    Strengthening salt reduction in low- and middle-income countries: Is the current evidence on the influences and effects of salt consumption, and strategies for salt reduction generalisable to low- and middle-income countries?

    Background: The totality of evidence shows that high salt intake increases cardiovascular risk, and a number of studies have suggested its link with diabetes mellitus. Many studies also indicate that reducing salt intake will reduce these risks; however, most of the evidence is from high income countries, and little is known about how this applies to LMICs. Methods: This PhD combined systematic reviews of existing evidence and Bayesian modelling to contribute evidence on the effectiveness of salt reduction interventions, the relationship between salt intake and cardiometabolic outcomes, and factors (individual, household, national-level) affecting salt intake, with a view to better informing future interventions in LMICs. The research comprises three components: (1) a review of national salt reduction initiatives around the world; (2) an in-depth examination of evaluated salt reduction interventions at all levels of implementation in LMICs, and; (3) secondary analysis of data from LMICs using Bayesian methods. Results: The number of national salt reduction initiatives globally increased from 75 in 2014 to 96 in 2019, with a 43% and 18% increase in the number of initiatives in upper-middle- and lower-middle-income countries, respectively. Evaluations of salt reduction interventions in LMICs, implemented at all levels (i.e. not just national), showed that they were successful in producing positive outcomes such as decreased salt intake, lower sodium levels in foods, improved knowledge, attitudes and behaviours towards salt, or decreased blood pressure. The secondary analysis found limited data on cardiometabolic outcomes, apart from systolic blood pressure, and most datasets only contained individual-level factors. The Bayesian analysis showed that salt intake was positively associated with systolic blood pressure (β 0.15, 95% credible interval 0.07 to 0.22) and the factors that influenced salt intake in LMICs were sex, age, BMI, education level, history of diabetes, and salt-related attitudes. Conclusion: While there was little data on other cardiometabolic outcomes, this research reaffirms the relationship between high salt intake and blood pressure, and shows the factors that influence salt intake in LMICs. Combined with existing global evidence on the effectiveness of salt reduction interventions, the findings from this PhD produce a strong case for urgent action to reduce salt intake in LMICs

    Bayesian Meta-Analysis of Health State Utility Values: A Tutorial with a Practical Application in Heart Failure.

    Researchers incorporate health state utility values as inputs to inform economic models. However, for a particular health state or condition, multiple utility values derived from different studies typically exist and a single study is often insufficient to represent the best available source of utility needed to inform policy decisions. The purpose of this paper is to provide an introductory guidance for conducting Bayesian meta-analysis of health state utility values to generate a single parameter input for economic evaluation, using R. The tutorial is illustrated using data from a systematic review of health state utilities of patients with heart failure, with 21 studies that reported utilities measured using the EuroQol-5D (EQ-5D). Explanations, key considerations and suggested readings are provided for each step of the tutorial, adhering to a clear workflow for conducting Bayesian meta-analysis: (1) setting-up the data; (2) employing methods to impute missing standard deviations; (3) defining the priors; (4) fitting the model; (5) diagnosing model convergence; (6) interpreting the results; and (7) performing sensitivity analyses. The posterior distributions for the pooled effect size (i.e. mean health state utility) and between-study heterogeneity are discussed and interpreted in light of the data, priors and models used. We hope that this tutorial will foster interest in Bayesian methods and their applications in the meta-analysis of utilities

    Compressive Strength and Bulk Density of Concrete Hollow Blocks (CHB) Infused with Low-density Polyethylene (LDPE) Pellets

    Infusing plastic waste to concrete and masonry structures is an increasingly common industry practice that has the potential to create an environment-friendly material that can improve some of the material’s properties, craft a novel means to repurpose plastic waste, and reduce the need for mining aggregates in the environment. This concept has been studied extensively in different forms of concrete, as shown by several studies; however, there is a dearth of studies focusing on the incorporation plastic waste in concrete hollow blocks (CHB). In this study, we aim to fill that gap by investigating on the effects of incorporating low-density polyethylene (LDPE), a commonly used plastic material, to CHB on its compressive strength and bulk density. Samples of varying percentages of LDPE replacement by volume (0, 10, 20, 30 and 40%) were fabricated and tested. Results showed a general trend of decreasing compressive strength and bulk density upon increasing the amount of LDPE pellets in CHB, which was also observed in previous studies. However, the compressive strength of CHB increased at 10% LDPE replacement, a result similar to a previous study. It was inferred that the strength of the plastic material could have a direct contribution to the compressive strength of CHB at low percentage of aggregate replacement. Statistical analysis showed that the mix with 10% LDPE pellets as replacement to sand was the best among the samples tested. It was shown that CHB infused with LDPE pellets has a higher compressive strength than what is normally used in the Philippines. It was concluded that based on compressive strength and bulk density, LDPE pellets is a viable material to use as partial replacement to sand in non-load bearing CHB

    Outcomes of a state-wide salt reduction initiative in adults living in Victoria, Australia

    Purpose: To assess any effects of a state-wide sodium reduction intervention on sodium intake, sources of dietary sodium and discretionary salt use at a population level. Methods: Data (24-h urinary sodium excretion, self-report survey, a 24-h dietary recall) were collected cross-sectionally at baseline (2016/2017) and follow-up (2020) from adults in Victoria, Australia. Intervention activities included consumer awareness advertising campaign, public debate generation via mass media, strengthening existing policy initiatives and supporting food innovation with industry. Results: There were 339 participants at baseline and 211 at follow-up, with 144 and 90 of participants completing a 24-h dietary recall, respectively. There was no difference in adjusted 24-h urinary sodium excretion between baseline and follow-up (134 vs 131 mmol/24 h; p = 0.260). There were no differences in the percentage of participants adding salt during cooking (63% vs 68%; p = 0.244), adding salt at the table (34% vs 37%; p = 0.400) or regularly taking action to control salt/sodium intake (22% vs 21%; p = 0.793). There were large differences in the quantity of dietary sodium sourced from retail stores (57% vs 77%, p < 0.001), and less sodium was sourced from foods at fresh food markets (13% vs 2%; p ≤ 0.001) at follow-up. No large differences were apparent for foods with different levels of processing or for food groups. Conclusion: There was no clear population-level effect of the 4-year multi-component Victorian Salt Reduction Intervention on sodium intake with Victorian adults continuing to consume sodium above recommended levels. The findings indicate that more intensive and sustained efforts aiming at the retail and food industry with national level support are likely to be required to achieve a measurable improvement in sodium intake at a state level

    From 2015 to 2020 a state-wide salt-reduction initiative was launched in Victoria, Australia, including an awareness campaign focused on parents with children <18 years of age. To evaluate the impact of the campaign on salt-related knowledge, attitudes and behaviors (KABs) we have assessed trends in salt-related KAB pre- and post-delivery of the campaign in parents, as well as within the wider adult population. Cross-sectional surveys of adults aged 18–65 years were undertaken pre- (2015: n = 821 parents; n = 1527 general sample) and post-campaign (2019: n = 935 parents; n = 1747 general sample). KABs were assessed via an online survey. Data were analyzed with regression models and adjusted for covariates. Among parents, around one-quarter of salt-related KABs shifted in a positive direction, but changes were small: there was a 6% (95% CI 2, 11%) increase in the percentage who knew the main source of salt in the diet and reductions in the percentage who reported placing a salt shaker on the table (−8% (95%CI −12, −3)) and that their child added salt at the table (−5% (95% −9, −0.2)). Among the wider adult sample, even fewer shifts in KAB were observed, with some behaviors worsening at follow-up. These findings indicate that this consumer awareness campaign had minimum impact

    Evaluation of sex differences in dietary behaviours and their relationship with cardiovascular risk factors:a cross-sectional study of nationally representative surveys in seven low- and middle-income countries

    Background: Cardiovascular diseases (CVD) are the leading causes of death for men and women in low-and-middle income countries (LMIC). The nutrition transition to diets high in salt, fat and sugar and low in fruit and vegetables, in parallel with increasing prevalence of diet-related CVD risk factors in LMICs, identifies the need for urgent action to reverse this trend. To aid identification of the most effective interventions it is crucial to understand whether there are sex differences in dietary behaviours related to CVD risk. Methods: From a dataset of 46 nationally representative surveys, we included data from seven countries that had recorded the same dietary behaviour measurements in adults; Bhutan, Eswatini, Georgia, Guyana, Kenya, Nepal and St Vincent and the Grenadines (2013-2017). Three dietary behaviours were investigated: positive salt use behaviour (SUB), meeting fruit and vegetable (F&V) recommendations and use of vegetable oil rather than animal fats in cooking. Generalized linear models were used to investigate the association between dietary behaviours and waist circumference (WC) and undiagnosed and diagnosed hypertension and diabetes. Interaction terms between sex and dietary behaviour were added to test for sex differences. Results: Twenty-four thousand three hundred thirty-two participants were included. More females than males reported positive SUB (31.3 vs. 27.2% p-value < 0.001), yet less met F&V recommendations (13.2 vs. 14.8%, p-value< 0.05). The prevalence of reporting all three dietary behaviours in a positive manner was 2.7%, varying by country, but not sex. Poor SUB was associated with a higher prevalence of undiagnosed hypertension for females (13.1% vs. 9.9%, p-value = 0.04), and a higher prevalence of undiagnosed diabetes for males (2.4% vs. 1.5%, p-value = 0.02). Meeting F&V recommendations was associated with a higher prevalence of high WC (24.4% vs 22.6%, p-value = 0.01), but was not associated with undiagnosed or diagnosed hypertension or diabetes. Conclusion: Interventions to increase F&V intake and positive SUBs in the included countries are urgently needed. Dietary behaviours were not notably different between sexes. However, our findings were limited by the small proportion of the population reporting positive dietary behaviours, and further research is required to understand whether associations with CVD risk factors and interactions by sex would change as the prevalence of positive behaviours increases

    Assessment of a salt reduction intervention on adult population salt intake in Fiji

    Reducing population salt intake is a global public health priority due to the potential to save lives and reduce the burden on the healthcare system through decreased blood pressure. This implementation science research project set out to measure salt consumption patterns and to assess the impact of a complex, multi-faceted intervention to reduce population salt intake in Fiji between 2012 and 2016. The intervention combined initiatives to engage food businesses to reduce salt in foods and meals with targeted consumer behavior change programs. There were 169 participants at baseline (response rate 28.2%) and 272 at 20 months (response rate 22.4%). The mean salt intake from 24-h urine samples was estimated to be 11.7 grams per day (g/d) at baseline and 10.3 g/d after 20 months (difference: -1.4 g/day, 95% CI -3.1 to 0.3, p = 0.115). Sub-analysis showed a statistically significant reduction in female salt intake in the Central Division but no differential impact in relation to age or ethnicity. Whilst the low response rate means it is not possible to draw firm conclusions about these changes, the population salt intake in Fiji, at 10.3 g/day, is still twice the World Health Organization\u27s (WHO) recommended maximum intake. This project also assessed iodine intake levels in women of child-bearing age and found that they were within recommended guidelines. Existing policies and programs to reduce salt intake and prevent iodine deficiency need to be maintained or strengthened. Monitoring to assess changes in salt intake and to ensure that iodine levels remain adequate should be built into future surveys

    Process evaluation and costing of a multifaceted population-wide intervention to reduce salt consumption in Fiji

    This paper reports the process evaluation and costing of a national salt reduction intervention in Fiji. The population-wide intervention included engaging food industry to reduce salt in foods, strategic health communication and a hospital program. The evaluation showed a 1.4 g/day drop in salt intake from the 11.7 g/day at baseline; however, this was not statistically significant. To better understand intervention implementation, we collated data to assess intervention fidelity, reach, context and costs. Government and management changes affected intervention implementation, meaning fidelity was relatively low. There was no active mechanism for ensuring food companies adhered to the voluntary salt reduction targets. Communication activities had wide reach but most activities were one-off, meaning the overall dose was low and impact on behavior limited. Intervention costs were moderate (FJD 277,410or277,410 or 0.31 per person) but the strategy relied on multi-sector action which was not fully operationalised. The cyclone also delayed monitoring and likely impacted the results. However, 73% of people surveyed had heard about the campaign and salt reduction policies have been mainstreamed into government programs. Longer-term monitoring of salt intake is planned through future surveys and lessons from this process evaluation will be used to inform future strategies in the Pacific Islands and globally. <br /
