    Prevention of Elder Mistreatment in Nursing Homes: Competencies for Direct-Care Staff

    Existing training on elder mistreatment in nursing homes focuses on detection and reporting of abuse, with little training specifically targeted toward prevention of mistreatment before it occurs. We used qualitative interviews with nursing home staff, policymakers, and related professionals to identify training needs. Based on participant accounts, we drafted a number of competencies essential for caregiver training to prevent mistreatment in nursing homes. Competencies include those dealing with: definitions and policies; risks for mistreatment; communication and respect; and development of a cooperative working environment. Competencies are discussed along with illustrative examples, and implications for practice and policy are addressed

    Toward sustainable development: Implications for population aging and the wellbeing of elderly women in developing countries

    Attaining sustainable development has significant implications for population age structure, family structure and the wellbeing of elderly women. If one of the primary goals of sustainable development is reducing fertility to attain a population growth rate which can be supported by the Earth's resources, then working toward sustainable development will lead to an aging population. This demographic change coupled with other impacts of working toward sustainable development could significantly affect the status and wellbeing of elderly women.     Labor force status transitions at older ages in the Philipines, Singapore, Taiwan, and Thailand, 1970--1990

    While there is considerable evidence of a trend toward earlier retirement in the United States and Europe, trends in rapidly growing economies in other parts of the world have not been closely examined. This paper traces the labor force participation rates of older men and women in four Asian countries, the Philippines, Singapore, Taiwan, and Thailand, over the period of 1970 to 1990. Aggregate census data are used to calculate net transitions into and out of the labor force so as to permit the analysis of differences by country and cohort as well as change over time. Results show that, although men exhibit a general trend toward earlier net labor force exit, labor force participation rates at older ages remain high.     Integrating Mental Health and HIV Services in Zimbabwean Communities: AĀ Nurse and Community-led Approach toĀ ReachĀ theĀ Most Vulnerable

    Alcohol use and depression negatively impact adherence, retention in care, and HIV progression, and people living with HIV (PLWH) have disproportionately higher depression rates. In developing countries, more than 76% of people with mental health issues receive no treatment. We hypothesized that stepped-care mental health/HIV integration provided by multiple service professionals in Zimbabwe would be acceptable and feasible. A three-phase mixed-method design was used with a longitudinal cohort of 325 nurses, community health workers, and traditional medicine practitioners in nine communities. During Phase 3, 312 PLWH were screened by nurses for mental health symptoms; 28% were positive. Of 59 PLWH screened for harmful alcohol and substance use, 36% were positive. Community health workers and traditional medicine practitioners screened 123 PLWH; 54% were positive for mental health symptoms and 29% were positive for alcohol and substance abuse. Findings indicated that stepped-care was acceptable and feasible for all provider types

    Do family relationships buffer the impact of disability on older adults' daily mood? An exploration of gender and marital status differences

    OBJECTIVE: We evaluate whether non-spousal family support and strain moderate the effect of disability on two daily emotions (happiness and frustration) among older adults, and whether these patterns differ by gender among married persons, and by marital status among women. BACKGROUND: Stress buffering perspectives predict that harmful effects of stress on well-being are buffered by family support, whereas stress proliferation models suggest these effects are intensified by family strain. The extent to which family relationships moderate associations between stress and well-being may vary on the basis of gender and marital status, as non-spousal family ties are considered especially salient for women and those without a romantic partner. METHOD: Daily diary data are from the 2013 Disability and Use of Time supplement to the Panel Study of Income Dynamics (n=1,474), a national sample of adults ages 60+. Multivariate regression models are estimated for married/partnered men and women, and formerly married women. RESULTS: Neither family support nor strain moderated the effect of severe impairment on married men's daily emotions. Family support buffered the effect of severe impairment on frustration among divorced and widowed women, but not their married counterparts. Counterintuitively, family arguments mitigated against frustration and increased happiness among married women with severe impairment. CONCLUSION: Consistent with stress buffering perspectives, family support was most protective for the vulnerable population of formerly married older women with severe impairment.     An experimental comparison of composite and grab sampling of stream water for metagenetic analysis of environmental DNA

    Use of environmental DNA (eDNA) to assess distributions of aquatic and semi-aquatic macroorganisms is promising, but sampling schemes may need to be tailored to specific objectives. Given the potentially high variance in aquatic eDNA among replicate grab samples, compositing smaller water volumes collected over a period of time may be more effective for some applications. In this study, we compared eDNA profiles from composite water samples aggregated over three hours with grab water samples. Both sampling patterns were performed with identical autosamplers paired at two different sites in a headwater stream environment, augmented with exogenous fish eDNA from an upstream rearing facility. Samples were filtered through 0.8 Ī¼m cellulose nitrate filters and DNA was extracted with a cetyl trimethylammonium bromide procedure. Eukaryotic and bacterial community profiles were derived by amplicon sequencing of 12S ribosomal, 16S ribosomal, and cytochrome oxidase I loci. Operational taxa were assigned to genus with a lowest common ancestor approach for eukaryotes and to family with the RDP Classifier software for prokaryotes. Eukaryotic community profiles were more consistent with composite sampling than grab sampling. Downstream, rarefaction curves suggested faster taxon accumulation for composite samples, and estimated richness was higher for composite samples as a set than for grab samples. Upstream, composite sampling produced lower estimated richness than grab samples, but with overlapping standard errors. Furthermore, a bimodal pattern of richness as a function of sequence counts suggested the impact of clumped particles on upstream samples. Bacterial profiles were insensitive to sample method, consistent with the more even dispersion expected for bacteria compared with eukaryotic eDNA. Overall, samples composited over 3 h performed equal to or better than triplicate grab sampling for quantitative community metrics, despite the higher total sequencing effort provided to grab replicates. On the other hand, taxon-specific detection rates did not differ appreciably and the two methods gave similar estimates of the ratio of the common fish genera Salmo and Coregonus at each site. Unexpectedly, Salmo eDNA dropped out substantially faster than Coregonus eDNA between the two sites regardless of sampling method, suggesting that differential settling affects the estimation of relative abundance. We identified bacterial patterns that were associated with eukaryotic diversity, suggesting potential roles as biomarkers of sample representativeness

    Socioeconomic Status and Biological Markers of Health: An Examination of Adults in the United States and Taiwan

    Abstract Objective:The study documents whether socioeconomic status (SES) differentials in biological risk are more widely observed and larger in the United States than Taiwan. Method: Data come from the Social Environment and Biomarkers of Aging Study in Taiwan and the Midlife in the United States study. We use regression analyses to test whether four summary measures of biological risk are significantly related to categorical measures of education, income, and subjective social status among four country-sex-specific subgroups. Results: Physiological dysregulation is significantly, negatively related to SES in both the United States and Taiwan, especially for males. The prevalence and magnitude of the relationships are similar in the two countries: 12 of 24 possible SES-biological summary score relationships are significant in the United States and 11 of 24 are significant in Taiwan.
