Paracoccidioidomicosis diseminada con insuficiencia suprarrenal: reporte de un caso de autopsia
RESUMEN
La paracoccidioidomicosis producida por el Paracoccidioides brasiliensis es una micosis sistémica que afecta principalmente los pulmones y puede comprometer mucosas, piel, sistema reticuloendotelial, hueso y glándulas suprarrenales. Esta enfermedad es una importante causa de insuficiencia suprarrenal en Brasil y en estudios posmortem se encuentra compromiso de estas hasta en el 80% de los casos.
Se presenta el caso de un campesino, con masa de seis centímetros de diámetro en ojo izquierdo, hospitalizado por alteración del estado de conciencia, deshidratación e hipotensión. Las pruebas de laboratorio indican alteración electrolítica. Recibe manejo con líquidos endovenosos sin mejoría y posteriormente fallece. El estudio de autopsia evidencia compromiso pulmonar, linfático y suprarrenal por Paracoccidioides brasiliensis.

SUMMARY
Paracoccidioides brasiliensis causes a systemic infection that primarily affects the lungs, but can also target mucous membranes, skin, reticuloendothelial system, bone, and adrenal glands. This disease constitutes an important cause of adrenal insufficiency in Brazil and studies posmortem show that the adrenal glands are compromised in as many as 80% of cases.
This is the case of a farmer who presented with a mass of 6 cms in diameter in his left eye. He was hospitalized for altered state of consciousness, dehydration, and hypotension. Laboratory tests indicated electrolytes disturbance. He received management with intravenous fluids without improvement and subsequently died. The autopsy evidenced lung, lymphatic, and adrenal compromise by Paracoccidioides brasiliensis.

Aspergilosis cerebral en un paciente postrasplante cardiaco: reporte de caso
Los pacientes trasplantados son susceptibles a complicaciones neurológicas derivadas entre otros aspectos, de las complicaciones mismas del procedimiento, el cuidado crítico que requieren los pacientes, y de los efectos secundarios a los medicamentos utilizados y el efecto inmunoderpresor de los mismos. Este último hace que los pacientes estén expuestos a infecciones oportunistas. Dentro de estas, las Infecciones del Sistema Nervioso Central en el paciente trasplantado constituyen un reto diagnóstico.
Dentro de los procesos infecciosos es importante tener en cuenta aquellos relacionados con hongos, especialmente en los 3 primeros meses posterior al trasplante.
Se presenta el caso de un paciente 67 años con antecedente de trasplante cardíaco, quien ingresa con un proceso febril y posterior compromiso neurológico
Transferosomas conteniendo ftalocianina de aluminio clorada como alternativa terapéutica en leishmaniasis cutánea: permeabilidad y biodistribución en ratas Wistar
Objetivo: Evaluar la permeabilidad, retención y biodistribución de los LUD-PcAlCl in vivo.Metodología: Los transferosomas fueron obtenidos mediante rehidratación de película lipídica. Ratas Wistar fueron tratadas tópica e intraperitonealmente con los transferosomas por 5 días. La penetración ex vivo fue determinada mediante el ensayo en celdas de Franz y la retención por el método de la cinta adhesiva. Cinco y treinta días postratamiento se obtuvo la piel y órganos para determinar la retención del compuesto y realizar estudios histopatológicos. La PcAlCl fue extraída con solventes y cuantificada por fluorometría. Los resultados se expresaron en nM PcAlCl/mg órgano.Resultados: La PcAlCl no penetró la piel en los ensayos ex vivo, reteniéndose principalmente en el estrato córneo. Cinco días post-tratamiento tópico la PcAlCl fue retenida en estrato córneo (41,76±0,02), mostrando concentraciones mínimas en bazo (0,09±0,02), epidermis-dermis (0,06±0,17), hígado (0,03±0,02) y pulmón (0,02±0,01 nM). Por vía intraperitoneal se encontró PcAlCl en bazo (0,58±0,4), cerebro (0,07±0,07), corazón (0,07±0,12), pulmón (0,012±0,01) y piel (0,021±0,02 nM). Treinta días postratamiento no se encontró PcAlCl en ningún órgano. Los estudios histopatológicos fueron negativos.Conclusión: La PcAlCl contenida en transferosomas fue retenida principalmente en estrato córneo, mostrando bajas concentraciones en la dermis, sitio donde se aloja el parásito. Se sugiere modificar los componentes vesiculares del sistema para aumentar la permeación del compuesto
Immunomodulation and Antioxidant Activities as Possible Trypanocidal and Cardioprotective Mechanisms of Major Terpenes from Lippia alba Essential Oils in an Experimental Model of Chronic Chagas Disease
In the late phase of Trypanosoma cruzi infection, parasite persistence and an exaggerated immune response accompanied by oxidative stress play a crucial role in the genesis of Chronic Chagasic Cardiomyopathy (CCC). Current treatments (Benznidazole (BNZ) and Nifurtimox) can effect only the elimination of the parasite, but are ineffective for late stage treatment and for preventing heart damage and disease progression. In vivo trypanocidal and cardioprotective activity has been reported for Lippia alba essential oils (EOs), ascribed to their two major terpenes, limonene and caryophyllene oxide. To investigate the role of antioxidant and immunomodulatory mechanisms behind these properties, chronic-T. cruzi-infected rats were treated with oral synergistic mixtures of the aforementioned EOs. For this purpose, the EOs were optimized through limonene-enrichment fractioning and by the addition of exogenous caryophyllene oxide (LIMOX) and used alone or in combined therapy with subtherapeutic doses of BNZ (LIMOXBNZ). Clinical, toxicity, inflammatory, oxidative, and parasitological (qPCR) parameters were assessed in cardiac tissue. These therapies demonstrated meaningful antioxidant and immunomodulatory activity on markers involved in CCC pathogenesis (IFN-γ, TNF-α, IL-4, IL-10, and iNOS), which could explain their significant trypanocidal properties and their noteworthy role in preventing, and even reversing, the progression of cardiac damage in chronic Chagas disease
Evolution over Time of Ventilatory Management and Outcome of Patients with Neurologic Disease∗
OBJECTIVES: To describe the changes in ventilator management over time in patients with neurologic disease at ICU admission and to estimate factors associated with 28-day hospital mortality. DESIGN: Secondary analysis of three prospective, observational, multicenter studies. SETTING: Cohort studies conducted in 2004, 2010, and 2016. PATIENTS: Adult patients who received mechanical ventilation for more than 12 hours. INTERVENTIONS: None. MEASUREMENTS AND MAIN RESULTS: Among the 20,929 patients enrolled, we included 4,152 (20%) mechanically ventilated patients due to different neurologic diseases. Hemorrhagic stroke and brain trauma were the most common pathologies associated with the need for mechanical ventilation. Although volume-cycled ventilation remained the preferred ventilation mode, there was a significant (p < 0.001) increment in the use of pressure support ventilation. The proportion of patients receiving a protective lung ventilation strategy was increased over time: 47% in 2004, 63% in 2010, and 65% in 2016 (p < 0.001), as well as the duration of protective ventilation strategies: 406 days per 1,000 mechanical ventilation days in 2004, 523 days per 1,000 mechanical ventilation days in 2010, and 585 days per 1,000 mechanical ventilation days in 2016 (p < 0.001). There were no differences in the length of stay in the ICU, mortality in the ICU, and mortality in hospital from 2004 to 2016. Independent risk factors for 28-day mortality were age greater than 75 years, Simplified Acute Physiology Score II greater than 50, the occurrence of organ dysfunction within first 48 hours after brain injury, and specific neurologic diseases such as hemorrhagic stroke, ischemic stroke, and brain trauma. CONCLUSIONS: More lung-protective ventilatory strategies have been implemented over years in neurologic patients with no effect on pulmonary complications or on survival. We found several prognostic factors on mortality such as advanced age, the severity of the disease, organ dysfunctions, and the etiology of neurologic disease
