    Abnormal ossified structures around the hip joint and its clinical implications

    Background: The hip joint is the body’s second largest weight-bearing joint forms a connection from the lower limb to the pelvic girdle. It is formed by an articulation between the pelvic acetabulum and the head of the femur. Ankylosis or fusion of the joint, ossification of the adjacent ligaments and calcific tendinitis of adjacent muscles can decrease the mobility of the joint. The study was undertaken to evaluate the incidence of abnormal ossified structures around the hip joint.Methods: This study was carried out on 228 dry human hip bones (right- 114 and left-114) and 228 dry human femur bones (right- 114 and left-114) irrespective of age and sex at Varun Arjun medical college-Banthra, UP, KMCT Medical College, Manassery-Calicut and Melaka Manipal Medical College-Manipal. All the hip and femur bones were macroscopically inspected for the abnormal ossified structures around the pelvic acetabulum and upper end of the femur. Photographs of the abnormal ossified structures were taken for proper documentation.Results: Very rare and unusual unilateral ossified and complete fused left hip joint was noted (0.43%). Unilateral ossified acetabular labrum with ossified transverse acetabular ligament was noted in seven right hip bones (3.07%). Unusual ossified fibrous capsule on the posterior aspect of neck of femur was noted in 4 left sided femur bones (1.75%). Unusual unilateral ossified deposits near the greater trochanter and intertrochanteric line of femur was noted in nine left sided femur bones (3.94%).Conclusions: Anatomical knowledge of ankylosis of hip joint, ossification of the ligaments and muscles tendons around the hip joint as found in the present study made this study unique such abnormal ossifications may be helpful for clinicians, radiologists and surgeons for differential diagnosis and can be implicated in the development of innovative treatments of hip joint and hip pains

    Anatomical and congenital variations of human dry sternum bone: its embryogenesis and clinical implications

    Background: The sternum is one of the skeleton parts with frequently detected variation in cross-sectional images or autopsy series. The anatomical or congenital variations of the sternum in the anterior chest wall may involve malignancies, injuries or severe traumas. The aim of the study was undertaken to evaluate the incidence of anatomical and congenital variations of human dry sternum bones.Methods: This study was carried out on 120 dry human sternum bones irrespective of age and sex at Varun Arjun medical college- Banthra, UP, KMCT Medical College, Manassery-Calicut and Melaka Manipal Medical College-Manipal. All the sternum bones were macroscopically inspected for the anatomical and congenital variations of human dry sternum bones. Photographs of the anatomical and congenital variations were taken for proper documentation.Results: Complete sternal foramina in the body of the sternum were noted in 9 bones (7.5%), with an average vertical diameter of 17mm and transverse diameter of 16mm (The highest vertical diameter of 19mm and transverse diameter of 17mm was noted); Incomplete sternal foramina in the body of the sternum were noted in 4 bones (3.3%);Complete sternal foramina in the xiphoid process of the sternum were noted in 7 bones (5.8%) with an average vertical diameter of 6mm and transverse diameter of 8mm; Unusual complete sternal foramina in the body and incomplete sternal foramina in the xiphoid process of the sternum were noted in 8 bones (6.6%); Very rare longer xiphoid process (7.3 cm) with complete sternal foramina was noted in 7 bone (5.8%); Unusual Longer xiphoid process with an average length of 6.7cm with sharp bifid ends was noted in 8 bones (6.6%).Conclusions: The knowledge of existence of anatomical variants and congenital foramina of sternum and xiphoid process found in our study is essential, especially for bone marrow sampling, radiology (X - ray, CT, MRI, and USG) reporting, pathology autopsy and forensic medicine post-mortem reporting and patoacupuncture practice to avoid complications during various surgical procedures

    Lymphomatoid granulomatosis masquerading as interstitial pneumonia in a 66-year-old man: a case report and review of literature

    Abstract Lymphomatoid granulomatosis (LG) is a rare, Epstein-Barr virus (EBV)-associated systemic angiodestructive lymphoproliferative disorder that may progress to a diffuse large B cell lymphoma. Pulmonary involvement may mimic other more common lung pathologies including pneumonias. Therapeutic standards have not been established for LG, but rituximab, interferon-α2b (INF-α2b), and chemotherapy have shown to improve symptoms and long term prognosis. We report a case of rapid respiratory deterioration in a 66-year-old man with clinical presentation, chest radiography, pulmonary function testing and high resolution computed tomography (HRCT) findings consistent with idiopathic interstitial pneumonia, but very poor response to antibiotics and low dose steroids. Lung biopsy showed histopathology consistent with LG that was confirmed by a positive in situ hybridization for Epstein - Barr virus encoded RNA (EBER). The patient was treated with rituximab and combination chemotherapy and showed significant initial clinical improvement with gradual resolution of abnormal findings on imaging. However, the patient developed pancytopenia as a complication of chemotherapy and died secondary to septic shock and renal failure that were refractory to medical management. Autopsy showed diffuse alveolar damage but no evidence of any residual LG within the lungs. This case demonstrates that an open lung biopsy or video-assisted thoracoscopic surgical (VATS) biopsy is often necessary to rule out the presence of LG in order to determine the appropriate therapeutic strategy early in the course of illness to improve prognosis

    Globalization and innovation are revolutionizing the higher education forcing to create new market trends.Different nations have their own pattern and framework of education in delivering the educational services.Educational institutions are also seeking different organizational and behavioural changes for their better future as they hunt for new financial resources, face new competition and seek greater prestige domestically and internationally. The coming future will decide which universities would survive the market trends, competition and expectations of the students (Clients). The survival-of-the-fittest paradigm framework plays a prominent role in ideas of how the higher education would be delivered to the students in future with the Instruction Technology and distance education. According to us the education trend has changed its phase of delivery of services form the management point of view to student’s point of view.Leading to delivery of educational service’s which would have more impact on student’s education, knowledge and experience within the institution. In our thesis we try to provide some information about how to support and manage the information in Virtual Organizations. We also explore the frameworks of the university and discussed a case study about the different ways of providing better support for information management resulting in delivery of best students driven services and unique facilities. We would be looking at the different aspects of the university work flows and procedures and gain an insight on the student’s expectation from the organization. This investigation would be helpful for the students to know what are the services they should expect from the universities and also helpful for management to know better the needs of the students and their needs and to develop a framework for proper execution of these services.In an educational environment students should be the first priority. Trusting students is we feel the most important aspect of an educational institution also providing the best possible support to students.after being through the total framework of an organization and different Non- Functional requirements, we conclude that the primary task for better information management and service delivery in any organization is to get the better understanding of the requirements engineering. An organization should have clearly defined stakeholders who would be qualified and filled with commitment to manage their responsibilities with higher priority; also they have to take up the responsibilities for the students.So to achieve a balance among stakeholders there should be a proper negotiation and communication between the stakeholders. In order to achieve that there should be common set of defined rules (say services) through which they could correspond with each other. The stakeholder should be well equipped with the technical nature used to develop and maintain the system.In short the management is the key player in understanding and validating the system and determine whether or not the existing system/process is in control. The most powerful designs are always the result of a continuous process of simplification and refinement. The system could be validated by taking regular feedback from students by handing out a paper for evaluation at various occasions of the different courses. An online, end-of-course plus the experiences of students questions should be designed and should be made a part of students work to fill it in order to get their right perception on the system. These feedbacks could lead to proper student dialogue evaluation. Therefore, eventually controlling and smoothly managing the stakeholders and the services could provide efficient support for the information management in virtual organizations.BTH has the staffs who are concerned about student problems but there is no person responsible for the students whom the students could approach. So BTH should have defined stakeholders who are responsible for the students and to whom the students could approach with their difficulties and problems.
