    Acupuncture for treatment of alcohol dependence - establishment of collaboration with St George’s Mental Health NHS Trust and Community Drug Service South London

    Alcohol dependence is an increasing social problem. In the UK, the NHS estimates that around 9% of men and 4% of women show signs of alcohol dependence. Acupuncture, as a treatment for addiction, can be integrated into a comprehensive programme to combat alcohol dependence. There have been research studies into the effectiveness of acupuncture to treat alcohol dependence and its related mental health disorders. However, although some findings are positive, they are still not sufficient or conclusive. Two questions need to be answered. The first is whether or not acupuncture is effective in treating alcohol addiction and its associated physiological and psychological symptoms, and the second is whether acupuncture is effective in preventing recidivism. We have established cooperation between our School, South West London and St George’s Mental Health NHS Trust and Community Drug Service South London (CDS). Since January 2015, some alcohol dependent clients have been given acupuncture, and their clinical records show that acupuncture may be effective in helping prevent recidivism. The hypothesis of this research is that acupuncture is effective in treating alcohol dependence by helping addicts quit alcohol drinking and maintain abstinence, and acupuncture has positive effects on symptoms of withdrawal and quality of life. This paper will present the aims and stages of this practical research and will demonstrate how we will measure the effect of treatment on the management of the clients’ addiction to alcohol

    Inflation and initial conditions in the pre-big bang scenario

    The pre-big bang scenario describes the evolution of the Universe from an initial state approaching the flat, cold, empty, string perturbative vacuum. The choice of such an initial state is suggested by the present state of our Universe if we accept that the cosmological evolution is (at least partially) duality-symmetric. Recently, the initial conditions of the pre-big bang scenario have been criticized as they introduce large dimensionless parameters allowing the Universe to be "exponentially large from the very beginning". We agree that a set of initial parameters (such as the initial homogeneity scale, the initial entropy) larger than those determined by the initial horizon scale, H^{-1}, would be somewhat unnatural to start with. However, in the pre-big bang scenario, the initial parameters are all bounded by the size of the initial horizon. The basic question thus becomes: is a maximal homogeneity scale of order H^{-1} necessarily unnatural if the initial curvature is small and, consequently, H^{-1} is very large in Planck (or string) units? In the impossibility of experimental information one could exclude "a priori", for large horizons, the maximal homogeneity scale H^{-1} as a natural initial condition. In the pre-big bang scenario, however, pre-Planckian initial conditions are not necessarily washed out by inflation and are accessible (in principle) to observational tests, so that their naturalness could be also analyzed with a Bayesan approach, in terms of "a posteriori" probabilities.Comment: 4 pages, Latex, one figure.     Asymptotic Behaviour of Inhomogeneous String Cosmologies

    The asymptotic behaviour at late times of inhomogeneous axion-dilaton cosmologies is investigated. The space-times considered here admit two abelian space-like Killing vectors. These space-times evolve towards an anisotropic universe containing gravitational radiation. Furthermore, a peeling-off behaviour of the Weyl tensor and the antisymmetric tensor field strength is found. The relation to the pre-big-bang scenario is briefly discussed.Comment: 15 pages, Late

    The importance of particle dispersion in electrical treeing and breakdown in nano-filled epoxy resin

    The addition of nano-fillers has been widely proposed as a method to enhance the dielectric properties of high voltage polymeric insulation, though there are mixed reports in the literature. Here the potential of silica nano-particles to extend the time to failure specifically through resistance to electrical tree growth in epoxy resin is determined. The benefit of silane treating the nano-particles before compounding is clearly established with regard to slowing tree growth and subsequent time to failure. The growth of trees in needle-plane samples is measured in the laboratory with loadings of 1, 3 and 5 wt% nano-filler. In all cases the average times to failure are extended, but silane treatment of the nano-particles prior to compounding yields much superior results. The emergence of a pronounced inception time before tree growth is also noted for the higher-filled, silane-treated cases. The average time to failure of silane-treated 5 wt% filled material was 28 times that of the unfilled resin. The improvement in performance between the nanocomposites with untreated and treated fillers is attributed to fewer agglomerations and improved dispersion of the filler in the treated cases. Measurements of Partial Discharge (PD) indicated significant differences in PD patterns during the growth of trees in the treated and untreated cases. This distinction may provide a quality control method for monitoring materials. In particular, long periods in which PDs were not measured were observed in the silane-treated cases. Visual imaging of tree growth in the unfilled material allowed the changing nature of the tree from fine to tree to dark tree to be observed as it grew. Corresponding PD measurements suggest the dark tree is gradually becoming conductive, and that growth of maximum PD measured is dependent on the relative rates of the growth of the tree and its carbonization. X-ray computer tomography identified significant differences in average tree channel diameters (a reduction from 2.8 µm to 2.0 µm for 1 wt% and 3 wt% cases). This implies that in addition to tree length changes, evaporated tree volumes also change and may explain the change in partial discharge characteristics observed

    Activation of stress-activated protein kinase in osteoarthritic cartilage: evidence for nitric oxide dependence

    AbstractObjective We have demonstrated in bovine chondrocytes that nitric oxide (NO) mediates IL1 dependent apoptosis under conditions of oxidant stress. This process is accompanied by activation of c-Jun NH2-terminal kinase (JNK; also called stress-activated protein kinase). In these studies we examined activation of JNK in explant cultures of human osteoarthritic cartilage obtained at joint replacement surgery and we characterized the role of peroxynitrite to act as an upstream trigger.Design A novel technique to isolate chondrocyte proteins (<10% of total cartilage protein) from cartilage specimens was developed. It was used to analyse JNK activation by a western blot technique. To examine the hypothesis that chondrocyte JNK activation is a result of increased peroxynitrite, in vitro experiments were performed in which cultured chondrocytes were incubated with this oxidant.Results Activated JNK was detected in the cytoplasm of osteoarthritis (OA) affected chondrocytes but not in that of controls. In vitro, chondrocytes produce NO and superoxide anion. IL-1 (48h), which induces nitric oxide synthase, resulted in an activation of JNK; this effect was reversed by N-monomethylarginine (NMA). TNFα treated chondrocytes at 48h produce superoxide anion (EPR method). Exposure of cells to peroxynitrite led to an accumulation of intracellular oxidants, in association with JNK activation and cell death by apoptosis.Conclusion We suggest that JNK activation is among the IL-1 elicited responses that injure articular chondrocytes and this activation of JNK is dependent on intracellular oxidant formation (including NO peroxynitrite). In addition, the extraction technique here described is a novel method that permits the quantitation and study of proteins such as JNK involved in the signaling pathways of chondrocytes within osteoarthritic cartilage

    Assessing the functional performance of post-call hospital doctors using a Nintendo Wii.

    Sleep deprivation is an established part of the working life for Non-Consultant Hospital Doctors (NCHDs) in Ireland. Concern exists about the effect of extended NCHD work hours. We utilised a Nintendo Wii to evaluate motor function of NCHDs both prior to their on-call shift and the day afterwards. Data was exported to SPSS ver. 15 for statistical analysis with p \u3c 0.05 considered significant. A total of 72 NCHDs were invited to participate in this study. There was a 62.5% (45) rate of follow-up. Overall 27 (60%) NCHDs were on medical call, with 18 (40%) on surgical call. There was no statistically significant difference between NCHDs pre-and post-call motor assessment scores. The majority of study participants (75.5%, n = 34) had four or more hours sleep. On-call duty allows for a greater than anticipated amount of sleep per on-call shift and therefore has a negligible effect on the motor skills of medical staff
