    Ex Post Welfare under Alternative Health Care Systems

    This paper studies the implications of a societal aversion to inequality for the optimal design of a public health care system. Inequality aversion is introduced by postulating a strictly concave ex post social welfare function. Illnesses are characterized by three factors: the agent's health with treatment, the agent's health without treatment, and the cost of treatment. It is shown that the optimal public health care system allocates health care differently than would private health insurance; speci?cally, people who are relatively unhealthy with and without treatment receive more health care, and people who are relatively healthy with and without treatment receive less health care. The aggregate quantity of health care under the optimal public health care system might be either greater or less than under private health care insurance. If the public health care system is optimally designed, allowing agents to purchase supplementary private health care insurance cannot raise social welfare and is likely to decrease it.

    Equalization Payments in a Bargaining Model of Tax Competition

    A model in which a high-productivity region and a low-productivity region bargain with each firm in a group of mobile firms is constructed. It differs from the Han and Leach [7] model in that the firms are identical, so that its comparative statics are more tractable. The model is used to examine the allocative effects of equalization payments (both non-contingent payments and "corrective subsidies"). The equilibrium is characterized by misallocation of capital and underprovision of public goods. Underprovision is more severe in the low-productivity region than the high-productivity region. A transfer of revenue from the high-productivity region to the low-productivity region augments public goods provision in the low-productivity region, allowing that region to make more generous offers to the firms. Likewise, underprovsion becomes more severe in the high-productivity region, so that its offers become less generous. Equilibrium is attained by a movement of firms from the high-productivity region to the low-productivity region, reducing the misallocation of capital.

    Sudden unexpected death in epilepsy is a recurring calamity, yet there is little evidence to guide standards of care for supporting the bereaved. Grief in bereavement includes loss, feelings of guilt, anger and blame. There is also the shock and trauma of the sudden event. How can this be alleviated? This paper focuses on guiding the physician to support the bereaved, while recognising the limited evidence and the varying circumstances that may not always facilitate this. We propose a pathway of care and mode of communication with the deceased's family, with whom contact is currently limited. We suggest timely contact by telephone or in person, followed by ongoing support and referral to voluntary organisations and specialist services, as needed. Clarification and discussion may mitigate inappropriate feelings of guilt and blame, and may help the family with their sudden and unexpected loss

    A Bargaining Model of Tax Competition

    This paper develops a model in which competing governments offer financial incentives to individual firms to induce the firms to locate within their jurisdictions. Equilibrium is described under three specifications of the supplementary taxes. There is no misallocation of capital under two of these specifications, and there might or might not be capital misallocation under the third. This result contrasts strongly with that of the standard tax competition model, which does not allow governments to treat firms individually. That model almost always finds that competition among governments leads to the misallocation of capital.

    The Emergent Middle Classes in Timor-Leste and Melanesia: Conceptual Issues and Developmental Significance

    The emergence of a middle class has been identified as an important factor driving economic and political transitions in Asia and Africa. Class has been 'happening' in the broader Pacific region1 for some time, as Gewertz and Errington (1999:2) observed of Papua New Guinea (PNG). Nevertheless, to date, little systematic policy attention has been given to questions of class. We believe that the concept of an emerging middle class provides a useful entry point for understanding significant developmental and political transformations in Timor-Leste and Melanesia and is important for informing development policies. However, in order to explore the potential consequences that an emergent middle class may have, it is necessary to first consider how such a class can be identified in the region. We have chosen to focus on Timor-Leste, PNG and Solomon Islands because these countries are undergoing comparable social and economic transitions, many of which correlate strongly with the emergence of a middle class. Such changes include rapid economic growth driven by resource booms in Timor-Leste and PNG, associated formalisation of regional economies, deepening urbanisation, increasing social integration with metropolitan powers through growing diaspora communities, changing consumption patterns, and the transformative impacts of social media following recent internet and mobile telephone penetration. This paper has three substantive sections. First, it considers contemporary discussions of class and development in Asia and other developing regions. Second, the paper develops a multidimensional framework for identifying an emergent middle class, drawing on a range of economic, political and social criteria. Finally, the paper uses these criteria to examine recent developments in each of three case study countries and then draws some conclusions on the developmental and political significance of an emergent middle class in the broader Pacific region with a view to establishing a longer term research agenda.AusAI
