    Quality of Life in Children with Asthma versus Healthy Children

    BACKGROUND: Asthma is the most prevalent chronic disease in the pediatric age group. The disease affects different aspects of the children's lives, such as physical, emotional, social and educational aspects. Thus, more focus has been on the quality of life in these patients rather than the duration of their illness in recent years.AIM: This study examined the different aspects of quality of life in asthmatic children for the first time in this geographic area.METHODS: The study was cross-sectional conducted in 2015-2016. The asthmatic group was 100 patients aged 8 to 12 admitted to the Asthma and Allergy Clinic of Ghaem Hospital (as) in Mashhad with the control group composed of 100 healthy children of the same age and gender. The standard questionnaire pedsQLTM was used for comparing the quality of life of children in the two groups. Statistical analysis was SPSS23 with P-value less than 0.05, which was statistically significant.RESULTS: In each group, 58 patients were boys, and 42 were girls. In a comparison of the quality of life of children, the asthma group with a mean total score of Peds QL 20.99 ± 12.54 compared to the healthy children with a mean total score of Peds QL of 8.8 ± 5.41 had a lower quality of life (P < 0.001). Moreover, regarding various aspects of quality of life asthma group had a lower quality of life in physical performance, emotional performance and performance in school (P < 0.001). Nonetheless, there was no significant difference between the two groups considering social function (P = 0.267). Examining the relationship between Peds QL score of patients with asthma with various variables was indicative of the fact that Peds QL scores were significantly correlated with the gender of the patients, showing better quality of life in the girls (P = 0.001).CONCLUSION: The results indicated that children with asthma have a significantly lower quality of life compared with healthy children of the same age. Also, in examining the different aspects of quality of life, these children had a lower quality of life in physical performance, emotional performance, and performance at school, and were at the level as that of healthy children only in social performance

    Immunotherapy in Allergic Rhinitis: It's Effect on the Immune System and Clinical Symptoms

    BACKGROUND: Allergic rhinitis is one of the most common allergic diseases and characterised by sneezing, rhinorrhea, nasal congestion and nasopharyngeal itching. Subcutaneous immunotherapy (SCIT) for specific allergens is an effective treatment and induces the inhibitory effect of T regulatory lymphocytes and decreases clinical symptoms in allergic rhinitis. AIM: In this study effect of subcutaneous immunotherapy with specific allergens on clinical symptoms and T regulatory and T Helper cells cytokines, in patients with allergic rhinitis are evaluated. METHODS: In this study, 30 patients with moderate to severe allergic rhinitis according to clinical criteria and positive skin prick test for aeroallergens were selected and treated by SCIT. Clinical symptoms and T cells cytokines IL4, IL17, IFN gamma, TGF beta, GITR, FOXP3 and IL-10 (by RT-PCR) were evaluated before and one year after initiation of treatment. RESULTS: Thirty (30) patients with allergic rhinitis at age range 15-45 years old were treated by SCIT, and 23 (14 female, 9 male) patients continued the study, and 7 patients did not continue treatment. After immunotherapy, clinical symptoms decreased significantly. The specific cytokines TGF beta and IL10 levels increased and changes were statistically significant. (Respectively P = 0.013 and P = 0.05) The IL17 level was also increased, but not statistically significant. (P = 0.8) IFN gamma, IL4, GITR, FOXP3, all decreased, but the changes were not statistically significant (P > 0.05). CONCLUSION: Subcutaneous Immunotherapy for specific allergens decreases clinical symptoms in patients with allergic rhinitis and induces tolerance in T lymphocytes, especially by increasing T regulatory cells cytokines, TGF beta and IL10

    The effect of positive end-expiratory pressure during anesthesia on arterial oxygen saturation after surgery in patient undergoing cesarean section

    One of the major complications of general anesthesia in the recovery room is arterial oxygen desaturation and hypoxemia. Positive end-expiratory pressure (PEEP) can improve arterial oxygen saturation by increasing FRC. This study aims to evaluate the effects of applying PEEP on arterial oxygen saturation and hemodynamic parameters in the patient undergoing cesarean section in VALIASR hospital. In this double blind clinical trial we randomly allocated 120 patients of class1 and 2 ASA scheduled to undergoing cesarean section into 3 group (in 40).Different levels of PEEP (0, 5 and 10 CmH2o) were applied to each group while zero PEEP was established as control. All other variables (anesthesia and surgery techniques) were the same for all patients SPO2, noninvasive mean arterial pressure and heart rate were measured before, during and after surgery (Recovery room). The comparison of noninvasive arterial blood pressure and heart rate during and after surgery did not show significant differences but mean o2 saturation in group B (5 cmH2o PEEP) and C (10 cm H2o PEEP) in PACU was higher than control group (98.30±0.93 and 98.50±0.90 as opposed to97.12±1.15 respectively) P0<001. In light of results applying PEEP is effective in preventing desaturation after surgery and improving respiratory indexes without the significant hemodynamic changes, the result of using five cmH2o PEEP is more efficient and satisfying

The most cause of Nosocomial infection include: bloodstream infection, urinary tract infection, pneumonia, and wound infection. The purpose of this study was to determine the epidemiology of the three most common NI in the Pediatric department.        Materials and Methods: We performed a prospective study in a single Pediatric department during 12 months. Children were assessed for 3 NI: wound infections, pneumonia and urinary tract infections (UTI), as the same method as Center of Disease Control criteria. All patients were followed up and individuals who had have NI and their treatment was entered in this study.          Results: In this study 811 patients were hospitalized that 60% of them were male and were older than 60 months. The main causes of hospitalization include: toxicity, seizure, respiratory infection and fever. Among them 15 cases had NI (1.87%). The most NI occurred in pediatric intensive care unit (PICU) and it was followed in aspect of intubation. The most cultured organism was pseudomonas that they suspected to ceftazidime and isolate from blood and endotracheal tube.           Conclusion:  NI presence was associated with increased mortality and length of stay in hospital. This study highlights the importance of NIs in children admitted to a pediatric department especially PICU in a developing country. Clinical monitoring of NIs and bacterial resistance profiles are required in all pediatric units

    Assessment of Pain Management in Pediatric Emergency Department in Mashhad -Iran

    Introduction: Pain may be described as a sensation of hurt or strong discomfort and is the body's way of sending message to the brain that an injury has occurred. Pain medicines block these messages or reduce their effect on the brain. Accurate administration of analgesia have a long –lasting effect on children whole experience of medical care and affects parents' and children's future reaction to pediatrics emergency departments. The purpose of this study was to evaluate pain management on children in our emergency department. Materials and Methods: In this study we evaluated the relief of pain and anxiety on 100 children who referred to our pediatric Emergency Department (ED) in Imam Reza Hospital- Mashhad .The patients were assessed based on the American Academy of Pediatrics (AAP) recommendations about pain.  Results: Patients were gone under IV Line 97%, Intubation 5% and Lumbar Puncture 28%. Training had been provided to 70% participants in the Emergency Department. Nonpharmacologic stress reduction was used in 35% of cases. Family presence was allowed only in 5%. Prehospital pain controlling was began on 20% of patients and continued in ED on 40%. At the time of discharge 40% prescribed analgesics. Sedation and pain prophylaxis was provided for 10% of patients undergoing painful procedures in ED.  Conclusion: According to results, pain management in our Pediatric Emergency Department was inadequate. Physicians and prehospital EMS providers should be justified about the importance of pain relieving and trained how to use all available analgesic and sedative options

    The Epidemiology of Cold–weather Injuries in Infants in Mashhad, Iran

    Background: Injury is a leading cause of morbidity and mortality in children. There is insufficient data on the epidemiology or incidence of injuries in the first two years of life, especially in Iran. The main objective of this study was to describe the incidence and causes of injuries in a sample group of infants and toddlers in Mashhad, Iran. Methods: This prospective, observational study, was conducted on 1000 infants (6-24 months old), admitted to 46 healthcare centers of Mashhad. The infants were observed in terms of the occurrence of injuries and the outcomes over six months. Results: At the beginning of the study, the mean age of the infants was 13.8±7.8 months. During six months of observation, 382 episodes of injuries were detected. The injuries were burning (134 episodes), falling (132 episodes), cuts and lacerations (63 episodes), choking due to foreign body aspiration (40 episodes), poisoning (9 episodes), traffic accidents and electrical injuries (2 episodes each) and drowning (no case), respectively. During the study period, 42 injury-related outpatient visits (by a physician) were recorded, which were caused by burning (28 visits), falling (13 visits) and traffic accidents (one visit), respectively. Twenty-one cases of hospital admission were reported, although only two of them were caused by accidents (a case of falling and a case of burning). Conclusion: During the cold months, burns and falls were the main causes of injury in children, aged 6-24 months in Mashhad. However, injury was not a major cause of hospital admission in this group

    A survey of anaphylaxis etiology and treatment

    Identifying the causes of anaphylaxis which is an acute, potentially fatal systemic reaction is very important in every community. Treatment strategies and pitfalls should also be determined. We sought to determine the most common triggers of anaphylaxis, clinical manifestations and treatment strategies in Mashhad, northeast of Iran. An observational cross-sectional study was conducted to evaluate all patients with a history of anaphylactic reaction who were referred to University Allergy Clinics between 2006 and 2016 in Mashhad Iran. We used a combination of patient's clinical history and allergy diagnostic testing including radioallergosorbant test and skin prick test in order to determine the etiology of anaphylaxis. We identified 172 anaphylactic reactions in 70 patients. Median age was 15 years with a range from 6 months to 48 years. The triggers included: foods, 61.4%; drugs, 15.7%; hymenoptera venom, 8.6%; idiopathic, 5.7%; immunotherapy, 4.3% and other etiologies: 5.7%. Nuts and seeds were the most important triggers of food induced anaphylaxis, especially in school children, adolescents and young adults, followed by fruits. However, Cow's milk and hen's egg were the main triggers of anaphylaxis in children aged under 2 years. The most common symptoms were cutaneous and cardiovascular. Corticosteroids (94.3%) and/or antihistamines (85.7%) were used most frequently for treatment followed by intravenous fluids (54.3%), whereas epinephrine was only used in 17.1% of the cases. Food related anaphylaxis and other typical triggers of anaphylaxis are age dependent and the risks and triggers change with age. Epinephrine injection should be increased by improving the awareness of physician and medical teams.
