    A Personal Navigation System for Sightseeing across Multiple days

In this paper, we propose a new scheduling function to compose sightseeing tours across multiple days. Scheduling for multiple days is challenging since the number of possible sequences to travel destinations becomes huge and accommodation places have to be carefully chosen considering the schedules before and after each stay. So, some techniques are required to calculate the sightseeing schedule across multiple days at practically short time. In our method, we divide the target area into several sub-areas, and we restrict each day trip to be in a limited number of areas with infrequent passing of area boundary, in order to decrease the number of sequences. We have designed and implemented a genetic algorithm to solve this scheduling problem at a practical time. In this algorithm, accommodation places are represented in chromosome as exclusive alleles, to make schedules which contain only one accommodation place for each day. In order to evaluate our method, we made various sightseeing schedules across multiple days using the digital map of the Tohoku area (Japan Geographical Survey Institute numeric map 25000), and confirmed that the proposed algorithm could compute the near best 6 days schedule in several tens of seconds

    Multiple Retinal Vascular Occlusion after Vitrectomy

    Retinal vascular occlusion after ocular surgery is a rare but serious complication. A history of cardiovascular diseases, retrobulbar anesthesia injection, high intraocular pressure during the perioperative period, and drug toxicity have been reported as possible causative factors. We report here two cases of multiple retinal vascular occlusions after the subconjunctival injection of gentamicin at the end of uncomplicated 25-gauge vitrectomy. Case 1 was a 61-year-old man who developed a macular hole in the right eye. Phacovitrectomy with gas tamponade was performed. On postoperative day (POD) 1, dot hemorrhage was observed on the temporal side of the optic disk. On POD10, macular whitening, retinal hemorrhage, and multiple occlusion of retinal arteries and veins were observed. Case 2 was a 51-year-old woman who was diagnosed with rhegmatogenous retinal detachment in the right eye and underwent phacovitrectomy with gas tamponade. On POD3, macular whitening with cotton wool spots and retinal hemorrhage were observed with macular ischemia owing to occlusion of retinal arteries and veins. In both cases, subconjunctival injection of gentamicin given at the end of surgery was the most suspected cause of retinal vascular occlusion

    Case report: Rehabilitation course in thrombocytopenia, anasarca, fever, reticulin fibrosis/renal failure, and organomegaly syndrome complicated by cerebral infarction in the left parabolic coronary region

    Although thrombocytopenia, anasarca, fever, reticulin fibrosis/renal failure, and organomegaly (TAFRO) syndrome was first reported in 2010, its pathogenesis and prognosis are still unknown. Moreover, reports on rehabilitation in patients with TAFRO are limited. In severe cases, dyspnea and muscle weakness could impede improvements in activities of daily living (ADL). However, reports on exercise intensity showed no worsening of TAFRO within the load of 11–13 on the Borg scale. Herein, we describe the rehabilitation and progress in a 61-year-old woman with TAFRO syndrome complicated by cerebral infarction from early onset to discharge. After cerebral infarction onset in the perforating artery, she was admitted to the intensive care unit due to decreased blood pressure and underwent continuous hemodiafiltration. Two weeks following transfer to a general ward, the patient started gait training using a brace due to low blood pressure, respiration, and tachycardia. After initiating gait training, increasing the amount of training was difficult due to a high Borg scale of 15–19, elevated respiratory rate, and worsening tachycardia. Furthermore, there was little improvement in muscle strength on the healthy side after continuous training, owing to long-term steroid administration. On day 100 after transfer, the patient was discharged home with a T-cane gait at a monitored level. The patient had severe hemiplegia due to complications with severe TAFRO syndrome delaying early bed release and gait training; tachycardia; and respiratory distress. Additionally, delayed recovery from muscle weakness on the non-paralyzed side made it difficult for the patient to walk and perform ADLs. Despite these issues, low-frequency rehabilitation was useful. However, low-frequency rehabilitation with gait training, using a Borg scale 15–19 orthosis, did not adversely affect the course of TAFRO syndrome

    We investigated the rates of the use of steroids in Japanese central serous chorioretinopathy (CSC) cases and differences in the characteristics of CSC with and without steroids. A total of 538 eyes of 477 patients diagnosed with CSC, with 3 months or more of follow-up between April 2013 and June 2017 at 8 institutions. Patients with CSC with more than 3 months of follow-up were identified by OCT and fluorescein angiography at 8 institutions. Data collected included patient demographics, history of corticosteroid medication and smoking, spherical errors, findings of angiography, subfoveal choroidal thickness, and changes through the follow-up period. Differences in these findings were analyzed in cases with and without corticosteroid treatment. Among the 477 patients (344 men,133 women), 74 (15.5%) (39 men, 35 women) underwent current or prior steroid treatment. Cases with steroids were higher age (p = 0.0403) and showed no male prevalence, more bilateral involvement (p < 0.0001), and the affected eyes had multiple pigment epithelial detachment (p <0.0001), more fluorescein leakage sites (p < 0.0001), greater choroidal thickness (p = 0.0287) and a higher recurrence rate (p = 0.0412). Steroids can cause severer CSC through an effect on choroidal vessels and an impairment of retinal pigment epithelium

    Changes in Choroidal Structures in Eyes with CSC after Photodynamic Therapy

    Purpose To determine the structural changes in the choroid after half-dose photodynamic therapy (hPDT) in eyes with chronic central serous chorioretinopathy (CSC). Methods This was a retrospective interventional study of 29 eyes of 29 patients who underwent hPDT for chronic CSC with serous retinal detachment (SRD) and were followed for ≥3 months. Enhanced depth imaging optical coherence tomographic (EDI-OCT) images of the subfoveal choroid were converted to binary images. The central choroidal thickness (CCT), the cross sectional subfoveal choroidal area, the hyporeflective and hyperreflective areas of the inner, outer, and whole choroid were determined at the baseline, and at 1, 3, and 12 months after the hPDT. Results The SRDs were resolved in 26 (89.7%) eyes at 3 months after the hPDT. The mean CCT (P = 0.001), the total choroidal area (P = 0.001), and the hypo-reflective area (P = 0.003) of the whole choroid were significantly decreased from the baseline at 3 months. The hyperreflective area of whole choroid was not significantly changed during the study period (P = 0.083). The hyperreflective but not the hyporeflective area of the inner choroid was significantly decreased at 3 months (P = 0.001, P = 1.000, respectively). The hyporeflective but not the hyperreflective area of the outer choroid was significantly decreased at 3 months (P = 0.001, P = 1.000, respectively). Conclusions The hyperreflective area of the inner choroid and hyporeflective area of the outer choroid were significantly decreased after hPDT for chronic CSC. Because the hyperreflective and hyporeflective area correspond to the choroidal stroma and vessels, respectively, the decreased CCT and subfoveal choroidal area after hPDT may be attributed to a decrease in the exudative changes in the inner choroidal stroma and the reduction of the dilation of the outer choroidal vessels

    Mechanosensory trichome cells evoke a mechanical stimuli–induced immune response in Arabidopsis thaliana

    Perception of pathogen-derived ligands by corresponding host receptors is a pivotal strategy in eukaryotic innate immunity. In plants, this is complemented by circadian anticipation of infection timing, promoting basal resistance even in the absence of pathogen threat. Here, we report that trichomes, hair-like structures on the epidermis, directly sense external mechanical forces, including raindrops, to anticipate pathogen infections in Arabidopsis thaliana. Exposure of leaf surfaces to mechanical stimuli initiates the concentric propagation of intercellular calcium waves away from trichomes to induce defence-related genes. Propagating calcium waves enable effective immunity against pathogenic microbes through the CALMODULIN-BINDING TRANSCRIPTION ACTIVATOR 3 (CAMTA3) and mitogen-activated protein kinases. We propose an early layer of plant immunity in which trichomes function as mechanosensory cells that detect potential risks

    Exercise on the Structure and Circulation of Choroid

    Aims To determine the effects of dynamic exercise on the circulation and the luminal and stromal areas of the choroid in normal eyes. Methods This was a prospective interventional study of 38 eyes of 38 normal subjects enrolled by invitation. The systolic and diastolic blood pressures, heart rate, intraocularpressure, mean ocular perfusion pressure (MOPP), choroidal blood velocity, and enhanced depth imaging optical coherence tomographic (EDI-OCT) images were recorded before, and immediately after mild dynamic exercise. The same measurements were recorded after 10 min of rest. The choroidal blood velocity was measured bylaser speckle flowgraphy, and the mean blur rate was used for the evaluations. The horizontal EDI-OCT images of the subfoveal choroid were converted to binary images. The central choroidal thickness (CCT), total cross sectional choroidal area, luminal areas, stromal areas, and the ratio of luminal area to total choroidal area (L/C ratio) were determined from these images. Results The systolic and diastolic blood pressures, heart rate, MOPP, and the mean blur rate were significantly increased immediately after the exercise and significantly decreased 10 minutes after the exercise. There wereno significant changes in the mean CCT, the mean total choroidal area, the mean luminal and stromal areas, and the mean L/C ratio after the exercise. Conclusions Our results suggest that a rest period is needed before measurements of blood flow velocity but not necessary for the EDI-OCT imaging to determine the choroidal thickness and area

    Many commercially important fishes associate with drifting seaweeds in their juvenile stage, however, the ecological significance of drifting seaweeds for juvenile fishes is still unclear. We postulated that the following two hypotheses may be applicable for juvenile fishes associate with drifting seaweeds, the “concentration of food supply” hypothesis: juvenile fishes are attracted by phytal animals on the drifting seaweeds and the “indicator-log” hypothesis: fish use accumulations of drifting seaweed as an indicator of productive areas (e.g. frontal areas) for food. We investigated the frontal areas, zooplankton abundance around the drifting seaweed, and the food availability of fish juveniles associated with drifting seaweed accumulations in the East China Sea in 2012 and 2013. A total of 14 drifting seaweed mass and 22 species (n = 408) of fish juveniles were collected. We found that 49.7 - 99.7 % of the individual fed on planktonic food and the feeding incidence on phytal animals was less than 50 %. Although drifting seaweeds were aggregated around the frontal areas of surface currents, the zooplankton abundance was not significantly different between these frontal areas and other areas. Our findings indicate that ecological significance of drifting seaweeds as feeding habit is relatively low for juvenile fishes associated with drifting seaweeds.流れ藻には多くの水産上重要種の稚魚が付随するが,流れ藻の稚魚にとっての生態学的意義は明らかにされていない。筆者らは次の2仮説のいずれかが流れ藻付随稚魚に当てはまると考え,“concentration of food supply hypothesis”(流れ藻葉上生物を摂餌するため)と“indicator log hypothesis”(流れ藻をフロント域のような餌豊度の高い海域の目印とするため)を検証するため,2012年と2013年に東シナ海の流れ藻周辺の海洋環境,フロント域,動物プランクトン豊度,流れ藻付随稚魚の摂餌個体率を調べた。流れ藻は表層流の収束帯に集積されていたが,収束帯のプランクトン豊度は高くなかった。合計14個の流れ藻を採集し,合計22種(408尾)の稚魚の胃内容物を調査した結果,稚魚の49.7 - 99.7 %の個体はプランクトンを選択的に摂餌していたが,葉上生物の摂餌個体率は高くなかった(50 %未満)。以上の結果から流れ藻の稚魚にとっての生態学的意義は摂餌場でないことが示唆された
