Seroprevalence of 34 Human Papillomavirus Types in the German General Population
The natural history of infections with many human papillomavirus (HPV) types is poorly understood. Here, we describe for the first time the age- and sex-dependent antibody prevalence for 29 cutaneous and five mucosal HPV types from 15 species within five phylogenetic genera (alpha, beta, gamma, mu, nu) in a general population. Sera from 1,797 German adults and children (758 males and 1,039 females) between 1 and 82 years (median 37 years) were analysed for antibodies to the major capsid protein L1 by Luminex-based multiplex serology. The first substantial HPV antibody reactions observed already in children and young adults are those to cutaneous types of the genera nu (HPV 41) and mu (HPV 1, 63). The antibody prevalence to mucosal high-risk types, most prominently HPV 16, was elevated after puberty in women but not in men and peaked between 25 and 34 years. Antibodies to beta and gamma papillomaviruses (PV) were rare in children and increased homogeneously with age, with prevalence peaks at 40 and 60 years in women and 50 and 70 years in men. Antibodies to cutaneous alpha PV showed a heterogeneous age distribution. In summary, these data suggest three major seroprevalence patterns for HPV of phylogenetically distinct genera: antibodies to mu and nu skin PV appear early in life, those to mucosal alpha PV in women after puberty, and antibodies to beta as well as to gamma skin PV accumulate later in life
Factors affecting efficiency of cotton producers in rural Khorezm, Uzbekistan: Re-examining the role of knowledge indicators in technical efficiency improvement
Few efficiency studies, to date, have been conducted for Central Asian countries. This article fills the gap in the literature by conducting a frontier efficiency analysis using survey data obtained from the cotton producing farms in North-western Uzbekistan. The results of the restricted stochastic frontier model, which follows a three-stage procedure, demonstrate the presence of technical inefficiency in cotton production. The findings illustrate that the results from the traditional frontier models, which use black box tools, lead to biased outcomes. The model displays that farmers' educational background, farm size, water availability, the application of manure, access to formal credit, Water User Association's services, farmers' participation in off-farm work and poor drainage systems, significantly contribute to input use efficiency. A quantile regression also shows that knowledge indicators play a significant role in improving farmers' efficiency in cotton production. The impact of agricultural experience on technical efficiency is positive, but not significant, in the middle and higher efficiency percentiles. Interestingly, having a basic education is not sufficient in achieving higher efficiency, based on the results obtained. The findings suggest that the provision of agricultural training and the development of agricultural extension services will help farmers acquire new technologies and enhance their decision-making capabilities in farm production which subsequently improve resource use efficiency in cotton production
Trauma and mental health of medics in eastern Myanmar’s conflict zones: a cross-sectional and mixed methods investigation
BACKGROUND: In conflict and disaster settings, medical personnel are exposed to psychological stressors that threaten their wellbeing and increase their risk of developing burnout, depression, anxiety, and PTSD. As lay medics frequently function as the primary health providers in these situations, their mental health is crucial to the delivery of services to afflicted populations. This study examines a population of community health workers in Karen State, eastern Myanmar to explore the manifestations of health providers’ psychological distress in a low-resource conflict environment. METHODS: Mental health screening surveys were administered to 74 medics, incorporating the 12-item general health questionnaire (GHQ-12) and the posttraumatic checklist for civilians (PCL-C). Semi-structured qualitative interviews were conducted with 30 medics to investigate local idioms of distress, sources of distress, and the support and management of medics’ stressors. RESULTS: The GHQ-12 mean was 10.7 (SD 5.0, range 0–23) and PCL-C mean was 36.2 (SD 9.7, range 17–69). There was fair internal consistency for the GHQ-12 and PCL-C (Cronbach’s alpha coeffecients 0.74 and 0.80, respectively) and significant correlation between the two scales (Pearson’s R-correlation 0.47, P<0.001). Qualitative results revealed abundant evidence of stressors, including perceived inadequacy of skills, transportation barriers, lack of medical resources, isolation from family communities, threats of military violence including landmine injury, and early life trauma resulting from conflict and displacement. Medics also discussed mechanisms to manage stressors, including peer support, group-based and individual forms of coping. CONCLUSIONS: The results suggest significant sources and manifestations of mental distress among this under-studied population. The discrepancy between qualitative evidence of abundant stressors and the comparatively low symptom scores may suggest marked mental resilience among subjects. Alternatively, the discrepancy may reflect the inadequacy of standard screening tools not validated for this population and potential cultural inappropriateness of established diagnostic frameworks.
