Immune Regulation in the Male Genital Tract
Spermatozoa are not produced until puberty, long after the establishment of tolerance to self-antigens.
Therefore, sperm-specific antigens are immunogenic in men. Most men, however, do not
produce antibodies to their own gametes. Development of mechanisms to prevent or limit autoimmune
responses to spermatozoa were essential for preservation of reproductive capacity. Tight
junctions between adjacent Sertoli cells, as part of the blood-testis barrier, prevent sperm-immune
cell contact. In some portions of the genital tract this barrier is thin or incomplete. Immune mechanisms
have evolved to actively suppress the autoimmune response to spermatozoa within the genital
tract. Unlike in the circulation where CD4+ helper T lymphocytes predominate, CD8+ suppressor/cytotoxic T lymphocytes are the most prominant T cells in the epididymis and vas deferens.
In addition, spermatozoa suppress pro-inflammatory lymphocyte immune responses, possibly by inducing
production of anti-inflammatory cytokines. Antisperm antibody production is induced in the
male genital tract when a local infection or disruption in the genital tract physical barrier leads to
an influx of CD4+ T cells. In response to induction of a productive immune response, two additional
mechanisms downregulate humoral immunity within the genital tract. T lymphocytes possessing
the γσ form of the antigen receptor (γσ T cells) are concentrated in the male genital tract and in
semen. These cells become activated and proliferate in men with evidence of sperm autoimmunity.
Activated γσ T cells inhibit production of antibodies by activated B lymphocytes, thereby limiting
antisperm antibody production. Heat shock proteins (hsps) are also present in semen in association
with infection and antisperm antibody formation. Hsp gene transcription leads to inhibition of
transcription of the genes coding for pro-inflammatory cytokines and, conversely, to activation of
γσ T cells. Activated γσ T cells also promote hsp synthesis. The mechanisms to inhibit immunity
to sperm may hinder effective immune elimination of microoganisms in the male genital
tract
Immune Recognition of the 60kD Heat Shock Protein: Implications for Subsequent Fertility
The 60kD heat shock protein (hsp60) is a highly conserved protein and a dominant antigen of
most pathogenic bacteria. In some women, chronic or repeated upper genital tract infections with
Chlamydia trachomatis, and possibly with other microorganisms, induces immune sensitization to
epitopes of hsp60 that are present in both the microbial and human hsp60. Once a woman becomes
sensitized to these conserved epitpes, any subsequent induction of human or bacterial hsp60
expression will reactivate hsp60-sensitized lymphocytes and initiate a pro-inflammatory immune
response. Hsp60 is expressed during the early stages of pregnancy, by both the embryo and the
maternal decidua. We examined, therefore, whether women who were sensitized to hsp60 experienced
less successful pregnancy outcomes compared to women who were not sensitized to this
antigen. In women undergoing in vitro fertilization (IVF), the presence of cervical IgA antibodies
reactive with the C. trachomatis hsp60 correlated with implantation failure after embryo transfer.
Further analysis revealed that an immunodominant epitope for these IgA antibodies was an hsp60
epitope shared between C. trachomatis and man. In subsequent studies of women not undergoing
IVF, cervical IgA antibodies to the human hsp60 were identified in 13 of 91 reproductive age
women. This antibody was most prevalent in those women with a history of primary infertility
(p = 0.003). In addition, cervical anti-hsp60 IgA correlated with the detection of the pro-inflammatory
cytokines interferon-γ (p = 0.001) and tumor necrosis factor-α (p = 0.02) in the cervix.
Conversely, women with proven fertility had the highest prevalence of the anti-inflammatory cytokine,
interleukin 10, in their cervices (p = 0.001). In an analysis of serum samples in a third study,
women with a history of two or more consecutive first trimester spontaneous abortions had a higher
prevalence (p = 0.01) of IgG antibodies to the human hsp60 (36.8%) than did age matched fertile
women (11.1%) or women with primary infertility (11.8%). Immune sensitization to epitopes expressed
by the human hsp60 may reduce the probability of a successful pregnancy outcome due to
reactivation of hsp60-reactive lymphocytes, induction of a pro-inflammatory cytokine response and
interference with early embryo development and/or implantation
Experimentieren im Physikunterricht: Wechselwirkung zwischen Überzeugungen von Lehrkräften und unterschiedlichen Experimentiersituationen
Im Rahmen einer umfangreichen Längsschnittstudie zum Kompetenzerwerb im Physikunterricht (KoPhy) wurde unter anderem der Lernzuwachs durch unterschiedliche Experimentiersituationen untersucht. Aktuelle Forschungsergebnisse lassen die Vermutung gerechtfertigt erscheinen, dass nicht das Experimentieren an sich – als Schülerexperiment oder als Demonstrationsexperiment – für den Lernerfolg ausschlaggebend ist. Vielmehr scheint eine Wechselwirkung zwischen der jeweilig unterrichtenden Lehrkraft und der Experimentiersituation zu bestehen. In der hier vorgestellten Studie wird versucht, diese Wechselwirkung mit Hilfe der physikbezogenen Überzeugungen der Lehrkräfte zu erklären. In diesem Beitrag werden erste Ergebnisse präsentiert und mögliche Konsequenzen für die Unterrichtsgestaltung sowie die Physiklehrerausbildung diskutiert

Dem schließt sich eine Darstellung der Hauptstudie an, in welcher sowohl die grundlegende Fragestellung als auch die Messinstrumente vorgestellt werden. Die bisher abgeschlossenen Vorstudien zur Entwicklung eines IRT-skalierten Fachwissenstest als Messinstrument zum Einsatz in der Hauptstudie stellen den Abschluss dieses Beitrags dar

Entwicklung eines Fachwissenstests zur geometrischen Optik
Im BMBF-geförderten Projekt 'Kompetenzmessung und Kompetenzförderung in leistungsheterogenen Lerngruppen im experimentierbasierten Physikunterricht' (KoPhy) werden etwa 2000 Schüle- rinnen und Schüler in einer längsschnittlich angelegten Interventionsstudie auf den Kompetenzzuwachs in drei Experimentiersituationen untersucht. Anhand einer im Frühjahr 2016 durchgeführten Pilotstudie mit 310 Teilnehmerinnen und Teilnehmern konnten für die Hauptstudie IRT-skalierte Testhefte zur geometrischen Optik für den Kompe- tenzbereich 'Fachwissen' zusammengestellt werden. Zusammen mit vorhandenen Items zur „Erfassung prozessbezogener naturwissenschaftlicher Grundbildung“ (Glug, 2009) und dem aktuellen In- teresse der Schülerinnen und Schüler an Physik (Schulz, 2011) werden die Teilnehmerinnen und Teilnehmer der Hauptstudie umfassend befragt, um Auswirkungen der Intervention auf diese drei Aspekte zu erfassen. Die teilnehmenden Lehrkräfte erhalten zusätzlich einen Fragebogen zu ihrer Einstellung zum Fach und zur Wissenschaft Physik. Im vorliegenden Beitrag werden die Rahmenbedingungen der Hauptstudie sowie ihr Aufbau kurz skizziert. Danach folgt ein Überblick über die Erkenntnisse der Pilotstudie und deren Einfluss auf die Hauptstudie. Die Vorstellung erster deskriptiv statistischer Ergebnisse aus der Hauptstudie schließt sich daran an. Abschließend soll ein Ausblick auf die weiteren Forschungsschritte und die bisher gewonnenen Erkenntnisse gegeben werden.
Fractional Flow Reserve/ Instantaneous Wave-Free Ratio Discordance in Angiographically Intermediate Coronary Stenoses: An Analysis Using Doppler-Derived Coronary Flow Measurements
OBJECTIVES The study sought to determine the coronary flow characteristics of angiographically intermediate stenoses classified as discordant by fractional flow reserve (FFR) and instantaneous wave-free ratio (iFR).
BACKGROUND Discordance between FFR and iFR occurs in up to 20% of cases. No comparisons have been reported between the coronary flow characteristics of FFR/iFR discordant and angiographically unobstructed vessels.
METHODS Baseline and hyperemic coronary flow velocity and coronary flow reserve (CFR) were compared
across 5 vessel groups: FFRþ/iFRþ (108 vessels, n 1�4 91), FFR–/iFRþ (28 vessels, n 1�4 24), FFRþ/iFR– (22 vessels, n 1�4 22), FFR–/iFR– (208 vessels, n 1�4 154), and an unobstructed vessel group (201 vessels, n 1�4 153), in a post hoc analysis of the largest combined pressure and Doppler flow velocity registry (IDEAL [Iberian-Dutch-English] collaborators study).
RESULTS FFRdisagreedwithiFRin14%(50of366).Baselineflowvelocitywassimilaracrossall5vesselgroups,includingthe unobstructed vessel group (p 1�4 0.34 for variance). In FFRþ/iFR– discordants, hyperemic flow velocity and CFR were similar to both FFR–/iFR– and unobstructed groups; 37.6 (interquartile range [IQR]: 26.1 to 50.4) cm/s vs. 40.0 [IQR: 29.7 to 52.3] cm/s and 42.2 [IQR: 33.8 to 53.2] cm/s and CFR 2.36 [IQR: 1.93 to 2.81] vs. 2.41 [IQR: 1.84 to 2.94] and 2.50 [IQR: 2.11 to 3.17], respectively (p > 0.05 for all). In FFR–/iFRþ discordants, hyperemic flow velocity, and CFR were similar to the FFRþ/iFRþ group;
28.2 (IQR: 20.5 to 39.7) cm/s versus 23.5 (IQR: 16.4 to 34.9) cm/s and CFR 1.44 (IQR: 1.29 to 1.85) versus 1.39 (IQR: 1.06 to 1.88), respectively (p > 0.05 for all).
CONCLUSIONS FFR/iFR disagreement was explained by differences in hyperemic coronary flow velocity. Furthermore, coronary stenoses classified as FFRþ/iFR– demonstrated similar coronary flow characteristics to angiographically unobstructed vessels

Response to a sexual risk reduction intervention provided in combination with hepatitis C treatment by HIV/HCV co-infected men who have sex with men: a reflexive thematic analysis.
BACKGROUND: Hepatitis C virus reinfections in HIV-positive men-who-have-sex-with-men (MSM) challenge the effectiveness of antiviral treatment. To fight this problem, an adapted sexual risk reduction intervention was implemented within a hepatitis C treatment trial. Following this, the current study had two aims and describes 1) how the program was received by participants; and 2) their responses to the program regarding sexual risk taking. Based on the participants' input, we hoped to judge the intervention's potential for scale-up. METHODS: Seventeen participants who received the sexual risk reduction intervention in addition to hepatitis C treatment were recruited for semi-structured interviews six to 12 months post-intervention. We evaluated the responses via reflexive thematic analysis and applied the concept of sense-making. RESULTS: Giving hepatitis C a place and living without it again illustrates how participants received the program and how their experiences were altered by the impact of sense-making. Based on their responses, we allocated participants to three groups: 1. Avoid risks: get rid of hepatitis C for life. For these men, hepatitis C remained a life-threatening disease: they actively modified their risk behavior and felt supported by the intervention in maintaining their behavioral changes. 2. Minimize risks: live as long as possible without hepatitis C. In contrast to group 1, these men saw hepatitis C as a manageable disease. The intervention facilitated reflection on risks and how to develop behavioral changes that suited them individually. 3. Accept risks; live with the risk of hepatitis C. These men perceived behavioral changes as much more difficult than "easy" medical treatment. They expected to either undergo repeated rounds of treatment or stay HCV re-infected. CONCLUSION: These results illustrate the diversity of men's responses and their decisions regarding sexual risk behavior after participating in a combination of antiviral treatment and a sexual risk reduction intervention. Two major aspects were identified: 1) Teachable moments, particularly at the time of diagnosis/treatment, could offer an opportunity to develop openness for behavioral change; 2) adapting sexual risk reduction interventions to sense-making patterns could help to improve its effectiveness. Support for reducing infection risk and raising awareness of preventative measures are additional benefits. TRIAL REGISTRATION: Clinical Trial Number: NCT02785666 , 30.05.2016
