    Prova de Trabalho de Parto Após uma Cesárea Anterior

    Objetivo: estudar a validade da prova de trabalho de parto (PTP) em gestantes com uma cesárea anterior. Métodos: estudo retrospectivo, tipo coorte, incluindo 438 gestantes com uma cesárea anterior ao parto em estudo e seus 450 recém-nascidos (RN), divididas em dois grupos - com e sem PTP. O tamanho amostral mínimo foi de 121 gestantes/grupo. Considerou-se variável independente a PTP e as dependentes relacionaram-se à ocorrência de parto vaginal e à freqüência de complicações maternas e perinatais. Foram efetuadas análises uni e multivariada, respectivamente. A comparação entre as freqüências (%) foi analisada pelo teste do qui-quadrado (chi²) com significância de 5% e regressão logística com cálculo do odds ratio (OR) e do intervalo de confiança a 95% (IC95%). Resultados: a PTP associou-se a 59,2% de partos vaginais. Foi menos indicada nas gestantes com mais de 40 anos (2,7% vs 6,5%) e nas portadoras de doenças associadas e complicações da gravidez: síndromes hipertensivas (7,0%) e hemorragias de 3º trimestre (0,3%). A PTP não se relacionou às complicações maternas e perinatais. As gestantes que tiveram o parto por cesárea, independente da PTP, apresentaram maior risco de complicações puerperais (OR = 3,53; IC95% = 1,57-7,93). A taxa de mortalidade perinatal foi dependente do peso do RN e das malformações fetais e não se relacionou à PTP. Ao contrário, as complicações respiratórias foram mais freqüentes nos RN de mães não testadas quanto à PTP (OR = 1,92; IC95% = 1,20-3,07). Conclusões: os resultados comprovaram que a PTP em gestantes com uma cesárea anterior é estratégia segura - favoreceu o parto vaginal em 59,2% dos casos e não interferiu com a morbimortalidade materna e perinatal. Portanto, é recurso que deve ser estimulado.Purpose: to study trial of labor (TOL) for vaginal birth after one previous cesarean section. Methods: this is a retrospective cohort study that included 438 pregnant women with one previous cesarean section and their 450 newborns. They were divided into two groups - with and without TOL. The minimum sample size was 121 pregnant mothers per group. TOL was considered as an independent variable and vaginal birth and maternal and perinatal complication frequency as dependent variables. Both univariate and multivariate analyses were performed. The comparison of observed frequencies (%) was analyzed by the chi-squared test (chi²) with 5% significance, and linear regression from the odds ratio (OR) and confidence interval of 95% (CI95%). Results: TOL was used in 59.2% of vaginal deliveries. It was less used in women over 40 years (2.7% vs 6.7%) and in those with clinical or obstetrical diseases such as arterial hypertension (7.0%) and bleeding in the third trimester (0.3%). There was a higher risk for puerperal complications with cesarean deliveries (OR = 3.53, CI 95% = 1.57-7.93), independent of TOL. Perinatal mortality was dependent on neonatal weight and fetal malformations, not on TOL. Newborns from mothers not submitted to TOL were at a higher risk for developing breathing complications (OR = 1.92 CI 95% = 1.20-3.07). Conclusions: The results confirm that trial of labor after a previous cesarean section is a safe method - assisting vaginal delivery in 59.2% of births and not interfering with maternal and perinatal mortality. It is a treatment that should be stimulated

    Pro-angiogenic approach for skeletal muscle regeneration

    The angiogenesis process is a phenomenon in which numerous molecules participate in the stimulation of the new vessels' formation from pre-existing vessels. Angiogenesis is a crucial step in tissue regeneration and recovery of organ and tissue function. Muscle diseases affect millions of people worldwide overcome the ability of skeletal muscle to self-repair. Pro-angiogenic therapies are key in skeletal muscle regeneration where both myogenesis and angiogenesis occur. These therapies have been based on mesenchymal stem cells (MSCs), exo-somes, microRNAs (miRs) and delivery of biological factors. The use of different calls of biomaterials is another approach, including ceramics, composites, and polymers. Natural polymers are use due its bioactivity and biocompatibility in addition to its use as scaffolds and in drug delivery systems. One of these polymers is the natural rubber latex (NRL) which is biocompatible, bioactive, versatile, low-costing, and capable of promoting tissue regeneration and angiogenesis.     Pressão arterial e freqüência cardíaca avaliadas pela MAPA em primigestas durante o trabalho de parto e puerpério imediato

    OBJETIVO: analisar os valores da pressão arterial e da freqüência cardíaca durante o trabalho de parto e puerpério imediato de primigestas normais. MÉTODOS: foram incluídas no estudo 60 parturientes, às quais foi aplicada a monitorização ambulatorial da pressão arterial (MAPA) com uso do aparelho modelo SpaceLabs 90207, durante o trabalho de parto e nas primeiras 12 horas após o mesmo. A pressão arterial e a freqüência cardíaca foram registradas a cada 15 minutos durante o trabalho de parto e na primeira hora após o parto, e a cada 30 minutos até a 12ª hora após o parto. Esses parâmetros foram avaliados em três momentos do trabalho de parto (com dilatação cervical até 7 cm, entre 8 cm e dilatação total e durante o período expulsivo) e em dois momentos do puerpério (na primeira e décima segunda hora). Primeiramente os resultados foram analisados sem levar em consideração o tipo de procedimento anestésico, e depois, dividindo-os em grupos conforme o tipo de procedimento realizado: anestesia local, analgesia peridural lombar e anestesia subaracnóidea. Para comparação dentro de cada grupo foram realizados análise de variância (ANOVA) e teste t de Student pareado e, entre os grupos, o teste t não-pareado. Foi considerado o limite de significância estatística de 5%. RESULTADOS: quando os resultados foram avaliados sem levar em consideração o procedimento anestésico, os valores médios da pressão arterial sistólica, registrados durante o trabalho de parto, foram significativamente mais elevados que no puerpério. Durante o trabalho de parto esses valores foram significativamente mais elevados nos períodos de dilatação final e expulsivo que no período de dilatação inicial e, na 12ª hora, inferiores ao da primeira hora após o parto. Os valores médios da pressão arterial diastólica registrados durante o trabalho de parto foram significativamente mais altos que no puerpério. Estes valores não apresentaram alteração durante os diferentes períodos do trabalho de parto ou durante as primeiras doze horas do puerpério. A freqüência cardíaca aumentou progressivamente durante o trabalho de parto, diminuindo nas primeiras 12 horas após o parto. Quando a pressão arterial e a freqüência cardíaca foram avaliadas conforme o tipo de procedimento anestésico empregado, ambas mostraram-se com mesmo comportamento na anestesia local e peridural, embora naquelas submetidas à anestesia subaracnóidea, tanto a pressão arterial sistólica como a diastólica não apresentaram alteração durante o trabalho de parto. CONCLUSÕES: o trabalho de parto determinou aumento da pressão arterial sistólica e da freqüência cardíaca. Durante o trabalho de parto os valores das pressões arteriais sistólica e diastólica foram mais altos que nas primeiras 12 horas do puerpério, havendo queda significativa entre a primeira e a décima segunda hora do mesmo. Diferentes procedimentos anestésicos não interferiram nos valores das pressões arteriais sistólica e diastólica ou da freqüência cardíaca, durante o trabalho de parto e nas primeiras doze horas após o parto.OBJECTIVE: to analyze the maternal blood pressure and heart rate variation of primigravid women during labor and early puerperium. METHODS: sixty primigravid women were included in the study, and submitted to ambulatory blood pressure monitoring (ABPM) with SpaceLabs 90207 monitor during labor and the first 12 h of puerperium. The records of blood pressure and heart rate were done every 15 min during labor and every 30 min during the first 12 h of puerperium. Three periods during labor (until cervix dilated 7 cm, cervix dilated between 8 cm and total dilatation, and delivery period) and two during puerperium (first and twelfth hours), were analyzed. First of all the results were analyzed without considering the kind of analgesia used and then the patients were divided into three groups, according to the anesthetic technique: local, lumbar extradural or subarachnoid. Results were analyzed by one-way analysis of variance (ANOVA) and paired Student's t-test for blood pressure and heart rate in each group during labor and puerperium. The nonpaired Student's t-test was used to compare different groups. A p value < 0.05 was regarded as statistically significant. RESULTS: when the results were analyzed without considering the analgesic procedure, the values of systolic blood pressure during labor were significantly higher than in early puerperium. During labor, systolic blood pressure values were higher in the periods of later cervical dilatation and delivery than during early cervical dilatation. In the 12th h of puerperium the systolic blood pressure was lower than in the first hour. Diastolic blood pressure did not change during labor and was higher than in early puerperium. Heart rate increased during labor and decreased during puerperium. The systolic and diastolic blood pressure and heart rate were the same both in local or lumbar extradural anesthesia groups; however, in the subarachnoid group the systolic and diastolic blood pressure did not change during labor. CONCLUSIONS: labor increased systolic blood pressure and heart rate. During labor, systolic and diastolic blood pressure were higher than in early puerperium. Both blood pressure and heart rate significantly fell from the first to the 12th hour of puerperium. The different anesthetic techniques did not affect blood pressure or heart rate, as compared with the primigravid group when the anesthetic technique was not taken into consideration

    OBJECTIVE - To assess drug reactions and report the drugs involved and the most frequent types of skin reactions.METHODS - A retrospective and descriptive study. Data of inpatients at the Dermatology Ward with initial diagnosis of adverse drug reactions were evaluated from January 1999 to June 2004. Patients with confirmed diagnosis were included in the study based on clinical and histopathological criteria, after analysis of medical charts.RESULTS - Initial diagnosis of adverse drug reactions was confirmed in 121 patients. Forty-three patients were included in the study; 51.2% were females and 86% were caucasians. A total of 48,8% were on one drug only. Antibiotics were the most commonly used drug (20%) and accounted for 33% of the drug eruptions. The second group comprised anti-inflammatory drugs (16 7%), followed by anticonvulsants (13%), analgesic/antipyretic (13%.) agents. Skin eruption manifested as maculopapular exanthema in 41.9% patients, erythrodermia in 25.6%, and urticaria in 23.3%.CONCLUSION - Maculopapular exanthema was the main type of skin reaction triggered by use of drugs, and these reactions were most frequently caused by antibiotics

    Gestantes vivendo com HIV/AIDS: Características antropométricas e peso ao nascer dos seus recém-nascidos

    PURPOSE: To describe the anthropometric and pregnancy characteristics of women with HIV/AIDS, assisted by the Brazilian National Health System and the birth weight of their newborns. METHODS: The participants were women assisted at public STD/AIDS clinics of the Municipal Health system of São Paulo. The anthropometric characteristics were evaluated by trained nutritionists and other information was obtained from the medical records. For comparison of the survey data to those of the general population, secondary maternal and pregnancy data were obtained from live birth certificates through the Live Birth Information System. Continuous variables were summarized as mean and standard deviation or as the 25th, 50th and 75th percentiles and minimum and maximum values. The other variables are presented as percentages. Means were compared by the Student's t-test or Kruskal-Wallis test depending on the fulfillment of assumptions, with the decision based on the p value. RESULTS: We found the presence of inadequate maternal nutrition according to triceps skinfold (60.9%). The BMI/gestational age showed the presence of underweight (18.5%) and overweight or obesity (40%). There was no association between disease status (HIV or AIDS) and weight, height, and lean or fat mass. Mean newborn birth weight was lower than the value for the general population without infection or disease. The results of this study indicate the need to develop adapted curves to allow a more accurate nutritional assessment of this population group

    Markers of Endothelial Dysfunction Are Attenuated by Resveratrol in Preeclampsia

    Preeclampsia (PE) is characterized by great endothelial dysfunction, decreased nitric oxide (NO) bioavailability, and higher levels of arginase activity. In the present study, we investigated the potential modulatory effects of trans-resveratrol (RSV) on arginase and endothelial dysfunction biomarkers in endothelial cells exposed to plasma from patients with PE and healthy pregnant (HP) women, and umbilical arteries from patients with PE. Human umbilical vein endothelial cells (HUVECs) were incubated with pooled plasma from 10 HP or 10 PE pregnant women and RSV; umbilical arteries from patients with PE were incubated with RSV; intracellular NO and total reactive oxygen species (ROS) levels were assessed using a probe that interacted with these radicals; total arginase activity was evaluated measuring the urea produced; total antioxidant capacity was measured using the ferric reduction ability power (FRAP) assay; and endothelial dysfunction biomarkers were assessed using qPCR in endothelial cells and umbilical arteries. RSV increased NO levels and decreased total arginase activity in endothelial cells incubated with plasma from patients with PE. In addition, RSV increased total antioxidant capacity and downregulated endothelial dysfunction biomarkers, such as intercellular adhesion molecule-1 (ICAM-1), von Willebrand factor (vWF), and Caspase-3, (CASP-3), in endothelial cells and umbilical arteries from PE patients. RSV treatment positively modulated the L-arginine&ndash;NO pathway, decreased arginase activity, and increased antioxidant capacity, in addition to downregulating endothelial dysfunction biomarkers

    Effect of maternal hydration on the increase of amniotic fluid index

    The objective of this study was to determine the effect of maternal hydration with oral isotonic solution and water on the amniotic fluid (AF) index of women with normohydramnios. Women with a normal AF index and gestational age between 33 and 36 weeks without maternal complications were randomized into three groups [isotonic solution (Gatorade (R)), water, control]. The isotonic solution and water groups were instructed to drink 1.5 L of the respective solution and the control group was instructed to drink 200 mL water over a period of 2 to 4 h. AF index was measured before and after hydration by Doppler ultrasonography. The investigator performing the AF index measurement was blind to the subject's group. Ninety-nine women completed the study without any adverse maternal effects. The median increase in AF index after hydration was significantly greater for the isotonic solution and water groups than for the control group. There was no significant difference between the isotonic solution and water groups. Hydration with isotonic solution and water caused a 10-fold (95% CI: 2.09-49.89) and 6-fold (95% CI: 1.16-30.95) increase in the chance of a 20% increase of AF index, respectively. Maternal hydration with isotonic solution or water increased the AF index in women with normohydramnios

    Hipertireoidismo na gravidez: repercussões materno-fetais

    OBJETIVO: avaliar a experiência do Hospital das Clínicas da Faculdade de Medicina de Botucatu da Universidade Estadual Paulista Júlio de Mesquita Filho, no acompanhamento de gestantes com hipertireoidismo. MÉTODOS: foram estudadas, retrospectivamente, 60 pacientes, divididas em grupos com hipertireoidismo compensado (GHC=24) e com hipertireoidismo descompensado (GHD=36) e comparadas quanto a características clínico-laboratoriais e intercorrências. Para análise dos resultados, foram utilizados o teste t de Student, as tabelas de contingência, regressão linear múltipla e regressão logística múltipla, com nível de significância de 5,0%. RESULTADOS: propiltiouracil (PTU) foi usado por 94,0% do GHD e 42,0% do GHC (p<0,0001); complicações maternas próximas ao parto ocorreram em 20,6% do GHD e 11,8% do GHC, sendo que o GHD apresentou três óbitos fetais. Influenciaram nestes: idade materna, nível de T4L (nT4L) e dose de PTU (dPTU) mais elevados, no terceiro trimestre (p=0,007); restrição de crescimento intra-uterino, influenciada por nT4L e dPTU do terceiro trimestre, ocorreu em nove casos do GHD e três do GHC, e oligoâmnio ocorreu em 12 pacientes (83,3% do GHD, 16,7% do GHC), influenciado por idade e nT4L do terceiro trimestre (p=0,04); a idade gestacional no parto foi de 34,4±4,6 semanas no GHD e 37,0±2,5 no GHC, influenciada pelo nT4L do terceiro trimestre (p<0,05). CONCLUSÕES: o GHD apresentou resultados menos satisfatórios que o GHC, influenciados por nT4L e dPTU elevados no terceiro trimestre e por idade mais avançada de algumas gestantes.PURPOSE: to evaluate the experience of Hospital das Clínicas da Faculdade de Medicina de Botucatu da Universidade Estadual Paulista Júlio de Mesquita Filho, in the follow-up of pregnant women with hyperthyroidism. The t-Student test, contingency tables, multiple linear regression and multiple logistic regression with significance level at 5.0% were used. RESULTS: propylthiouracil (PTU) was used by 94.0% of UHG and by 42.0% of CHG (p<0.0001); maternal complications close to delivery have occurred in 20.6% of UHG and in 11.8% of CHG, and UHG presented three fetal deaths, influenced by the mother age, higher level of T4L (lT4L) and of PTU dose (PTUd) in the third trimester (p=0.007); restriction of intra-uterine growth, influenced by lT4L and PTUd in the third trimester has occurred in nine UHG and in three CHG cases, and oligoamnios has occurred in 12 patients (83.3% of UGH and 16.7% of CGH), influenced by age and lT4L in the third trimester (p=0.04); the gestational age at delivery was 34.4±4.6 weeks in UHG and 37.0±2.5 in CHG, influenced by the T4Ll in the third trimester (p<0.05).
