Evaluation of outcomes following thermal, open and arthroscopic glenohumeral capsulorrhaphy for recurrent anterior instability
PURPOSE: To compare glenohumeral joint position sense (JPS), concentric internal (IR) and external rotation (ER) strength, functional ability, and level of satisfaction in patients who underwent three types of glenohumeral capsulorrhaphy with age-matched controls. RESEARCH DESIGN: Four 4x2 and two 4x3 ANOVAs were used to identify differences in JPS and concentric IR/ER strength between groups: Open Capsulorrhaphy (n=21), Thermal Capsulorrhaphy (n=16), Arthroscopic Capsulorrhaphy (n=14) and Controls (n=22). Pearson correlation analyses were performed to determine the relationship between objective American Shoulder and Elbow Surgeons (ASES) evaluations and subjective Shoulder Rating Questionnaire (SRQ) scores. Stepwise multiple regression analyses were performed to predict ASES and SRQ scores from various objective and subjective outcome measures. SUBJECTS: 73 adults (51 postsurgical patients, 22 healthy controls; mean age, 23.7 ± 6.8 yrs) participated in this retrospective study. The 51 patients who underwent capsulorrhaphy for recurrent, anterior glenohumeral instability were evaluated at an average of 32.1 months postsurgery. MEASUREMENTS: JPS was measured bilaterally using a reproduction of passive positioning protocol at 2 target angles: 60% and 90% of maximum passive external rotation (60% and 90% ER[subscript max]). Concentric IR and ER peak torques were measured bilaterally at 90°/sec, 180°/sec and 270°/sec. Objective postoperative function was quantified with the clinician-based ASES form, while functional status and patient satisfaction were assessed with the patient-based SRQ form. RESULTS: The accuracy of JPS in patients' surgical limbs was similar to that present in their contralateral, uninjured shoulders at both target angles. The Open group demonstrated significantly better involved-limb JPS acuity (4.2° ± 1.9°) than the Arthroscopic group (6.8° ± 3.2°) and Control group (6.6° ± 3.5°) (p<.05). However, the Open group had 31% less IR strength than Control subjects and 33% less than the Arthroscopic group, with IR peak torques significantly less in their postsurgical shoulders than their uninvolved limbs (p<.002). There was a strong, positive correlation (r =.64, p≤.001) between objective ASES and subjective SRQ scores. Patients' postoperative level of pain and ASES scores were significant predictors of their SRQ clinical scores (R=.81, p<.003). CONCLUSIONS: Glenohumeral JPS and rotator cuff strength were similar in both the postsurgical and uninvolved shoulders of the Arthroscopic and Thermal groups. Patients in the Open capsulorrhaphy group demonstrated significantly better involved-limb JPS than Arthroscopic and Control groups. The large strength deficits observed in the Open group, particularly in IR, were of significant concern. We observed a higher failure rate, more revision surgeries, and lower patient satisfaction with the Thermal capsulorrhaphy technique. Patient-based outcomes were significant predictors of operative success as measured by clinician-based evaluation. Prospective, randomized controlled studies are still needed to evaluate the outcomes of these glenohumeral capsulorrhaphy procedures over the longer term

The Interagency Coordinating Committee for Airborne Geosciences Research and Applications (ICCAGRA) was established to improve cooperation and communication among agencies sponsoring airborne platforms and instruments for research and applications, and to serve as a resource for senior level management on airborne geosciences issues. The Interagency Working Group for Airborne Data and Telecommunications Systems (IWGADTS) is a subgroup to ICCAGRA for the purpose of developing recommendations leading to increased interoperability among airborne platforms and instrument payloads, producing increased synergy among research programs with similar goals, and enabling the suborbital layer of the Global Earth Observing System of Systems
Type IIn Supernova Detections in z ~ 2 Lyman Break Galaxies: Probing the IMF Directly
Type IIn supernovae (SNe IIn) exhibit luminous ultraviolet continua during outburst and luminous, long-lived narrow ultraviolet and optical emission lines attributed to circumstellar interaction. These properties have enabled successful detections at z ~ 2 in archival imaging and continued investigations from late-time spectroscopy. Because SNe IIn are believed to have massive (≳50M_☉) progenitors, searches in the well-studied Lyman break galaxy (LBG) host population offer the prospect of testing the form of the high-redshift stellar initial mass function (IMF) in a high density star formation environment directly. I briefly discuss our z ~ 2 photometric detection method targeting LBGs in the Canada-France-Hawaii Telescope Legacy Survey (CFHTLS) and present data from the first 6 confirmed z ~ 2 SNe IIn pulled from 30 photometric SN candidates. A comparison of the color and magnitude distributions of the SN host galaxies to that of the full LBG sample finds that z ~ 2 SNe preferentially occur in bluer, fainter galaxies. I conclude with a discussion of an approach that uses the CFHTLS pilot sample to provide a first estimate of the form of the high-redshift IMF. Upcoming deep synoptic imaging surveys will greatly improve z ~ 2 SNe IIn statistics from ~10^5 expected detections and future large aperture space- and ground-based telescopes will have the sensitivities to extend this work to z ≳ 6

Managing terror when self-worth and worldviews collide: Evidence that mortality salience increases reluctance to self-enhance beyond authorities.
a b s t r a c t Terror management theory posits that one's self-esteem and worldview operate jointly to manage mortality concerns. Accordingly, past research shows that mortality salience (MS) increases selfenhancement and worldview defense. The current research is the first to examine MS effects when self-enhancement threatens to undermine aspects of the worldview, in this case the credibility and status of worldview-representative authorities. MS led to reluctance to self-enhance following positive personality test feedback when the test was judged negatively by institutional authorities (Study 1a), as well as unwillingness to contradict self-esteem threatening feedback sanctioned by authorities (Study 1b). Mortality salient participants also rated themselves higher on valued dimensions unless it meant viewing themselves more positively than their parents (Study 2) and admired political icons (Study 3). Taken together, these results show that MS increases self-enhancement unless doing so challenges important representatives of the worldview. Theoretical and practical implications are discussed
From development to implementation with a fully integrated downstream bioprocess
Boehringer Ingelheim and Pfizer have developed a unique continuous bioprocess consisting of a short duration perfusion upstream and fully integrated downstream. The process is designed to generate at least 1 kg of material from a 100 L bioreactor in approximate 2 weeks. The upstream strategy utilizes a non-steady state, short duration perfusion process to achieve volumetric productivities ranging from 0.5 to 4 g/L/day. This creates a unique challenge for the downstream process as the titer and impurity load change over the duration of the culture. To accommodate upstream, a fully integrated downstream process was created using a combination of traditional batch unit operations and continuous bioprocessing. The system design includes a pair of small proteins A columns operated consecutively, a continuous low pH inactivation chamber (cVI), an anion exchange chromatography column, a single pass tangential flow filter, a virus filter and batch UFDF. The key to the system is three distinct operation modes including continuous, periodic and batch phases. The resulting hybrid system provides flexible and robust downstream processing of the continuous perfusion bioreactor. We have successfully used this new downstream process at the predicted manufacturing scale to generate clinical quality drug substance for multiple monoclonal antibodies. With the success of the new integrated system, our team is shifting its focus to implementation on a clinical program. A key element of the ongoing work is to establish the control and robustness of novel elements of our process such as confirming that the critical pH has been achieved during cVI and demonstrating robust impurity removal for dynamic loading on an AEX polishing step. In this talk, we will explain our approach to integrated continuous downstream processing and our strategy for future implementation. Data from at-scale demonstration runs will show the robustness of the process over a wide range of loading conditions. This will include product quality data including HCP, DNA, charge variants and aggregate removal that is consistent with batch processes.
