    This chapter discusses the accreditation requirements of mediators with reference to selected jurisdiction. The reasons and the benefits for accreditation, who should regulate and the qualifications required for accreditation among others are also addressed in this chapter. At this juncture, it is worth noting that the Mediation Act 2012 provides that only a person who possesses the relevant qualifications, knowledge or experience in mediation through training or formal tertiary education or satisfies the requirements of an organisation which provides mediation services, can be appointed as a mediator under the Act

    Ethnic differences in cardiovascular risk in rheumatic disease: Focus on Asians

    Rheumatic diseases are associated with high cardiovascular morbidity and mortality. Considerable differences exist in the frequency of cardiovascular disease (CVD) risk factors and events among people of different ethnic origins, but little is known of the ethnic variations in the relative distribution of CVD risk factors and the degree of atherosclerosis in patients with rheumatic diseases. Understanding this variation will provide insight into the underlying pathogenesis of CVD in patients with rheumatic diseases, and aid in future studies of the detection and management of this complication. In general, although Asian patients seem to have fewer background CVD risk factors and are less affected by metabolic syndrome (MetS) than their non-Asian counterparts, those with rheumatic disease are equally as susceptible to CVD. Furthermore, it seems that systemic inflammation and mechanisms that do not involve conventional CVD risk factors and MetS have an important role in the development of atherosclerosis in patients with rheumatic diseases. Here we examine the frequency of conventional CVD risk factors and the prevalence of MetS in both Asian and non-Asian patients with selected rheumatic diseases. We also discuss the burden of CVD, as evaluated using various surrogate markers in these patients, and their overall CVD mortality rate. ยฉ 2011 Macmillan Publishers Limited.     Flexible fibreoptic bronchoscope has been employed in the examination of the nasopharynx, pharynx, larynx, trachea, bronchus and intrathoracic oesophagus under local anaesthesia. It is used either for diagnostic or therapeutic purposes. A total of 1690 procedures have been performed with no complication. The indications and findings in these procedures are listed. The advantages and limitations of the use of flexible fibreoptic bronchoscope as a 'panendoscopic examination' instrument are discussed.published_or_final_versio

    Incremental predictive value of vascular assessments combined with the Framingham Risk Score for prediction of coronary events in subjects of low-intermediate risk

    Background: In patients with low-intermediate risk, the use of the Framingham Risk Score (FRS) may not allow accurate prediction of the occurrence of coronary events. Objective: To determine whether non-invasive vascular sonographic assessments add value to the FRS for prediction of coronary events. Methods: Brachial artery flow-mediated dilatation (FMD), carotid intima-media thickness (IMT) and the presence of carotid plaque in 70 male subjects (mean (SD) age 62 (9) years) with a low-intermediate FRS who presented with a recent coronary event were evaluated and compared with those in 35 male controls matched for age (mean age 60 (9) years). Results: Patients with a recent coronary event had a significantly higher FRS than controls. They had a significantly lower FMD (3.56 (2.41)% vs 5.18 (2.69)%, p = 0.003) and significantly higher prevalence of carotid plaque (67% vs 40%, p = 0.008), but there was no significant difference in mean maximum IMT between the two groups (1.01 (0.28) vs 0.96 (0.14) mm, p = 0.32). Multivariate analysis revealed that FMD โ‰ค 4.75% was an independent predictor of an acute coronary event. Of the three vascular markers, FMD โ‰ค 4.75% and presence of carotid plaque provided the best diagnostic accuracy for a coronary event, with area under the curve (AUC) of 0.70 and 0.64 (p = 0.001 and p = 0.033), respectively, based on receiver operating characteristic curve analysis. Furthermore, incorporating carotid plaque or FMD โ‰ค 4.75% into the FRS (AUC = 0.72 and AUC = 0.78) provided incremental benefit in risk stratification over FRS alone (AUC = 0.66) (p = 0.008 and p = 0.007, for comparison of difference in two receiver operating characteristic curves). Conclusions: Incorporating a measure of FMD or carotid plaque burden with FRS significantly increases the accuracy of predicting coronary events in subjects of low-intermediate risk and hence should be considered as additional investigations to improve coronary risk assessment.published_or_final_versio

    Optimal designs for an additive quadratic mixture model involving the amount of mixture

    This paper is concerned with D- and A-optimal designs for a quadratic additive model for experiments with mixtures, in which the response depends not only on the relative proportions but also on the actual amounts of the mixture components. It is found that the origin and vertices of the simplex are support points of these optimal designs, and when the number of mixture components increases, other support points shift gradually from barycentres of depth 1 to barycentres of higher depths. It is shown that the D-optimal designs have high efficiency in terms of A-optimality, and vice versa.published_or_final_versio

    Influenza-related hospitalizations among children in Hong Kong

    Background: It has been difficult to define the burden of influenza in children because of confounding by the cocirculation of respiratory syncytial virus (RSV). In Hong Kong, China, the influenza and RSV infection seasons sometimes do not overlap, thus providing an opportunity to estimate the rate of influenza-related hospitalization in a defined population, free from the effects of RSV. Methods: In a retrospective, population-based study, we estimated the influenza-associated excess rate of hospitalization among children 15 years old or younger in the Hong Kong Special Administrative Region from 1997 to 1999. Data from a single hospital with intensive use of virologic analyses for diagnosis were obtained to define and adjust for underestimation of the model. Results: Peaks of influenza and RSV infection activity were well separated in 1998 and 1999 but overlapped in 1997. The adjusted rates of excess hospitalization for acute respiratory disease that were attributable to influenza were 278.5 and 288.2 per 10,000 children less than 1 year of age in 1998 and 1999, respectively; 218.4 and 209.3 per 10,000 children 1 to less than 2 years of age; 125.6 and 77.3 per 10,000 children 2 to less than 5 years of age; 57.3 and 20.9 per 10,000 children 5 to less than 10 years of age; and 16.4 and 8.1 per 10,000 children 10 to 15 years of age. Conclusions: In the subtropics, influenza is an important cause of hospitalization among children, with rates exceeding those reported for temperate regions.     Predictive value of high-sensitivity troponin-I for future adverse cardiovascular outcome in stable patients with type 2 diabetes mellitus

    INTRODUCTION: High-sensitivity cardiac troponin I(hs-TnI) and T levels(hs-TnT) are sensitive biomarkers of cardiomyocyte turnover or necrosis. Prior studies of the predictive role of hs-TnT in type 2 diabetes mellitus(T2DM) patients have yielded conflicting results. This study aimed to determine whether hs-TnI, which is detectable in a higher proportion of normal subjects than hsTnT, is associated with a major adverse cardiovascular event(MACE) in T2DM patients. METHODS AND RESULTS: We compared hs-TnI level in stored serum samples from 276 consecutive patients (mean age 65 +/- 10 years; 57% male) with T2DM with that of 115 age-and sex-matched controls. All T2DM patients were prospectively followed up for at least 4 years for incidence of MACE including heart failure(HF), myocardial infarction(MI) and cardiovascular mortality. At baseline, 274(99%) patients with T2DM had detectable hs-TnI, and 57(21%) had elevated hs-TnI (male: 8.5 ng/L, female: 7.6 ng/L, above the 99th percentile in healthy controls). A total of 43 MACE occurred: HF(n = 18), MI(n = 11) and cardiovascular mortality(n = 14). Kaplan-Meier analysis showed that an elevated hs-TnI was associated with MACE, HF, MI and cardiovascular mortality. Although multivariate analysis revealed that an elevated hs-TnI independently predicted MACE, it had limited sensitivity(62.7%) and positive predictive value(38.5%). Contrary to this, a normal hs-TnI level had an excellent negative predictive value(92.2%) for future MACE in patients with T2DM. CONCLUSION: The present study demonstrates that elevated hs-TnI in patients with T2DM is associated with increased MACE, HF, MI and cardiovascular mortality. Importantly, a normal hs-TnI level has an excellent negative predictive value for future adverse cardiovascular events during long-term follow-up.published_or_final_versio
