące trudności w diagnostyce przedoperacyjnej guzów
neuroendokrynnych przewodu pokarmowego. Przedstawiono
również nieoczekiwane powikłania neurologiczne
w przebiegu leczenia.Neuroendocrine tumors are rare gastrointestinal tract disorders,
in which diagnosis and treatment are often difficult.
The aim of the paper is to present two cases of patients
with neuroendocrine tumor of gastrointestinal tract, who
underwent surgical procedure in II Department of General
and Gastroenterological Surgery of Medical University of
Białystok in 2005.
A 63-year-old female patient with primary diagnosis of neuroendicrine
tumor metastases in liver was not successfully
investigated for primary tumor in the preoperative period.
The laparotomy procedure indicated the malignant neuroendocrine
tumor in the terminal ileum and metastases to
the liver and to the greater omentum. The right hemicolectomy
and liver metastatic segment VII and VIII resection
were performed. The neurological disturbances of obscure
origin were observed in the postoperative period and the
patient suddenly died on the 15th day after surgery.
A 57-years-old male patient was operated on for lymph node
recurrence of gastric tumor. Pathologic examination of tissue
sample revealed the diagnosis of carcinoid. The patient
underwent subtotal gastric resection for a pyloric ulcer, diagnosed as Adenocarcinoma G2 pT2N0M0 6 years before.
Liver and abdominal node metastases, confirmed by octreoscan,
were observed after lymphadenectomy, The treatment
of somatostatin analogues (LAR octreotide) was used.
In spite of therapy the patient died.
The authors present their own experiences and show the
preoperative diagnostic difficulties in patients with neuroendocrine
gastrointestinal tumors. Unexpected neurological
complications in the treatment course were described
Bcl3 Couples Cancer Stem Cell Enrichment With Pancreatic Cancer Molecular Subtypes
[Background & Aims]: The existence of different subtypes of pancreatic ductal adenocarcinoma (PDAC) and their correlation with patient outcome have shifted the emphasis on patient classification for better decision-making algorithms and personalized therapy. The contribution of mechanisms regulating the cancer stem cell (CSC) population in different subtypes remains unknown.
[Methods]: Using RNA-seq, we identified B-cell CLL/lymphoma 3 (BCL3), an atypical nf-κb signaling member, as differing in pancreatic CSCs. To determine the biological consequences of BCL3 silencing in vivo and in vitro, we generated bcl3-deficient preclinical mouse models as well as murine cell lines and correlated our findings with human cell lines, PDX models, and 2 independent patient cohorts. We assessed the correlation of bcl3 expression pattern with clinical parameters and subtypes.
[Results]: Bcl3 was significantly down-regulated in human CSCs. Recapitulating this phenotype in preclinical mouse models of PDAC via BCL3 genetic knockout enhanced tumor burden, metastasis, epithelial to mesenchymal transition, and reduced overall survival. Fluorescence-activated cell sorting analyses, together with oxygen consumption, sphere formation, and tumorigenicity assays, all indicated that BCL3 loss resulted in CSC compartment expansion promoting cellular dedifferentiation. Overexpression of BCL3 in human PDXs diminished tumor growth by significantly reducing the CSC population and promoting differentiation. Human PDACs with low BCL3 expression correlated with increased metastasis, and BCL3-negative tumors correlated with lower survival and nonclassical subtypes.
[Conclusions]: We demonstrate that bcl3 impacts pancreatic carcinogenesis by restraining CSC expansion and by curtailing an aggressive and metastatic tumor burden in PDAC across species.
