    Local Glucocorticoids in Treatment of Children with Dermatitis

    Local glucocorticoids (GCs) are the gold standard of anti-inflammatory topical therapy. The description of the main pharmaceutics properties and classification of local GCs, as well as its indications and contradictions are presented in the article. This article also provides fundamentals for application of local GCs which should be implemented for effective and safe use of these drugs in children. The problems of steroid-phobia (one of the reasons of the wrong and untimely administration of local GCs) are discussed. Advantages of 0.1% methylprednisolone aceponate (new generation GCs) use are discussed. This medication has optimal efficiency/safety ratio and unique bioactivation pathway that enables its effective administration in management of various dermatitis in children from 4 months of age. Results of different comparative and double blind placebo-controlled clinical trials of 0.1% methylprednisolone aceponate administration are discussed in this article. High efficiency and safety of this medication are noted. The use of modern local GCs at the first signs of skin inflammatory changes allows to relieve aggravation quickly and achieve remission of the disease

    Awareness and Competence of Pediatricians on Child Abuse: Preliminary Results of the International Cross-Sectional Study

    Background. Many social problems have worsened nowadays especially in the context of the COVID-19 pandemic. It is important that child health professionals have all the necessary knowledge for detection of child abuse in modern society. Aim of the study is to study the awareness and competence of pediatricians on child abuse. Methods. The cross-sectional study was conducted using the international standardized questionnaire (22 questions). The study has included practicing pediatricians from inpatient and outpatient (95% of all respondents) clinics in Kazan. Results. The data from questionnaire survey of 73 pediatricians has shown that 29 (40%) and 39 (53%) of all respondents encountered physical and psychological abuse against children, respectively. Physical abuse was considered as the most common form of abuse by 38 (52%) pediatricians. The child abuse was revealed during routine examination in 33 (45%) cases. 20 (28%) respondents have initiated proceedings for every case of physical abuse, and 23 (31%) — for psychological abuse. One in three pediatricians, 23 (31%), consider the present legislation comprehensive enough in the sphere of children protection against abuse. Meanwhile, 50 (68%) pediatricians regarded their own level of awareness in this topic as insufficient. Conclusion. The study has shown insufficient competence of pediatricians (especially in the visiting services) in the detection and maintenance of children abuse cases (physical and psychological). It is crucial to enhance and continuously improve education of pediatricians on pre-graduate and continuing professional education levels in issues of children abuse

В современном обществе, особенно в условиях пандемии COVID-19, обострились многие социальные проблемы. В этой связи важно, чтобы специалисты детского здравоохранения имели необходимые знания для выявления распространившегося насилия над детьми. Цель исследования — изучить осведомленность и уровень компетенций педиатров первичного звена г. Казани по вопросам жестокого и ненадлежащего обращения с детьми. Методы. Проведено одномоментное исследование с использованием международной стандартизированной анкеты (22 вопроса). В исследование включали практикующих педиатров стационарных и амбулаторных (95% всех опрошенных) служб г. Казани. Результаты. Анкетирование 73 педиатров показало, что с физическим и психологическим видами насилия над детьми встречались 29 (40%) и 39 (53%) опрошенных соответственно. Физическое насилие считали самой распространенной формой жестокого обращения 38 (52%) педиатров. Жестокое/ненадлежащее обращение с ребенком в 33 (45%) случаях педиатры выявляли в рамках рутинного осмотра. Инициировали разбирательство по каждому случаю физического насилия 20 (28%) опрошенных, по случаям психологического насилия — 23 (31%) респондентa. Каждый третий педиатр — 23 (31%) — считает, что существующее законодательство исчерпывающим образом обеспечивает защиту детей, пострадавших от жестокого/ненадлежащего обращения. Вместе с тем 50 (68%) педиатров расценили собственный уровень осведомленности в обсуждаемой теме как недостаточный. Заключение. Проведенное исследование продемонстрировало недостаточную компетентность педиатров, прежде всего участковой службы, в вопросах выявления и сопровождения случаев насилия (физического и психологического) над детьми. Необходимо активизировать и непрерывно совершенствовать обучение на додипломном этапе и в процессе дополнительного профессионального образования педиатров по проблемам жестокого/ненадлежащего обращения с детьми
