    Peritoneal Dialysis for Chronic Congestive Heart Failure

    Maladaptive responses between a failing heart and the kidneys ultimately lead to permanent chronic kidney disease, referred to as cardiorenal syndrome type 2. In this narrative review, we discuss the pathophysiological pathways in the progression of cardiorenal failure and review the current evidence on peritoneal dialysis as a treatment strategy in cardiorenal syndrome type 2. A patient with heart failure can present with clinical symptoms related to venous congestion even in the absence of end-stage renal disease. Diuretics remain the cornerstone for the treatment of fluid overload related to heart failure. However, with chronic use, diuretic resistance can supervene. When medical therapy is no longer able to relieve congestive symptoms, ultrafiltration might be needed. Patients with heart failure tolerate well the gentle rate of fluid removal through peritoneal dialysis. Recent publications suggest a positive impact of starting peritoneal dialysis in patients with cardiorenal syndrome type 2 on the hospitalisation rate, functional status and quality of life

    Abstract Background Encapsulating peritoneal sclerosis (EPS) is classically described as progressive sclerosis and cocooning of the entire peritoneum; however, there has been limited number of reported cases of localized fibrosis as a variant form. Case presentation We describe two cases of acute bowel obstruction with isolated transition points in the setting of long-term peritoneal dialysis. Conclusion We postulate that some of the cases of small bowel obstruction with an obvious transition point in long-term peritoneal dialysis patients may represent a unique and localized form of EPS. We aim to emphasize the presence of macroscopic variations in presentation of EPS

    Treatment of asymptomatic hyperuricemia in chronic kidney disease: A new target in an old enemy – A review

    Asymptomatic hyperuricemia is increasing in prevalence. There is a growing body of literature suggesting that uric acid has deleterious effects on vascular health and renal histological integrity. Several trials, reviewed herein, suggest that lowering the serum uric acid level is associated with a slowing in the rate of renal deterioration in those with chronic kidney disease. Given that there is little available in the general armamentarium to slow the rate of kidney deterioration, strong consideration could be given to the administration of agents or lifestyle changes that decrease uric acid production in hyperuricemic patients with deteriorating kidney function
