Avaliaçao microbiológica entre cultura semiquantitativa de biópsia e cultura de swab e sua correlaçao com aspectos histopatológicos nas queimaduras

Defesa: Curitiba, 21/02/2006Inclui bibliografia e anexosO diagnóstico e a etiologia de uma infecção em queimados apresentam inúmeras dificuldades, sendo difícil determinar a presença e o grau de infecção na ferida. A freqüência e a gravidade de complicações sépticas impõem a necessidade de um acurado diagnóstico e de uma apropriada terapêutica. O estudo avaliou as características microbiológicas da queimadura com o uso de biópsia, examinando a associação entre resultados de cultura semiquantitativa e achados histopatológicos. Foi coletado um total de 140 biópsias e 70 swabs, entre o sétimo e o nono dia após a queiamdura, de 70 pacientes admitidos na unidade de queimados do Hospital Universitário Evangélico de Curitiba (HUEC) - Curitiba-PR, no período de um ano. A biópsia, obtida com punch (4mm), foi encaminhada para análise microbiológica e histopatológica. Pela cultura semiquantitativa, o número de UFC/g de tecido foi obtido por meio da fórmula N x V x v/w, e foram consideradas como indício de infecção contagens > 10 elevado a 5 UFC/g de tecido. Os resultados histopatológicos foram feitos pelas colorações de hematoxilina-eosina e de Gram, quanto à visualização de microrganismos no tecido necrótico e no tecido viável, indicando infecção. A predominância de homens (59%) foi observada em quase todas as faixas de idade, sendo o álcool (46%) o agente envolvido na maioria dos casos. A média do tamanho da
queimadura foi de 32% SCQ. A taxa de mortalidade observada foi de 10%. Os microrganismos foram identificados e testados quanto à sensibilidade a antimicrobianos pelo método de microdiluição em ágar. P. aeruginosa (44%) foi o patógeno causador de infecção mais comum, seguido por A. baumannii (31%), S. aureus (13%) e outros (12%). As drogas mais ativas contra os microrganismos comumente isolados foram
polimixina (100% de sensibilidade) para P. aeruginosa, e A. baumannii e vancomicina (100%) e linezolide (100%) para S. aureus. Infecções causadas por MRSA foram freqüentes (67%) do total de S. aureus. Das 28 análises histológicas positivas 21 apresentaram infecção, e nessas, bactérias gram-negativas estiveram envolvidas em 81% dos casos. Concordâncias de 100% e 90% foram encontradas entre culturas negativas ou com baixos níveis de crescimentos e achados histológicos negativos, respectivamente. Em contraste, infecção histológica ocorreu em apenas 43% de amostras com cultura > 10 elevado a 5 UFC/g de tecido. A boa concordância entre identificações histológicas e microbiológicas mostra que a cultura semiquantitativa e a análise histológica de biópsia poderiam ser vistas como adjuntas no entendimento de possíveis infecções no paciente queimado, fornecendo informações úteis com relação à quantificação da microbiota predominante, seus padrões de sensibilidade e sua profundidade, na ferida por queimadura. HistopatológicoThe diagnoastic and the etiology of infection in burned tissued present countless
difficulties, being dificult to define the presence and the grade of the infection on the
wound. The frequency and gravity of the sepsis complications impose the necessity of
an accurate diagnosis and proper therapeutic. The study evaluated the microbiological
characteristics of the burn wound by the usage of biopsy, examining the association
between results from semiqutitaive culture and histopatologic findings. It has been
colected a total of 140 biopsies and 70 swabs, between the seventh and ninth day after
of burned, of 70 patients admitted in the burning unit from Hospital Universitário
Evangélico de Curitiba – Curitiba-PR (HUEC), during a period of one year. The biopsy
has been obtained with punch (4 mm) and sent to microbiologic and histopathology
analysis. Through semiquantitative culture the number of UFC/g of tissue was obtained
by the N x V x v/w formula, and considering as infecion indication the countings with >
10 elevado a 5 UFC/g of tissue. The histopatological results has been made by the hematoxylineosin(
HE) and Gram colorations to the visualization of microorganisms in the necrotic
tissue and viable tissue, indicating infection. The predominancy of men (59%) has been
observed in most of age ranges, being alcohol (46%) the agent involved in most of
cases, the medium size of burn wound were 32% of TSBA and the mortality rate
observed reached 10%. The microorganims has been identified and tested upon
susceptibility to antimicrobial agents by agar diluition method. P. aeruginosa (44%) has
been the patogen more usual, causing infection, followed by A. baumannii (31%), S.
aureus (13%) and others(12%). The most active drugs against the microorganisms
comonly isolated were polimix (100% of sensibility) to P. aeruginosa and A. baumannii
and vancomycin (100%) and linezolid(100%) to S. aureus. Infections caused by MRSA
were frequents (67%) of S. aureus. From the 28 positive histopatological analisys 21 has
been infection, and in these gram-negative bacterias were involved in 81% of cases.
Agreement of 100% and 90% has been encountered between negative cultures or low
levels of growing and negative histologic findings, respectively. In contrast, histologic
infection happenden only in 43% of samples with culture > 10 elevado a 5 UFC/g of tissue. The
good agreement between the histologic and microbiologic identifications show that the
semiquantitative culture and the biopsy histologic analisys could be viewed as an adjunt
in the understanding of possible infections in the burned patient, suppling useful
informations with relation of predominant flora quantification, their standarts of sensibility
and the depth of this burn wound

Systematic review on fecal calprotectin in cystic fibrosis
Objectives: Fecal calprotectin is an inflammatory marker used for monitoring intestinal diseases. It has been studied as a marker of intestinal inflammation in cystic fibrosis (CF), a multi-systemic genetic disease caused by alterations to the CFTR gene. Manifestations of the disease favor a systemic inflammation not limited to the respiratory tract, therefore, calprotectin is a non-invasive and effective diagnostic method. The aim of the study was to perform a systematic review of the literature with a qualitative synthesis of studies. Sources: The articles were selected from PubMed, Web of Science, Scielo and Lilacs. Summary of the findings: Nine studies were selected for that qualitative synthesis, one was a randomized clinical trial, and eight were case-control or cohort designs. Most studies have indicated that calprotectin is a marker of systemic inflammation in CF and not just intestinal inflammation. Calprotectin is an aid in monitoring inflammatory bowel conditions in patients with cystic fibrosis. Conclusion: Further studies should be conducted to investigate the role of this marker in the systemic inflammation of CF
