    Characteristics of breast cancer support groups within hospitals in Japan

    施設内の乳がん患者会の有無とその特徴を明らかにする目的で,日本乳癌学会参加564施設(認定347施設,非認定217施設)を対象に郵送質問紙調査を行った.236施設(回収率41.8%)から回答があり,以下の結果を得た.1)乳がん患者会がある施設は,236施設中52施設(22.0%)であった.2)乳がん患者会がある施設の設置主体,規模及び日本乳癌学会認定の有無に違いは見られなかった.しかし,年間乳がん手術件数は,患者会がある施設が有意に多かった.3)患者会は,医療者が発起している会が52施設中22施設(42.3%),患者が発起している会が17施設(32.7%),患者と医療者で発起している会が8施設(15.4%)であった.施設内の患者会は,医療者と患者がコミュニケーションを図る場所,退院直後や外来で活発な治療を受けている時期の患者にとっては利用しやすい身近なサポート資源であると考えられた.4)施設内の患者会は会の内容,運営など,医療者主導の会が多く,セルフヘルプ性は低かった.また,患者の年齢,術後年数など幅があるため,患者個々のニーズには対応できない部分もあり,術後経過と共に支援の内容や必要性が変化してきた際には,施設外の患者会も利用していくシステムも今後必要ではないかと示唆された.The purpose of this study was to determine the number of breast cancer support groups withinhospitals and to describe their characteristics. A questionnaire was mailed to 564 member hospitals of theJapanese Breast Cancer Society (347 accredited, 217 non-accredited). 236 hospitals responded (a responserate of 41.8 percent), and we obtained the following results.1) 52 of 236 hospitals (22.0 percent) had a breast cancer support group.2) No significant differences were found among the hospitals concerning their administrative entity, size,or accredited versus non-accredited status by the Japanese Breast Cancer Society. However, thehospitals with a support group performed a significantly larger number of breast cancer operationsannually than those without a group.3) 22 (42.3 percent), 17 (32.7 percent) and 8 (15.4 percent) of the 52 hospitals had a support groupestablished by physicians, patients, or by both physicians and patients, respectively. A support groupwithin a hospital is considered to provide opportunities for physicians and patients to communicate, aswell as a patient-friendly support resource for those who have just left the hospital and those who areunder intensive treatment on an outpatient basis.4) Many of the support groups were headed by physicians in content and administration and played arather small role as a self-help resource. Additionally, some support groups could not cover patients'individual needs because the ages and postoperative periods of patients varied widely. Therefore, it issuggested that a system be devised where patients can utilize outside support groups when the contentand requirements of needed support change during the postoperative period

    Diffusion kurtosis imaging as a biomarker of breast cancer

    Diffusion kurtosis imaging (DKI) is a diffusion-weighted imaging method that describes non-Gaussian signal behavior using a relatively simple mathematical model. A parameter, kurtosis K, describes the deviation of the diffusion signal decay from a Gaussian pattern. The deviation reflects the complexity of the tissue microstructure affecting water diffusion. Several studies have investigated the diagnostic performance of DKI in distinguishing malignant from benign breast lesions. DKI has been reported to correlate with subtypes and with several molecular and other factors related to the treatment and prognosis of breast cancer. Some technical considerations remain to be resolved for the clinical application of DKI in the breast

    Y-shaped Muscular Wrapping Technique Avoiding Re-infection of a Replaced Aortic Graft: A Cadaveric Study

    Replacing an infected prosthetic thoracic aorta graft carries a high re-infection risk. We previously reported two clinical cases successfully treated with a new muscular wrapping technique: latissimus dorsi (LD) muscle flap with a distally based serratus anterior (SA) extension; however, a cadaveric study to prove the regular existence of the distal attachment area was lacking. We tried to establish an appropriate way of elevating the combined muscle flap safely. All of the cadavers were preserved using the Thiel embalming technique to retain flexibility. We checked for the existence of the distal attachment area between the LD and SA. Combined muscle flaps were elevated proximally while identifying the thoracodorsal artery, including the LD and SA branches. After the SA branch was ligated and cut, the SA muscle was manually peeled from the LD muscle with only the distal tight attachment area remaining. Contrast-enhanced computed tomography was performed using a multislice computed tomography system. Six human cadavers (three men, three women: 91 years old, on average) were examined. All six LD and SA combined muscle flaps showed a distal tight attachment area at the level from the seventh rib to the ninth rib. The tip of the SA muscle easily reached the sternum. Contrast-enhanced computed tomography failed to reconfirm the distal vascular flow from the LD to the reverse SA muscle, which we had visualized in a clinical case. We demonstrated the anatomical reliability of the new Y-shaped muscular flaps, which are suitable for preventing re-infection of aortic graft replacement

    Biomarkers Predictive of Distant Disease-free Survival Derived from Diffusion-weighted Imaging of Breast Cancer

    Purpose: To investigate whether intravoxel incoherent motion (IVIM) and/or non-Gaussian diffusion parameters are associated with distant disease-free survival (DDFS) in patients with invasive breast cancer. Methods: From May 2013 to March 2015, 101 patients (mean age 60.0, range 28-88) with invasive breast cancer were evaluated prospectively. IVIM parameters (flowing blood volume fraction [ɪᴠɪᴍ] and pseudodiffusion coefficient [D*]) and non-Gaussian diffusion parameters (theoretical apparent diffusion coefficient [ADC] at a b value of 0 s/mm² [ADC₀] and kurtosis [K]) were estimated using a diffusion-weighted imaging series of 16 b values up to 2500 s/mm². Shifted ADC values (sADC₂₀₀-₁₅₀₀) and standard ADC values (ADC₀-₈₀₀) were also calculated. The Kaplan-Meier method was used to generate survival analyses for DDFS, which were compared using the log-rank test. Univariable Cox proportional hazards models were used to assess any associations between each parameter and distant metastasis-free survival. Results: The median observation period was 80 months (range, 35-92 months). Among the 101 patients, 12 (11.9%) developed distant metastasis, with a median time to metastasis of 79 months (range, 10-92 months). Kaplan-Meier analysis showed that DDFS was significantly shorter in patients with K > 0.98 than in those with K ≤ 0.98 ( = 0.04). Cox regression analysis showed a marginal statistical association between K and distant metastasis-free survival ( = 0.05). Conclusion: Non-Gaussian diffusion may be associated with prognosis in invasive breast cancer. A higher K may be a marker to help identify patients at an elevated risk of distant metastasis, which could guide subsequent treatment

    Investigation of breast cancer microstructure and microvasculature from time-dependent DWI and CEST in correlation with histological biomarkers

    We investigated the associations of time-dependent DWI, non-Gaussian DWI, and CEST parameters with histological biomarkers in a breast cancer xenograft model. 22 xenograft mice (7 MCF-7 and 15 MDA-MB-231) were scanned at 4 diffusion times [Td = 2.5/5 ms with 11 b-values (0-600 s/mm2) and Td = 9/27.6 ms with 17 b-values (0-3000 s/mm2), respectively]. The apparent diffusion coefficient (ADC) was estimated using 2 b-values in different combinations (ADC0-600 using b = 0 and 600 s/mm2 and shifted ADC [sADC200-1500] using b = 200 and 1500 s/mm2) at each of those diffusion times. Then the change (Δ) in ADC/sADC between diffusion times was evaluated. Non-Gaussian diffusion and intravoxel incoherent motion (IVIM) parameters (ADC0, the virtual ADC at b = 0; K, Kurtosis from non-Gaussian diffusion; f, the IVIM perfusion fraction) were estimated. CEST images were acquired and the amide proton transfer signal intensity (APT SI) were measured. The ΔsADC9-27.6 (between [Formula: see text] and [Formula: see text] and ΔADC2.5_sADC27.6 (between [Formula: see text] and [Formula: see text]) was significantly larger for MCF-7 groups, and ΔADC2.5_sADC27.6 was positively correlated with Ki67max and APT SI. ADC0 decreased significantly in MDA-MB-231 group and K increased significantly with Td in MCF-7 group. APT SI and cellular area had a moderately strong positive correlation in MDA-MB-231 and MCF-7 tumors combined, and there was a positive correlation in MDA-MB-231 tumors. There was a significant negative correlation between APT SI and the Ki-67-positive ratio in MDA-MB-231 tumors and when combined with MCF-7 tumors. The associations of ΔADC2.5_sADC27.6 and API SI with Ki-67 parameters indicate that the Td-dependent DW and CEST parameters are useful to predict the histological markers of breast cancers

    Phase Ib/II study of nivolumab combined with palliative radiation therapy for bone metastasis in patients with HER2-negative metastatic breast cancer

    Radiation therapy (RT) can enhance the abscopal effect of immune checkpoint blockade. This phase I/II study investigated the efficacy and safety of nivolumab plus RT in HER2-negative metastatic breast cancer requiring palliative RT for bone metastases. Cohort A included luminal-like disease, and cohort B included both luminal-like and triple-negative disease refractory to standard systemic therapy. Patients received 8 Gy single fraction RT for bone metastasis on day 0. Nivolumab was administered on day 1 for each 14-day cycle. In cohort A, endocrine therapy was administered. The primary endpoint was the objective response rate (ORR) of the unirradiated lesions. Cohorts A and B consisted of 18 and 10 patients, respectively. The ORR was 11% (90% CI 4–29%) in cohort A and 0% in cohort B. Disease control rates were 39% (90% CI 23–58%) and 0%. Median progression-free survival was 4.1 months (95% CI 2.1–6.1 months) and 2.0 months (95% CI 1.2–3.7 months). One patient in cohort B experienced a grade 3 adverse event. Palliative RT combined with nivolumab was safe and showed modest anti-tumor activity in cohort A. Further investigations to enhance the anti-tumor effect of endocrine therapy combined with RT plus immune checkpoint blockade are warranted

    Grading Meningioma : A Comparative Study of Thallium-SPECT and FDG-PET

    The purpose was to compare capability of fluorine-18 fluorodeoxyglucose (FDG)-PET and thallium-201 (Tl)-SPECT for grading meningioma. This retrospective study was conducted as a case-control study under approval by the institutional review board. In the hospital information system, 67 patients (22 men and 45 women) who had both FDG-PET and Tl-SPECT preoperative examinations were found with histopathologic diagnosis of meningioma. The maximum FDG uptake values of the tumors were measured, and they were standardized to the whole body (SUVmax) and normalized as gray matter ratio (SUVRmax). Mean and maximum Tl uptake ratios (TURmean and TURmax, respectively) of the tumors were measured and normalized as ratios to those of the contralateral normal brain. Receiver-operating characteristic curve analyses of the 4 indexes were conducted for differentiation between low- and high-grade meningiomas, and areas under the curves (AUCs) were compared. Correlation coefficients were calculated between these indexes and Ki-67. Fifty-six meningiomas were classified as grade I (low grade), and 11 were grade II or III (high grade). In all 4 indexes, a significant difference was observed between low- and high-grade meningiomas (P<0.05). AUCs were 0.817 (SUVmax), 0.781 (SUVRmax), 0.810 (TURmean), and 0.831 (TURmax), and no significant difference was observed among the indexes. Their sensitivity and specificity were 72.7% to 90.9% and 71.4% to 87.5%, respectively. Correlation of the 4 indexes to Ki-67 was statistically significant, but coefficients were relatively low (0.273-0.355). Tl-SPECT, which can be used at hospitals without a cyclotron or an FDG distribution network, has high diagnostic capability of meningioma grades comparable to FDG-PET

    Feasibility of Dedicated Breast Positron Emission Tomography Image Denoising Using a Residual Neural Network

    Objective(s): This study aimed to create a deep learning (DL)-based denoising model using a residual neural network (Res-Net) trained to reduce noise in ring-type dedicated breast positron emission tomography (dbPET) images acquired in about half the emission time, and to evaluate the feasibility and the effectiveness of the model in terms of its noise reduction performance and preservation of quantitative values compared to conventional post-image filtering techniques.Methods: Low-count (LC) and full-count (FC) PET images with acquisition durations of 3 and 7 minutes, respectively, were reconstructed. A Res-Net was trained to create a noise reduction model using fifteen patients’ data. The inputs to the network were LC images and its outputs were denoised PET (LC + DL) images, which should resemble FC images. To evaluate the LC + DL images, Gaussian and non-local mean (NLM) filters were applied to the LC images (LC + Gaussian and LC + NLM, respectively). To create reference images, a Gaussian filter was applied to the FC images (FC + Gaussian). The usefulness of our denoising model was objectively and visually evaluated using test data set of thirteen patients. The coefficient of variation (CV) of background fibroglandular tissue or fat tissue were measured to evaluate the performance of the noise reduction. The SUVmax and SUVpeak of lesions were also measured. The agreement of the SUV measurements was evaluated by Bland–Altman plots.Results: The CV of background fibroglandular tissue in the LC + DL images was significantly lower (9.10 2.76) than the CVs in the LC (13.60  3.66) and LC + Gaussian images (11.51  3.56). No significant difference was observed in both SUVmax and SUVpeak of lesions between LC + DL and reference images. For the visual assessment, the smoothness rating for the LC + DL images was significantly better than that for the other images except for the reference images.Conclusion: Our model reduced the noise in dbPET images acquired in about half the emission time while preserving quantitative values of lesions. This study demonstrates that machine learning is feasible and potentially performs better than conventional post-image filtering in dbPET denoising

    The association of C-reactive protein with an oxidative metabolite of LDL and its implication in atherosclerosis

    C-reactive protein (CRP) is one of the strongest independent predictors of cardiovascular disease. We have previously reported that oxidized LDL (oxLDL) interacts with beta 2-glycoprotein I (beta 2GPI), implicating oxLDL/P2GPI complexes as putative autoantigens in autoimmune-mediated atherosclerotic vascular disease. In this study, we investigated the interaction of CRP with oxLDL/beta 2GPI complexes and its association with atherosclerosis in patients with diabetes mellitus (DM). CRP/oxLDL/R2GPI complexes were predominantly found in sera of DM patients with atherosclerosis. In contrast, noncomplexed CRP isoforms were present in sera of patients with acute/chronic inflammation, i.e., various pyrogenic diseases, rheumatoid arthritis (RA), and DM. Immunohistochemistry staining colocalized CRP and beta 2GPI together with oxLDL in carotid artery plaques but not in synovial tissue from RA patients, strongly suggesting that complex formation occurs during the development of adierosclerosis. Serum levels of CRP correlated with soluble forms of intercellular adhesion molecule-1 and vascular cell adhesion molecule-1, and oxLDL/beta 2GPI complexes correlated with total cholesterol and hemoglobin Al c. Thus, the generation of CRP/oxLDL/beta 2GPI complexes seems to be associated with arterial inflammation, hyperglycemia, and hypercholesterolemia. CRP/oxLDL/R2GPI complexes can be distinguished from pyrogenic noncomplexed CRP isoforms and may represent a more specific and predictive marker for atherosclerosis

    Optimization of prediction methods for risk assessment of pathogenic germline variants in the Japanese population

    Predicting pathogenic germline variants (PGVs) in breast cancer patients is important for selecting optimal therapeutics and implementing risk reduction strategies. However, PGV risk factors and the performance of prediction methods in the Japanese population remain unclear. We investigated clinicopathological risk factors using the Tyrer-Cuzick (TC) breast cancer risk evaluation tool to predict BRCA PGVs in unselected Japanese breast cancer patients (n = 1, 995). Eleven breast cancer susceptibility genes were analyzed using target-capture sequencing in a previous study; the PGV prevalence in BRCA1, BRCA2, and PALB2 was 0.75%, 3.1%, and 0.45%, respectively. Significant associations were found between the presence of BRCA PGVs and early disease onset, number of familial cancer cases (up to third-degree relatives), triple-negative breast cancer patients under the age of 60, and ovarian cancer history (all P < .0001). In total, 816 patients (40.9%) satisfied the National Comprehensive Cancer Network (NCCN) guidelines for recommending multigene testing. The sensitivity and specificity of the NCCN criteria for discriminating PGV carriers from noncarriers were 71.3% and 60.7%, respectively. The TC model showed good discrimination for predicting BRCA PGVs (area under the curve, 0.75; 95% confidence interval, 0.69-0.81). Furthermore, use of the TC model with an optimized cutoff of TC score ≥0.16% in addition to the NCCN guidelines improved the predictive efficiency for high-risk groups (sensitivity, 77.2%; specificity, 54.8%; about 11 genes). Given the influence of ethnic differences on prediction, we consider that further studies are warranted to elucidate the role of environmental and genetic factors for realizing precise prediction
