A data mining approach in home healthcare: outcomes and service use
BACKGROUND: The purpose of this research is to understand the performance of home healthcare practice in the US. The relationships between home healthcare patient factors and agency characteristics are not well understood. In particular, discharge destination and length of stay have not been studied using a data mining approach which may provide insights not obtained through traditional statistical analyses. METHODS: The data were obtained from the 2000 National Home and Hospice Care Survey data for three specific conditions (chronic obstructive pulmonary disease, heart failure and hip replacement), representing nearly 580 patients from across the US. The data mining approach used was CART (Classification and Regression Trees). Our aim was twofold: 1) determining the drivers of home healthcare service outcomes (discharge destination and length of stay) and 2) examining the applicability of induction through data mining to home healthcare data. RESULTS: Patient age (85 and older) was a driving force in discharge destination and length of stay for all three conditions. There were also impacts from the type of agency, type of payment, and ethnicity. CONCLUSION: Patients over 85 years of age experience differential outcomes depending on the condition. There are also differential effects related to agency type by condition although length of stay was generally lower for hospital-based agencies. The CART procedure was sufficiently accurate in correctly classifying patients in all three conditions which suggests continuing utility in home health care

Meeting the home-care needs of disabled older persons living in the community: does integrated services delivery make a difference?
<p>Abstract</p> <p>Background</p> <p>The PRISMA Model is an innovative coordination-type integrated-service-delivery (ISD) network designed to manage and better match resources to the complex and evolving needs of elders. The goal of this study was to examine the impact of this ISD network on unmet needs among disabled older persons living in the community.</p> <p>Methods</p> <p>Using data from the PRISMA study, we compared unmet needs of elders living in the community in areas with or without an ISD network. Disabilities and unmet needs were assessed with the Functional Autonomy Measurement System (SMAF). We used growth-curve analysis to examine changes in unmet needs over time and the variables associated with initial status and change. Sociodemographic characteristics, level of disability, self-perceived health status, cognitive functioning, level of empowerment, and the hours of care received were investigated as covariates. Lastly, we report the prevalence of needs and unmet needs for 29 activities in both areas at the end of the study.</p> <p>Results</p> <p>On average, participants were 83 years old; 62% were women. They had a moderate level of disability and mild cognitive problems. On average, they received 2.07 hours/day (SD = 1.08) of disability-related care, mostly provided by family. The findings from growth-curve analysis suggest that elders living in the area where ISD was implemented and those with higher levels of disability experience better fulfillment of their needs over time. Besides the area, being a woman, living alone, having a higher level of disability, more cognitive impairments, and a lower level of empowerment were linked to initial unmet needs (r<sup>2 </sup>= 0.25; p < 0.001). At the end of the study, 35% (95% CI: 31% to 40%) of elders with needs living in the ISD area had at least one unmet need, compared to 67% (95% CI: 62% to 71%) in the other area. In general, unmet needs were highest for bathing, grooming, urinary incontinence, walking outside, seeing, hearing, preparing meals, and taking medications.</p> <p>Conclusions</p> <p>In spite of more than 30 years of home-care services in the province of Quebec, disabled older adults living in the community still have unmet needs. ISD networks such as the PRISMA Model, however, appear to offer an effective response to the long-term-care needs of the elderly.</p

Individual, task, and environmental influences on balance recovery: a narrative review of the literature and implications for preventing occupational falls
Balance recovery is a complex, multi-factorial task. When examining occupational environments for fall safety hazards, practitioners must be aware of how a workerâs ability to recover from balance loss and avoid a fall depends on their unique individual characteristics, the task they are performing, and their work environment. Balance recovery can be negatively affected by factors related to the individual (e.g., aging, obesity, arthritis, low back pain, fatigue, and peripheral neuropathies); the task (e.g., holding objects or performing multiple tasks); and the environment (e.g., slopes or stairs). Conversely, balance recovery can be enhanced by exposure to balance disturbances in the context of perturbation training, and by environmental design (e.g., appropriately-designed handrails). By understanding how individual, task, and environmental factors influence balance recovery and overall fall risk, occupational health and safety practitioners will be in a stronger position to design and implement safety controls to prevent occupational slips, trips, and falls
Quantifying segmental contributions to center-of-mass motion during dynamic continuous support surface perturbations using simplified estimation models
Investigating balance reactions following continuous, multidirectional, support surface perturbations is essential for improving our understanding of balance control in moving environments. Segmental motions are often incorporated into rapid balance reactions following external perturbations to balance, although the effects of these motions during complex, continuous perturbations have not been assessed. This study aimed to quantify the contributions of body segments (ie, trunk, head, upper extremity, and lower extremity) to the control of center-of-mass (COM) movement during continuous, multidirectional, support surface perturbations. Three-dimensional, whole-body kinematics were captured while 10 participants experienced 5 minutes of perturbations. Anteroposterior, mediolateral, and vertical COM position and velocity were calculated using a full-body model and 7 models with reduced numbers of segments, which were compared with the full-body model. With removal of body segments, errors relative to the full-body model increased, while relationship strength decreased. The inclusion of body segments appeared to affect COM measures, particularly COM velocity. Findings suggest that the body segments may provide a means of improving the control of COM motion, primarily its velocity, during continuous, multidirectional perturbations, and constitute a step toward improving our understanding of how the limbs contribute to balance control in moving environments
