    AZALEP a randomized controlled trial of azathioprine to treat leprosy nerve damage and Type 1 reactions in India: Main findings.

    BACKGROUND: Leprosy Type 1 reactions are difficult to treat and only 70% of patients respond to steroid treatment. Azathioprine has been used as an immune-suppressant and we tested its efficacy in treating leprosy T1R. METHODOLOGY: Randomised controlled trial adding azathioprine to steroid treatment for leprosy reactions. This trial was conducted in four leprosy hospitals in India. Patients with a new leprosy Type 1 reaction affecting either skin or nerve were recruited. They were given a 20 week course of oral prednisolone either with placebo or azathioprine 50mg for 24, 36 or 48 weeks. Outcomes were measured using a verified combined clinical reaction severity score (CCS) and the score difference between baseline and end of study calculated. An intention to treat analysis was done on the 279 patients who had an outcome. PRINCIPAL FINDINGS: 345 patients were recruited, 145 were lost due to adverse events, loss to follow up or death. 36% needed extra steroids due to a recurrence of their skin and/or nerve reaction. 76% of patients had improvements in their CCS the end of the study, 22% had no change and 1.1% deteriorated. Adding azathioprine to steroid treatment did not improve CCS. So the improvements were attributable to treatment with steroids. We analysed the skin, sensory and motor scores separately and found that skin improvement contributed most with 78.9% of patients having skin improvement, azathioprine treatment for 48 weeks improved sensory scores it also improved motor scores but so did treatment with prednisolone alone. We identified significant adverse effects attributable to steroid treatment. When azathioprine and Dapsone were given together significant numbers of patients developed significant anaemia. CONCLUSIONS: Azathioprine is not recommended for the treatment of leprosy reactions and does not improve steroid treatment. Recurrent reactions are a major challenge. We have also identified that 65% of patients with sensory and 50% with motor nerve damage do not improve. Future studies should test giving azathioprine in the treatment of nerve damage and giving a higher dose for 48 weeks to patients. These findings highlight the difficulty in switching off leprosy inflammation and the need for better treatments for reactions and nerve damage. There is also a research need to identify patients who have recurrences and optimize treatments for them. Patients with recurrences may benefit from combined treatment with steroids and azathioprine. We have also shown that significant numbers of patients treated with steroids develop adverse effects and this needs to be highlighted in leprosy programmes. Research is needed to identify patients who do not respond to steroid treatment and develop alternative treatments for them.     Vibrational analysis of Ag3(PO2NH)3, Na3(PO2NH)3.H2O, Na3(PO2NH)3.4H2O, [C(NH2)3]3(PO2NH)3.H2O and (NH4)4(PO2NH)4.4H2O

    FT IR and FT Raman spectra of Ag3(PO2NH), (Compound I), Na3(PO2NH)3.H2O (Compound II), Na3(PO2NH)3.4H2O (Compound III), [C(NH2)3]3(PO2NH)3.H2O (Compound IV) and (NH4)4(PO2NH)4.4H2O (Compound V) are recorded and analyzed on the basis of the anions, cations and water molecules present in each of them. The PO2NH− anion ring in compound I is distorted due to the influence of Ag+ cation. Wide variation in the hydrogen bond lengths in compound III is indicated by the splitting of the v2 and v3 modes of vibration of water molecules. The NH4 ion in compound V occupies lower site symmetry and exhibits hindered rotation in the lattice. The correlations between the symmetric and asymmetric stretching vibrations of P-N-P bridge and the P-N-P bond angle have also been discussed

    Temperature dependent polarized Raman spectra of nonaaqualanthanoid (Pr) single crystal

    Polarized Raman spectral changes with respect to temperature were investigated for Pr(BrO3)3·9H2O single crystals. FTIR spectra of hydrated and deuterated analogues were also recorded and analysed. Temperature dependent Raman spectral variation have been explained with the help of the thermograms recorded for the crystal. Factor group analysis could propose the appearance ofBrO3 ions at sites corresponding to C3v (4) and D3h (2). Analysis of the vibrational bands at room temperature confirms a distorted C3v symmetry for the BrO3 ion in the crystal. From the vibrations of water molecules, hydrogen bonds of varying strengths have also been identified in the crystal. The appearance υ1 mode of BrO3− anion at lower wavenumber region is attributed to the attachment of hydrogen atoms to the BrO3− anion. At high temperatures, structural rearrangement is taking place for bothH2Omolecule and BrO3 ions leading to the loss ofwater molecules and structural reorientation of bromate ions causing phase transition of the crystal at the temperature of 447 K.Spectrochimica Acta Part A 64 (2006) 518–52

    Structural, Optical, and Compactness Characteristics of Nanocrystalline CaNb2O6 Synthesized through an Autoigniting Combustion Method

    Nanoparticles of calcium metaniobate compound are prepared by an autoigniting combustion technique and its structural, optical, and dielectric properties are investigated. The X-ray diffraction, Fourier-transform Raman, and infrared studies reveal that calcium metaniobate possesses phase pure orthorhombic columbite structure with space group of Pbcn. The average particle size of the as-prepared nanoparticles obtained from both the Scherrer formula and transmission electron microscopy is ~37 nm. The optical band gap calculated from Tauc's Plot is 3.25 eV. Photoluminescence studies reveal that Calcium metaniobate can be used as an idealphotoluminarmaterial. The powders are pelletised and sintered at an optimized temperature of 1350∘C in a short duration of two hours, yielding a high density. The morphology of the sintered pellet is further examined using scanning electron microscopy. The dielectric constant and loss factor values measured at 5 MHz for a well-sintered Calcium metaniobate pellet are found to be 27.6 and 5.3×10−4 respectively, at room temperature

    Ziel des Verbundprojektes war die Verbesserung der Hochtemperaturbestaendigkeit von Werkstoffen auf Basis Siliciumnitrid (Si_3N_4) sowie die technische, oekonomische und prozesssichere Herstellbarkeit daraus gefertigter Teile. Es wurden zwei Werkstoffqualitaeten entwickelt, ein auf #beta#-Si_3N_4 basierender Werkstoff mit kristalliner Korngrenzphase ('cgb-#beta#-SN') sowie ein Yb-#alpha#'/#beta#'-SiAlON, die beide deutlich verbesserte Hochtemperatureigenschaften gegenueber dem Stand zu Beginn des Projektes aufweisen.
