    Anaerobic co-digestion of acetate-rich with lignin-rich wastewater and the effect of hydrotalcite addition

    The methane potential and biodegradability of different ratios of acetate and lignin-rich effluents from a neutral sulfite semi-chemical (NSSC) pulp mill were investigated. Results showed ultimate methane yields up to 333 ± 5 mL CH4/gCOD when only acetate-rich substrate was added and subsequently lower methane potentials of 192 ± 4 mL CH4/gCOD when the lignin fraction was increased. The presence of lignin showed a linear decay in methane production, resulting in a 41% decrease in methane when the lignin-rich feed had a 30% increase. A negative linear correlation between lignin content and biodegradability was also observed. Furthermore, the effect of hydrotalcite (HT) addition was evaluated and showed increase in methane potential of up to 8%, a faster production rate and higher soluble lignin removal (7–12% higher).     New alternative energy pathway for chemical pulp mills: from traditional fibers to methane production

    Chemical pulp mills have a need to diversify their end-product portfolio due to the current changing bio-economy. In this study, the methane potential of brown, oxygen delignified and bleached pulp were evaluated in order to assess the potential of converting traditional fibers; as well as microcrystalline cellulose and filtrates; to energy. Results showed that high yields (380 mL CH4/gVS) were achieved with bleached fibers which correlates with the lower presence of lignin. Filtrates from the hydrolysis process on the other hand, had the lowest yields (253 mL CH4/gVS) due to the high amount of acid and lignin compounds that cause inhibition. Overall, substrates had a biodegradability above 50% which demonstrates that they can be subjected to efficient anaerobic digestion. An energy and cost estimation showed that the energy produced can be translated into a significant profit and that methane production can be a promising new alternative option for chemical pulp mills.Postprint (author's final draft

    Galectin-3 and fibulin-1 in systolic heart failure:relation to glucose metabolism and left ventricular contractile reserve

    Abstract Background Heart failure (HF) patients with diabetes (DM) have an adverse prognosis and reduced functional capacity, which could be associated with cardiac fibrosis, increased chamber stiffness and reduced left ventricular (LV) contractile reserve. Galectin-3 (Gal-3) and fibulin-1 are circulating biomarkers potentially reflecting cardiac fibrosis. We hypothesize that plasma levels of Gal-3 and fibulin-1 are elevated in HF patients with DM and are associated with reduced LV contractile reserve in these patients. Methods A total of 155 patients with HF with reduced ejection fraction underwent a low-dose dobutamine echocardiography and blood sampling for biomarker measurements. Patients were classified according to history of DM and an oral glucose tolerance test (OGTT) as: normal glucose tolerance (NGT) ( n \u2009=\u200970), impaired glucose tolerance (IGT) ( n \u2009=\u200925) and DM ( n \u2009=\u200960). Results Galectin-3 levels were elevated in DM patients as compared to non-diabetic patients ( P \u2009=\u20090.02), while higher fibulin-1 levels were observed in HF patients with IGF and DM ( P \u2009=\u20090.07). Reduced LV contractile reserve was associated with increasing Gal-3 levels (\u3b2\u2009=\u2009\u22120.19, P \u2009=\u20090.03) although, this association was attenuated after adjustment for estimated glomerular filtration rate ( P \u2009=\u20090.66). Fibulin-1 was not associated with LV contractile reserve ( P \u2009=\u20090.71). Conclusions Galectin-3 and fibulin-1 levels were elevated in HF patients with impaired glucose metabolism. However, reduced LV contractile reserve among HF patients with DM does not to have an independent impact on plasma Gal-3 and fibulin-1 levels

    Hemodynamic changes during aortic valve surgery among patients with aortic stenosis

    Introduction. Patients with severe aortic stenosis (AS) undergoing surgery are at increased risk of hypotension and hypoperfusion. Although treatable with inotropic agents or fluid, little is known about how these therapies affect central hemodynamics in AS patients under general anesthesia. We measured changes in central hemodynamics after dobutamine infusion and fluid bolus among patients with severe AS and associated these changes with preoperative echocardiography. Methods. We included 33 patients with severe AS undergoing surgical AVR. After induction of general anesthesia, hemodynamic measurements were obtained with a pulmonary artery catheter, including Cardiac index (CI), stroke volume index (SVi) and pulmonary capillary wedge pressure (PCWP). Measurements were repeated during dobutamine infusion, after fluid bolus and lastly after sternotomy. Results. General anesthesia resulted in a decrease in CI and SVi compared to preoperative values. During dobutamine infusion CI increased but mean SVi did not (38 ± 12 vs 37 ± 13 ml/m², p = .90). Higher EF and SVi before surgery and a larger decrease in SVi after induction of general anesthesia were associated with an increase in SVi during dobutamine infusion. After fluid bolus both CI, SVi (48 ± 12 vs 37 ± 13 ml/min/m², p < .0001) and PCWP increased. PCWP increased mostly among patients with a larger LA volume index. Conclusion. In patients with AS, CI can be increased with both dobutamine and fluid during surgery. Dobutamine’s effect on SVI was highly variable and associated with baseline LVEF, and an increase in CI was mostly driven by an increase in heart rate.     Plasma osteoprotegerin is related to carotid and peripheral arterial disease, but not to myocardial ischemia in type 2 diabetes mellitus

    <p>Abstract</p> <p>Background</p> <p>Cardiovascular disease (CVD) is frequent in type 2 diabetes mellitus patients due to accelerated atherosclerosis. Plasma osteoprotegerin (OPG) has evolved as a biomarker for CVD. We examined the relationship between plasma OPG levels and different CVD manifestations in type 2 diabetes.</p> <p>Methods</p> <p>Type 2 diabetes patients without known CVD referred consecutively to a diabetes clinic for the first time (n = 305, aged: 58.6 Âą 11.3 years, diabetes duration: 4.5 Âą 5.3 years) were screened for carotid arterial disease, peripheral arterial disease, and myocardial ischemia by means of carotid artery ultrasonography, peripheral ankle and toe systolic blood pressure measurements, and myocardial perfusion scintigraphy (MPS). In addition, plasma OPG concentrations and other CVD-related markers were measured.</p> <p>Results</p> <p>The prevalence of carotid arterial disease, peripheral arterial disease, and myocardial ischemia was 42%, 15%, and 30%, respectively. Plasma OPG was significantly increased in patients with carotid and peripheral arterial disease compared to patients without (p < 0.001, respectively), however, this was not the case for patients with myocardial ischemia versus those without (p = 0.71). When adjusted for age, HbA1c and U-albumin creatinine ratio in a multivariate logistic regression analysis, plasma OPG remained strongly associated with carotid arterial disease (adjusted OR: 2.12; 95% CI: 1.22-3.67; p = 0.008), but not with peripheral arterial disease or myocardial ischemia.</p> <p>Conclusions</p> <p>Increased plasma OPG concentration is associated with carotid and peripheral arterial disease in patients with type 2 diabetes, whereas no relation is observed with respect to myocardial ischemia on MPS.     Impact of mandatory preoperative dental screening on post-procedural risk of infective endocarditis in patients undergoing transcatheter aortic valve implantation: a nationwide retrospective observational study

    BackgroundGuidelines recommend preoperative dental screening (PDS) prior to cardiac valve surgery, to reduce the incidence of prosthetic valve infective endocarditis (IE). However, limited data support these recommendations, particular in patients undergoing transcatheter aortic valve implantation (TAVI). We aimed to investigate the effect of mandatory PDS on risk of IE in patients undergoing TAVI.MethodsIn this observational study, a total of 1133 patients undergoing TAVI in Western-Denmark from 2020 to 2022 were included. Patients were categorized based on two implemented PDS practices: mandatory PDS (MPDS group), and no referral for PDS (NPDS group). Outcome data were retrieved from Danish registries and confirmed using medical records. The primary outcome was incidence of IE. Secondary outcomes were all-cause mortality and composite outcome of all-cause mortality and IE.FindingsOf 568 patients in the MPDS group 126 (22.2%) underwent subsequent oral dental surgery, compared to 8 (1.4%) among 565 patients in the NPDS group. During a median follow-up of 1.9 years (interquartile range 1.4–2.5 years), 31 (2.7%) developed IE. The yearly incidence IE rate was 1.4% (0.8–2.3) and 1.5% (0.8–2.4) in MPDS and NPDS, respectively, p = 0.86. All-cause mortality rates were similar between groups (estimated 2-year overall mortality of 6.7% (4.8–9.2) vs. 4.7% (3.2–6.9), MPDS and NPDS, respectively, p = 0.15). Consistent findings were found in 712 propensity score-matched patients.InterpretationMandatory PDS did not demonstrate reduced risk of IE or all-cause mortality compared to targeted PDS in patients undergoing TAVI
