Erythema multiforme in a patient with recurrent non-hodgkins lymphoma/chronic lymphocytic leukemia

Erythema multiforme major (EMM) is a hypersensitivity reaction usually secondary to medications, viruses or other infections. Its presentation is fairly typical with a symmetrical distribution of vesicles, bullae or targeted lesions on the upper body, arms, legs, palms, feet and oral mucosa. The authors present a delineated case of EMM in association with chronic lymphocytic leukemia (CLL) and non-Hodgkin′s lymphoma (NHL) with a very unusual clinical presentation evolving overtime into a unique, almost dermatomal distribution. Typical therapies were not initially helpful and intravenous immunoglobulin antibody had to be administered

    Smart Roofing For Terrace Gardening

The world has turned into a great place for our existence. Though our life has become well sophisticated in all means still there is a problem in producing our food. Since the population is growing exponentially we cannot depend on farmers to produce food for everyone. And so now-a-days it is possible to meet our own needs by ourselves with to help of roof top gardening. But this system needs a good, proper care at right time. To eliminate this struggle we have come with SMART ROOFING SYSTEM. This system takes the input from a moisture sensor that is being inserted into the every single pot. The output of the sensor signal is taken to the microprocessor. The average value of values from the each sensor is compared with the threshold that is fixed manually. If the threshold value increases beyond the fixed threshold then an output from the microprocessor goes high. So that the motor receives the supply. The angle of rotation is already fed into to the motor. As soon as it receives the supply it starts rotating. It rotates to a certain extend that the shaft of the shelter system that is attached to it also rotates. As the angle is already fed the shaft comes to rest position at particular angle. At the end of this action the shaft makes an enough displacement to cover the given area. Thus the shelter is raised to cover the given area

DRUG UTILIZATION AND PRESCRIBING PATTERN ANALYSIS IN ORTHOPAEDIC OUTPATIENT DEPARTMENT OF TERTIARY CARE HOSPITAL

Objective: The objective of conducting prescribing pattern study is to monitor, evaluate, and if necessary, recommend modifications in the prescribing behavior of medical practitioners. Hence, this study was conducted to analyze the drug utilization and prescribing pattern of drugs in orthopedic outpatient department (OPD). Methods: This prospective observational study was conducted in the orthopedic OPD of tertiary care hospital for a period of 3 months. Data were obtained from the prescriptions through one-to-one interviews and recorded in a separately designed case report form. Results: Out of the 260 patients enrolled in the study, 181 were female and 79 were male. The patients between the ages of 50 and 60 years constituted the higher number with a mean age of 52.2 years. The total number of drugs prescribed in the analyzed prescriptions was 1012 which includes orthopedic drugs as well as drugs prescribed to treat various comorbidities. The World Health organization drug prescribing indicators were used to assess the appropriate use of drugs. Conclusion: Our study concludes that drug utilization and prescribing quality should be improved by promoting medical education on current updates. The periodic audit should be conducted by a clinical pharmacist to reduce an error in the prescription as well as to check the compliance of prescription with standard treatment guidelines for rationalizing the therapy

Immunomorphological assessment of regional lymph nodes for predicting metastases in oral squamous cell carcinoma

Background: Oral squamous cell carcinoma is the most common neoplasm and comprises of approximately 80% of the cancers occurring in the oral cavity. The role of the host response to this neoplasm has been recognized, and for many years the regional lymph node in tumor-bearing hosts has been considered as an anatomic barrier to the systematic dissemination of tumor cells. Morphological evaluation of the regional nodes has aided in understanding the immune response. Aim: The current study was carried out to observe the morphological changes occurring in the regional lymph nodes and to evaluate whether these features could be helpful in assessing the immunological status of the patient, and thereby, the prognosis of the patient. Materials and Methods: The study was based on lymph nodes from 63 patients with oral squamous cell carcinoma, who underwent radical neck dissection or modified neck dissection. In the lymph node, four morphological patterns were observed that included lymphocyte predominance, germinal center predominance, mixed pattern (sinus Histiocytosis), and an unstimulated pattern. The cases were then divided into four groups according to the predominant immunoreactivity pattern based on the World Health Organization (WHO) standardized system for reporting human lymph node morphology. Results: Revealed that risk of metastases to cervical lymph nodes in patients with lymphocyte predominance was less (28.6%) when compared to the high risk of metastases with germinal center predominance (68%), and these results were statistically significant (P < 0.05). Patients with a mixed pattern showed less risk of metastases (45.4%), while those with an unstimulated pattern had increased risk of metastases (66.6%), but the results were not statistically significant. It was also found that in the positive nodes, germinal center hyperplasia (50.2%) was the predominant pattern. Conclusion: The present study revealed that patients with lymphocyte predominance had less risk of metastases and patients with germinal center predominance had a high risk of metastases to the lymph node
