Mechanizm działania niesteroidowych leków przeciwzapalnych
Niesteroidowe leki przeciwzapalne (NLPZ) stanowią różnorodną grupą
związków. Leki te wykazują działanie przeciwbólowe, przeciwzapalne i przeciwgorączkowe
w następstwie hamowania aktywności cyklooksygenaz (COX). W zależności od
właściwości hamowania aktywności cyklooksygenaz, COX-1 i COX-2 NLPZ można
podzielić na cztery grupy: leki swoiście hamujące COX-1, leki nieswoiście hamujące
COX-1 i COX-2 oraz koksyby I i II generacji. Jednym z najpowszechniej stosowanych
NLPZ jest aspiryna. Lecznicze funkcje aspiryny spełnia tylko niezhydrolizowana forma
tego związku. Niesteroidowe leki przeciwzapalne poprzez wielokierunkowość mechanizmu
działania powodują wystąpienie licznych, nie do końca poznanych, działań niepożądanych.
Wiadome jest, że znoszą one działanie ochronne prostaglandyn dla błony śluzowej
przewodu pokarmowego, zwiększają prawdopodobieństwo wystąpienia krwotoków,
mogą doprowadzać do skurczu oskrzeli, wywoływać reakcje uczuleniowe, powodować
hepatotoksyczność, neurotoksyczność i zmiany skórne.
Następstwa niepożądane przy przewlekłym stosowaniu NLPZ mogą być groźne dla zdrowia,
a nawet życia pacjenta. Wszyscy członkowie zespołu terapeutycznego powinni zatem
szczegółowo znać szerokie spektrum działania tych leków.Non-steroidal anti-infl ammatory drugs (NSAIDs) are a diverse group of compounds.
These drugs have analgesic, antiinfl ammatory and antipyretic activity following
the inhibition of cyclooxygenase (COX). NSAIDs are classifi ed into four groups, depending
on the inhibition of cyclooxygenase activity of COX-1 and COX-2: drugs specifi -
cally inhibiting COX-1, the drugs that inhibit non-specifi c COX-1 and COX-2 and coxibs
and second generation. One of the most commonly used NSAID is aspirin. The curative
features of aspirin has only the hydrolysed form of this compound. NSAIDs mechanism
of action results is not fully known, by multifunctionality. It is known that they abolish
the protective eff ect of prostaglandin for the lining of the gastrointestinal tract, increase
the likelihood of bleeding, may lead to bronchoconstriction, cause allergic reactions,
cause liver toxicity, neurotoxicity, and skin changes. Consequences of the chronic use
of NSAIDs may be dangerous to health and even life of the patient. All members of the
therapeutic team should therefore detail to know the broad spectrum of action of these
drugs

BCC rarely methasizes but can be locally invasive and destructive to neighboring tissues. It is composed of cells that arise from the epidermis and its appendages. Basal cell carcinoma is mostly seen in elderly person, especially those over 60 years of age. Its frequency is slightly higher in males than in females. Cumulative UV exposure and severe sunburn during childhood and adolescence are risk factors for basal cell carcinoma. Th e incidents of BCC also increases with Fitzpatrick skin type I and II, fair or red hair, blue eyes, exposures to trivalent arsenic and ionizing radiation. 90% of BCC occurs on sun-exposed areas such as the face, neck, ears and scalp. Tumor suppressor genes and oncogenes are two basis classes of genes that undergo mutations leading to BCC. Disruption of the Hedgehog signaling pathway, Ras genes and p53 supressing functions are the most important in this tumor. It appears as a slowly growing, translucent papule or nodule. It shows a small capillaries fi lled with blood, known as telangiectasia. This painless change is in many cases ignored by patients, what signifi cantly worsens the prognosis for further treatment. Not without signifi cance in the context of medical treatment of patients with BCC is correct diagnosis. Heterogeneity of cells forming metastatic cancer, diff erentiated location or tumor size is out of the sensitivity of diagnostic imaging methods available are the features of diff use malignant contributing to high mortality among cancer patients.

Porównanie aktywności esterazy aspirynowej i cholinesterazy w grupach osób zdrowych i chorych
Artykuł recenzowany / peer-reviewed articleEsteraza aspirynowa (EC 3.1.1.55) jest enzymem katalizującym hydrolizę
kwasu acetylosalicylowego (aspiryny) do kwasu salicylowego i kwasu octowego. Esteraza
aspirynowa jest obecna między innymi w wątrobie, krwinkach czerwonych i osoczu.
Uważa się powszechnie, że jest ona różna od karboksylesterazy (EC 3.1.1.1), arylesterazy
(EC 3.1.1.7) i cholinesterazy (EC 3.1.1.8). Pomiędzy cholinesterazą a esterazą aspirynową
podobieństwa są tak znaczne, że według niektórych autorów, te dwa enzymy są
identyczne. Sporu dotąd nie rozstrzygnięto.
W pracy mierzono równolegle aktywność esterazy aspirynowej i cholinesterazy
w 106 próbkach surowicy pochodzącej od zdrowych dawców krwi oraz w 120 próbkach
surowicy pacjentów, aktywności wyrażano w μkat/L. Stwierdzono bardzo wysoką, pozytywną korelację pomiędzy tymi dwiema aktywnościami – aktywność esterazy aspirynowej była około 10-krotnie niższa niż aktywność cholinesterazy. Wyniki badań wskazują, że występują dwie różne aktywności, choć niekoniecznie dwa różne białka. Warto wziąć pod uwagę możliwość, że cząsteczka enzymu posiada dwa niezależne centra aktywne. Molekularne podstawy podobieństw i różnic tych dwóch enzymów będą wyjaśniane
Zmiany hiperpigmentacyjne jako niepożądany efekt działania wybranych leków
W pracy przedstawiono mechanizm powstawania pigmentu skóry – melaniny
oraz przemiany tego barwnika, które pod wpływem czynników wewnętrznych i zewnętrznych,
mogą predysponować do powstania zmian hiperpigmentacyjnych w obrębie
skóry. Zmiany barwnikowe będące efektem niepożądanym działania leków stanowią
10–20% wszystkich zmian hiperpigmentacyjnych. Defekt ten związany jest ze zwiększoną
syntezą melaniny, zwiększoną produkcją lipofuscyny, odkładaniem się w skórze
związków farmakologicznych, a także bardzo często stanowią efekt ustępowania stanu
zapalnego. Czynnikiem o istotnym znaczeniu w mechanizmie powstawania tego typu
zmian jest współdziałanie promieniowania UV. Istnieje bardzo duża grupa leków oraz
substancji pochodzenia roślinnego, które uwrażliwiają skórę na działanie promieniowania.
Zaliczyć można tu głównie antybiotyki, leki przeciwarytmiczne, hormonalne, cytostatyki
oraz suplementy diety zawierające w swoim składzie dziurawiec, seler, arcydzięgiel
czy rutę zwyczajną.The work shows the mechanism of creating pigmentation i.e. melanin as well
as its transformation which due to external or internal factors could predispose to skin
hyperpigmentation. The changes in the skin color that are the side eff ect of drugs are
10–20% of all hyperpigmentations. It is a result of hypersynthesis of melanin, increased
formation of lipofuscin, cumulating pharmacological compounds and often it can outgoing
infl ammatory condition. UV radiation has been found an important concomitant factor
in causing this type of skin change. There is a large group of pharmaceutical drugs and
botanical extracts that could sensitize skin to UV radiation. This includes mainly antibiotics,
antiarrhythmic drugs, hormonal drugs, cytostatic as well as supplements containing
St John’s wort, celery, Angelica archangelica or rue

Wielonienasycone kwasy tłuszczowe z rodziny omega-3 stanowią niezbędny
składnik fosfolipidów błon komórkowych tkanki mózgowej i włókien nerwowych,
przez co mają wpływ na rozwój i funkcjonowanie mózgu, na proces widzenia,
zdolności poznawcze oraz intelektualne. Ze względu na istotność pełnionych funkcji wielonienasycone
kwasy tłuszczowe stały się przedmiotem badań i obserwacji w dziedzinie
neuropsychiatrii, z uwzględnieniem zaburzeń rozwojowych, chorób o podłożu emocjonalnym
i psychicznym oraz pogorszenia się zdolności poznawczych w procesie starzenia
się organizmu. Liczne badania wykazały zaburzenia metabolizmu wielonienasyconych
kwasów tłuszczowych, wynikające najczęściej z ich niedoboru w takich stanach chorobowych,
jak depresja, schizofrenia czy autyzm. Ponadto patogeneza demencji starczej,
choroby Alzheimera oraz osłabienie funkcji poznawczych w procesie starzenia się została
powiązana z niedoborami w diecie wielonienasyconych kwasów tłuszczowych. Uważa
się, że suplementacja kwasami tłuszczowymi z rodziny omega-3 jest zasadnicza zarówno
w prewencji, jak i leczeniu chorób układu nerwowego.Omega-3 polyunsaturated fatty acids are essential component of the membrane

Due to the importance of their functions, polyunsaturated fatty acids have been the subject
of numerous studies and observations in the fi eld of neuropsychiatry including the
developmental disorders, emotional and mental disorders, decreased cognitive abilities
associated with the aging.
Several studies have found abnormal metabolism of polyunsaturated fatty acids, mostly
due to their defi ciency in diseases such as depression, schizophrenia and autism. In addition,
the pathogenesis of dementia, Alzheimer’s disease and cognitive decline in the aging
was associated with defi ciencies in dietary polyunsaturated fatty acids. It is believed that
supplementation with the fatty acids omega-3 family is essential in the prevention and
