    The efficacy of preoperative PET/CT for prediction of curability in surgery for locally advanced gastric carcinoma

    <p>Abstract</p> <p>Background</p> <p>The benefits of preoperative <sup>18</sup>FDG-PET/CT for gastric cancer remain uncertain. The aim of this study was to investigate the effects of preoperative <sup>18</sup>FDG-PET/CT on the surgical strategy for locally advanced gastric cancer retrospectively.</p> <p>Methods</p> <p>From January 2007 to November 2008, <sup>18</sup>FDG-PET/CT was performed in 142 patients who had been diagnosed with advanced gastric cancer by computed tomography or gastrofiberscope findings.</p> <p>Results</p> <p>Detection rates were 88.7% (126/142) for primary tumors and 24.6% (35/142) for local lymph nodes (LN). Nine patients with metastatic lesions underwent induction chemotherapy without operation. Of 133 patients subjected to operation, positive FDG uptake in primary tumors (<it>p </it>= 0.047) and local lymph nodes (<it>p </it>< 0.001) was related to non-curable operations. The mean standard uptake value (SUV) of primary tumors of patients who underwent non-curable operations was significantly higher than that of patients with curable operations (<it>p </it>= 0.001). When the SUV was greater than 5 and FDG uptake of LN was positive, non-curable operations were predicted with a sensitivity of 35.2%, a specificity of 91.0% and an accuracy of 76.7%.</p> <p>Conclusions</p> <p>High SUV of the primary tumor and positive FDG uptake in local lymph nodes at PET/CT could predict non-curative resection in locally advanced gastric cancer. Therefore, information from preoperative PET/CT can help physician decisions regarding other modalities without laparotomy.</p

    Impact of Socioeconomic Status on 30-Day and 1-Year Mortalities after Intensive Care Unit Admission in South Korea: A Retrospective Cohort Study

    Background Socioeconomic status (SES) is closely associated with health outcomes, including mortality in critically ill patients admitted to intensive care unit (ICU). However, research regarding this issue is lacking, especially in countries where the National Health Insurance System is mainly responsible for health care. This study aimed to investigate how the SES of ICU patients in South Korea is associated with mortality. Methods This was a retrospective observational study of adult patients aged ≥20 years admitted to ICU. Associations between SES-related factors recorded at the time of ICU admission and 30-day and 1-year mortalities were analyzed using univariable and multivariable Cox regression analyses. Results A total of 6,008 patients were included. Of these, 394 (6.6%) died within 30 days of ICU admission, and 1,125 (18.7%) died within 1 year. Multivariable Cox regression analysis found no significant associations between 30-day mortality after ICU admission and SES factors (P>0.05). However, occupation was significantly associated with 1-year mortality after ICU admission. Conclusions Our study shows that 30-day mortality after ICU admission is not associated with SES in the National Health Insurance coverage setting. However, occupation was associated with 1-year mortality after ICU admission

    Development of Formation Flying CubeSats and Operation Systems for the CANYVAL-C Mission: Launch and Lessons Learned

    The CubeSat Astronomy NASA and Yonsei using Virtual telescope ALignment for Coronagraph (CANYVAL-C) is a technology demonstration mission that shows the concept of a virtual space telescope using two CubeSats in formation flying. The final goal of the mission is to obtain several images of the solar corona during an artificial solar eclipse created by the two CubeSats, Timon (1U CubeSat) and Pumbaa (2U CubeSat). To implement this mission, two CubeSats in formation flying and a ground segment have been developed. The CubeSats were constructed mainly with commercial off the shelf components, sharing the bus architecture. The payload of each CubeSat is a visible camera and an occulter to block the light from the photosphere of the Sun. The occulter is composed of tape measures and a black-colored polyimide film; the system size is smaller than 0.5U (10 × 10 × 5 cm3) while it stowed and enlarged to 0.75 × 0.75 m2 after spreading the film. The 3D-printed propulsion system is smaller than 0.5U and facilitates accurate positioning maneuvers of Pumbaa. The on-board computer has multi-task processing capabilities and a space-saving configuration which is integrated with the GNSS receiver and the UHF transceiver. The core technology for the mission implementation is the precise formation flying guidance, navigation, and control system with a cold-gas propulsion system and an inter-satellite link system. The specification of each CubeSat system was evaluated using numerical simulations and ground testing. To operate CubeSats, the ground segment was constructed with some components, including the UHF ground station (UGS), flight dynamics system (FDS), mission analysis and planning system (MAPS), and spacecraft operation system (SOS). Each component works under the environment of an integrated graphic user interface. In particular, the UGS handles the RF communication, data storage, and instrument control for tracking CubeSats. The FDS processes the navigation data to precisely estimate absolute position and velocity. Then, the MAPS determines the allowable mission schedule and parameter set for implementing maneuvers of each CubeSat. Using the MAPS, feasibility of the mission operation canbe ensured through numerical simulations based on the solutions from the FDS. Finally, the SOS is the interface system between each component, processing telemetry and generating telecommand. The CubeSats were launched on March 22, 2021, by Soyuz-2.1a with a Fregat stage
