    Can guidelines improve referral to elective surgical specialties for adults? A systematic review

    Aim To assess effectiveness of guidelines for referral for elective surgical assessment. Method Systematic review with descriptive synthesis. Data sources Medline, EMBASE, CINAHL and Cochrane database up to 2008. Hand searches of journals and websites. Selection of studies Studies evaluated guidelines for referral from primary to secondary care, for elective surgical assessment for adults. Outcome measures Appropriateness of referral (usually measured as guideline compliance) including clinical appropriateness, appropriateness of destination and of pre-referral management (eg, diagnostic investigations), general practitioner knowledge of referral appropriateness, referral rates, health outcomes and costs. Results 24 eligible studies (5 randomised control trials, 6 cohort, 13 case series) included guidelines from UK, Europe, Canada and the USA for referral for musculoskeletal, urological, ENT, gynaecology, general surgical and ophthalmological conditions. Interventions varied from complex (“one-stop shops”) to simple guidelines. Four randomized control trials reported increases in appropriateness of pre-referral care (diagnostic investigations and treatment). No evidence was found for effects on practitioner knowledge. Mixed evidence was reported on rates of referral and costs (rates and costs increased, decreased or stayed the same). Two studies reported on health outcomes finding no change. Conclusions Guidelines for elective surgical referral can improve appropriateness of care by improving prereferral investigation and treatment, but there is no strong evidence in favour of other beneficial effects

An informed approach to the evaluation of the patients with stable ischemic chest pain can lead to efficient use of resources and better outcomes.     Heralded state preparation in a superconducting qubit

    We demonstrate high-fidelity, quantum nondemolition, single-shot readout of a superconducting flux qubit in which the pointer state distributions can be resolved to below one part in 1000. In the weak excitation regime, continuous measurement permits the use of heralding to ensure initialization to a fiducial state, such as the ground state. This procedure boosts readout fidelity to 93.9% by suppressing errors due to spurious thermal population. Furthermore, heralding potentially enables a simple, fast qubit reset protocol without changing the system parameters to induce Purcell relaxation.Comment: 5 pages, 5 figure

    Signatures of Pseudoscalar Photon Mixing in CMB Radiation

    We model the effect of photon and ultra-light pseudoscalar mixing on the propagation of electromagnetic radiation through the extragalactic medium. The medium is modelled as a large number of magnetic domains, uncorrelated with one another. We obtain an analytic expression for the different Stokes parameters in the limit of small mixing angle. The different Stokes parameters are found to increase linearly with the number of domains. We also verify this result by direct numerical simulations. We use this formalism to estimate the effect of pseudoscalar-photon mixing on the Cosmic Microwave Background (CMB) polarization. We impose limits on the model parameters by the CMB observations. We find that the currently allowed parameter range admits a CMB circular polarization up to order 10710^{-7}.Comment: 17 pages, 5 figure

    Dispersive readout of a flux qubit at the single photon level

    A superconducting flux qubit is inductively coupled to a Superconducting QUantum Interference Device (SQUID) magnetometer, capacitively shunted to form a 1.294-GHz resonator. The qubit-state-dependent resonator frequency is weakly probed with a microwave signal and detected with a Microstrip SQUID Amplifier. At a mean resonator occupation nˉ\bar{n} = 1.5 photons, the readout visibility is increased by a factor of 4.5 over that using a cryogenic semiconductor amplifier. As nˉ\bar{n} is increased from 0.008 to 0.1, no reduction in T1T_1 is observed, potentially enabling continuous monitoring of the qubit state.Comment: 5 pages, 5 figure

    The Birth of High Mass Stars: Accretion and/or Mergers?

    The observational consequences of the merger scenario for massive star formation are explored and contrasted with the gradual accumulation of mass by accretion. Protostellar mergers may produce high luminosity infrared flares lasting years to centuries followed by a luminosity decline on the Kelvin-Helmholtz time-scale of the merger product. Mergers may be surrounded by thick tori of expanding debris, impulsive wide-angle outflows, and shock induced maser and radio continuum emission. Collision products are expected to have fast stellar rotation and a large multiplicity fraction. Close encounters or mergers will produce circumstellar debris disks with an orientation that differs form that of a pre-existing disk. The extremely rare merger of two stars close to the upper-mass end of the IMF may be a possible pathway to hypernova generated gamma-ray bursters. While accretional growth can lead to the formation of massive stars in isolation or in loose clusters, mergers can only occur in high-density cluster environments. It is proposed that the outflow emerging from the OMC1 core in the Orion molecular cloud was produced by a protostellar merger that released between 104810^{48} to 104910^{49} ergs less than a thousand years ago

    River Invertebrate Classification Tool

    Background to research The Regulatory Agencies in the UK (the Environment Agency; Scottish Environment Protection Agency; and the Environment & Heritage Service) currently use RIVPACS III+ software to classify the ecological quality of rivers. However, because RIVPACS III+ pre-dates the WFD, there has been a requirement to ensure that the RIVPACS reference sites are fully WFD compliant, to add new biotic indices to the RIVPACS models, and to improve the robustness of the RIVPACS software to fully meet the needs of the Agencies in their delivery of WFD monitoring. These issues have been addressed in this project and have led to the development of new RIVPACS IV predictive models that will be programmed into a new River Invertebrate Classification Tool being built by SEPA. This new system will be based on a modern software programming language, be compatible with the agencies’ computer systems and include the ability to predict new biological indices, produce biological status assessments based on these new indices and be able to estimate the errors involved in using these new indices. Because access to the new system will be essential for the UK Agencies to be able to implementation the WFD, the new tool will be readily and freely available to anyone who might seek to use it. Objectives of research • The overall objective of the project was to produce a new set of RIVPACS predictive models for use within a new River Invertebrate Classification Tool that will be used to classify the ecological status of rivers for Water Framework Directive compliance monitoring • The new RIVPACS models constructed with this project required considerably enhanced functionality compared to RIVPACS III+ to properly address the monitoring requirements of the UK Agencies in their implementation of the Water Framework Directive. Key findings and recommendations This project has produced new RIVPACS IV models with considerably enhanced functionality compared to RIVPACS III+. These models incorporate: • A full revision of the taxonomic framework of RIVPACS to bring the taxonomy up-to-date and enable compatiability across the revised Miatland, Furse code and National Biodiversity Network taxon coding systems used across the UK Agencies and beyond • Predictions that fully satisfy the WFD definition of ‘reference condition’ by adjusting predictions for certain stream types and by removal of sites that were not in reference condition when sampled • Allocation of actual abundance values to family level records in the RIVPACS reference data set. Lack of actual abundance data, especially at family level, has affected all versions of RIVPACS and has constrained the types of biotic indices that RIVPACS can predict • Extension to the suite of biotic indices so that the new system can predict a wider range of reference state “expected” index values. This enables full WFD quality reporting capabilities as well as providing the system with the general functionality to predict a much wider range of indices e.g. intercalibration indices (e.g. ICMi), stress-specific indices, and ecological and functional trait indices • Extension of the uncertainty/errors module to estimate and assess uncertainty in (i) assignment to status class and (ii) comparison of samples for temporal change in quality and status. This needs to be done for a wider range of biotic indices (including those incorporating abundance data) These new RIVPACS IV models can be used by the UK Agencies across Great Britain and Northern Ireland in their WFD compliance monitoring. All of the algorithms, variables and data necessary to build these models have been provided to SEPA for programming into a new River Invertebrate Classification Tool that will be disseminated made free of charge to all interested user

    Noiseless nonreciprocity in a parametric active device

    Nonreciprocal devices such as circulators and isolators belong to an important class of microwave components employed in applications like the measurement of mesoscopic circuits at cryogenic temperatures. The measurement protocols usually involve an amplification chain which relies on circulators to separate input and output channels and to suppress backaction from different stages on the sample under test. In these devices the usual reciprocal symmetry of circuits is broken by the phenomenon of Faraday rotation based on magnetic materials and fields. However, magnets are averse to on-chip integration, and magnetic fields are deleterious to delicate superconducting devices. Here we present a new proposal combining two stages of parametric modulation emulating the action of a circulator. It is devoid of magnetic components and suitable for on-chip integration. As the design is free of any dissipative elements and based on reversible operation, the device operates noiselessly, giving it an important advantage over other nonreciprocal active devices for quantum information processing applications.Comment: 17 pages, 4 figures + 12 pages Supplementary Informatio

    Test for interlayer coherence in a quasi-two-dimensional superconductor

    Peaks in the magnetoresistivity of the layered superconductor κ\kappa-(BEDT-TTF)2_2Cu(NCS)2_2, measured in fields 45\leq 45 T applied within the layers, show that the Fermi surface is extended in the interlayer direction and enable the interlayer transfer integral (t0.04t_{\perp} \approx 0.04 meV) to be deduced. However, the quasiparticle scattering rate τ1\tau^{-1} is such that /τ6t\hbar/\tau \sim 6t_{\perp}, implying that κ\kappa-(BEDT-TTF)2_2Cu(NCS)2_2 meets the criterion used to identify interlayer incoherence. The applicability of this criterion to anisotropic materials is thus shown to be questionable.Comment: 5 pages, 4 figure
