    Detection of Fraudulent Health Insurance Claims Based on Decision Tree with Principal Component Analysis

    Fraudulent health insurance claims pose a significant challenge to insurance companies and healthcare providers, leading to substantial financial losses and compromised service quality. In this study, we focused on detecting fraudulent health insurance claims using the decision tree algorithm and principal component analysis (PCA). The objective was to gain valuable insights and extract meaningful patterns from the dataset to enhance fraud detection capabilities. We developed a comprehensive method that employed the decision tree algorithm to build a decision tree-based model and the PCA for dimensionality reduction. By analyzing the data using these algorithms, we were able to capture important patterns and relationships within the dataset. The decision tree algorithm demonstrated reasonable performance, while the PCA exhibited even better results, leveraging the advantage of dimensionality reduction. The findings of this study have significant implications for fraud detection in the healthcare industry. The insights gained from applying the decision tree algorithms and PCA can aid in making informed decisions, identifying trends, and uncovering hidden patterns within the data. Our study recommends implementing advanced fraud detection systems that incorporate these algorithms, continuous monitoring and evaluation, collaboration and data sharing, further research and development, and adherence to regulatory compliance. By following these recommendations, stakeholders in the insurance and healthcare industries can strengthen their fraud detection capabilities, protect their organizations from financial losses, and maintain the integrity of their services. The use of decision tree algorithms and PCA, combined with effective strategies, can significantly contribute to the detection of fraudulent health insurance claims and the overall security and sustainability of the healthcare system

    Factors associated with viral load testing and viral suppression among HIV-positive female sex workers in Nigeria.

    BackgroundFemale sex workers (FSWs) are at high risk for HIV infection and face unique barriers to receiving and adhering to testing and treatment. Early viral suppression and consistent viral load testing are critical to optimizing health and reducing transmission in this population. However, the factors associated with testing and successful viral suppression among FSWs are poorly understood, especially in Sub-Saharan Africa. Our study aimed to examine factors, including social, demographic, and clinical characteristics, associated with viral load testing and suppression among female sex workers initiating antiretroviral therapy in Nigeria.MethodsIn this retrospective study, we analyzed routine programmatic data from FSWs enrolled in the National HIV Key Populations (KP) program in Nigeria. We included FSWs who were newly diagnosed with HIV and registered between January 2016 and January 2022. Primary outcomes of interest were a), receiving a viral load test at any point after treatment initiation and b), viral suppression (FindingsOut of 34,976 FSWs, 97.1% (n = 33,945) received at least one viral load test, with 94.5% (n = 32,092) indicating viral suppression. The odds of receiving at least one viral load test were higher for those who entered treatment in more recent years, those with formal education and those with advanced HIV stages at baseline (adjusted odds ratios [aOR]: 1.17 [1.14-1.19] for those who entered treatment in 2020 vs. 2016; 1.02 [1.01-1.03] for post-secondary vs. no education; and 1.05 [1.01-1.10] for WHO clinical stage 3/4 vs. stage 1 respectively). The odds of successful viral suppression were higher for those who entered treatment in more recent years, but lower for those with advanced HIV stages at baseline (aOR: 1.13 [1.09-1.18] for 2022 vs. 2016; and 0.92 [0.87-0.98] for WHO clinical stage 3/4 vs. stage 1 respectively).ConclusionsThe study underscored the relevance of timely diagnosis and ART initiation for optimal outcomes among HIV-positive FSWs in Nigeria. We also observed significant improvements in the likelihood of early viral load testing and suppression over the study period, reflecting advancements in the KP program. Further research should clarify factors driving these trends to further strengthen the HIV care pipeline for female sex workers

    COVID-19 preventive social-behavioural practices and exposure to SARS-CoV-2 among residents in the city of Yaounde: Lessons from the early phase of the pandemic in Cameroon

    Non-pharmaceutical interventions remain key in mitigating the spread of SARS-CoV-2. We sought to assess COVID-19 preventive, social-behavioural practices, and SARS-CoV-2 exposure through IgG rapid tests. This was a cross-sectional survey among 971 respondents residing in 180 households within the “Cite Verte” health district of Yaounde-Cameroon, from October-November 2020. Using a structured questionnaire, data on SARS-CoV-2 preventive and social behavioural practices were collected, while exposure to SARS-CoV-2 was determined by IgG profiling. p<0.05 was considered statistically significant. Overall, 971 participants were enrolled, among whom 56.5% were females. The age group 15–29 (33.5%) and those with a secondary level of education (44.7%) were most represented. Regarding preventive/social behavioural practices, the least respected measure was "stopped work", 49.1%, while the most respected was "Respect of hygiene rules", 93.8%. Women obeyed preventive measures more than men, with 87.6% vs 81.0% adhering to the lockdown, (p = 0.005) and 95.5% vs 91.7% to hygiene rules (p = 0.017). The age range 45–64 years was the least adherent to the lockdown rule, with 75.2% (38/153), p<0.0001. Only 24.7% (73/295) and 6.1% (59/295) of the symptomatic individuals reported having sought medical consultation and Covid-19 testing respectively. In addition, up to 69.8% (555/795) felt healthcare facilities were high-risk sites for getting infected, p = 0.002. Exposure to SARS-CoV-2 by IgG positivity was 31.1% (302/971), with men recording a higher proportion of viral exposure, 51.0% (154/302), p = 0.021. After adjusting for gender, age, education, and occupation; salaried worker (p = 0.029; OR: 0.29), and trading (p = 0.001; OR: 0.23) least complied with lockdown rule. In this community of Cameroonian residents highly exposed to COVID-19, many perceived healthcare facilities as high-risk zones for SARS-CoV-2 infection and consequently did not seek medical interventions. Thus, in the context of such a pandemic, advocacy on risk communication and community engagement for health-seeking attitudes should preferentially target men and those afraid of pandemics

    Evaluation of a quality improvement intervention to reduce anastomotic leak following right colectomy (EAGLE): pragmatic, batched stepped-wedge, cluster-randomized trial in 64 countries

    Background Anastomotic leak affects 8 per cent of patients after right colectomy with a 10-fold increased risk of postoperative death. The EAGLE study aimed to develop and test whether an international, standardized quality improvement intervention could reduce anastomotic leaks. Methods The internationally intended protocol, iteratively co-developed by a multistage Delphi process, comprised an online educational module introducing risk stratification, an intraoperative checklist, and harmonized surgical techniques. Clusters (hospital teams) were randomized to one of three arms with varied sequences of intervention/data collection by a derived stepped-wedge batch design (at least 18 hospital teams per batch). Patients were blinded to the study allocation. Low- and middle-income country enrolment was encouraged. The primary outcome (assessed by intention to treat) was anastomotic leak rate, and subgroup analyses by module completion (at least 80 per cent of surgeons, high engagement; less than 50 per cent, low engagement) were preplanned. Results A total 355 hospital teams registered, with 332 from 64 countries (39.2 per cent low and middle income) included in the final analysis. The online modules were completed by half of the surgeons (2143 of 4411). The primary analysis included 3039 of the 3268 patients recruited (206 patients had no anastomosis and 23 were lost to follow-up), with anastomotic leaks arising before and after the intervention in 10.1 and 9.6 per cent respectively (adjusted OR 0.87, 95 per cent c.i. 0.59 to 1.30; P = 0.498). The proportion of surgeons completing the educational modules was an influence: the leak rate decreased from 12.2 per cent (61 of 500) before intervention to 5.1 per cent (24 of 473) after intervention in high-engagement centres (adjusted OR 0.36, 0.20 to 0.64; P &lt; 0.001), but this was not observed in low-engagement hospitals (8.3 per cent (59 of 714) and 13.8 per cent (61 of 443) respectively; adjusted OR 2.09, 1.31 to 3.31). Conclusion Completion of globally available digital training by engaged teams can alter anastomotic leak rates. Registration number: NCT04270721 (http://www.clinicaltrials.gov)
