    Modernization as a Politico-Cultural Response and Modernity as a Culltural Mixture : An Alternative View of Korean Modernization

    Revised paper presented at the international conference on The Course of Modernization in the Countries and Regions of East Asia, organized by the International Committee of Historical Sciences(CISH), the Association of Chinese Historians, and the Institute of World History of the Chinese Academy of Social Sciences, April 7-9, 2004, Beijing, China.This paper presents an alternative view, an indigenized interpretation, of modernization and modernity from the perspective of the historical experience of Korea. Modernization is conceived as a twin process; first of international acculturation emanating from the initial modernization of the West around the turn of the 16th century; and second, of indigenous adaptive change in each society exposed to it; the former as the converging force and the latter as the divergent process. The nature of the dynamics of indigenous adaptation in each late modernizer society is explained by the Principles of Political and Cultural Selectivity which provide ideas about the interactions among different social forces and selective processes of the existing culture. The end result of this politico-cultural response to the global modernization originating in the West is conceived to be modernity of each non-western modernizing society. Modernity in this sense is a mixture of traditional elements, colonial legacies if relevant, and indigenous effort to adapt to the changing situation, and hence in some respect similar to, and yet in other respects different from Western modernity. The issue of rationality and rationalization, the central theme of Western modernity, is discussed in the context of Korean experience and prospect of change. Finally, some theoretical implications of Yin-Yang dialectics are touched upon for future reflections on the idea of challenge and response in global transformations.This research was supported by a grant from the Research Fund of the KDI School of Public Policy and Management

    Relation between Obesity and Bone Mineral Density and Vertebral Fractures in Korean Postmenopausal Women

    PURPOSE: The traditional belief that obesity is protective against osteoporosis has been questioned. Recent epidemiologic studies show that body fat itself may be a risk factor for osteoporosis and bone fractures. Accumulating evidence suggests that metabolic syndrome and the individual components of metabolic syndrome such as hypertension, increased triglycerides, and reduced high-density lipoprotein cholesterol are also risk factors for low bone mineral density. Using a cross sectional study design, we evaluated the associations between obesity or metabolic syndrome and bone mineral density (BMD) or vertebral fracture. MATERIALS AND METHODS: A total of 907 postmenopausal healthy female subjects, aged 60-79 years, were recruited from woman hospitals in Seoul, South Korea. BMD, vetebral fracture, bone markers, and body composition including body weight, body mass index (BMI), percentage body fat, and waist circumference were measured. RESULTS: After adjusting for age, smoking status, alcohol consumption, total calcium intake, and total energy intake, waist circumference was negatively related to BMD of all sites (lumbar BMD p = 0.037, all sites of femur BMD p < 0.001) whereas body weight was still positively related to BMD of all sites (p < 0.001). Percentage body fat and waist circumference were much higher in the fracture group than the non-fracture group (p = 0.0383, 0.082 respectively). Serum glucose levels were positively correlated to lumbar BMD (p = 0.016), femoral neck BMD (p = 0.0335), and femoral trochanter BMD (p = 0.0082). Serum high density lipoprotein cholesterol (HDLC) was positively related to femoral trochanter BMD (p = 0.0366) and was lower in the control group than the fracture group (p = 0.011). CONCLUSION: In contrast to the effect favorable body weight on bone mineral density, high percentage body fat and waist circumference are related to low BMD and a vertebral fracture.     Fabrication of Modified MMT/Glass/Vinylester Multiscale Composites and Their Mechanical Properties

    Montmorillonite (MMT) may become a preferred filler material for fiber-reinforced polymer (FRP) composites due to its high aspect ratio, large surface area, and low charge density. In the present paper, MMT/glass/vinylester multiscale composites are prepared with untreated and surface-treated MMT clay particles with an MMT content of 1.0โ€‰wt%. Effects of surface treatment on mechanical properties of MMT/glass/vinylester multiscale composites are investigated through tensile and bending tests, which revealed enhanced mechanical properties in the case of surface-treated MMT. Thermal properties are studied through thermogravimetric analysis (TGA) and dynamic mechanical analysis (DMA). X-Ray diffraction is performed to investigate the interaction between MMT and the matrix. Fourier Transform Infrared (FTIR) is also performed for both untreated and surface-treated MMT. Furthermore, Field Emission-Scanning Electron Microscope (FE-SEM) is conducted to investigate the path of fracture propagation within the composite surface, showing that the surface-treated MMT based multiscale composite has better interactions with the host matrix than the untreated MMT multiscale composites. These composites with enhanced mechanical strength can be used for various mechanical applications

    A comparative study of rubber band ligation versus BANANA-Clip in grade 1 to 3 internal hemorrhoids

    Purpose Rubber band ligation (RBL) for grade 1 to 3 internal hemorrhoids is a well-established modality of choice. But RBL is also a kind of surgical treatment; it is not free from complications (e.g., delayed bleeding [DB], rectal stenosis). This study aimed to investigate the results of the comparative treatment of RBL and BANANA-Clip (BC; Endovision). Methods Study participants were 632 consecutive patients with grade 1 to 3 internal hemorrhoids attended to Department of Colorectal Surgery at Wellness Hospital between January 2010 and May 2019. We retrospectively reviewed the incidence rate of complications, including DB between RBL and BC. Results There were 304 male and 328 female patients, whose ages ranged from 15 to 84 years, with a mean age of 45.7 years. The common symptom and cause of treatment was prolapse (70.1%). The number of ligated sites was 1.49ยฑ0.57 in the RBL group and 1.99ยฑ0.77 in the BC group. RBL showed a significantly higher incidence of DB (3.5%) compared to BC (0%) (P=0.001). The 1-year success rate was 95.9% in the RBL group and 99.7% in the BC group (P=0.005). Conclusion In our study, BC was more reliable in treating grade 1 to 3 internal hemorrhoids with higher success rates and less postligation complications, especially DB, compared to RBL

    A Protein Profile of Visceral Adipose Tissues Linked to Early Pathogenesis of Type 2 Diabetes Mellitus

    Adipose tissue is increasingly recognized as an endocrine organ playing important pathophysiological roles in metabolic abnormalities, such as obesity, cardiovascular disease, and type 2 diabetes mellitus (T2DM). In particular, visceral adipose tissue (VAT), as opposed to subcutaneous adipose tissue, is closely linked to the pathogenesis of insulin resistance and T2DM. Despite the importance of VAT, its molecular signatures related to the pathogenesis of T2DM have not been systematically explored. Here, we present comprehensive proteomic analysis of VATs in drug-naรฏve early T2DM patients and subjects with normal glucose tolerance. A total of 4,707 proteins were identified in LC-MS/MS experiments. Among them, 444 increased in abundance in T2DM and 328 decreased. They are involved in T2DM-related processes including inflammatory responses, peroxisome proliferator-activated receptor signaling, oxidative phosphorylation, fatty acid oxidation, and glucose metabolism. Of these proteins, we selected 11 VAT proteins that can represent alteration in early T2DM patients. Among them, up-regulation of FABP4, C1QA, S100A8, and SORBS1 and down-regulation of ACADL and PLIN4 were confirmed in VAT samples of independent early T2DM patients using Western blot.     Melanocortins induce interleukin 6 gene expression and secretion through melanocortin receptors 2 and 5 in 3T3-L1 adipocytes

    Interleukin 6 (IL6) is a pleiotropic cytokine that not only affects the immune system, but also plays an active role in many physiological events in various organs. Notably, 35% of systemic IL6 originates from adipose tissues under noninflammatory conditions. Here, we describe a previously unknown function of melanocortins in regulating Il6 gene expression and production in 3T3-L1 adipocytes through membrane receptors which are called melanocortin receptors (MCRs). Of the five MCRs that have been cloned, MC2R and MC5R are expressed during adipocyte differentiation. ฮฑ-Melanocyte-stimulating hormone (ฮฑ-MSH) or ACTH treatment of 3T3-L1 adipocytes induces Il6 gene expression and production in a time- and concentration-dependent manner via various signaling pathways including the protein kinase A, p38 mitogen-activated protein kinase, cJun N-terminal kinase, and IฮบB kinase pathways. Specific inhibition of MC2R and MC5R expression with short interfering Mc2r and Mc5r RNAs significantly attenuated the ฮฑ-MSH-induced increase of intracellular cAMP and both the level of Il6 mRNA and secretion of IL6 in 3T3-L1 adipocytes. Finally, when injected into mouse tail vein, ฮฑ-MSH dramatically increased the Il6 transcript levels in epididymal fat pads. These results suggest that ฮฑ-MSH in addition to ACTH may function as a regulator of inflammation by regulating cytokine production

    Femoral geometry, bone mineral density, and the risk of hip fracture in premenopausal women: a case control study

Methods The participants in this caseโ€“control study were 16 premenopausal women with minimal-trauma hip fractures (fracture group) and 80 age-and BMI-adjusted controls. Subjects underwent dual-energy X-ray absorptiometry (DXA) to assess BMD at the proximal femur and to obtain DXA-derived hip geometry measurements. Results The fracture group had a lower mean femoral neck and total hip BMD than the control group (0.721โ€‰ยฑโ€‰0.123 vs. 0.899โ€‰ยฑโ€‰0.115, p <0.001 for the femoral neck BMD and 0.724โ€‰ยฑโ€‰0.120 vs. 0.923โ€‰ยฑโ€‰0.116, p <0.001 for the total hip BMD). In addition, participants in the fracture group had a longer hip axis length (HAL; pโ€‰=โ€‰0.007), narrower neck shaft angle (NSA; pโ€‰=โ€‰0.008), smaller cross sectional area (CSA; pโ€‰<โ€‰0.001) and higher cross sectional moment of inertia (CSMI; pโ€‰=โ€‰0.004) than those in control group. After adjusting for BMD, the fracture group still had a significantly longer mean HAL (pโ€‰=โ€‰0.020) and narrower NSA (pโ€‰=โ€‰0.006) than the control group. Conclusions BMD is an important predictor of hip fracture in premenopausal women. Furthermore, HAL and NSA are BMD-independent predictors of hip fracture in premenopausal women. Hip geometry may be clinically useful for identification of premenopausal women for whom active fracture prevention should be considered

    Mind bomb 1 in the lymphopoietic niches is essential for T and marginal zone B cell development

    Notch signaling regulates lineage decisions at multiple stages of lymphocyte development, and Notch activation requires the endocytosis of Notch ligands in the signal-sending cells. Four E3 ubiquitin ligases, Mind bomb (Mib) 1, Mib2, Neuralized (Neur) 1, and Neur2, regulate the Notch ligands to activate Notch signaling, but their roles in lymphocyte development have not been defined. We show that Mib1 regulates T and marginal zone B (MZB) cell development in the lymphopoietic niches. Inactivation of the Mib1 gene, but not the other E3 ligases, Mib2, Neur1, and Neur2, abrogated T and MZB cell development. Reciprocal bone marrow (BM) transplantation experiments revealed that Mib1 in the thymic and splenic niches is essential for T and MZB cell development. Interestingly, when BM cells from transgenic Notch reporter mice were transplanted into Mib1-null mice, the Notch signaling was abolished in the double-negative thymocytes. In addition, the endocytosis of Dll1 was impaired in the Mib1-null microenvironment. Moreover, the block in T cell development and the failure of Dll1 endocytosis were also observed in coculture system by Mib1 knockdown. Our study reveals that Mib1 is the essential E3 ligase in T and MZB cell development, through the regulation of Notch ligands in the thymic and splenic microenvironments

    Locally-applied 5-fluorouracil-loaded slow-release patch prevents pancreatic cancer growth in an orthotopic mouse model

    To obtain improved efficacy against pancreatic cancer, we investigated the efficacy and safety of a locally-applied 5-fluorouracil (5-FU)-loaded polymeric patch on pancreatic tumors in an orthotopic nude-mouse model. The 5-FU-releasing polymeric patch was produced by 3D printing. After application of the patch, it released the drug slowly for 4 weeks, and suppressed BxPC-3 pancreas cancer growth. Luciferase imaging of BxPC3-Luc cells implanted in the pancreas was performed longitudinally. The drug patch delivered a 30.2 times higher level of 5-FU than an intra-peritoneal (i.p.) bolus injection on day-1. High 5-FU levels were accumulated within one week by the patch. Four groups were compared for efficacy of 5-FU. Drug-free patch as a negative control (Group I); 30% 5-FU-loaded patch (4.8 mg) (Group II); 5-FU i.p. once (4.8 mg) (Group III); 5-FU i.p. once a week (1.2 mg), three times (Group IV). The tumor growth rate was significantly faster in Group I than Group II, III, IV (p=0.047 at day-8, p=0.022 at day-12, p=0.002 at day-18 and p=0.034 at day-21). All mice in Group III died of drug toxicity within two weeks after injection. Group II showed more effective suppression of tumor growth than Group IV (p=0.018 at day-12 and p=0.017 at day-21). Histological analysis showed extensive apoptosis in the TUNEL assay and by Ki -67 staining. Western blotting confirmed strong expression of cleaved caspase-3 in Group II. No significant changes were found hematologically and histologically in the liver, kidney and spleen in Groups I, II, IV but were found in Group III.113Ysciescopu
