    Australian cardiac rehabilitation exercise parameter characteristics and perceptions of high-intensity interval training: a cross-sectional survey

    Purpose: This study explored current demographics, characteristics, costs, evaluation methods, and outcome measures used in Australian cardiac rehabilitation (CR) programs. It also determined the actual usage and perceptions of high-intensity interval training (HIIT). Methods: A cross-sectional observational web-based survey was distributed to 328 Australian CR programs nationally. Results: A total of 261 programs completed the survey (79.6% response rate). Most Australian CR programs were located in a hospital setting (76%), offered exercise sessions once a week (52%) for 6ā€“8 weeks (49%) at moderate intensity (54%) for 46ā€“60 min (62%), and serviced 101ā€“500 clients per annum (38%). HIIT was reported in only 1% of programs, and 27% of respondents believed that it was safe while 42% of respondents were unsure. Lack of staff (25%), monitoring resources (20%), and staff knowledge (18%) were the most commonly reported barriers to the implementation of HIIT. Overall, Australian CR coordinators are unsure of the cost of exercise sessions. Conclusion: There is variability in CR delivery across Australia. Only half of programs reassess outcome measures postintervention, and cost of exercise sessions is unknown. Although HIIT is recommended in international CR guidelines, it is essentially not being used in Australia and clinicians are unsure as to the safety of HIIT. Lack of resources and staff knowledge were perceived as the biggest barriers to HIIT implementation, and there are inconsistent perceptions of prescreening and monitoring requirements. This study highlights the need to educate health professionals about the benefits and safety of HIIT to improve its usage and patient outcomes

    Retrofitting and Rehabilitation of Vernacular housing in Flood Prone Areas in Sri Lanka

    This paper presents findings from an investigation into applications to improve the structural resilience and safety of low-rise vernacular masonry homes when subject to extreme flooding. In 2016 and 2017 flooding brought devastation throughout many areas in Sri Lanka. Findings from field investigations to evaluate, characterize, and quantify the extent and nature of structural damage to low rise vernacular masonry houses from these flood events are presented. Low cost solutions were developed to enhance the flexural capacity of masonry walls using reinforced plasters. Single storey homes in rural areas are particularly at risk from rapid flood events, and limited evacuation opportunities require a means of in-situ refuge. Focusing on these risks, a unique retrofitting project, including an elevated refuge area for occupants to escape and shelter during flood events, is also presented. This research will directly improve the welfare of vulnerable communities living in flood risk areas, minimizing the risk of flood induced structural failure, while enabling people to safely remain in their homes

    High-intensity interval training versus moderate-intensity continuous training within cardiac rehabilitation:a systematic review and meta-analysis

    Aerobic capacity has been shown to be inversely proportionate to cardiovascular mortality and morbidity and there is growing evidence that high-intensity interval training (HIIT) appears to be more effective than moderate-intensity continuous training (MICT) in improving cardiorespiratory fitness within the cardiac population. Previously published systematic reviews in cardiovascular disease have neither investigated the effect that the number of weeks of intervention has on cardiorespiratory fitness changes, nor have adverse events been collated.We aimed to undertake a systematic review and meta-analysis of randomized controlled trials (RCTs) within the cardiac population that investigated cardiorespiratory fitness changes resulting from HIIT versus MICT and to collate adverse events.A critical narrative synthesis and meta-analysis was conducted after systematically searching relevant databases up to July 2017. We searched for RCTs that compared cardiorespiratory fitness changes resulting from HIIT versus MICT interventions within the cardiac population.Seventeen studies, involving 953 participants (465 for HIIT and 488 for MICT) were included in the analysis. HIIT was significantly superior to MICT in improving cardiorespiratory fitness overall (SMD 0.34 mL/kg/min; 95% confidence interval [CI; 0.2-0.48]; p6-week duration. Programs of 7-12 weeks' duration resulted in the largest improvements in cardiorespiratory fitness for patients with coronary artery disease. HIIT appears to be as safe as MICT for CR participants

In this review, we discuss classical human psychophysics studies as well as those using engineered systems that aim to develop models of motor control of the upper limb. We present evidence for a hybrid model of motor control, which has an evolutionary advantage due to division of labor between cerebral hemispheres. Stopping is a fundamental aspect of movement that deserves more attention in research than it currently receives. Such research may provide a basis for understanding arm stabilization deficits that can occur following central nervous system (CNS) damage

    Statistical comparisons of non-deterministic IR systems using two dimensional variance

    Retrieval systems with non-deterministic output are widely used in information retrieval. Common examples include sampling, approximation algorithms, or interactive user input. The effectiveness of such systems differs not just for different topics, but also for different instances of the system. The inherent variance presents a dilemma - What is the best way to measure the effectiveness of a non-deterministic IR system? Existing approaches to IR evaluation do not consider this problem, or the potential impact on statistical significance. In this paper, we explore how such variance can affect system comparisons, and propose an evaluation framework and methodologies capable of doing this comparison. Using the context of distributed information retrieval as a case study for our investigation, we show that the approaches provide a consistent and reliable methodology to compare the effectiveness of a non-deterministic system with a deterministic or another non-deterministic system. In addition, we present a statistical best-practice that can be used to safely show how a non-deterministic IR system has equivalent effectiveness to another IR system, and how to avoid the common pitfall of misusing a lack of significance as a proof that two systems have equivalent effectiveness

    Erosion of trust in humanitarian agencies: what strategies might help?

    Aid agencies (AAs) provide a range of humanitarian and health related assistance globally. However, the trust placed on them is eroding. Evidence for this includes accusations of a decline in their humanitarianism, and the increasing number of conflicts with host states. An analysis of the concerns expressed yields two possible reasons: a relative lack of transparency of their work and weak accountability mechanisms. This is further supported by the existing milieu: an absence of internationally accepted instrument or mechanism to check the credentials of INGOs and an opaque system of close links between some of the INGOs and their donors. The article suggests two global strategies to tackle these issues: (a) Increase transparency by establishing a global register of aid agencies. This should have basic information: their main goals and activities, countries they are active in, number of employees, annual turnover of funds (updated regularly), principal financing sources and nature of links with donors. This could also be available as printed manual that should be freely available to client countries. (b) Ensure accountability by developing templates of fair legal instruments (to facilitate and regulate work), and a set of generic rules and procedures of engagement for the interactions between agencies and client states. These should be institutionalized within the regulatory frameworks of countries and included in the Codes of Conduct of NGOs

    Land-Use Change and Its Impact on Urban Flooding: A Case Study on Colombo District Flood on May 2016

    Colombo district has become an increasingly congested urban society. It has been reported that the frequent flooding in the Colombo district occurs due to the shrinking of open spaces, illegal constructions, and lack of suitable waste disposal facilities. Therefore, this study focuses on analyzing the impact of land-use change on the flood of Colombo district in May 2016 in comparison to the land-use during the flood in 1989. Accordingly, Landsat images were utilized to identify the land-use by using NDVI, NDBI, and NDWI indices. Out of the several techniques examined, SVM classification was chosen, and change detection techniques in conjunction with remote sensing and GIS environment were adopted. SVM classification showed the highest accuracy for land-use classification, which was 99.0% in 1989 and 99.9% in 2016. The comparison of land-use changes of 1989 and 2016 with similar flood extent of the Colombo district proved that the area of the Kelani river watershed changed into urban area, having a significant impact on flood inundation. The Kelani river watershed includes 23% of the total urban area of the Colombo district. Similarly, the entire area of land-use transformation covered 37.7% of the area within the watershed region of the Colombo district. Eventually, this research identified the significant impact of Colombo district floods in May 2016 on land-use changes
