Acupuncture in Treatment of Chronic Functional Constipation
Constipation is not only a symptom but is a predominant presenting symptom in many diseases. The prevalence is between 3 and 27% worldwide, and is especially prevalent in the elderly population. The aetiology is multifactorial. Laxative abuse or enema use are usually a norm in patients’ constipation. Patients tend not to seek further medical aid after several unsuccessful therapies and this can seriously affect their quality of life
Analgesia for total knee arthroplasty: a meta-analysis comparing local infiltration and femoral nerve block
Patients frequently experience postoperative pain after a total knee arthroplasty; such pain is always challenging to treat and may delay the patient’s recovery. It is unclear whether local infiltration or a femoral nerve block offers a better analgesic effect after total knee arthroplasty.We performed a systematic review and meta-analysis of randomized controlled trials to compare local infiltration with a femoral nerve block in patients who underwent a primary unilateral total knee arthroplasty. We searched Pubmed, EMBASE, and the Cochrane Library through December 2014. Two reviewers scanned abstracts and extracted data. The data collected included numeric rating scale values for pain at rest and pain upon movement and opioid consumption in the first 24 hours. Mean differences with 95% confidence intervals were calculated for each end point. A sensitivity analysis was conducted to evaluate potential sources of heterogeneity.While the numeric rating scale values for pain upon movement (MD-0.62; 95%CI: -1.13 to -0.12; p=0.02) in the first 24 hours differed significantly between the patients who received local infiltration and those who received a femoral nerve block, there were no differences in the numeric rating scale results for pain at rest (MD-0.42; 95%CI:-1.32 to 0.47; p=0.35) or opioid consumption (MD 2.92; 95%CI:-1.32 to 7.16; p=0.18) in the first 24 hours.Local infiltration and femoral nerve block showed no significant differences in pain intensity at rest or opioid consumption after total knee arthroplasty, but the femoral nerve block was associated with reduced pain upon movement

Phase noise cancellation in coherent communication systems using a radio frequency pilot tone
Long-haul optical fiber communication employing digital signal processing (DSP)-based dispersion compensation can be distorted by the phenomenon of equalization-enhanced phase noise (EEPN), due to the reciprocities between the dispersion compensation unit and the local oscillator (LO) laser phase noise (LPN). The impact of EEPN scales increases with the increase of the fiber dispersion, laser linewidths, symbol rates, signal bandwidths, and the order of modulation formats. In this work, the phase noise cancellation (PNC) employing a radio frequency (RF) pilot tone in coherent optical transmission systems has been investigated. A 28-Gsym/s QPSK optical transmission system with a significant EEPN has been implemented, where the carrier phase recovery (CPR) was realized using the one-tap normalized least-mean-square (NLMS) estimation and the differential phase detection (DPD), respectively. It is shown that the RF pilot tone can entirely eliminate the LPN and efficiently suppress the EEPN when it is applied prior to the CPR
Low Serum Vitamin D Is Associated with Anti-Thyroid-Globulin Antibody in Female Individuals
Objectives. Some evidence has pointed out that vitamin D plays a significant role in reducing the incidence of autoimmune diseases, especially autoimmune thyroid diseases. The authors aimed to examine the relationship between circulating 25-hydroxyvitamin D and thyroid autoantibody in a population-based health survey of Xinjiang Chinese population. Subjects and Methods. A total of 1714 Chinese adults were analyzed. 25(OH)D, anti-thyroid antibodies, and thyroid function were measured. Results. The prevalence of vitamin D insufficiency was 28.3% in Hans and 9.3% in Uyghurs, and the prevalence of vitamin D deficiency was 61.6% in Hans and 87.6% in Uyghurs. Overall prevalence of TgAb positivity was 6.2% (0.9% males; 5.3% females). In female subjects, mean serum 25(OH)D levels were significantly lower in Hans and Uyghurs compared with males, and the difference was statistically significant. Importantly, after adjusting for age and ethnicity, a negative correlation (r=-0.121, P=0.014) was recognized between 25(OH)D and TgAb levels only in female subjects. Conclusion. Vitamin D insufficiency and deficiency are prevalent among Chinese adults. Low serum 25(OH)D is related to the presence of TgAb in females. The causal effect of low vitamin D level on thyroid autoimmunity should be studied further more

Investigation into antiepileptic effect of ganoderic acid A and its mechanism in seizure rats induced by pentylenetetrazole
Ganoderic acid A (GAA) exhibited neuron protection in in vitro epilepsy study, but no study has been done in vivo. Rats were
administered (i.p.) pentylenetetrazole daily for 28 days to induce seizure. Rats with grade II or above of epileptic score were
divided into three groups and given placebo, sodium valproate, or GAA treatment, respectively, for 7 days. The electrical
signals of brain were monitored with electroencephalography (EGG); epileptic behavior was assessed using the Racine scale;
morphological changes and apoptosis rate of cortical neurons were assessed with H&E staining and TUNEL staining,
respectively. Protein expression of calcium-sensing receptor, p-ERK, p-JNK, and p-p38 in hippocampal tissue and Bcl-2,
cleaved caspase-3, and Bax in cortical tissues was observed by Western blot and immunohistochemistry assay, respectively.
After GAA treatment, apparent seizure-like EEG with significant arrhythmic disorder and spike waves was reduced or
disappeared, and wave amplitude of EEG was reduced significantly. GAA showed similar effect with sodium valproate
treatments on epilepsy. There were an apparent improvement of the epileptic behavior and a significant increase in the
epileptic latency and shortening of the epileptic duration in the treatment group compared to control. GAA treatment
ameliorated the nuclear pyknosis of neurons which appeared seriously in the epilepsy group. GAA treatment significantly
reduced the cortical neuron apoptosis of epilepsy and the expression of calcium-sensing receptor, p-P38, p-JNK, cleaved
caspase-3, and Bax but increased the expression of both p-ERK and Bcl-2. In conclusion, GAA treatment showed strong
antiepileptic effect by decreasing apoptosis in cortical neuron and the expression of calcium-sensing receptor and stimulating
the MAPK pathwa
Detection of mismatch repair gene germline mutation carrier among Chinese population with colorectal cancer
<p>Abstract</p> <p>Background</p> <p>Hereditary nonpolyposis colorectal cancer (HNPCC) is an autosomal dominant syndrome. The National Cancer Institute (NCI) has recommended the Revised Bethesda guidelines for screening HNPCC. There has been a great deal of research on the value of these tests in other countries. However, literature about the Chinese population is scarce. Our objective is to detect and study microsatellite instability (MSI) and mismatch repair (MMR) gene germline mutation carriers among a Chinese population with colorectal cancer.</p> <p>Methods</p> <p>In 146 prospectively recruited consecutive patients with clinically proven colorectal cancer, MSI carriers were identified by analysis of tumor tissue using multiplex fluorescence polymerase chain reaction (PCR) using the NCI recommended panel and classified into microsatellite instability-low (MSI-L), microsatellite instability-high (MSI-H) and microsatellite stable (MSS) groups. Immunohistochemical staining for MSH2, MSH6 and MLH1 on tissue microarrays (TMAs) was performed, and methylation of the MLH1 promoter was analyzed by quantitative methylation specific PCR (MSP). Germline mutation analysis of blood samples was performed for MSH2, MSH6 and MLH1 genes.</p> <p>Results</p> <p>Thirty-four out of the 146 colorectal cancers (CRCs, 23.2%) were MSI, including 19 MSI-H CRCs and 15 MSI-L CRCS. Negative staining for MSH2 was found in 8 CRCs, negative staining for MSH6 was found in 6 CRCs. One MSI-H CRC was negative for both MSH6 and MSH2. Seventeen CRCs stained negatively for MLH1. MLH1 promoter methylation was determined in 34 MSI CRCs. Hypermethylation of the MLH1 promoter occurred in 14 (73.7%) out of 19 MSI-H CRCs and 5 (33.3%) out of 15 MSI-L CRCs. Among the 34 MSI carriers and one MSS CRC with MLH1 negative staining, 8 had a MMR gene germline mutation, which accounted for 23.5% of all MSI colorectal cancers and 5.5% of all the colorectal cancers. Five patients harbored MSH2 germline mutations, and three patients harbored MSH6 germline mutations. None of the patients had an MLH1 mutation. Mutations were commonly located in exon 7 and 12 of MSH2 and exon 5 of MSH6. Right colonic lesions and mucinous carcinoma were not common in MSI carriers.</p> <p>Conclusion</p> <p>Our data may imply that the characteristics of HNPCC in the Chinese population are probably different from those of Western countries. Application of NCI recommended criteria may not be effective enough to identify Chinese HNPCC families. Further studies are necessary to echo or refute our results so as to make the NCI recommendation more universally applicable.</p
In-plane uniaxial strain in black phosphorus enables the identification of crystalline orientation
The identification of the crystalline axis of anisotropic black phosphorus
(BP) is key for the study of its physical properties and for its optical and
electronic applications. Herein, we show that by applying an in-plane uniaxial
strain and measuring the changes of Raman shifts, the crystalline axis of BP
can be reliably determined. The strain effects on Raman shifts are
angle-dependent, which can be expressed as a combination of Raman responses
under zigzag and armchair strains. Different from previous polarized optical
spectroscopic means, the proposed method does not rely on the laser
polarization, the excitation wavelength, the sample thickness and the material
of the stretchable substrate. Besides, the effective strain applied on BP from
the stretched substrate is estimated, showing that only 20% to 40% of the
strain can be effectively transferred to BP flakes from polyethylene
terephthalate substrates. Our method provides not only an effective and
universal approach to identify the crystalline orientation of layered BP, but
also a model to extract additional information in strain-related studies, and

Classification of young males’ shoulder shapes based on cross-sectional morphological characteristics
This paper focused on the different characteristics of the shoulder cross-section curves closely related to the shape to subdivide the shoulder shapes. In this paper, 213 young college male students aged 18-26 were selected to measure the shoulder data with three-dimensional body scanner. With the help of imageware12.0 and matlabr2012b software, the cross-section curves which could be used to classify the shoulder shapes were extracted, and the method of subdividing the shoulder shapes with the curvature radius of the characteristic points of the cross-section curve and the ratio of sagittal to frontal diameter was established. K-means clustering method was used through dynamic clustering, the optimal classification number of shoulder shapes was determined to be 4 categories by variance analysis, and the shape differences of each shoulder shape were quantified; by comparing the curve shape of shoulder section, the curve change characteristics of 4 categories of shoulder section were further qualitatively described
