    Recipient iNOS but Not eNOS Deficiency Reduces Luminal Narrowing in Tracheal Allografts

    Chronic airway rejection is characterized by prolonged inflammation, epithelial damage, and eventual luminal obliterative bronchiolitis (OB). In cardiac allografts, the inducible nitric oxide synthase (iNOS) promotes acute rejection but paradoxically reduces neointimal formation, the hallmark of chronic rejection. The specific roles of NOS isoforms in modulating lymphocyte traffic and airway rejection are not known. Using a double lumen mouse tracheal transplant model, tracheal grafts from B10.A (allo) or C57BL/6J (iso) mice were transplanted into cyclosporine-treated wild-type (WT) iNOS−/− or endothelial NOS (eNOS)−/− recipients. OB was observed in WT tracheal allografts at 3 weeks (53 ± 2% luminal occlusion vs. 17 ± 1% for isografts, P < 0.05) with sites of obstructive lesion formation coinciding with areas of CD3+ CD8+ T cell–rich lymphocytic bronchitis. In contrast, allografts in iNOS−/− recipients exhibited reductions in local expression of proinflammatory chemokines and cytokines, graft T cell recruitment and apoptosis, and luminal obliteration (29 ± 2%, P < 0.05 vs. WT allografts). Recipient eNOS deficiency, however, suppressed neither chemokine expression, lymphocyte infiltration, nor airway occlusion (54 ± 2%). These data demonstrate that iNOS exacerbates luminal obliteration of airway allografts in contrast with the known suppression by iNOS of cardiac allograft vasculopathy. Because iNOS−/− airways transplanted into WT allograft hosts are not protected from rejection, these data suggest that iNOS expressed by graft-infiltrating leukocytes exerts the dominant influence on airway rejection

    Searching for Extra Dimensions in the Early Universe

    We investigate extra spatial dimensions (D=3+ϵD = 3+\epsilon) in the early universe using very high resolution molecular rotational spectroscopic data derived from a large molecular cloud containing moderately cold carbon monoxide gas at Z 6.42\approx 6.42. It turns out that the ϵ\epsilon-dependent quantum mechanical wavelength transitions are solvable for a linear molecule and we present the solution here. The CO microwave data allows a very precise determination of =0.00000657±.10003032 = -0.00000657 \pm .10003032. The probability that 0 \neq 0 is one in 7794, only 850 million years (using the standard cosmology) after the Big Bang.Comment: 17 pages, 2 figure

    Perceptions of patients regarding quality nursing care (QNC) at a tertiary hospital in Karachi, Pakistan

    Background: Patients’ experiences regarding the quality of care continue to be a hot topic of discussion in the healthcare industry. Exploring the quality of nursing care from the patients’ perspectives is a vital element of quality evaluation. The purpose of this study was to explore the patients’ perspectives regarding quality nursing care. Methodology: A qualitative descriptive exploratory design was used. A total of twelve participants were recruited from one medical and one surgical unit at a tertiary care hospital in Karachi, Pakistan. Purposive sampling was used to select the participants. Data was collected through semi-structured interviews. A thematic analysis approach was used for data analysis. Results: Data analysis generated three main themes: (1) meaning of QNC, (2) characteristics of nurses, and (3) participants’ recommendations. Conclusion: Nursing care continues to be the foundation of healthcare and QNC could have a great effect on patients’ positive outcome of recovery and well- being.     Campylobacter jejuni PflB is required for motility and colonisation of the chicken gastrointestinal tract.

    Campylobacter jejuni is the leading cause of foodborne bacterial gastroenteritis worldwide. Although the mechanisms by which C. jejuni causes disease are not completely understood, the presence of functional flagella appears to be required for colonisation of the gastrointestinal tract of humans and animals. Therefore much attention has been given to understanding the synthesis and role of flagella in C. jejuni. In this study we report insights into the function of PflB that is essential for Campylobacter motility. We have explored the function of this gene by constructing deletion mutants in C. jejuni strains NCTC11168 and M1, in the genes cj0390 and CJM1_0368, respectively. The mutants were non-motile yet assembled flagella that appeared structurally identical to the wild type.     Case report: a case of intractable Meniere's disease treated with autogenic training

    <p>Abstract</p> <p>Background</p> <p>Psychological stress plays an important role in the onset and course of Meniere's disease. Surgical therapy and intratympanic gentamicin treatment are options for cases that are intractable to conventional medical therapy. Psychotherapy, however, including autogenic training (AT), which can be used for general relaxation, is not widely accepted. This paper describes the successful administration of AT in a subject suffering from intractable Meniere's disease.</p> <p>Case presentation</p> <p>A 51-year-old male patient has suffered from fluctuating right sensorineural hearing loss with vertigo since 1994. In May 2002, he was first admitted to our hospital due to a severe vertigo attack accompanied by right sensorineural hearing loss. Spontaneous nystagmus toward the right side was observed. Since April 2004, he has experienced vertigo spells with right-sided tinnitus a few times per month that are intractable to conventional medical therapy. After four months, tympanic tube insertion was preformed in the right tympanic membrane. Intratympanic injection of dexamethasone was ineffective. He refused Meniett therapy and intratympanic gentamicin injection. In addition to his vertigo spells, he suffered from insomnia, tinnitus, and anxiety. Tranquilizers such as benzodiazepines and antidepressants such as serotonin selective re-uptake inhibitors (SSRIs) failed to stop the vertigo and only slightly improved his insomnia. In December 2006, the patient began psychological counseling with a psychotherapist. After brief psychological counseling along with cognitive behavior therapy (CBT), he began AT. He diligently and regularly continued his AT training in his home according to a written timetable. His insomnia, tinnitus, and vertigo spells disappeared within a few weeks after only four psychotherapy sessions. In order to master the six standard formulas of AT, he underwent two more sessions. Thereafter, he underwent follow-up for 9 months with no additional treatment. He is now free from drugs, including tranquilizers, and has continued AT. No additional treatment was performed. When we examined him <b>six </b>and nine months later for follow-up, he was free of vertigo and insomnia.</p> <p>Conclusion</p> <p>AT together with CBT can be a viable and palatable treatment option for Meniere's disease patients who are not responsive to other therapies.</p

    Geometrically Induced Gauge Structure on Manifolds Embedded in a Higher Dimensional Space

    We explain in a context different from that of Maraner the formalism for describing motion of a particle, under the influence of a confining potential, in a neighbourhood of an n-dimensional curved manifold M^n embedded in a p-dimensional Euclidean space R^p with p >= n+2. The effective Hamiltonian on M^n has a (generally non-Abelian) gauge structure determined by geometry of M^n. Such a gauge term is defined in terms of the vectors normal to M^n, and its connection is called the N-connection. In order to see the global effect of this type of connections, the case of M^1 embedded in R^3 is examined, where the relation of an integral of the gauge potential of the N-connection (i.e., the torsion) along a path in M^1 to the Berry's phase is given through Gauss mapping of the vector tangent to M^1. Through the same mapping in the case of M^1 embedded in R^p, where the normal and the tangent quantities are exchanged, the relation of the N-connection to the induced gauge potential on the (p-1)-dimensional sphere S^{p-1} (p >= 3) found by Ohnuki and Kitakado is concretely established. Further, this latter which has the monopole-like structure is also proved to be gauge-equivalent to the spin-connection of S^{p-1}. Finally, by extending formally the fundamental equations for M^n to infinite dimensional case, the present formalism is applied to the field theory that admits a soliton solution.     Real-time monitor of geomagnetic field in the near-pole regions as an index of magnetospheric electric field

    Solar wind electric field that penetrates into the magnetosphere is an important key for causes of magnetosphenc disturbances. Since PC index, produced from the variation of the magnetic field in the near-pole regions is known to be a good indicator of magnetospheric electric field, monitoring the magnetic activity in the near-pole regions in real-time is useful for nowcasting/forecasting space weather. From the comparison of horizontal components of the magnetic field data between two stations, Eureka and Thule, it is found that the correlation between these two stations are quite high except for the summer months. This result suggests that magnetic field variations in the near-pole region are uniform, and the index can be produced throughout the year using magnetic field data in the northern and southern near-pole region

    Defining the cultural milieu for implementing faculty practice in Pakistan

    Faculty practice can promote a collaborative partnership mutually beneficial to both nursing education and service. However, little is known about its implementation in developing countries. The purpose of this study was to explore the potential for introducing faculty practice within the cultural milieu of Karachi, Pakistan. Focus groups of nursing faculty, staff and students were conducted in various settings: government, semi government and private institutions to elicit the data. Data analysis revealed that a more comprehensive definition of faculty practice is needed that will provide the foundation for a culturally acceptable model of faculty practice in Pakistan. Hierarchy dominates the current cultural milieu and must be addressed before faculty practice can be implemented

    Light Curves and Event Rates of Axion Instability Supernovae

    It was recently proposed that exotic particles can trigger a new stellar instability which is analogous to the e-e+ pair instability if they are produced and reach equilibrium in the stellar plasma. In this study, we construct axion instability supernova (AISN) models caused by the new instability to predict their observational signatures. We focus on heavy axion-like particles (ALPs) with masses of ~400 keV--2 MeV and coupling with photons of g_{ag}~10^{-5} GeV^{-1}. It is found that the 56Ni mass and the explosion energy are significantly increased by ALPs for a fixed stellar mass. As a result, the peak times of the light curves of AISNe occur earlier than those of standard pair-instability supernovae by 10--20 days when the ALP mass is equal to the electron mass. Also, the event rate of AISNe is 1.7--2.6 times higher than that of pair-instability supernovae, depending on the high mass cutoff of the initial mass function.Comment: 9 pages, 6 figures, 1 table, submitted to Ap
