    Development and application of a real-time quantitative PCR assay for determining expression of benzo-apyrene- Inducible cytochrome P450 1A in Nile tilapia (Oreochromis niloticus)

    Cytochrome P4501A’s (CYP1A) constitute a ubiquitous family of proteins associated with the detoxification of organic compounds such as PCB (polychlorinated biphenyl), PAH (polyaromatic hydrocarbons) and dioxin. These compounds are documented to induce the CYP1A gene in a variety of tissues of many fish species. Consequently, changes in CYP1A gene expression have been used as a biomarker for contaminant exposure in fish populations using a variety of techniques. Of all of thesemethods, quantitative PCR appears to be the most sensitive. It has been used to assess impact of environmental pollution in marine ecosystems using different fish models. Subsequently, for measuring benzo-a-pyrene (BaP) induction of CYP1A mRNA in different organs of tilapia (Oreochromis niloticus), ribosomal protein large P0-like protein (RPLP0-like protein) and -actin genes as internal controls were selected based on previous studies to assess their expression variability. Real-time polymerase chainreaction (real-time PCR) analysis of liver, intestine, gills and kidney revealed a distinct induced expression in liver and intestine (127.1 and 79.3 in liver, 26 and 56.1 in intestine using RPLP0 and -actin genes respectively as internal controls) with no detectable expression in the other organs studied

    Cloning and sequence analysis of benzo-a-pyreneinducible cytochrome P450 1A in Nile tilapia (Oreochromis niloticus)

    Polycyclic aromatic hydrocarbons (PAHs), dioxins, dibenzofurans and polychlorinated biphenyls (PCBs) present in polluted environment induce cytochrome P4501A (CYP1A) isozyme in fish which inturn results in a marked increased production of carcinogenic metabolites. The induction of hepatic CYP1A in fish by certain classes of chemicals has been suggested as an early warning system, a “mostsensitive biological response” for assessing environmental contamination conditions. This has implications for human fish consumption as well as for the health status of aquatic organisms.Considering the importance of Oreochromis niloticus fish as a laboratory animal, the common CYP1A sequence was determined from cDNA and genomic DNA after intraperitoneal injection with benzo-apyrene (BaP). The full-length cDNA was 2530 bp long and contained an open reading frame of 1566 bp encoding a protein of 521 amino acids and a stop codon. The sequence exhibited 5' and 3' noncodingregions of 134 and 830 bp, respectively. The deduced amino acid sequence of O. niloticus CYP1A shows similarities of 80.5, 79.3, 79.1, 77.8, 77.6, 74.3, 72.4, 77.2, 71.8, 70.7 and 50.8% with Europeanflounder CYP1A, scup CYP1A, killifish CYP1A, butterfly fish CYP1A, European sea bass CYP1A, rainbow trout CYP1A, Japanese eel CYP1A, toad fish CYP1A, European eel CYP1A, red sea bream CYP1A and common carp CYP1A, respectively. The phylogenetic tree based on the amino acid sequences clearly shows tilapia CYP1A and killifish CYP1A to be more closely related to each other than to the other CYP1A subfamilies. Sequence analysis of 3727 bp of genomic DNA showed that the clone obtained was the structural gene of CYP1A which consists of seven exons and six introns, the initiation codon was not found in the first exon but in the second one as was reported for the CYP1A genes of fish and mammals

    Gastric cancer treatment in Japan: 2008 annual report of the JGCA nationwide registry

    The Japanese Gastric Cancer Association (JGCA) started a new nationwide gastric cancer registry in 2008. Approximately 50 data items, including surgical procedures, pathological diagnoses, and survival outcomes, for 12004 patients with primary gastric cancer treated in 2001 were collected retrospectively from 187 participating hospitals. Data were entered into the JGCA database according to the JGCA Classification of gastric carcinoma, 13th edition and the International Union Against Cancer (UICC) TNM Classification of malignant tumors, 5th edition by using an electronic data collecting system. Finally, data of 11261 patients with gastric resection were analyzed. The 5-year follow-up rate was 83.5%. The direct death rate was 0.6%. TNM 5-year survival rates (5YSRs)/JGCA 5YSRs were 91.8/91.9% for stage IA, 84.6/85.1% for stage IB, 70.5/73.1% for stage II, 46.6/51.0% for stage IIIA, 29.9/33.4% for stage IIIB, and 16.6/15.8% for stage IV. The proportion of patients more than 80 years old was 7.0%, and their 5YSR was 48.7%. Compared to the JGCA archived data, though the follow-up rate needs to be improved, these data suggest that the postoperative results of patients with primary gastric carcinoma have improved in those with advanced disease and in the aged population in Japan

    Clinical experience with the Bicarbon heart valve prosthesis

    BACGROUND: We have previously reported mid-term results of a study, which ended in January 2000, on the Bicarbon valve. The study concluded that the valve showed excellent clinical results, associated with a low incidence of valve-related complications. In the present study, the same patients were prospectively followed for an additional 5 years. METHODS: Forty-four patients had aortic valve replacement (AVR), 48 had mitral valve replacement (MVR), and 13 had both aortic and mitral valve replacement (DVR). The mean age of the 105 patients was 61.2 ± 11.3 years. The mean follow-up was 6.1 ± 1.9 years with a cumulative follow-up of 616 patient-years. RESULTS: There were 5 early deaths (4.7%: 4 in the AVR group and 1 in the MVR group) and 21 late deaths (3.4%/patient-year: 5 valve related deaths and 16 valve unrelated deaths). Survival at 8 years was 75.2 ± 7.0% in the AVR group, 76.6 ± 6.2% in the MVR group, and 55.4 ± 16.1% in the DVR group. The linearized incidence of thrombo-embolic complications, hemorrhagic complications, and paravalvular leaks in all patients was 0.65 ± 1.48%, 0.81 ± 1.69%, and 0.16 ± 0.54%/patient-year respectively. No other complications were observed. CONCLUSION: The Bicarbon prosthetic heart valve has shown excellent long-term clinical results, associated with a low incidence of valve-related complications

    Frequent loss of RUNX3 gene expression in remnant stomach cancer and adjacent mucosa with special reference to topography

    Our previous studies suggest that a lack of RUNX3 function is causally related to the genesis and progression of human gastric cancer. This study was conducted to determine whether alteration of RUNX3 gene expression could be detected in the normal-looking gastric remnant mucosa, and to ascertain any difference in the potential of gastric carcinogenesis between the anastomotic site and other areas in the remnant stomach after distal gastrectomy for peptic ulcer (RB group) or gastric cancer (RM group), by analysing RUNX3 expression with special reference to topography. A total of 89 patients underwent distal gastrectomy for gastric cancer from the intact stomach (GCI group) and 58 patients underwent resection of the remnant stomach for gastric cancer (RB group: 34 cases, RM group: 24 cases). We detected RUNX3 and gene promoter methylation by in situ hybridisation, quantitative reverse transcriptase–polymerase chain reaction (RT–PCR), and methylation-specific PCR. The interval between the initial surgery and surgery for remnant gastric cancer (interval time) was 10.4 years in the RM group, and 27.5 years in the RB group. Cancers in the RB group were significantly more predominant in the anastomosis area (P<0.05). Within the tumour, downregulation of RUNX3 expression ranged from 74.7 to 85.7% in the three groups. The rate of downregulation of RUNX3 of adjacent mucosa was 39.2% (11 in 28 cases) in RB and 47.6% (10 in 21 cases) in RM, which are significantly higher than that of the GCI group (19.5%, 17 in 87 cases). In noncancerous mucosa of the remnant stomach in the RB group, RUNX3 expression decreased more near the anastomosis area. In the RM group, however, there were no significant differences in RUNX3 expression by sampling location. Based on RUNX3 downregulation and clinical features, residual stomach mucosa of the RM group would have a higher potential of gastric carcinogenesis compared to the RB or GCI group. Gastric stump mucosa of the RB group has higher potential especially than other areas of residual stomach mucosa. Measurement of RUNX3 expression and detection of RUNX3 methylation in remnant gastric mucosa may estimate the forward risk of carcinogenesis in the remnant stomach

    Upper abdominal body shape is the risk factor for postoperative pancreatic fistula after splenectomy for advanced gastric cancer: A retrospective study

    <p>Abstract</p> <p>Background</p> <p>Postoperative pancreas fistula (POPF) is a major complication after total gastrectomy with splenectomy. We retrospectively studied the effects of upper abdominal shape on the development of POPF after gastrectomy.</p> <p>Methods</p> <p>Fifty patients who underwent total gastrectomy with splenectomy were studied. The maximum vertical distance measured by computed tomography (CT) between the anterior abdominal skin and the back skin (U-APD) and the maximum horizontal distance of a plane at a right angle to U-APD (U-TD) were measured at the umbilicus. The distance between the anterior abdominal skin and the root of the celiac artery (CAD) and the distance of a horizontal plane at a right angle to CAD (CATD) were measured at the root of the celiac artery. The CA depth ratio (CAD/CATD) was calculated.</p> <p>Results</p> <p>POPF occurred in 7 patients (14.0%) and was associated with a higher BMI, longer CAD, and higher CA depth ratio. However, CATD, U-APD, and U-TD did not differ significantly between patients with and those without POPF. Logistic-regression analysis revealed that a high BMI (≥25) and a high CA depth ratio (≥0.370) independently predicted the occurrence of POPF (odds ratio = 19.007, p = 0.002; odds ratio = 13.656, p = 0.038, respectively).</p> <p>Conclusion</p> <p>Surgical procedures such as total gastrectomy with splenectomy should be very carefully executed in obese patients or patients with a deep abdominal cavity to decrease the risk of postoperative pancreatic fistula. BMI and body shape can predict the risk of POPF simply by CT.</p
